Survey of haemorrhagic erosive gastritis.

Fluorescent-antibody studies in 22 patients out of a total of 40 cases of acute erosive gastritis showed no antibodies to gastric parietal cells. Hence erosive gastritis seems unlikely to be an autoimmune phenomenon.In this series 57.5% of cases belonged to blood group A. This suggests that there is a correlation between the instability of the gastric mucosa and blood group A.The operative mortality in erosive gastritis is high. Operation should be delayed as long as possible in the hope that the bleeding will stop spontaneously. Erosive gastritis may be idiopathic or precipitated by drugs, particularly aspirin. The highest risk was found to occur in those cases where bleeding was drug-induced.

Drug testing in the workplace--are methods legally defensible? A survey of experts, arbitrators, and testing laboratories.

Urinalysis results are increasingly being used by employers to detect use of prohibited substances in the workplace. Actions taken on the basis of positive urinalysis results can have a significant effect on employees' lives and careers, and may face legal challenge. Important aspects of urinalysis are choice of analytical methods and their legal defensibility. Medical directors are frequently required to select testing laboratories, but have had few data correlating analytical methods with legal defensibility. To evaluate the legal defensibility of the methods used for drug testing, we conducted a survey of technical experts, testing laboratories, and arbitrators. Experts reported wide differences in the legal defensibility of methods rated. Arbitrators had considerable experience in drug use cases and understood the critical role of urinalysis results, but were unable to distinguish legal defensibility of analytical methods. Commercial laboratory practice in urinalysis varies widely among laboratories. An objective of this article is to provide company medical directors with information that will enable them to make informed decisions when choosing urinalysis laboratories and methods.

ECMO for left ventricular assist in a newborn with critical aortic stenosis.

Extracorporeal membrane oxygenation (ECMO) has been used in neonates for a variety of disease states including congenital diaphragmatic hernia, meconium aspiration syndrome, sepsis, and postoperative cardiac compromise. To our knowledge, ECMO has not been employed prior to cardiac catheterization in critical aortic stenosis (CAS). We report a neonatal case of CAS where ECMO was used early as a form of left ventricular assist to achieve adequate systemic perfusion and oxygenation and reduce myocardial ischemia. The patient was maintained on ECMO during subsequent attempts at cardiac catheterization, balloon valvuloplasty, and operative valvotomy.
