HLA DR4 and rheumatoid arthritis in Japanese people.

Eighty-eight Japanese patients with rheumatoid arthritis and 104 normal Japanese persons were typed for HLA A, B, C, and DR antigens. The frequency of HLA DR4 was 70.5% in patients compared with 46.1% in normal controls (P less than 0.001). However, a sex difference in the frequency of HLA DR4 in patients was noted. HLA DR4 was found in 80.6% of male patients, which was highly significant compared with controls (P less than 0.0005), while only a borderline increase of 60.5% was found in female patients (P less than 0.05). In addition, the frequency of HLA DR2 was remarkably low in male patients. These suggest the possible heterogeneity of rheumatoid arthritis in Japanese.

Study of HLA antigens among Japanese schizophrenics.

HLA antigens were typed among 136 Japanese schizophrenics. Increased frequencies were seen in A9 (Aw24), A10 (A26) and Bw54, and decreased frequency in B40 antigens when compared to 187 Japanese controls. It is suggested that there may be an association between A9 (Aw24) and schizophrenia with a chronic-progressive course and also an association between A10 (A26) and hebephrenia.

HLA-DRw3 in juvenile onset diabetes mellitus in Chinese.

Thirty-nine juvenile onset diabetes mellitus (JOD) patients and 57 healthy Chinese were HLA-typed in Taiwan. The incidence of HLA-DRw3 was significantly increased in patients when compared with the control group (R.R. = 5.8, P = 0.0027). The frequencies of HLA-B17, which showed linkage disequilibrium with DRw3, and Bw54 were increased in patients, but these increases were not significant because of the small number tested. The DR antigen associated with JOD was the same among Chinese and Caucasians, which suggested that at least one of the susceptibility genes to JOD would be commonly in linkage disequilibrium with DRw3 in the two populations.

Human leukocyte antigen (HLA) in patients with idiopathic portal hypertension (IPH).

Both cryptogenic liver cirrhosis and idiopathic portal hypertension without pathological findings of liver cirrhosis (IPH) are relatively common in Japan. To find differrence between IPH and cryptogenic liver cirrhosis of Japanese, we investigated human leukocyte antigen (HLA), familial clustering of liver diseases as well as prevalence of HBs antigen in 31 patients with IPH. The age of patients with IPH ranged between 6 and 67 years old (mean, 40 years). The ratio of female to male in patients with IPH was 3.1. The incidence of chronic liver diseases in the relatives of patients with cryptogenic liver cirrhosis was significantly high (29.1%), whereas that in the relatives of patients with IPH was not high (12.9%) as compared with hospitalized controls (8.9%). The phenotype frequencies of the specificities of HLA A and B loci in patients with IPH did not differ from those in healthy controls. And the incidence of HBs antigen and antibody in patient with IPH (3.2% and 20.7%, respectively) was similar to those in normal subjects in Japan. These findings suggest that IPH in Japan is a different clinical entity from cryptogenic liver cirrhosis and that HB virus may not be one of the main causes of IPH.

Haplotype analysis of the linkage group HLA-A: HLA-B : Bf in Japanese.

Three hundred four HLA-A : HLA-B : Bf haplotypes of the Japanese population as deduced by family analysis are described. Several linkage disequilibriums were observed in the following two-factor haplotypes: HLA-A and HLA-B, HLA-A and Bf, and HLA-B and Bf. Positive linkage disequilibriums between HLA-A and HLA-B noted in the present study seem to be Japanese specific when compared with the results obtained from other ethnic groups reported so far. The striking finding is that three HLA-B : Bf haplotypes, namely B12-BfF, B15-BfF, and B7-BfS, are common and show linkage disequilibrium in both Japanese and European Caucasians (German). This finding not only confirms the proximity of the HLA-B and Bf loci but also suggests that the interaction of the two genes plays an important role in the maintenance of stable linkage disequilibrium.

Ankylosing spinal hyperostosis (ASH) and ossification of the posterior longitudinal ligament (OPLL).

The clinical and radiological features of 70 patients with ankylosing spinal hyperostosis are presented. Pain in some region of the back was found in 49 patients. No significant deviations were found in HLA typing. Radiologically, the most characteristic features were bony spurs or bridges in the dorsal spine, where they predominated on the right and anterior aspects of the spine, especially in the lower dorsal region. Ossification of the posterior longitudinal ligament was found in 21 patients.
