[Evaluation of para-aortic lymph node metastases in patients with cervical cancer].

From 1979 to 1986, 355 patients with cervical cancer underwent paraaortic lymph node (PAN) biopsy before the start of initial treatment. Nineteen of these 355 patients (5.4%) had a positive PAN, and the percentage incidence of positive PAN in each clinical stage was as follow: stage Ib, 2.4%; IIa, 0%; IIb, 3.7%; IIIb, 19.0%; and IV, 20%. Eighteen of these 19 patients also had positive pelvic lymph nodes. Positive PANs were found to be more frequent in cases of an adenocarcinoma than in cases of squamous cancer. The palpation of the PANs seemed to be too insufficient to predict the metastasis. Patients with positive PAN who had received paraaortic irradiation survived longer than those who did not receive such treatment. It is concluded that the evaluation of the PAN is important for the management of a cervical cancer.

Sebaceous gland tumor of the ovary.

A sebaceous gland tumor of the ovary was detected in a 60-year-old woman who underwent right salpingo-oophorectomy for a right ovarian cyst. The cyst was unilocular, weighed 820 g, and was filled with sebaceous material containing a few hair shafts. There was a protruded mass composed of lobules of mature or immature sebaceous cells over the inner surface of the cyst wall. She has been well for 4 years and 2 months after the surgery. This is the second well-documented case of this extremely rare type of tumor. This lesion is teratogenic with unilateral development of the sebaceous glands and malignant characteristics are nil.

Clinical and EEG studies of zotepine, a thiepine neuroleptic, on schizophrenic patients.

The overall effect of zotepine was a "slightly improved" or better response in 20 patients (64.5%), "unchanged" in 10 (32.3%) and "worsened" in 1 (3.2%). Zotepine exhibited some degree of improvement in 54.5% of patients unresponsive to prior drugs. The onset of effect of zotepine was within one month in 19 patients. The improvement rate in the hebephrenic type (66.7%) was almost the same as in the paranoid type. The improvement rate classified by psychopathology was highest for hypobulia, followed by restlessness-excitement and hallucination, depressive mood, hypochondria and delusion. The side-effects were subjective complaints, such as general fatigue, dryness of mouth, sleepiness or fainting in a small number of cases. There was a slight increase in S-GPT in one patient and a slightly increased blood platelet count, also in one patient. Serial EEG changes associated with zotepine studied in another 17 chronic schizophrenics could be classified into three groups: those with increased slow waves, those with enhanced alpha waves and those with unchanged EEGs. There was a positive correlation between the incidence of slow waves and higher plasma levels of zotepine.

Studies on cyclic AMP-dependent protein kinase inhibitor from human heart.

Heat stable adenosine 3',5'-monophosphate (cAMP)-dependent protein kinase (PK) inhibitor was extracted and partially purified from human heart tissue. One unit of inhibitor was defined as the amount necessary to produce 50% inhibition of the PK activity of 50 p mole/min. The eluate from Sephadex G-75 showed a specific activity of 41 units/mg, with purification of 390-fold and recovery of 23%. The molecular weight was 29,000 by gel filtration and S20,w was 1.4, similar to that of rabbit skeletal muscle PK inhibitor. At the purification step involving trichloracetic acid precipitation, the specific activity was not significantly different between tissue from the atria and ventricles. However, the particulate fraction of ventricular homogenate yielded a specific activity that was 3 times higher than that of the supernatant. Kinetic analysis showed that the inhibition was noncompetitive with respect to ATP, histone and cAMP. In the presence of inhibitor the binding of cAMP to PK was increased. This is consistent with the concept that the inhibitor directly interacts with the catalytic subunit of PK, and modifies the physiological action of cyclic AMP.

[Gliomatosis peritonei].

Gliomatosis peritonei, the miliary implants of mature glial tissues on the peritoneum or omentum, is a rare complication of solid ovarian teratoma. Our case is reported and 38 previously reported cases are reviewed. The grade of the primary tumors varied from grade 0 to grade 3. Only five cases were composed entirely of mature tissues. Five of the 39 patients died. Despite of varied therapy, the rest of the patients were alive from 3 months to 38 years later. Inspite of intraperitoneal implants, the prognosis in patients with these tumors is good, irrespective of the mode of therapy. On the basis of this study, we recommended a conservative therapy for the primary tumor and therapy for the implants is not required.

[Evaluation of the efficacy of cisplatin-based chemotherapy in epithelial ovarian cancer; comparison with treatments employed before the cisplatin era].

Seventy-five patients with epithelial ovarian cancers received cisplatin-based chemotherapy as a post-surgical treatment in the 1980s. In the 1970s, fifty-eight patients received various treatments without cisplatin, which included radiotherapy, chemotherapy and their combination. In the patients with stage III disease, cisplatin-based chemotherapy achieved a median survival of 24 months, whereas other treatments employed in the 1970s achieved a median survival of 8 months. In the patients with early disease, however, cisplatin-based chemotherapy did not improve the survival. Since it is clear that cisplatin-based chemotherapy achieved an improved survival in the treatment of advanced epithelial ovarian cancer, it is concluded that this therapy is worth continuing until a more effective treatment is available.

[An evaluation of para-aortic and pelvic lymph node metastases in patients with endometrial carcinoma].

During 1979-1986, 101 patients with an endometrial carcinoma were treated. Eighty-four of these patients underwent a para-aortic lymph node (PAN) biopsy at the time of their initial surgery, eight of whom (9.5%) were found to have a positive PAN. The incidence of positive PAN as per each clinical stage, was as follows: stage I, 2.0%; II, 11.5%; III, 50%; and, IV, 33.3%. Positive pelvic nodes were found in 19.2% of the patients, per the following stages: stage I, 6.1%; II, 32%; and, III & IV, 100%. This finding suggests that PAN metastasis occurs after the pelvic nodes are involved. A 5-year survival rate of approximately 50% is expected even in patients with a positive PAN. Thus our study concludes that an evaluation of para-aortic and pelvic lymph nodes is important for the management of an endometrial carcinoma.

[Treatment of epithelial ovarian carcinoma with cisplatin and adriamycin: analysis of factors influencing prognosis in advanced cases].

Thirty previously untreated patients with epithelial ovarian carcinoma were treated with cisplatin and adriamycin (PA). Of eight evaluable patients, six were responders (two CRs and four PRs). The three-year survival rates were 83% for stage I, 67% for stage II, 50% for stage III, and none for stage IV. Toxicities included moderate myelo-suppression, mild nephrotoxicity, alopecia, and severe vomiting in almost all patients. Patients with residual lesions smaller than 2 cm had excellent prognosis when PA was given for more than five courses. Patients with residual lesions larger than 2 cm, however, had poor prognosis irrespective of the number of courses. No specific relation between histological grade and prognosis could be found. From the present data, aggressive cytoreductive surgery followed by PA therapy repeated more than five times is recommended for achieving a good outcome in the treatment of epithelial ovarian carcinoma.

Extremely well-differentiated adenocarcinoma ("adenoma malignum") of the cervix in a patient with Peutz-Jeghers syndrome.

In a 29-year-old woman with the Peutz-Jeghers syndrome (PJS), an extremely well differentiated adenocarcinoma (adenoma malignum) of the uterine cervix was detected. The cervical lesion consisted of a polypoid mass, measuring 3.5 cm in greatest diameter, composed of extremely well differentiated tubules resembling those of the endocervical glands, yet containing a few Paneth cells. Immunohistochemical stains displayed cytoplasmic carcinoembryonic antigen in this tumor. The ovaries had no apparent abnormality. The diagnosis of the PJS was based on the presence of numerous hamartomatous polyps of the rectum and cutaneous pigmentation around the lips, fingers, and toes. The patient underwent a simple total hysterectomy and was subsequently treated with chemotherapy. In an 11 year follow-up, there has been no recurrence of the cervical tumor and she is currently well. The clinicohistopathologic differences of this cervical tumor in patients with and without PJS are briefly discussed.

Adenomatoid tumor of the uterus. Ultrastructural, histochemical, and immunohistochemical analysis.

A uterine tumor from a 34-year-old woman was investigated morphologically and immunohistochemically. The tumor tissue was composed of multiple cystic spaces of various sizes. The cyst wall was covered by a single layer or occasionally multiple layers of flattened cells. Electron micrographs of the tumor tissue revealed that the cells were delimited with conspicuous basal lamina from the stroma, possessed numerous microvilli at the apical surface, and connected with desmosomes. Furthermore, immunohistochemical staining with antikeratin antibody disclosed the strong positivity of the tumor cells, whereas anti-UEA-I antibody could not stain the tumor cells. The results did not favor the theory of endothelial origin of the tumor and confirmed the mesothelial origin, namely, adenomatoid tumor of the uterus.

[Adenomatoid tumor of the uterus--a light-microscopical, electronmicroscopical, histochemical and immunohistochemical study].

A 34-year-old woman with an adenomatoid tumor of the uterus is reported. Light-microscopic examination showed the tumor to be cystic with a canalicular pattern. The ultrastructural findings were numerous microvilli, various intercellular spaces, desmosomes, and rich 10-nm microfilaments like tonofilament. Histochemically, the tumor cells secreted hyaluronic acids. Keratin was demonstrated diffusely in the cytoplasms of tumor cells by PAP staining. From these findings, we concluded that the histogenesis of the adenomatoid tumor was of mesothelial origin.
