Effects of tumor necrosis factor alpha on replication of varicella-zoster virus.

Replication of varicella-zoster virus (VZV) and expression of VZV nuclear antigen are inhibited in human embryonic lung fibroblast (HEL) cells pretreated with recombinant tumor necrosis factor (TNF) alpha for 24 h. This antiviral activity is completely blocked by the addition of monoclonal antibodies against TNF. TNF acts synergistically with interferons alpha and gamma. When TNF is added to HEL cells after VZV adsorption, virus replication is still inhibited. When VZV-infected HEL cells are co-cultured with HEL cells which have been pretreated with TNF or grown in the presence of TNF, spread of VZV from VZV-infected HEL cells to uninfected cells is unaffected. No interferon is detected in the supernatants or cell lysates of HEL cells cultured with TNF and antibodies to alpha-, beta- and gamma-interferon have no effect on the antiviral action of TNF.

Alpha2-plasmin-inhibitor deficiency (Miyasato disease).

A 25-year-old man, born in Okinawa, Japan, had a haemorrhagic diathesis characterised by prolonged bleeding and ecchymoses after minor trauma and spontaneous joint haemorrhage. The frequency and severity of these episodes were reduced by an antiplasminic drug. Routine coagulation studies revealed no abnormalities except for significantly sshortened euglobulin-lysis time and whole-blood clot lysis time. Activities of all known clotting and fibrinolytic factors were within normal ranges but no circulating alpha2-plasmin inhibitor was found in the plasma. alpha2-plasmin inhibitor is a potent and fast-acting protease inhibitor. Studies of family members indicated that this abnormality was inherited as an autosomal and recessive gene.

Postprandial electrogastrographic changes with or without parasympathetic nerve blockade.

To investigate the relationship between postprandial electrogastrographic changes and parasympathetic nervous activity, 10 healthy adult males (20 to 29 years old) volunteered for EGG recording in the following conditions: 1) butylscopolamine bromide (scopolamine) administration to block parasympathetic nervous activity; 2) after food intake; and 3) during a postprandial period after the parasympathetic blockade. Power spectral analysis of EGG was performed according to Akaike's autoregressive model. When the parasympathetic nervous activity was blocked, there were no changes in the dominant frequency of the EGG. During the postprandial period, the dominant frequency in EGG increased significantly, and postprandial dip (transient frequency decrease after the food intake) was observed in 8 of the 10 subjects. During the postprandial period after scopolamine administration, the dominant frequency of EGG did not increase, and postprandial dip was observed in only two subjects. These results suggest that the parasympathetic nervous activity is involved in occurrence of postprandial gastric motor function and postprandial electrogastrographic changes.

Development of a new transvenous patent ductus arteriosus occlusion technique using a shape memory polymer.

A novel percutaneous patent ductus arteriosus (PDA) occlusion technique, without the potential problems associated with conventional techniques, has been long awaited. The development of a novel transvenous PDA occlusion technique using a temperature-shape changeable occluder device, and the verification of its in vitro performance, were demonstrated in this study. The principles of this technique are: 1) the bar form device was inserted into the PDA transvenously by a guide wire and a pushing catheter, 2) the device was dashed with hot water through catheters, 3) the device was expanded by hot water in the PDA, and the PDA was occluded. The occluder device, made of a shape memory polymer (polynorbornene), was designed to have the monobloc configuration of a thin disk with a hole for a guide wire in the center and a cone. The barlike device by hot press was fully expanded within 10 sec upon immersion into 45 degrees C water. The mock-circulation test with the "great arteries" and a specially designed "PDA" showed 1) the introduction of hot water (45 degrees C) through the catheters caused expansion of the device which stayed in the "PDA" without any support, 2) the "aortic" pressure and the distal "aortic" flow increased rapidly, and the "pulmonary" flow reduced promptly. This resulted in a drastically reduced shunt ratio at the PDA from 68% to 30%. Thus, the novel PDA occlusion technique developed here functioned well to occlude the PDA in a great arteries model.

Results from an international survey of Kawasaki disease in 1979-82.

Kawasaki disease is a febrile illness of unknown etiology affecting mainly children younger than five years, 20% of whom develop coronary artery aneurysms. The disease was first reported in Japan, but case reports have come from several countries; epidemics apparently occur every two to three years. A committee of the International Society and Federation of Cardiology (ISFC) collaborated on an international, retrospective survey in mid-1983 through 1984, asking about Kawasaki disease cases seen in 1979-82 and their outcomes. Responses were received from 53 countries; reports from 20 countries indicated that no cases were seen in the years surveyed. Kawasaki disease did not always follow the Japanese pattern. An indication of 'epidemic' increases was found in some years in some countries. Recognition of Kawasaki disease varies considerably, but further educational efforts are likely to uncover additional cases, and intensive epidemiological efforts would be possible in a number of countries.

Clinical features of familial hypercholesterolemia.

The clinical consequences of familial hypercholesterolemia (FH) result from its metabolic peculiarities that persist from very early childhood, leading to the accumulation of cholesterol in the form of xanthomas in skin and tendons and atheromatous lesions in the arterial wall, in particular, in the aorta and the stem of coronary arteries. Plasma cholesterol concentration markedly increases during the suckling period, soon attaining a level near 1000 mg/dl in homozygotes and 200 to 400 mg/dl in heterozygotes. By age 50, about 80% of FH males suffer from ischemic heart diseases, while only 20% to 30% of the females are moderately affected by coronary atherosclerosis. In addition to the low density lipoprotein (LDL) cholesterol level, higher triglyceride and lower high density lipoprotein (HDL) cholesterol levels correlate with an increased risk of ischemic heart diseases. A very important problem is that most of these patients do not feel themselves to be seriously ill until a severe myocardial infarction (often leading to sudden death) takes place during the third to fifth decades of life. Among the different types of receptor mutations, the incidence of ischemic heart diseases was much higher and more extensive among patients with the receptor-negative type than among those with the receptor-defective type with a residual receptor activity.

[Transcatheter closure of atrial septal defect with a prototype clamshell septal umbrella: one year follow-up].

Transcatheter closure of a secundum atrial septal defect was attempted using a prototype clamshell septal umbrella closure system in 11 patients (six males, five females, aged 4.4-15.0 years and weighing 16-51 kg). The balloon stretched atrial septal defects were 10.3 to 20 mm in diameter, and 23 to 40 mm diameter umbrellas were used. All patients improved clinically after the procedure. Fixed splitting of the second heart sound disappeared after closure in 10 patients, systolic murmur disappeared in nine, diastolic rumble in nine, and paradoxical ventricular septal motion in seven. Doppler color flow imaging immediately after implantation showed significant residual shunt in all patients; however, no shunt flow was observed in seven patients and minimal residual shunt was present in four one year after the procedure. Nine of 11 umbrellas demonstrated fracture of the stainless steel arm confirmed by chest radiography and cinefluorography. Arm fracture occurred between 1 week and 12 months after implantation. None of the devices were dislodged and no other complications were noted despite the fractures. Transcatheter closure of atrial septal defect is a feasible and useful procedure but the present device needs further modification.
