Cicatricial pemphigoid (Brunsting-Perry type). Case report and immunofluorescence findings.

We describe clinical and immunofluorescence findings of a patient with Brunsting-Perry-type cicatricial pemphigoid. Direct immunofluorescence showed tissue-fixed basement membrane zone antibodies similar to those characteristic of bullous pemphigoid. Circulating antibodies to the basement membrane zone were not found. Brunsting-Perry-type cicatricial pemphigoid probably represents a clinical variation midway in the cicatricial pemphigoid-bullous pemphigoid spectrum of disease. Management with intralesional corticosteroids was successful in controlling the skin lesions.

Antinuclear antibodies. II. The prognostic significance of nuclear immunofluorescent patterns in lupus erythematosus.

Nuclear immunofluorescent patterns in lupus erythematosus (LE) were correlated with the clinical picture. Patterns were classified into three groups of prognostic significance: (1) Poor-Shrunken peripheral fluorescence was found in patients with severe systemic lupus erythematosus (SLE), 60% demonstrating renal involvement. (2) Intermediate-This included the peripheral and therapy patterns, associated with 33% renal involvement in patients with SLE. (3) Good-Large and small speckle-like threads were seen in cases of very benign LE.

Indirect cutaneous immunofluorescence. II. Clinical significance.

Sera of 532 patients with bullous diseases, connective tissue diseases and malignancies were tested for pemphigus epidermal intercellular fluorescence (ICF) and for the bullous pemphigoid ;tubular' band by the indirect fluorescent antibody technique. Human normal skin cryostat sections were used. The band and ICF were seen primarily only in bullous pemphigoid and pemphigus respectively, Some indirect band and ICF-negative patients demonstrated positive direct results in involved skin. suggesting that direct tests should be performed in indirect negative patients clinically thought to have pemphigus or bullous pemphigoid. No close correlation was found between disease activity and positive or negative indirect tests in bullous pemphigoid and pemphigus. Steroids did not interfere with positive results of this diagnostically extremely valuable test.
