Plasma cell dyscrasia in 105 Japanese patients with systemic amyloidosis.

Total 105 Japanese cases of systemic amyloidosis were analysed in terms of Congo red staining with potassium permanganate, anti-amyloid serum staining by immunoperoxidase and the occurrence of monoclonal proteins (ie. plasma cell dyscrasia, PCD). Thirty three cases of primary type amyloidosis and 22 cases of myeloma-associated amyloidosis were mostly associated with PCD, with 3 exceptional cases without PCD. In contrast, 45 cases of secondary type amyloidosis were demonstrated as AA type, with no association of PCD. Predominance of lambda light chain in amyloidosis was contrasted to kappa chain predominance among the non-amyloidotic myeloma cases.

Plasma cell myeloma showing progressive bone destruction during long-term clinical remission.

A case of plasma cell myeloma initially producing Bence Jones protein of lambda type was presented. BJ protein disappeared within two months of cyclophosphamide and betamethasone administration. A complete remission continued for the following 2 years and 8 months with no medication, although the patient insidiously developed osteolytic lesions. The discrepancy of long-term negative BJP and clinical remission without medication and progressive bone destruction was reported.

Pseudothrombocytopenia with cold-type platelet agglutinins.

Pseudothrombocytopenia is an in vitro phenomenon of platelet agglutination and clumping in the presence of EDTA on the electronical measurement of platelet count. Agglutinins reacting with platelet antigens belong to one heavy and one light chain of immunoglobulins with or without complement by immunofluorescence. The platelets are markedly agglutinated in EDTA, but scarcely in the presence of other anticoagulants. Agglutinations seem to occur more readily at cold temperature rather than at 37 degrees C.

Computed tomographic diagnosis of strangulated obstruction.

A case of strangulated obstruction with computed tomographic appearance is reported. CT may be useful for the demonstration of the strangulated loop in a patient with obstructive symptoms and a soft tissue mass in the pelvis shown on plain films of the abdomen.

A rapid separation of S100 subunits by high performance liquid chromatography: the subunit compositions of S100 proteins.

The subunits of S100 protein were dissociated and separated from each other by high performance liquid chromatography on a macroreticular poly-styrene resin, using an acetonitrile-trifluoroacetic acid solvent system. The separation was completed within 80 min in monitoring the effluent at 210 and 280 nm, allowing a rapid and sensitive identification of each S100 protein in terms of subunit composition, and a simultaneous purification of subunits. The method enabled to estimate the subunit compositions of S100a, S100b, and S100a0 protein purified from bovine brain, and revealed a micro-heterogeneity of S100 subunits as applied to the mixtures of S100 protein prepared from bovine, human and rat brain.

Purification of chicken gizzard myosin light-chain kinase, and its calcium and strontium sensitivities as compared with those of superprecipitation and ATPase activities of actomyosin.

1. A purified preparation of myosin light-chain kinase (MLCK) was obtained from chicken gizzard, and it was shown to consist of two subunits; 130,000 (130 K)-dalton subunit and 17,000 (17 K)-dalton subunit. In amino acid composition the 130 K and 17 K subunits were identical with the 105 K and 17 K subunits of Dabrowska et al. (1977 and 1978), respectively. In disc gel electrophoresis, the 17 K subunit of our MLCK preparation responded to Ca2+ ions in the same way as bovine calmodulin, and differently from skeletal troponin C. There appeared to be one minor difference between 17 K subunit and calmodulin in the primary structure of the C-terminal region. 2. The Ca2+ and Sr2+ concentrations required for the three activities (ATPase and superprecipitation activities and MLCK activity) were measured. Two types of "reconstituted" myosin B were used; one contained 17 K subunit of gizzard MLCK and the other contained bovine brain calmodulin. The two types of "reconstituted" myosin B were practically identical with "natural" myosin B in the Ca2+ and Sr2+ requirements for the three activities measured above. 3. Both the extent and the activity of superprecipitation, and both the limited and steady activities of ATPase were measured. The MLCK activity was estimated in two ways; by urea gel electrophoresis and by measuring 32 P incorporation from [gamma-32P]ATP into myosin. The results thus obtained favor the kinase-phosphatase mechanism of calcium regulation of gizzard muscle contraction.

Separation of extremely acidic proteins, S-100 proteins and calmodulin, in some bovine tissues and mammalian brains by two-dimensional electrophoresis in the absence of denaturing agents.

Extremely acidic proteins in bovine brain extracts, including S-100 proteins and calmodulin, were separated by two-dimensional electrophoresis in the absence of denaturing agents. The pattern was compared with the patterns of other bovine tissue extracts and also with those of brain extracts of other species. S-100 proteins (S-100 a and S-100 b) were specifically abundant in nerve tissues. A small amount of S-100 proteins was detected in heart extract. Extracts of liver and kidney did not have S-100 proteins, but each showed the distribution pattern of acidic proteins characteristic to the tissue. Calmodulin seemed to be present in all tissue extracts examined. Comparisons of the patterns of acidic proteins in brain extracts of various animal species suggest that S-100 proteins are more "variable" in structure than calmodulin.

Localization of Bence Jones proteins in the kidney of myeloma patients.

Six cases of multiple myeloma were studied regarding functions and pathology of the kidney. Glomerular changes were minimal, whereas tubular and interstitial changes were noted in all cases. Tubular cells were degenerative and atrophic in cases excreting more than 3 g BJP daily in the urine. Cases without BJP had only minor changes. The most severe impairment of renal function was found in a patient with Fanconi's syndrome, with hypercalcemia, calcium in the tubules, tubular atrophy and interstitial fibrosis. In attempting to correlate immunoglobulin abnormalities with renal histology, immunoperoxidase staining of immunoglobulin heavy and light chains were done on kidney sections. Glomerular and interstitial tissues gave a definitely negative reaction. In contrast, positive staining of light chain of corresponding type wrs found in the cytoplasma of the tubules and most tubular casts, with different degrees possibly in accord with the amounts of BJP.

Intradermal tests in Behçet's disease.

Intradermal tests were compared among 4 groups of Behçet's disease, other uveitis caused by unknown agent, sarcoidosis and SLE. The size of indurations of PPD, SK-SD and Candida was the largest in Behçet, followed by other uveitis, sarcoidosis and SLE in a descending order. Pustule formation was frequently seen in Behçet and other uveitis, in contrast to low incidence in sarcoidosis and not at all in SLE. Using saline solution or phosphate-buffered saline reaction, however, Behçet group shows erythema formation rather frequently which differentiate Behçet disease from other uveitis, sarcoidosis and SLE.

Indirect immunofluorescent antibody test in chicken leucocytozoonosis.

The indirect immunofluorescent antibody technique was applied to detection of antigens of different developmental stages of Leucocytozoon caulleryi and antibodies in the sera of chickens infected with L. caulleryi by using second generation merozoite and gametocyte antigens. Zygotes, ookinetes and sporozoites in midges, and second generation merozoites and gametocytes in chickens indicated specific fluorescence. Indirect immunofluorescent antibody titers were higher than agar gel precipitation antibody titers. So the detection of the antibody by the indirect immunofluorescent antibody technique was possible in the sera in which the antibody could not be detected by the agal gel precipitation test. Therefore, the indirect immunofluorescent antibody technique was applicable to chicken leucocytozoonosis as a highly sensitive serological diagnostic method.
