Apomorphine effects on behavioral response to ethanol in mice selectively bred for differential sensitivity to ethanol.

Two lines of mice selectively bred for differences in response to a hypnotic dose of ethanol were administered apomorphine alone or in combination with ethanol. When administered by itself, apomorphine produced similar dose-dependent depression of locomotor activity and increases in stereotypy in the two lines. Doses of apomorphine (0.5 microM/kg and 2 microM/kg) thought to bind only presynaptic dopamine receptors blocked the slight locomotor activation to 1.5 g/kg ethanol in the ethanol-sensitive Long-Sleep (LS) mice; in the ethanol-insensitive Short-Sleep (SS) mice which show marked activation to all subhypnotic doses of ethanol, these doses of apomorphine only attenuated the activation. A higher apomorphine dose (8 microM/kg) antagonized the locomotor depressant effects of 2.0 and 2.5 g/kg of ethanol in LS mice but did not alter the shape of the SS ethanol dose response curve for locomotor activity. Apomorphine (2 and 8 microM/kg) potentiated ethanol-induced loss of the righting reflex in LS mice in a dose dependent fashion, but did not alter this soporific effect of ethanol in SS mice. These findings extend the data base suggesting a role for dopamine both in the mechanism(s) differentiating the LS and SS mice and the stimulant and intoxicating properties of ethanol.

Locomotor stimulant and intoxicant properties of methanol, ethanol, tertiary butanol and pentobarbital in Long-Sleep and Short-Sleep mice.

Methanol, ethanol, t-butanol and pentobarbital all produced marked dose dependent activation of locomotor activity in Short-Sleep mice which were selectively bred for relative insensitivity to the hypnotic properties of ethanol. The locomotor activity of alcohol sensitive Long-Sleep mice was depressed in a dose dependent fashion by all four drugs. Simultaneous assessment of intoxication in a grid test indicated that all four drugs disrupted coordination, in a dose dependent manner, to a greater degree in Long-Sleep mice than in Short-Sleep mice. The line differences in response to all alcohols was greater than for pentobarbital, indicating that the previous assumption of specificity of the selection for alcohols may be a question of degree rather than a qualitative effect.

A university of department of family medicine after ten years.

The Department of Family Medicine at the University of Washington was started ten years ago after a major curriculum change in the medical school placed new emphasis on education and training of family physicians for the surrounding region in Washington, Alaska, Montana and Idaho (WAMI). This department has organized active programs in patient care, teaching and research at the university base and in a number of affiliated community sites throughout the region. The department is well accepted within the mission of the medical school. Almost a third of graduating medical students choose postgraduate training in family practice. Almost 90 percent of the graduates of the ten programs within the department's network of family practice residencies are established in active family practices, with more than two thirds settling in the WAMI region. Follow-up studies show that these graduates feel well prepared for their practices; are providing a broad range of services in rural, suburban and urban settings; are typically involved in partnership or group practice; and are generally well satisfied with their personal and professional lives.

Comparison of diagnostic methods of family practice and internal medicine residents.

The diagnostic methods of third year residents in internal medicine (N=23) and family practice (N=22) were compared with respect to common ambulatory patient problems. Five written simulated patients were presented and the dependent variables were: initial and revised diagnostic hypotheses, physical examination items, and laboratory charges. The two groups considered the same number and type of diagnostic hypotheses. There were large differences in the selection of physical examination items (P less than .001), with the family practice group selecting fewer items. Laboratory charges were significantly greater for the internal medicine group with two patients (P less than .05), and the charges were nearly identical with two patients. A high degree of patient-specific behavior was demonstrated by both groups. These findings have implications for the future training of primary care physicians.

Family physician pathway and medical student career choice. Ten years after curriculum change at the University of Washington.

In 1968 the University of Washington School of Medicine initiated a new curriculum that included a family physician pathway. Six of these classes have now graduated. Two thirds of the planned strategies for the pathway were actually implemented. Approximately half of the students in each second-year class now choose this pathway. Ninety percent of those graduating have done so in the traditional four years. Seventy-three percent of those in the pathway actually pursued family practice training on graduation. Approximately one third of all University of Washington graduates now enter training for family practice, one third enter internal medicine, 10% enter surgical fields, and 8% enter pediatrics. These rates represent significant increases in the proportion of students entering directly into family practice, internal medicine, and pediatrics training.

The role of the family physician in the care of obstetrics and gynecology patients.

Primary care is a major interest of all medical specialties today. That level of medical care stresses management of problems common in the population served. Studies of problems presented to general practitioners/family physicians indicate that a high proportion of the most common problems are obstetric/gynecologic. Departments of family medicine and of obstetrics and gynecology can base their mutual training efforts on care of these problems. The complementary relationship thus developed between residents in the two fields could lead to appropriate regionalization of services and optimum use of current knowledge and technology. Quality of care for the obstetric/gynecologic patient is a shared goal and can be achieved through col-laborative efforts of physicians in these two specialties.

Comparative effect of pethidine, trichloroethylene, and Entonox on fetal and neonatal acid-base and PO2.

The second stage of labour is associated with relative fetal hypoxia and progressive metabolic acidosis. Maternal analgesia can increase the danger, especially for the high-risk fetus. In 152 patients the effect on the fetus of pethidine alone, pethidine + trichloroethylene, or pethidine + Entonox was assessed by fetal scalp blood sampling. Fetal pH, Pco(2), and Po(2) were measured and base excess was calculated. Capillary samples were also taken 45 to 60 minutes after birth in 88 of the babies to estimate the rate of neonatal recovery.Entonox proved safer than trichloroethylene, and babies treated with it maintained their Po(2) before birth, had better Apgar scores at birth, and one hour later were significantly less acidotic and much better oxygenated.
