Superselective cisplatin (CDDP)-carboplatin (CBDCA) combined infusion for head and neck cancers.

A group of 26 patients with carcinoma of the head and neck region was treated by superselective intra-arterial infusion of cisplatin (CDDP)-carboplatin (CBDCA). The tumor locations included the tongue (n = 4), oral base (n = 2), nasopharynx (n = 2), oropharynx (n = 8), hypopharynx (n = 4) and larynx (n = 4). Using the coaxial technique, a microcatheter was placed in the lingual, ascending pharyngeal, facial and superior thyroidal arteries according to the location of the tumor. Under imaging studies, CDDP (50 mg/m2)-CBDCA (300 mg/m2) was infused into the vessel, via injectors at the rate of 12.5 ml/min. One to five injections were given every 4 weeks. During and following the chemotherapy the patients received radiotherapy (n = 22), surgery (n = 4) or both (n = 3). Sixty-six sessions of intra-arterial chemotherapy were given with no major complications. Drug-related systemic side effects were mild. The overall response rate was 96% (complete response (CR) 50% and partial response (PR) 46%). Superselective CDDP-CBDCA combined infusion is feasible, relatively non-toxic, and important as multimodality therapy.

Desquamative interstitial pneumonia in sibs.

We report on 2 sibs with desquamative interstitial pneumonia. The female died at age 1 7/12 years despite use of prednisolone and methylprednisolone, while the male, now age 3 years. is alive with oxygen support. The occurrence of desquamative interstitial pneumonia in sibs born to normal parents suggests that in some cases the disease is an autosomal-recessive trait.

N-[2-[N'-pentyl-(6,6-dimethyl-2,4-heptadiynyl)amino]ethyl]- (2-methyl-1-naphthylthio)acetamide (FY-087). A new acyl coenzyme a:cholesterol acyltransferase (ACAT) inhibitor of diet-induced atherosclerosis formation in mice.

FY-087 (N-[2-[N'-pentyl-(6,6-dimethyl-2,4-heptadiynyl)amino]ethyl]- (2-methyl-1-naphthylthio)acetamide) was found to be a competitive inhibitor of human microsomal acyl coenzyme A:cholesterol acyltransferase (ACAT) with an IC50 value of 0.11 microM. Under our assay conditions, other ACAT inhibitors tested, specifically YM-750, E-5324, and melinamide, all of which are now in phase I clinical trials or in clinical use in Japan, inhibited this enzyme with IC50 values of 0.18, 0.14, and 3.2 microM, respectively. FY-087 also inhibited ACAT in acetyl-low density lipoprotein loaded human macrophages (THP-1 cells) with an IC50 of 0.17 microM. Following the oral administration of FY-087 (30 mg/kg) to rats, the plasma concentration of FY-087 reached 0.42 microgram/mL after 2 hr. This concentration of FY-087 was enough to inhibit blood vessel ACAT activity. Cholesterol-lowering and anti-atherogenic effects of FY-087 were examined using C57BL/6J mice fed an atherogenic diet. In this mouse model, treatment with FY-087 (28 mg/kg) inhibited the increase in plasma cholesterol levels by about 20% and decreased the hepatic accumulation of free and esterified cholesterol by 61 and 67%, respectively. FY-087 also significantly inhibited the atherogenic diet-induced increase in the fatty-streak lesion area of the proximal aorta by 57% in C57BL/6J mice. These results indicate that FY-087 is not only a therapeutically bioavailable ACAT inhibitor that lowers plasma cholesterol levels, but also an effective anti-atherogenic agent in mice fed an atherogenic diet.

A case of multiple eccrine porocarcinoma with stepping-stone distribution on the foot.

An 89-year-old woman had three tumors of several years' duration in a stepping-stone distribution on the dorsum of her right foot. The two larger ones were diagnosed histologically as eccrine porocarcinomas, and the smallest of the three shared many clinical similarities with them. Eccrine porocarcinoma is a rare malignant tumor. We describe this peculiar case in which three tumors developed in the same region and discuss how they might have occurred. It is most likely that the three tumors of this case originated from nevoid tumors, and that the individual lesions exhibited malignant changes.

A novel adrenaline derivative, AZ002, and its hypoglycemic action in yellow KK mice.

AZ002 (L-threo-(3,4-dihydroxy phenyl)-N-methyl serine methyl ester) is a newly synthesized adrenaline derivative. AZ002 caused relaxation of rat jejunum (beta 3-receptors) (ED50 = 18 microM), but did not affect the atrial rate (beta 1) or tracheal relaxation (beta 2) at a concentration of 0.3 mM. The pA2 values for propranolol in inhibiting the isoproterenol- and AZ002-stimulated relaxation of rat jejunum were 6.27 and 6.33, respectively. Thus, AZ002 is a selective agonist for beta 3-adrenoceptor. AZ002 stimulated lipolysis (ED50 = 10 microM) and glucose uptake (ED50 = 1 microM) in rat adipocytes. In both cases, stimulation was antagonized by high concentrations of the beta-adrenoceptor antagonist propranolol, but not by the alpha-adrenoceptor antagonist phentolamine. The effect of AZ002 on glucose uptake was synergistic with that of insulin. AZ002 was also assessed in vivo by using genetically obese mice (KK/Ay strain) with hyperglycemia. Administration of AZ002 in the diet for a week decreased blood glucose and non-esterified fatty acids.

Afternystagmus, its specific features.

There are important issues and new observations concerning 'afternystagmus'. Recent findings were mostly supplied by 3-dimensional records of eye movements. 'After-nystagmus' appears to be secondary to the reactions to the visual and vestibular inputs. The beginning of afternystagmus, however, is not always clear, and the distinction from the primary reaction is not clear either. The axis of eyeball rotation during afternystagmus is not necessarily the same as that of the primary reaction, but frequently changes gradually with time. The change in the plane of eyeball rotation during afternystagmus appears to follow a certain rule, which appears to be under specific influences from the otolith organs. Afternystagmus and its related phenomena seem to be explained by a hypothetical 'velocity storage'.

Stage-assessment of the progress of continuous vertigo of peripheral origin by means of spontaneous and head-shaking nystagmus findings.

The stages of continuous vertigo of peripheral origin were classified into nine categories according to the findings of spontaneous nystagmus (SPN) and head-shaking nystagmus (HSN). The patients analysed were 18 with vestibular neuronitis, 6 with sudden deafness and 6 with unilateral inner ear disorders. 1) Irritative SPN (Stage I) was rarely encountered. 2) SPN of the paralytic type (Stage II) was usually observed in the period less than one month after the onset of diseases. 3) HSN directed toward the intact side with or without a reversal phase (Stages III-2 and III-1, respectively) were the common types of central compensation. 4) The progress of recovery to complete cure (Stage V) was usually rapid once it had actually begun. 5) During the process of recovery, HSN could disappear transitorily (Stage III-3), or be directed toward the affected side without a reversal phase (Stage III-5). Spontaneous recovery nystagmus (Stage IV) could also occur. 6) About 40% of patients recovered to Stage V within about 4 months after the onset of vertigo, but about 30% of patients remained in Stage III-1 or III-2 even after 4 months.

A quantitative analysis of head-shaking nystagmus of peripheral vestibular origin.

A quantitative analysis of horizontal head-shaking nystagmus (HSN) was made on 48 patients with unilateral peripheral vestibular lesions in conjunction with stimulus intensity. Each patient underwent three head-shaking tests with 10, 30 and 50 horizontal head-excursions at a frequency of approximately 2 Hz, and HSN was recorded on ENG with eyes open in total darkness. i) HSN appeared in a biphasic or monophasic pattern. ii) The maximal slow-phase eye velocity (MSV) of the 1st phase (PI) of biphasic HSN increased significantly in proportion to stimulus intensity, and was significantly greater than that of monophasic HSN. iii) The duration of HSN was greater in the 2nd phase (PII) of biphasic HSN than in PI and increased markedly in proportion to stimulus intensity. iv) As the stimulus intensity rose to a high level, the interval between PI and PII (2nd phase latency) shortened, and the PII tended to appear more quickly after head-shaking. It was especially noteworthy that in response to an increase in stimulus intensity, both the MSV in PI and the duration of PII of biphasic HSN increased, but the duration of PI was reversely suppressed by the PII.

An intraperitoneal tumorous mass caused by granulomas of microfibrillar collagen hemostat (Avitene).

Microfibrillar collagen hemostat (Avitene) is an absorbable topical hemostatic agent prepared from purified bovine corium collagen. Herein, we report the case of a 56-year-old woman who had undergone an ovariohysterectomy for an ovarian mucinous cystadenocarcinoma 3 months previously and then required a second operation for an intraperitoneal mass associated with microfibrillar collagen hemostat. Microscopically, the resected mass was composed mostly of foreign-body granulomas with microfibrillar collagen hemostat materials. Microfibrillar collagen hemostat was used to control bleeding during the first operation. In a very small portion, however, a recurrent mucinous cystadenocarcinoma was also present. This case is worthy of note in that microfibrillar collagen hemostat may produce huge granulomatous masses that clinically mimic a neoplasm, inducing unnecessary and extensive treatment.

Purification and characterization of recombinant squalene epoxidase.

Recombinant rat squalene epoxidase (rSE) was expressed in E. coli and purified to an apparent homogeneity. This expression system was constructed using squalene epoxidase (SE) cDNA in which nucleotides coding 99 amino acids in the N-terminal were deleted and nucleotides coding hexa-histidine in the C-terminal were added. Purification was carried out using Ni-chelate affinity agarose and Cibacron Blue Sepharose column chromatography. Purification was achieved 100-fold over the crude E. coli extract with a yield of about 50%. The purified enzyme demonstrated a single band on SDS-polyacrylamide gel electrophoresis. The enzyme showed no distinct absorption spectrum in the visible regions. The properties of rSE were compared with those of rat liver microsomal SE. The requirement of the co-factors, the S105 fraction or Triton X-100, and NADPH-cytochrome c reductase, the pH dependency for enzyme activity, and the sensitivity to NB-598 seen with both enzymes suggest that rSE has properties very similar to rat microsomal SE. 2,3-Oxi-dosqualene (OSQ) and 2,3;22,23-dioxidosqualene (DOSQ) were formed by rSE in a completely reconstituted system. It is suggested that recombinant squalene epoxidase catalyzes the conversion of squalene to 2,3-oxidosqualene and of 2,3-oxidosqualene to 2,3;22,23-dioxidosqualene.

[An intravenously injectable emulsified iodinated oil contrast agent for liver CT: experimental study of lipiodol emulsion emulsified by lecithin (LEL38) in rabbits].

LEL38 (lipiodol emulsion emulsified by lecithin 38 mgI/ml, mean diameter 200 nm) is a new intravenously injectable oil contrast agent for liver CT. The aim of this report was to evaluate its ability to enhance contrast in the liver of 46 rabbits with regard to the correlation of density with time at an injected dose of 76 mgI/kg (before to 120 minutes after), the correlation of density with dose (0-760 mgI/kg) and the detectability of liver mass. The time-density correlation of LEL38 in the liver, that is, peak density, was achieved after 30 minutes, and it was elevated to 20.9 H. U. Thereafter, it decreased slowly. In the blood vessels, it reached a sharp peak after immediately being elevated to 14. 7 H. U. There after detectability decreased quickly. The maximum difference in density between liver and blood vessels was 34 H. U. after 60 minutes. The dose-density correlation in the liver and blood vessels was linear. Tumors were detected as clear areas of low density. The minimal detectability was about 3 mm. LEL38 may be an effective contrast agent for screening CT studies of liver disease.

Diffuse increased renal uptake on bone scintigraphy in acute tubular necrosis.

Acute renal failure after exercise with frank pain and patchy renal vasoconstriction is a clinical syndrome that occurs in young, previously healthy persons. The authors describe a 24-year-old man who had this syndrome. CT scan showed wedge-shaped contrast enhancement of both kidneys, which suggested patchy renal vasoconstriction. Tc-99m MDP imaging revealed diffuse increased uptake of both kidneys. After renal function was improved, Tc-99m MDP imaging showed normal uptake of both kidneys. Acute renal failure due to acute tubular necrosis was confirmed by renal biopsy. The authors conclude that Tc-99m MDP may be useful in evaluation of acute renal failure after exercise with flank pain and patchy renal vasoconstriction due to acute tubular necrosis.

Radiation exposure in endovascular surgery of the head and neck.

PURPOSE: To evaluate the radiation risk to the operator and the patient during endovascular surgery of the head and neck. METHODS: The dose was measured using thermoluminescence dosimeters attached at the body surface of the operator and the patient during 15 endovascular surgeries (3 for arteriovenous malformation, 8 for dural arteriovenous fistulas, and 4 for other disorders of the head and neck). The dose was measured at seven sites on the operator and at five sites on the patient. RESULTS: The mean number of digital subtraction angiography studies and fluoroscopy time were 21 +/- 10 and 73 +/- 24 minutes, respectively. The equivalent dose range at each site in the operator was 0.12 to 0.88 mSv (glabella), 0.06 to 1.1 and 0 to 0.09 mSv (neck, outside and inside the protector, respectively), 0 to 0.20 mSv (left should, inside the protector), 0.09 to 1.99 mSv (left arm), 0.05 to 3.55 mSv (left hand), and 0 to 0.49 mSv (pubis, inside the protector). Those in the patients were 3.1 to 136 mSv (glabella), 13 to 5441 mSv (right temporal area), 4 to 186 mSv (left temporal area), 0.1 to 51 mSv (neck), and 0 to 0.62 mSv (pubis). CONCLUSIONS: The total doses at the operator's eyes and left hand during the course of a year may exceed the dose limits recommended by the International Commission on Radiological Protection. Operators should wear not only body protectors, but also thyroid protectors and lead glass spectacles. The equivalent dose at the right temporal area of the patient may exceed the deterministic dose for transient erythema or alopecia of the skin even in one endovascular procedure.

Assessment of residual tumor viability in thymic carcinoma by sequential thallium-201 SPECT: comparison with CT and biopsy findings.

We present a case of 201TI accumulating thymic carcinoma, in which sequential CT scans demonstrated a steady decrease in tumor volume. The presence of a residual mass on CT scans after the completion of therapy presented the clinical dilemma of whether or not a viable tumor remained. Sequential 201TI SPECT images demonstrated a marked decrease in tumor uptake. At 2 wk after therapy, no significant accumulation of 201TI in the region of the residual mass was observed, indicating a lack of viable tumor. A biopsy specimen revealed no tumor cells. Sequential histopathologic findings were correlated well with the findings of 201TI SPECT rather than those of CT. Thallium-201 SPECT is of great clinical value in assessing tumor viability in the course of therapy.

Basilar artery migraine. Reversible ischemia demonstrated by Tc-99m HMPAO brain SPECT.

The authors present a case of basilar artery migraine in a 33-year-old woman who was initially misdiagnosed as having a cerebrovascular or mental disorder and subjected to MRI and cerebral angiography, which, however, did not show any pathologic findings. During admission to the university hospital, she lost consciousness. An emergency Tc-99m HMPAO brain SPECT showed a significant decrease of regional cerebral blood flow in the right temporal and occipital cortices, and right cerebellar hemisphere, where regional cerebral blood flow was decreased by 10-24% as compared to the left side. The second brain SPECT during a symptom-free phase showed the reversion of regional cerebral blood flow to normal in these areas. Basilar artery migraine was diagnosed by the finding of reversible ischemia in the territory of the right basilar artery on brain SPECT images and the clinical picture.
