[Computed tomography in developmental anomalies of craniocervical junction].

We reviewed 27 patients with developmental anomalies in craniovertebral junction, and special attention was paid to computed tomography (CT) findings in congenital atlantoaxial dislocation (AAD), basilar invagination and Chiari malformations. In ADD, CT clearly demonstrated the relationships of the atlas to the axis in axial plane. Four major types were distinguished; anteroposterior (6 cases), transverse (0 case), anteroposterior-transverse (4 cases) and rotatory (5 cases) dislocations. It was feasible by CT to analyse the pathomechanics in each individual with AAD. Soft tissue wad was shown on the posterior aspect of the odontoid in 7 out of 15 patients with AAD. Those two features, pathomechanics of AAD and intraspinal soft tissue wad, were felt to be important for patient's symptomatology. Basilar invagination presented in no patient as a single deformity and was always associated with other bony and neural abnormalities in 14 patients. Basilar invagination might be suggested of its existence in such a case as CT showing; (1) the odontoid at or above the level of the foramen magnum, (2) distinctive margin of the foramen magnum above the bottom of the posterior fossa, and (3) the posterior arch of the atlas at the same level as foramen magnum. Nevertheless, it is essential to confirm occipital hypoplasia by using either reformatting CT images in coronal and sagittal plane or conventional tomography; whereas high resolutional CT machine provides new dimension for demonstration of bony details. In Chiari malformations, plain CT scanned by high resolutional machine often makes it possible to delineate caudal migration of the cerebellar tonsils, so that it may be a choice of examination as a screening of the anomaly. In addition, metrizamide CT clearly demonstrated caudal migration both of cerebellar tonsils and medulla oblongata. Therefore, CT apparently exceeds other diagnostic modalities in evaluating this anomaly.

[Atlantoaxial dislocation in neurofibromatosis.--Report of three cases--].

Atlantoaxial dislocation has received little attention in many studies of spinal deformity in neurofibromatosis. The only four cases of atlantoaxial dislocation associated with neurofibromatosis has been previously reported in the literature. We reported three rare cases of atlantoaxial dislocation associated with neurofibromatosis. These characteristics in roentgenogram were as follows; (1) marked narrowing of sagittal diameter at C1 level without instability (instability index 0%) (2) association with other mesodermal dysplasia, such as posterior vertebral body scalloping, vertebral body dysplasia, dural ectasia etc. Neurofibromatous tissue was found around the anterior region of the odontoid process in one of our three patients. We speculate that atlantoaxial dislocation in neurofibromatosis may be due to mesodermal dysplasia. On the other hand, a neurofibroma was found around the anterior region of the odontoid process in our third case. Therefore, there is a possibility that atlantoaxial dislocation in the instance was caused by the neurofibroma involving transverse atlantal ligament.

[Computed tomographic metrizamide myelography in spinal disease (author's transl)].

METHODS: Either EMT Head Scanner, CT 1010 (slice thickness 10mm) or EMI Body Scanner, CT 5005 (slice thickness 13mm) was used. The concentration of metrizamide was 170-250 mgI/microliter and the amount was 7-10 microliters. Either lumbar puncture or lateral C puncture was made. MATERIALS: 26 cases were included in this study. 1) disc disease: 11 cases, 2) spinal cord tumor: 6 cases, 3) Arnold-Chiari malformation: 3 cases, 4) atlantoaxial dislocation: 3 cases, 5) ossification of the posterior longitudinal ligament (associated with ossification of the ligamentum flavum): 2 cases (1 case), 6) spinal foreign body (acupuncture needle): 1 case. RESULTS: 1) CT metrizamide myelography visualizes the subarachnoid space and makes it possible to know the lesion in the spinal canal in relation to the spinal cord in transverse plane. 2) It is difficult to determine the exact level of the lesion in axial plane. 3) The present technique does not allow to visualize the root sleeves. 4) It is difficult to delineate a compression of the subarachnoid space by small localized lesions (esp., disc diseases) due to overlapping the patent adjacent subarachnoid space within a slice 10 mm to 13 mm thick.

[Three cases of brain injury due to windshield impact (author's transl)].

Three cases of brain injury due to windshield impact (tempered windshield) were reported. In the case of brain injury, multitude of small cracks injured the frontal lobe through the orbital roof with facial injuries due to windshield impact. Though we are apt to misdiagnose because of very small wounds, the diagnosis can be made easily from the plain film. The craniotomy, with complete removal of glass fragments and dural plasty, must be performed for the purpose of preventing infection and cerebrospinal fluid rhinorrhea. We believe that the use of seat belt and H.P.R. type laminated windshield is the best precaution against the windshield impact.

[Computed tomography in the diagnosis of spinal disease (author's transl)].

METHOD: During the period from October 1977 through February 1979, 41 cases of spinal diseases were studied by either EMI Whole Body Scanner, CT 5005 or EMI Head Scanner, CT 1010. Slices were 10 mm or 13 mm thick. MATERIALS: (1) disc diseases 15 cases (2) ossification of posterior longitudinal ligament 10 (3) congenital anomalies 5 (4) spinal cord tumors 6 (5) trauma 3 (6) narrow cervical spinal canal 3 (7) calcification of ligamentum flavum 1 (8) spinal arteriovenous malformation 1. Results obtained are as follows: 1) A precise determination of vertebral level scanned by C.T. is important. 2) Plain spinal C.T. is of diagnostic value in detecting bony lesions and lipoma. Meningioma showed positive images in the enhanced spinal C.T. 3) The spinal cord cannot be distinguished from the surrounding C.S.F. except for that at C1 level. 4) C.T. metrizamide myelography can appraise adjacent structures that may impinge on the spinal subarachnoid space, as well as structures contained in the space.

[Incidentally-discovered aneurysm--a report of eight cases (author's transl)].

There have been only a few reports of asymptomatic aneurysm clinically treated up to the present. The authors incidentally found eight cases of intracranial aneurysm: five cases with brain tumor, one with brain abscess and the remaining three with head injury. It may be considered that the careful observation of angiograms could reveal more asymptomatic aneurysms. The ideal treatment of aneurysm should be direct operation before rupture. But it is difficult to find out them before the onset of signs and symptoms of subarachnoid hemorrhage or cranial nerve involvement. The only chance of finding asymptomatic aneurysm is brought about by paying special attention to aneurysm even in angiography for other intracranial diseases such as tumors, head injuries, vascular diseases and so on. It was thus concluded that incidentally-discovered aneurysm should be directly treated if the prognosis of the original disease is thought to be good. The authors operated on four aneurysms of three patients and operative result was excellent.

[A case of intramedullary spinal schwanoma (author's transl)].

A rare case of intramedullary schwannoma of the spinal cord has been reported, The patient was a 30-year-old woman, who began to notice weakness in her right leg approximately 6 months prior to admission, followed 4 months later by numbness and weakness of the right arm. The above symptoms were progressively getting worse, and she was admitted to Hokkaido University Hospital on February 23, 1974. Neurological examination revealed slow speech, bilateral horizontal nystagmus, absent gag reflex and weakness of right trapezius muscle. Spasticity was noted in 4 extremities, in addition to right hemiparesis. All deep tendon reflexes were hyperactive, right more than left, with bilateral Hoffmann's and Babinski's signs. Vibration sense was diminished below the level of bilateral iliac crests. A tumor around the foramen magnum was suspected, however plain skull and neck, laminogram of cervical spines, vertebral arteriogram, fractional pneumoencephalogram and myodil myelogram failed to disclose abnormalities. Manometric Queckenstedt test showed a partial block on flexion, with CSF protein of 56 mg/dl. Air myelogram clearly visualized the presence of an intramedullary tumor at the level of the medullo-spinal junction. Subtotal removal of the intramedullary tumor at C1 was performed, which proved to be a schwannoma histologically. 14 such cases are reported in the literature and summarized on Table I, including our case. Clinical features of tumors around the foramen magnum are fairly complexed, and some radiological examinations might not be conclusive. It is stressed that air myelogram is extremely valuable in the diagnosis of lesions around the foramen magnum.

Photoluminescence imaging of phosphor particles using near-field optical microscope with UV light excitation.

We have developed fibre probes suitable for 325 nm UV light excitation and a photoluminescence near-field scanning optical microscope (NSOM) and demonstrated the photoluminescence imaging of phosphor BaMgAl10O17:Eu2+ (BAM) particles. The probe was fabricated by a two-step-etching method that we developed. The probe had a large taper angle at the top of the probe and a small taper angle at the root. The NSOM image was different from the topographical structure but roughly reflected the corresponding features of the particles. The inhomogeneity of the photoluminescence intensity between BAM particles was observed in the NSOM image. The photoluminescence intensity with various bandpass filters showed differences between the individual particles, which means that they have different spectra.
