Low serum thymic hormone levels in patients with chronic graft-versus-host disease.

We tested the hypothesis that chronic graft-versus-host disease (GVHD) is due to inadequate thymic function by examining pretransplant serum levels of facteur thymique serique (FTS). Four of five patients with no detectable FTS activity developed chronic GVHD, while one of four with some FTS activity did. Further patient numbers are needed to confirm or reject this hypothesis. We further postulated that chronic GVHD, whatever its cause, involves thymic epithelium as a target organ. When tested 11 mo or more posttransplant, patients with chronic GVHD had lower absolute FTS levels (p less than 0.02) and lower age-corrected levels (p = 0.05) than patients without chronic GHVD. Low values in chronic GVHD were associated with the disease itself and not its therapy. These findings show that thymic epithelial secretory function is impaired in chronic GVHD, and this may in part be responsible for the immunodeficiency characteristic of these patients.

[Adhesion of polysulfide rubber impression material to dental metals (author's transl)].

In order to investigate the adhesiveness of polysulfide rubber impression material to dental metals, we performed the tension test on the sample specimens which had been prepared by injecting the impression material into the gap between opposite metal surfaces finished brightly, thereafter observed the fractured surfaces. Pure metals decreased their adhesive strength to the impression material in order of Au, Pd greater than Ag greater than Cu. Some noble metal alloys containing gold and palladium in a large amount showed the good adhesiveness. In clinical using of 14 carat gold and gold-palladium-silver alloys for core metal, it should be noticed that polysulfide rubber adheres tightly to these alloys. Adhesive strength of copper was not so great comparing with noble metals, therefore the surface of copper tray should be roughned by emery paper above #500, aiming the mechanical bonding forth to increase. The surface of many metals which had left in regulated atmosphere for many hours showed the tendency to decrease their adhesiveness to the impression material. This tendency was not recognized on the metals which had appeared the failure of adhesion bond on the bright finished surfaces.

[A case of primary malignant lymphoma of the brain which showed a remarkable response to prednisolone].

We have recently had a case of a patient with primary malignant lymphoma in whom the diagnosis was difficult and prednisolone was remarkably effective. The patient was a 59-year-old man. Mental signs developed rapidly over a period of approximately 1 month. On admission, he was confused; his orientation was disturbed and his impressibility and understanding were markedly decreased. CT scan on admission revealed a remarkable enhancement of a nodular high density area in front of the lateral ventricles, accompanied by a surrounding diffuse low density. Angiography failed to reveal a tumor stain. When prednisolone was administered to the patient, the high density area disappeared and neurological findings returned to normal. After that, left hemiparesis occurred twice and disappeared following the administration of prednisolone. After that, however, he was readmitted to our hospital because his left hemiparesis advanced rapidly. CT scan revealed a high density area in the right basal ganglia. It also decreased in several days after the administration of prednisolone. Malignant lymphoma was strongly suspected by operation. He was then readmitted to our department because of a gait disturbance, decrease in impressibility, and incontinence of urine about one year and a half after onset. CT scan revealed symmetrical ventricular dilatation. Although a shunt procedure was considered, gastrointestinal bleeding occurred, followed by rapid deterioration of his general condition and neurological signs. CT scan 2 days before death revealed a mass lesion in the left basal ganglia. Autopsy revealed that a gelatinous tumor, primarily in the left basal ganglia, was verified macroscopically, but that there was no specific tumor at any other site. Light microscopic examination revealed a diffuse infiltration of tumors cells around the blood vessels in the subependymal area of the lateral ventricle. Because tumor cells were not verified in any other organ, primary malignant lymphoma of the brain was considered. These results suggest that in the development of malignant lymphoma, once a mass lesion is formed, it is widely invasive especially along blood vessel walls. As far as we know, there have been no reports which describe a tumor decreasing or disappearing on CT scan following administration of steroid hormone alone.

[The experimental and clinical studies on cefsulodin in the pediatric field].

A new cephalosporin cefsulodin (CFS) was studied basically and clinically and the following results were obtained. 1. The serum levels of 25 mg/kg of CFS administered intravenously were 39.5 mcg/ml after 30 minutes, 22.6 mcg/ml after 1 hour, 11.6 mcg/ml after 2 hours, 6.0 mcg/ml after 4 hours and 2.1 mcg/ml after 6 hours. The half life from serum was 84 minutes. 2. Clinical response on 4 cases of Pseudomonas aeruginosa infections were all good. 3. The slight elevations of GOT, GPT were observed by the drug administrations in 1 case. From the above results, CFS was effective drug to P. aeruginosa infections by intravenous administration of 25 mg/kg of CFS.

Primary immunodeficiency syndrome in Japan. I. Overview of a nationwide survey on primary immunodeficiency syndrome.

The results of a nationwide survey on primary immunodeficiency syndrome (PIS) in Japan are presented. By the repeated questionnaire method, 497 PIS cases were collected prior to February 1979 with clinical information. Numbers of each type of PIS, age at the time of diagnosis, patient's status at the time of registration, familial incidence of PIS, and development of malignancy, autoimmune diseases, and allergic diseases among all reported patients are presented and discussed.

In vitro differentiation of human marrow T cell precursors by thymic factors in severe combined immunodeficiency.

Marrow cells from 16 patients with severe combined immunodeficiency diseases (SCID) were examined for the presence of T cell precursors which could be induced to express surface markers and functions of T lymphocytes after exposure in vitro to thymic extracts or peptides of thymic origin (thymopoietin and thymopoietin32-36 (TP-5). Marrow cells from 14 patients studied before transplantation revealed three patterns of response. In five patients, inducible T cell precursors were not detected. In six patients, precursors were detected which could be induced to express a human T lymphocyte antigen (HTLA) but acquired little or no ability to rosette with sheep erythrocytes (SRBCs). Induction of HTLA-positive and E rosette-positive lymphocytes was normal in only two patients, both of whom were engrafted with maternal lymphocytes as an apparent result of an intrauterine infusion. Induction of mixed leukocyte reaction (MLR) responses following exposure to thymic hormones was also observed in one of these two patients. In no case could significant responses to mitogens be elicited. Following transplantation, marrow cells from fully engrafted, immunologically reconstituted patients could be induced to bear HTLA, to form E rosettes, and to respond to mitogens and allogeneic cells following exposure to thymic extracts, thymopoietin, or TP-5. Thus, most patients with SCID manifest differentiative abnormalities intrinsic to lymphoid precursors which are corrected following engraftment of functioning allogeneic lymphoid precursors from normal donors.

[Fundamental and clinical studies of cefotiam in the field of pediatrics (author's transl)].

The fundamental and clinical studies on cefotiam (CTM) were performed in the field of pediatrics, and the following results were obtained: 1. The peak MIC's of CTM against Gram negative rods such as E. coli, Klebsiella, Salmonella, Proteus were 1.56 mcg/ml. The MIC distribution against S. aureus was almost equal to the conventional cephalosporin antibiotics. The MICs against P. pseudomonas and Serratia were over 400 mcg/ml. 2. The mean serum levels of CTM after bolus intravenous injection of 25 mg/kg were 59.9 mcg/ml after 15 min., 30.0 mcg/ml after 30 min., 15.6 mcg/ml after 1 hour. 3. Administration of CTM to 6 pediatric patients produced the clinical responses which were good in all 6 cases and the bacterial effects of eradication in 3 cases and superinfection in the 2 cases in the 5 cases from whom the organism were isolated. No side effect was observed. From the above results, it is considered that a bolus injection of CTM 25 mg/kg t.i.d. to q.i.d. is a safe and useful treatment for pediatric cases.
