Benign prostatic hyperplasia: value of MR imaging for determining histologic type.

PURPOSE: To assess the value of magnetic resonance (MR) imaging for determining the histologic type of benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: Unenhanced T1- and T2-weighted images and gadolinium-enhanced T1-weighted images were obtained in the transverse plane at 1.5 T in 33 patients with BPH. Hyperphasia was classified as stromal or nonstromal. Nonstromal hyperplasia was diagnosed if (a) the nodules in the inner gland had heterogeneous high signal intensity on T2-weighted images and peripheral enhancement on gadolinium-enhanced images, (b) a surgical capsule was present, or (c) the inner gland volume to total volume ratio was greater than 0.75. RESULTS: A correct diagnosis was made with MR imaging in 23 of 24 patients with nonstromal hyperplasia and eight of nine with stromal hyperplasia (94% accuracy). In nonstromal hyperplasia, nodular lesions were seen in 20 patients and a surgical capsule was seen in 19. Eighteen patients had a transition zone ratio greater than 0.75. CONCLUSION: MR imaging appears to be useful for choosing the type of pharmacotherapy performed because accurate histologic assessment is possible.

Lack of effect of ouabain on calcium homeostasis in rat platelets: comparative study with human platelets.

The precise mechanisms of Ca2+ handling in rat platelets are not fully understood. We sought to determine whether rat platelets possess a Na(+)-Ca2+ exchanger. First, we investigated the time course of the effect of ouabain (10(-4) M) on cytosolic sodium concentration ([Na+]i) and Ca2+ homeostasis in platelets from Wistar rats in comparison with those from humans. Ouabain increased platelet [Na+]i in both rat and human platelets. Whereas ouabain induced a time-dependent increase in basal and thrombin-stimulated (0.3 units/ml) cytosolic calcium concentration ([Ca2+]i) in human platelets, no change was found in rat platelets. Furthermore, 90-min pretreatment of rat platelets with ouabain did not affect the [Ca2+]i response to thrombin (0.1-1.0 units/ml) or a maximal dose of ionomycin (5 microM). Also the decline in [Ca2+]i after the peak response evoked by these agonists in the absence of extracellular Ca2+ was not changed by ouabain pretreatment. Similarly, replacement of extracellular Na+ had no influence on any of these determinations. Thus decreasing the plasma membrane Na+ gradient did not affect basal [Ca2+]i, thrombin-induced mobilization of Ca2+ from intracellular stores, internal Ca2+ discharge capacity, or Ca2+ extrusion from cytosol of rat platelets. In contrast to human platelets, a Na(+)-Ca2+ exchange mechanism does not appear to play a significant role in Ca2+ homeostasis of rat platelets.

Lack of increase in platelet Ca2+ and Na+ in deoxycorticosterone acetate-salt hypertensive rats.

We have previously characterized abnormal Ca2+ handling in platelets of spontaneously hypertensive rats (SHRs). In this study, we investigated whether cellular Ca2+ metabolism and/or Na+ concentration is altered in platelets of deoxycorticosterone acetate-salt hypertensive rats (DOCA rats). The resting cytosolic Ca2+ concentration ([Ca2+]i) in platelets was significantly lower in DOCA rats than controls (54.5 +/- 1.4 vs. 61.2 +/- 2.3 nmol/l). The amplitude of the [Ca2+]i transient induced by thrombin was significantly increased in the absence, but not the presence, of external Ca2+ in DOCA rats compared with control rats. The [Ca2+]i response to 5 mumol/l ionomycin in the Ca(2+)-free buffer was greater in DOCA rats than in controls (546 +/- 23 vs. 470 +/- 18 nmol/l), indicating larger intracellular Ca2+ stores. The rate of recovery of [Ca2+]i after the peak response to thrombin was decreased in DOCA rats (79% at 0.1 U/ml and 91% at 1.0 U/ml thrombin of control rats). Cytosolic Na+ concentration ([Na+]i) in platelets was similar in DOCA and control rats. Altered Ca2+ levels are not correlated with [Na+]i in this salt-sensitive hypertensive model. Therefore, an increased [Ca2+]i is not an obligatory phenomenon in hypertension.

Regional assignment of the human immunoglobulin processed pseudogene C epsilon 3 (IGHEP2) to 9p24.2-->p24.1 by fluorescence in situ hybridization.

The human immunoglobulin processed pseudogene C epsilon 3 (IGHEP2), which was assigned to chromosome 9 by somatic cell hybrid analysis, has not been regionally localized as yet. In this study, using fluorescence in situ hybridization (FISH) combined with conventional QFQ-, RBG- or GTG-banding, IGHEP2 was assigned to the p terminus region of chromosome 9, at band 9p24.2-->p24.1. This result suggests that the C epsilon 3 gene is a novel telomeric DNA marker useful not only for constructing the physical map of human chromosome 9 but also for cytogenetic analyses such as cryptic translocations. In addition, comparative mapping of this gene in other catarrhine primates would contribute to investigations of human and other primate karyotype evolution.

Acute effects of hypoglycemia and hyperglycemia on hypothalamic growth hormone-releasing hormone and somatostatin gene expression in the rat.

Although previous studies have indicated that changes in the plasma glucose concentration alter GH secretion in the rat, the roles of hypothalamic GH-releasing hormone (GRH) and somatostatin (SS) in the glucose modulation of GH secretion have not yet been elucidated. We investigated the acute effects of hypo- and hyperglycemia on hypothalamic GRH and SS mRNA in the rat. Long term atrial catheters were implanted in male rats, and sequential blood sampling was performed in conscious animals. Insulin and 50% glucose were administered iv to induce hypo- and hyperglycemia, respectively. At the end of the experiment, the hypothalamus was removed, and total RNA was extracted for Northern blotting. Hypoglycemia completely inhibited pulsatile GH secretion, whereas hyperglycemia partially inhibited GH secretion. Direct effects of insulin itself on GH secretion in the insulin-induced hypoglycemia were ruled out by glucose clamp studies. Hypothalamic SS mRNA levels were increased dramatically by hypoglycemia (266 +/- 37%) and moderately by hyperglycemia (183 +/- 45%). Hypothalamic GRH mRNA levels were increased only by hyperglycemia (159 +/- 18%). The GH response to GRH was decreased dramatically by hypoglycemia and moderately by hyperglycemia. These results indicated that acute changes in the plasma glucose concentration stimulated hypothalamic SS mRNA in the rat.

Changes in annexin I and II levels during the postnatal development of rat pancreatic islets.

The expression patterns and the dynamic changes in content of both annexin I and annexin II in the rat pancreatic islets during postnatal development were investigated by both western blot analysis and immunohistochemistry. Immunohistochemical methods clearly demonstrated the presence of annexins I and II exclusively in pancreatic islets, while exocrine tissues were not stained by anti-annexin antibodies. Pancreatic islets were diffusely stained with no specific differences in distribution between different cell types. The expression of annexin I in pancreatic islets gradually increased with postnatal development. A developmental study of annexins I and II by western blot analysis essentially supported the results obtained by immunohistochemistry. In addition, the increasing expression of two protein tyrosine kinases, epidermal growth factor-receptor/kinase and pp60src, which phosphorylate annexin I and annexin II, respectively, and of protein kinase C, which phosphorylates both proteins, was also shown during postnatal development in rat pancreatic islets. Thus, a relationship between the expression of annexins I and II and the maturation of islet cell function is suggested.

The enantioselective metabolism of 4-isopropenyltoluene in rabbits.

4-Isopropenyltoluene (8) was administered orally to rabbits and the following four optically active metabolites, 2-(p-tolyl)propanoic acid (10), p-(1-carboxyethyl)benzoic acid (11), 2-hydroxy-2-(p-tolyl)propanoic acid (12), and 2-(p-tolyl)-1,2-propanediol (13) were isolated from urine in addition to an optically inactive metabolite, 4-isopropenylbenzoic acid (9). The enantiomeric ratios of the metabolites 10 and 11 were respectively R/S = 33:67 and 39:61 (S predominance), whereas those of the metabolites 12 and 13 possessing a tertiary hydroxyl group were R/S = 77:23 and 84:16 respectively (R predominance). The presumed metabolic pathways of 8 in rabbits leading to these metabolites are discussed.

Lipase-catalyzed hydrolysis of some racemic 1-acetoxy-2-arylpropanes.

Racemic 1-acetoxy-2-phenylpropane (12) and 1-acetoxy-2-(2-naphthyl)propane (33) were hydrolyzed with lipase at 35-36 degrees C for 2 and 24 h to give predominantly (S)-2-phenyl-1-propanol (11) and (S)-2-(2-naphthyl)-1-propanol (32), respectively. However, racemic 1-acetoxy-2-(1-naphthyl)propane (25) was recovered intact even when the reaction was carried out for 240 h. On the other hand, the enantioselectivities towards racemic 2-phenyl (16), 2-(p-tolyl) (20), 2-(1-naphthyl) (28), and 2-(2-naphthyl) (36) derivatives of 1-acetoxy-2-propanol were very low.

Chemical transformation of terpenoids. X. Ionophoretic activities of macrocyclic lactone epoxides synthesized from geraniol.

Two coronand-type 18-membered lactone epoxides, i.e., geranyl dimeric lactone diepoxide (GL2E2, 10) and tetraepoxide (GL2E4, 11), were synthesized from geraniol as diastereomeric mixtures. Among them, GL2E4 (11) was shown to exhibit ion-transport activity for Ca2+ ion in the test using a W-07 (liquid-membrane type) apparatus and ion-permeation activities for Ca2+ and K+ ions across the human erythrocyte membrane. Isolation of six component diastereomers of GL2E4 (11) [GL2E4-1 (11c), -2 (11d), -3 (11e), -4 (11f), -5 (11g), -6 (11h)], was effected by HPLC separation of two diastereomeric tetraepoxides (11a, 11b) which were prepared from two diepoxides (GL2E2-1, 10a and GL2E2-2, 10b). The relative stereostructures of these diastereomers were determined by a combination of X-ray diffraction and 1H-NMR analyses. Among the six diastereomers, S2-symmetrical GL2E4-4 (11f) exhibited the strongest ion-transport activity for Ca2+ ion while C2-symmetrical GL2E4-6 (11h) exhibited the strongest ion-permeation activity for Ca2+ ion across the human erythrocyte membrane.

Recognition of a nucleic acid base by tryptophan-containing peptides: spectroscopic comparison of the interaction of Trp-Gly-Gly-Glu and Trp-Gly-Gly-Gln with 7-methylguanine base.

As a part of a study to elucidate the functional difference between Glu and Gln side-chains in terms of the recognition of guanine base by tryptophan-containing peptides via cooperative stacking and hydrogen-bond pairing interactions, the binding of 7-methylguanine to Trp-Gly-Gly-Glu and Trp-Gly-Gly-Gln was examined by fluorescence and 1H-NMR methods. Comparison of fluorescence experiments showed a binding preference for Trp-Gly-Gly-Glu over Trp-Gly-Gly-Gln. The analyses of the downfield and upfield shifts of the C2 amino and N7 methyl protons of 7-methylguanine base showed there was hydrogen-bond pairing of Glu and Gln side chains with the base and a stacking interaction of the Trp residue with the base, respectively. However, the hydrogen-bond pairing was more effective in the case of the Glu residue than the Gln residue, indicating the preference of the carboxyl group over the carbamoyl group to form hydrogen-bond pairing with the guanine base.

Intrapelvic two-dimensional time-of-flight magnetic resonance angiography in healthy and diseased subjects.

We evaluated the use of magnetic resonance angiography (MRA) based on two-dimensional (2D) time-of-flight (TOF) technique in the pelvic vasculature. In the initial phase of this study, MRA was performed in 10 normal subjects for determination of the optimum imaging parameters. Systemically varied scan parameters included flip angle ranging from 20 degrees to 90 degrees by 10 degrees increments and repetition time (TR) of 30, 40, and 50 ms. The optimum imaging parameters for intrapelvic MR angiography were found to be flip angle of 90 degrees, pulse sequence of 30/4.9 (TR ms/TE ms). Using these imaging parameters, 70-100% of the second division branches of the internal iliac artery could be visualized in each subject. In the second phase of this study, we evaluated the use of MR angiography in patients with intrapelvic vascular disease by comparing its accuracy in evaluating the iliac arteries with that of conventional angiography. In patients with atheromatous disease, the MR angiographic determination was consistent with that of conventional angiography in 23 of 28 visualized arterial segments (82%). In eight lesions in patients with non-stenotic vascular disease or hypervascular pelvic tumours, all lesions were demonstrated clearly with MR angiography. 2D TOF technique with high flip angle excitation is suitable for the evaluation of main iliac branches and diseased vessels.

Changes in bile acid composition of serum and gallbladder bile in bile duct ligated dogs.

Biliary obstruction was produced by surgical ligation of the common bile duct to observe alterations in serum bile acid composition. The percent composition of serum bile acids was found to change with time. Taurocholic acid increased on day 3 and accounted for more than 90% of the total bile acids in all dogs, however it decreased after day 7. The percentage of taurochenodeoxycholic acid (TCDC) and taurodeoxycholic acid (TDC) decreased to 4.2-6.0% and 0.2-0.7% on day 3, respectively. However, the percentage of TCDC increased after day 7 in all dogs and reached greater than 20% on day 14 in 2 dogs, whereas the percent TDC after bile duct ligation remained low in all dogs. Glycolithocholic acid, which was not identified in normal dog sera, was detected on day 3 and remained throughout the study in all dogs. Bile acid composition of gallbladder bile sampled on day 35 was similar to the serum bile acid composition on the same day. This indicates that the bile acids refluxed into the circulation in these dogs. In the present study, total cholic acid to chenodeoxycholic acid (C:CDC) ratio increased to 15.5-22.3 at three days post bile duct ligation and after the day 14, the C:CDC ratio decreased to its pre-ligation value or below. In contrast, the glycine conjugated to taurine conjugated bile acids (G:T) ratio did not change. Therefore, at this time, the G:T ratio would not be usable as an indicator of liver disease in dogs while it may be possible to use the C:CDC ratio.

Histopathology and viral antigen distribution in lymph nodes of cats naturally infected with feline immunodeficiency virus.

Lymph nodes from cats with natural feline immunodeficiency virus (FIV) infection were examined histopathologically for characterization of lesions in the symptomatic and terminal stages. The localization of viral antigen was also studied in the nodes by immunohistochemistry using a monoclonal antibody to FIV p24. In a node of persistent generalized lymphadenopathy stage, follicular hyperplasia was prominent and the viral antigen was demonstrated predominantly in the follicular lymphocytes and tingible body macrophages. In cats died at AIDS related complex stage, the lymph nodes showed either mixed follicular hyperplasia and involution, involution, or mixed involution and depletion. The lymph nodes in those died at AIDS stage showed considerable destruction of the nodal architecture with involution and depletion of lymphoid follicles. In this terminal stage, the viral antigen was seen prominently in histiocytes/macrophages of the sinus. These findings observed in the dead cats were similar to the lymph node changes seen in human AIDS associated with HIV infection. It was suggested that sequential histologic changes from follicular hyperplasia to depletion via involution took place in the FIV infected lymph node with persistence of FIV antigen.

Sequential morphological changes of the constrictive basilar artery in a canine model of experimental cerebral vasospasm by talc injection.

To demonstrate the possible role of foreign-body reaction to extravasated blood in provoking chronic cerebral vasospasm, talc (crystallized hydrous magnesium silicate) was injected as a non-biologic foreign material into the canine-cisterna cerebellomedullaris, and pathologic changes were followed. Angiographically, this cisternal talc injection induced delayed and prolonged constriction of the basilar artery, without any evidence of so-called early phase cerebral vasospasm that should occur shortly after an insult. Pathologically, around the spastic artery in the subarachnoid space with talc injection, the appearance of a moderate cellular migration coincided with cerebral vasoconstriction, which took place 2 days after the talc injection. In the spastic basilar arterial wall, marked constrictive and degenerative changes including myonecrosis and subintimal proliferation were induced by cisternal talc injection as early as on day 2, and the changes were progressive with time. These pathologic changes were extremely analogous to those of the human autopsy cases with chronic cerebral vasospasm (chronic VS) but more prominent than those observed in the experimental autologous blood-induced model. The present study demonstrated that a foreign-body reaction to talc alone could induce chronic VS in the absence of extravasated blood. Thus, it is possible to consider that inflammatory reactions to extravasated autologous blood in subarachnoid hemorrhage may give rise to chronic VS.

Ultrastructural observation of Pneumocystis carinii in bronchoalveolar lavage fluid from non-AIDS patients with P carinii pneumonia.

To investigate the morphology of Pneumocystis carinii in vivo, we studied the ultrastructure of P carinii in bronchoalveolar lavage fluid (BALF) obtained from four non-AIDS patients with P carinii pneumonia (PCP), and observed the morphologic changes after treatment in one patient. Every form recognized in the normal life cycle of P carinii, ie, thick-walled cysts, crescent-shaped cysts, and trophozoites, was detected in the pretreatment BALF specimens. In the posttreatment BALF, P carinii revealed marked ultrastructural changes such as the elongation of thick-walled cysts, the degeneration of intracystic bodies, and the disappearance of trophozoites. Massive debris derived from P carinii was still found in the BALF even after the treatment. Examination of P carinii in BALF, including ultrastructural observations, might help us to evaluate the disease status and the effect of treatment in patients with PCP.

Relationship between thyroid functions and urinary growth hormone secretion in patients with hyper- and hypothyroidism.

Thyroid hormone plays an important role in growth hormone (GH) synthesis and secretion. To study the relationship between thyroid function and urinary GH secretion in the hyperthyroid and hypothyroid states, we measured thyroid hormones, simultaneously with serum and urinary GH levels, in 54 patients with thyroid diseases. GH-releasing hormone (GRH) test was performed in 18 patients in order to evaluate serum and urinary GH responses to GRH in hyper- and hypothyroid states. Serum thyroid hormone levels were strongly correlated with the urinary GH levels in the patients, and the correlation was greater than that between serum thyroid hormone and serum GH levels. Urinary GH levels were significantly higher in the hyperthyroid patients than in the euthyroid and hypothyroid patients, although serum GH levels were not significantly different among these three groups. Serum GH response to GRH was significantly decreased in hyperthyroid patients as compared to euthyroid patients. However, urinary GH levels after GRH administration were not decreased in the hyperthyroid patients. These results suggest that hyperthyroid states increase GH in urine and may accelerate the urinary clearance of GH.

Absence of HTLV-I infection in some Indian populations.

A total of 946 adults belonging to ten population groups of Uttar Pradesh and West Bengal were screened for HTLV-I using a gelatin particle agglutination test. The percentage of seroreactive cases varied from 0 to 5.8 per cent. The overall prevalence of seroreactivity was 2 per cent. Of the 19 seroreactive cases, 15 were subjected to Western blot analysis, none could be confirmed. HTLV-I infection is, therefore, absent in these populations.

Effects of age and sex on sodium chloride sensitivity: association with plasma renin activity.

The mechanism by which excessive NaCl intake raises blood pressure has not been fully clarified. The present study was undertaken in 87 Japanese inpatients with essential hypertension to investigate the interrelation among effects of age, sex and the renin-angiotensin system on NaCl sensitivity. After ingesting a regular NaCl diet (170 mmol/day) for one week, subjects were placed sequentially on a week of low NaCl diet (50 mmol/day) and a week of high NaCl diet (340 mmol/day). NaCl sensitivity defined as the difference in mean blood pressure between the low and high NaCl diets did not differ between genders. NaCl sensitivity was positively correlated with age and the change in PRA. The fall in PRA after NaCl loading was significantly smaller in women than in men. By multiple regression analysis, age and the change in PRA independently contributed to the change in mean blood pressure. Furthermore, the interaction between sex and the change in PRA was selected as a statistically significant variable. In conclusion, NaCl sensitivity of blood pressure is independently associated with age and the inadequate suppression of the renin-angiotensin system. Because the contribution of the change in PRA to NaCl sensitivity was greater in women than in men, the mechanism of blood pressure elevation after NaCl loading may differ between genders.
