[The effect of unilateral ureteral obstruction on the contralateral renal growth in weanling rats].

The effect of unilateral hydronephrosis on the growth of contralateral kidney were evaluated with weanling rats of which kidneys were in growing stage (body weighted between 80 g and 90 g). These rats underwent complete unilateral ureteral obstruction (CUUO) on day 0. They were divided into 3 groups and went through second operations on day 3 (CUUO-3) as follows; a) CUUO released, b) nephrectomy performed and c) CUUO continued. Sham-operation was performed on day 0 and on day 3 to the controls. Wet weights and bromodeoxy-uridine (BrdU) labeling index (L.I.) (index for cell proliferation in the uriniferous tubules) of the contralateral kidneys were measured for the evaluation. The wet weights were significantly higher in all the groups than that of the controls throughout the observation period on and after the day 3. The group that underwent nephrectomy demonstrated a sharp rise in L.I. after the day 7. In the group that was continued in CUUO, the L.I. started to rise later than its rise in nephrectomised group but the index caught up at the time when the blood flow completely disappeared in the kidneys of the ligated side. L.I. in the group of which CUUO was released became significantly higher than that of controls on the day 7 and remained its significance until the day 21 in spite of resolution of the hydronephrosis. To reveal the effect of length of obstructed term, the same trial was carried out that rats endured in CUUO for 5 days (CUUO-5) till the second operation. This turned out lower L.I. in the CUUO-5 than the CUUO-3. These results suggest that the presence of hydronephrosis made by CUUO suppress the growth (cell proliferation) of contralateral kidney and also duration of obstruction can be critical on the renal growth that shorter obstructed period turns out preferable for cell proliferation after relief of CUUO.

Intra- and extraoral prostheses using osseointegrated implants after maxillofacial surgery.

Maxillary and orbital defects due to the resection of maxillary tumors in six cases were treated utilizing maxillofacial prostheses employing pure titanium osseointegrated implants (Brãnemark system). A total of 17 fixtures were installed in the maxillary region, and 16 achieved osseointegration. For the orbital region, nine fixtures were installed, and all fixtures integrated well. Using these fixtures as anchors, four maxillary prostheses and three orbital prostheses were set. The stability of the prostheses were improved by anchors, and the prostheses were highly satisfactory to the patients.

Three-dimensional computed tomographic analysis for placement of maxillofacial implants after maxillectomy.

Three-dimensional computed tomography scanning was used to examine six patients who had undergone maxillectomy and who were being considered for endosseous dental implants in the residual maxillary bone. This technique allowed three-dimensional visualization of the precise structures of the deformed maxilla, which facilitated the operation procedure by indicating optimal sites for fixture installation.

Masticatory improvement using osseointegrated implants: analysis of Japanese patients' responses through questionnaires.

Masticatory improvement obtained by the use of osseointegrated implants (Brånemark System) was analyzed through questionnaires collected from 22 Japanese patients. The questionnaires were designed to assess mastication of different Japanese foods. The postoperative score of masticatory function obtained by analysis of the questionnaires was 90.9, as opposed to the preoperative score of 43.9. A multiple regression analysis indicated that the number of placed implants has no significant effect on the improvement of masticatory function, whereas preoperative performance does. Further, the degree of masticatory improvement showed variation according to the condition of the dental arch opposing the implant prostheses. The group having implant prostheses in the opposing arch showed the highest improvement.

A correlative immuno-light and electron microscopic study on the type I collagen in the bone morphogenetic protein-induced cartilage.

Bone morphogenetic protein (BMP) partially purified from bovine bone was implanted into the thigh muscles of AKR-strain mice to induce an ectopic endochondral ossification tissue. The extracellular matrix components around the hypertrophic chondrocytes in the induced tissue were examined by electron microscopy and immunohistochemistry for type I collagen. At 14 days after implantation, chondrocytes became hypertrophic with calcifying matrix around them. Some hypertrophic chondrocytes showed no degenerative appearances. They were ringed by a pericellular matrix with no metachromasia which was distinct from the metachromatic cartilage matrix. The localization of type I collagen was demonstrated by immuno-staining in this matrix. Ultrastructurally, ring-shaped matrix consisted of interwoven collagen fibrils on which a D-periodic banding pattern could be discerned. Immunoelectron microscopy demonstrated immunoreactivity of type I collagen on these fibrils. These results suggest that some of the BMP-induced hypertrophic chondrocytes maintain their activity and produce type I collagen-rich matrix before their lacunae are eroded.

Prosthodontic closure of palatal fistula with osseointegrated implants and onlay bone grafts--case report.

A case treated with onlay bone grafting and simultaneous osseointegrated implant insertion is reported. The patient had an oronasal fistula in the center of the premaxillary region due to failed repair of a bilateral cleft palate. Four fixtures were inserted with onlay bone grafting of the maxillary alveolar ridge. Twelve months postsurgery, one fixture was lost as a result of severe bone resorption around the fixture, but three fixtures integrated strongly. We constructed an overdenture stabilized with ball attachments connected to the implants 13 months after the first operation. Speech and masticatory function improved remarkably with closure of the fistula and good fixation of the denture.

Cervicofacial actinomycosis: report of two cases.

This article presents two cases of actinomycosis. Case 1 was a 39-year-old man who was first seen with the chief complaint of swelling around the left submandibular region. Case 2 was a 40-year-old woman who was first seen with the chief complaint of mass formation around the left buccal area. Both cases were initially diagnosed as malignant tumors and were later histopathologically interpreted as actinomycosis because of the presence of sulfur granules.

Solitary intraosseous neurofibroma of the mandible: report of a case.

A case of solitary intraosseous neurofibroma of the mandible is reported. The tumor was located in the body of the right mandible. Segmental mandibulectomy including the surrounding tissue was performed and the mandible was reconstructed with the free scapular osteocutaneous flap. From histological examination of the resected mandible, typical findings of a tumor invading the adjacent bone along the mandibular nerve were observed. Immunohistochemical examination of the tumor showed positive staining of the tumor cells for anti-S100 protein antibodies. Numerous nerve-like fibers were also noted by Bodian stain.

Presence of m3 subtype muscarinic acetylcholine receptors and receptor-mediated increases in the cytoplasmic concentration of Ca2+ in Jurkat, a human leukemic helper T lymphocyte line.

Recent studies have demonstrated the presence and the regulatory function of several neurotransmitters in the immune system. In the present study, we examined the presence of acetylcholine receptors, using pharmacological and molecular biological assays, and their transmembrane control and functions, using a biochemical assay, in a cloned human leukemic helper T lymphoma cell line, Jurkat. Several muscarinic agonists, such as acetylcholine, carbachol, muscarine, and oxotremorine-M (Oxo-M), at 100 microM caused a transient elevation of the free cytosolic Ca2+ concentration ([Ca2+]i), in contrast to the tonic elevation of [Ca2+]i induced by 10 micrograms/ml phytohemagglutinin (PHA). It appeared that the elevation induced by Oxo-M, the most potent [Ca2+]i elevator, was more effectively inhibited by p-fluorohexahydrosiladifenidol hydrochloride (p-F-HHSiD) and 4-diphenylacetoxy-N-methylpiperidine methiodine than by pirenzepine and 11-2[[2-[(diethylamino)methyl]-1-piperidinyl]acetyl]-5,11-dihydro- 6H-pyrido[2,3-b] [1,4]benzodiazepine-6-one (AF-DX 116), suggesting that a pharmacological M3 subtype of muscarinic receptors is involved in the elevation of [Ca2+]i. Northern blot analysis showed that the m3 type of receptors are expressed in Jurkat cells. Scatchard analysis of [3H]quinuclidinyl benzilate binding to intact cells indicated a Kd of 14.1 nM and a Bmax of 45,370 binding sites/cell. [3H]Quinuclidinyl benzilate binding to cell membranes was also inhibited by p-F-HHSiD rather than by pirenzepine and AF-DX 116. Oxo-M induced formation of inositol trisphosphate, and 5'-O-(2-thio)diphosphate inhibited the formation. Cholera toxin treatment inhibited the PHA-induced [Ca2+]i rise but did not affect the Oxo-M-induced rise. Neither pertussis nor butulinus (type C) toxin affected the rise induced by Oxo-M or PHA. Thus, bacterial toxin-insensitive GTP-binding proteins seem to be involved in the Oxo-M-induced increase in [Ca2+]i. Treatment with 12-O-tetradecanoylphorbol 13-acetate abolished the Oxo-M-induced [Ca2+]i rise but did not affect that induced by PHA. m3 Muscarinic receptors thus appear to cause Ca2+ mobilization from intracellular stores via bacteria toxin-insensitive GTP-binding proteins, phospholipase C activation, and inositol trisphosphate formation in Jurkat cells. Protein kinase C seems to negatively modulate the m3 receptor system.

Effect of hyperbaric oxygen therapy on osseointegration of titanium implants in irradiated bone: a preliminary report.

Four patients who had undergone combined surgical and radiotherapeutic treatment for maxillofacial cancer had osseointegrated titanium implants placed in the bone of the treated region at various intervals after irradiation. Preoperatively and postoperatively, hyperbaric oxygen therapy was administered to the patients at the level of 2 or 3 atmosphere absolute (ATA). Of the 21 fixtures placed, one was lost because of lack of osseointegration, rendering the survival rate 92.3%. Hyperbaric oxygen therapy seemed to be a viable method to improve the survival rate of fixtures in irradiated bone tissue.

Effects of ammonium chloride on membrane currents of acinar cells dispersed from the rat parotid gland.

In acinar cells freshly dispersed from rat parotid glands, the effects of ammonium chloride (NH4Cl) on membrane currents were studied using the whole-cell clamp method. When membrane currents were recorded with command pulses to 0 mV, applied at 2-s intervals from a holding potential of -70 mV, NH4Cl (5-20 mM) transiently decreased outward currents and then slowly increased both outward and inward currents. After reaching a peak in about 40-50 s, both outward and inward currents gradually decreased in the presence of NH4Cl and, on its wash-out, the currents returned to the control level. Butyrate (5-20 mM) had little effect on the resting membrane currents, but markedly inhibited the response to NH4Cl. Tetraethylammonium (5 mM) strongly reduced both the resting and NH4Cl-induced outward currents, whereas it slightly potentiated the NH4Cl-induced inward current without affecting the membrane current at the holding potential. Without ATP in the patch pipettes, carbachol-induced membrane currents were relatively resistant to Ca2+ removal from the external medium, but NH4Cl-induced currents were quickly abolished in the absence of Ca2+. We conclude that intracellular alkalinization with NH4Cl increases Ca2+ influx and activates Ca(2+)-dependent outward K+ and inward Cl- currents.

Chinese medicine, Coix seeds increase peripheral cytotoxic T and NK cells.

Coix seeds, a Chinese medicine have been used in Japan and reported to be effective in patients with verruca vulgaris and verrucae planae juveniles. We investigated the in vivo effects on lymphocyte subsets in seven healthy volunteers who took six tablets of Coix seeds three times a day (a typical dose) for four weeks. Leukocyte counts and the percentage of total lymphocytes did not change but the percentages of CD3+CD56+ cells and CD16+CD57- cells increased significantly. These results indicate that Coix seeds increase peripheral cytotoxic lymphocytes and may be effective to viral infection through the enhancement of cytotoxic activity.

Mitotic activity of cells in the fibrous zone of the rat mandibular condyle.

To investigate cell renewal in this fibrous zone, Wistar rats at 3 and 6 weeks of age were injected intraperitoneally with colchicine (1 mg/kg) three times, at 10:00, 13:00 and 16:00 h, and mitotic cells were examined after killing the rats at 18:00 h. Colchicine induced a significant increase in the number of arrested metaphases in the fibrous and the proliferating cell zone. The mitotic rates (per 1000 cells per h) in the fibrous zone at 3 weeks of age were 0.426 for the superficial layer and 0.266 for the deeper layer. These findings suggest that the fibrous zone of young rats grew with the proliferation of the cells within this zone. Age-associated histological changes in the fibrous zone were related to changes in mitotic rates within that zone.

Decreases in alpha beta T cell receptor negative T cells and CD8 cells, and an increase in CD4+ CD8+ cells in active Hashimoto's disease and subacute thyroiditis.

We examined peripheral lymphocyte subsets in patients with autoimmune thyroid disease, or subacute thyroiditis, in the active stage when possible. During destructive thyrotoxicosis arising from alpha beta T cell receptor (TCR) negative T (WT31-CD3+) cells and CD8 (CD4-CD8+) cells decreased and those of CD4+CD8+ cells increased slightly, resulting in proportional increases in CD4 (CD4+CD8-) cells, non-T, non-B (CD5-CD19-) cells, and the CD4/CD8 cell ratio. Changes were similar in active subacute thyroiditis. During stimulative thyrotoxicosis in active Graves' disease, the numbers of such T lymphocyte subsets were not changed, but only the number of CD5+ B (CD5+CD19+) cells increased markedly, resulting in proportional decreases in total T (CD3+) cells, alpha beta+ TCR T (WT31+CD3+) cells, CD8 cells, and non-T, non-B cells. A serial study of some of the patients showed opposite changes in alpha beta TCR- T cells, the CD4/CD8 cell ratio, and CD5+ B cells between the active stages of Graves' and Hashimoto's diseases. alpha beta TCR- T cells were mostly gamma delta TCR+ T (IIF2+ CD3+) cells in these patients. These data suggest that alpha beta TCR-T (gamma delta TCR+ T), CD8, and CD4+ CD8+ cells are important in thyroid destruction in Hashimoto's disease and subacute thyroiditis, and that CD5+ B cells are important in thyroid stimulation in Graves' disease.

Ameloblastoma in the maxillomandibular region: MR imaging.

Nine patients suspected of having primary ameloblastoma of the mandible or maxilla and five patients with clinical and/or radiologic indications of postoperative recurrence of ameloblastoma were examined with magnetic resonance (MR) imaging. In the former, results were compared with those of conventional radiography, computed tomography (CT), and pathologic analysis. Seven tumors were multilocular; two were unilocular. MR imaging showed a mixed pattern of solid and cystic components (n = 9), irregularly thick walls (n = 9), and papillary projections (n = 6). MR imaging was superior to conventional radiography and CT in demonstrating these findings. However, shell-like bulgings of the bone cortex were demonstrated more effectively with conventional radiography and/or CT in six cases, and soft-tissue invasion was overestimated with MR imaging in four cases. All tumors except one showed strong enhancement of solid components including papillary projections, walls, and septa at enhanced MR imaging. In the latter group of patients, MR imaging correctly demonstrated recurrence in two and no recurrence in three.
