Sensitive solid phase enzyme immunoassay for factor IX antigen and classification of hemophilia B.

A sensitive solid phase enzyme immunoassay (EIA) was developed for the measurement of factor IX antigen (IX:AG), using rabbit antihuman factor IX antiserum and beta-D-galactosidase, which enabled us to detect IX:AG as low as 10(-4)U/ml. 37 patients with severe hemophilia B have been investigated by EIA, inhibitor neutralization assay and bovine brain prothrombin time. They could be divided into four genetic variants. 25% had normal levels of IX:AG but decreased levels of factor IX clotting activity. On crossed immunoelectrophoresis of the hemophilia B+ and hemophilia BM, we could not find abnormalities in electrophoretic mobilities compared to normal subjects in the presence of 1 mM Ca++ lactate.

Surgical treatment for severe congenital heart diseases.

The surgical results in patients with severe congenital heart disease, who underwent surgical treatment between 1978 and 1981 at the National Cardiovascular Center, Japan, were analyzed. The surgical mortality rates were 4% for cases of ventricular septal defect associated with pulmonary hypertension in patients under 2 years of age, 16.7% for complete atrio-ventricular canal, 11% for coarctation of the aorta associated with ventricular septal defect, 40% for pure pulmonary atresia without Ebstein anomaly, 44% for total anomalous pulmonary venous return, 14.8% for transposition of the great arteries, 44% for double outlet right ventricle and 1.1% for tetralogy of Fallot. The surgical results have been improving and postoperative residua and sequelae have been decreasing through our persistent efforts. Today's main problem is a relatively poor performance in cases which need surgical intervention early in life, i.e., under 3 months of age. Therefore, we think that our effort to improve the surgical results for new-borns or very young infants is most necessary.

Myocarditis in children--Clinical findings and myocardial biopsy findings.

Eight children with myocarditis underwent a cardiac catheterization study and right ventricular endomyocardial biopsy. The degree of histological changes of the biopsy specimens correlated well with the severity of the clinical manifestations. Patients who had evidence of residual functional impairment had biopsy findings compatible with their clinical course.

Normal titer of functional and immunoreactive protein-C inhibitor in plasma of patients with congenital combined deficiency of factor V and factor VIII.

Protein-C inhibitor (PCI) is a newly described plasma inhibitor directed against a vitamin-K-dependent serine protease, activated protein-C, which is involved in the inactivation of factor V and factor VIII. Marlar and Griffin have reported that PCI activity is absent in the plasma of patients with congenital combined factor V/VIII deficiency. We have measured the levels of PCI in the plasma of seven unrelated patients with this disorder using both functional and immunologic methods. The rate at which the amidolytic activity of activated protein-C was neutralized in the patients' plasma was essentially identical to that observed in normal plasma. The titer of PCI antigen, as measured by an electroimmunoassay using a monospecific anti-PCI serum, was 5.3 +/- 1.6 micrograms/ml in the patients' plasma and was not significantly different from that of normal plasma (5.3 +/- 2.7 micrograms/ml, n = 30). The levels of factor-V-related antigen, factor V coagulant antigen, and factor VIII coagulant antigen were low in all patient plasma and were in good agreement with their respective coagulant activity. Our results do not appear to support the hypothesis that combined factor V/VIII defect is due to a lack of PCI.

Ca2+, Mg2+-dependent endonuclease and ADP-ribosylation.

The molecular mechanism of the inhibition of Ca2+, Mg2+-dependent endonuclease by ADP-ribosylation was studied by using purified bull seminal plasma Ca2+, Mg2+-dependent endonuclease, endonuclease-stimulating proteins, and poly-(ADP-ribose) polymerase. The activity of an essentially homogeneous preparation of the endonuclease was markedly suppressed by its preincubation with NAD+, poly-(ADP-ribose) polymerase, DNA, and Mg2+. These four components of the incubation mixture were all essential for the suppression of the activity. Analyses of the initial and the chased reaction product by Sephadex G-100 column chromatography and sodium dodecyl sulfate (SDS)-polyacrylamide gel electrophoresis revealed that Ca2+, Mg2+-dependent endonuclease was ADP-ribosylated during the incubation and its activity was markedly inhibited by the elongation of the ADP-ribose polymer covalently attached to the endonuclease. When the suppressed enzymes were mildly treated with an alkaline pH of 10.0, the activity was restored almost to the level of the unmodified control sample. These facts indicate that the linkage between the enzyme and poly(ADP-ribose) is hydrolyzed at this pH, and that the liberated polymer itself does not appreciably affect the endonuclease activity. These results also suggest that an electric repulsion between negative charges on DNA and poly(ADP-ribose) attached to Ca2+, Mg2+-dependent endonuclease is the basis for the observed suppression of the enzyme by ADP-ribosylation. Though histone H2B and H1 are shown to be as good endonuclease-stimulators (1) as they are good acceptors of ADP-ribose in poly(ADP-ribose) polymerase reaction (2), ADP-ribosylation of these two proteins did not affect their endonuclease-stimulating ability appreciably, at least under the conditions used.

Acceleration of ethanol metabolism by administration of uridine diphosphate(UDP) in rat.

To see if UDP accelerates the metabolism of ethanol, Wistar female rats were given UDP(100 mg/kg b.w.) intra-gastrically prior to a single dose of ethanol (3 g/kg b.w.) ingestion. UDP pretreatment accelerated blood ethanol disappearance rate by 50% (mg/kg animal/hr) by Widmark formula. Increases of the hepatic triglyceride concentration 6 hours and 20 hours after ethanol administration were significantly low in UDP pretreated animals suggesting the lipotropic effect of UDP. Neither the activity of alcohol dehydrogenase nor microsomal ethanol oxidizing system was affected by UDP pretreatment. The ratio of lactate to pyruvate in liver 6 hours after ethanol was not changed as compared with that of the control whether the rats were pretreated with UDP or not. There were significant increases in hepatic ATP content and the size of the adenylate pool in UDP pretreated group, whereas the energy charge was unchanged as compared with ethanol-alone group. These data suggest that UDP enhances ethanol metabolism possibly by causing a change in ATP metabolism.

Quantitative brain organisation in anteaters (Edentata-Tubilidentata).

We can state that the encephalization and the quantitative organisation of the brain in edentates sensu lato exhibit a certain degree of common patterning but also differences between groups that have long been separated. Orycteropus in Africa and the dasypodids in South America can be considered relatively primitive if evolutionary progress is linked essentially with encephalization and neocorticalization. But again, as pointed out by Pirlot (1980), the concept of primitiveness needs careful reconsideration. To this end, more genera from the classical edentate group ought to be worked upon. In particular, the South American bradypodid Choloepus would be of great interest, as well as the African pangolin (Manis). Above all, more data are needed on the behaviour of all those ill-toothed mammals, especially in connection with their olfaction and their balance control as well as with the relative use they make of vision and audition. This project is now at an early stage in Montreal.

[Evaluation of pulmonary regurgitation with a catheter-tip electromagnetic velocity probe].

Pulmonary regurgitation appeared after the surgical repair of pulmonary stenosis was investigated with an electromagnetic catheter-tip velocity probe in seven patients. Postoperative catheterization was performed by placing the probe in the pulmonary trunk transvenously and a velocity curve was recorded on a photographic recorder at a paper speed of 150 or 200 mm/sec with electrocardiograms and pulmonary arterial pressure pulse. The velocity curves were recorded under the baseline throughout diastole in patients with pulmonary regurgitation, while the curves in the normal controls were just on the baseline. The amount of regurgitation was evaluated with the ratio of the negative to positive area of velocity curves. The negative area (NA) was defined as an area circumscribed by the diastolic negative velocity curve and the baseline, and the positive area (PA) as an area circumscribed by the systolic positive curve and the baseline. The amount of the regurgitation was also evaluated from either the pulmonary cineangiogram or the volume measurement using the right arteriogram. The results were as follows: The severity of pulmonary regurgitation estimated by the pulmonary arteriogram (grade I to III) tended to be correlated with the ratio of the NA/PA. Right ventricular end-diastolic volume index (RVEDVI) and the NA/PA showed a fair correlation with a coefficient of 0.80. Right ventricular volume overload resulting from pulmonary regurgitation was reflected on the NA/PA. The subtraction of right and left ventricular systolic ejection indices (RVSI-LVSI) or its ratio (RVSI-LVSI)/RVSI, which were considered as the indices of the severity of pulmonary regurgitation from the volume measurement, had fairly good a correlation with the NA/PA.(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term follow-up study on gastric adenoma and its relation to gastric protruded carcinoma.

A total of 191 gastric adenomas in 178 patients was studied macroscopically by endoscopy and histopathologically by endoscopic biopsy. Among the lesions, 85 in 74 patients were followed-up for six months to 12 years. Gastric adenomas were found to be more frequent in the aged, with a rate of 0.1% in the third decade but 3.7% in the ninth decade, on gastroscopic examination. Gastric cancers coexistent with the gastric adenomas were seen in 14 cases (8%), and were more frequent in male than in female patients (sex ratio, 12:2). Only eight of the 85 lesions (9%) revealed macroscopic changes. Four of these showed a reduction in size, while the other four lesions showed enlargement. In 21 of the 85 lesions (25%), histologic changes were observed. Four (5%) changed from moderate dysplasia (Group III) to nondysplastic or intestinal metaplasia (Group I), eight lesions (9%) revealed histologic changes (Group III to IV, or vice versa) without malignant transformation, and nine lesions (11%) showed malignant changes. Neither submucosal invasion nor lymph node metastasis was found. These lesions consisted of carcinoma in situ with small foci in the lesions exhibiting moderate dysplasia, and a gradual transition from severe dysplasia to cancer was seen in resected lesions obtained by endoscopic polypectomy or surgical resection. In addition, a gradual increase in dysplasia of tissue from moderate to severe was revealed by repeated gastroscopic biopsy. These findings suggest that the gastric adenomas underwent malignant changes with gradual transformation from moderate through severe dysplasia.
