Detection of antibodies to Borrelia species among patients with confirmed sarcoidosis in a region where Lyme disease is nonendemic.

BACKGROUND: Lyme disease is a multisystemic disorder caused by the spirochete Borrelia burgdorferi, while sarcoidosis is a multisystemic granulomatous disease of unknown etiology. The purpose of this study was to evaluate the relationship between Lyme disease and sarcoidosis. METHODS: We examined the seroprevalence of antibody to Borellia species in patients with sarcoidosis. We performed the enzyme-linked immunosorbent assay, using three Japanese Borrelia species in addition to B. burgdorferi, and dotblot analysis using purified Borrelia-specific proteins in 38 patients with histopathologically confirmed sarcoidosis and 80 healthy controls. RESULTS: Two patients (5.3%) were positive for antibodies to Borrelia species according to one or both assays, and one (1.2%) healthy control was positive. In both patients it was suspected that Borrelia infection had developed prior to the development of sarcoidosis. CONCLUSION: Borrelia species were thought not to be responsible for the development of sarcoidosis in a nonendemic region in Japan. Since clinical manifestations of Lyme disease share certain similarities with those seen in sarcoidosis, ophthalmologists should be aware of the need to differentiate between the two diseases and the need for prompt treatment in each case.

Immunohistochemical expression of beta-human chorionic gonadotropin in colorectal carcinoma.

The presence of human beta-chorionic gonadotropin (HCG) was immunohistochemically studied in 123 cases of primary colorectal carcinoma using the avidin-biotin-peroxidase complex method. Positive staining for HCG was recognized in 45 (36.6%) tumors and a statistical difference was observed between the HCG-positive (n = 45) and -negative (n = 78) groups concerning the frequency of blood vessel invasion in the primary tumor (P < 0.01). The prognosis for patients with HCG-positive carcinoma was thus significantly worse than that for patients with HCG-negative carcinoma (P < 0.05). A multivariate analysis using the Cox hazards model demonstrated the positive or negative staining of HCG to be one of the independent prognostic factors. The above findings show that, in addition to various other prognostic factors, the HCG staining status may thus also help in determining the prognosis of patients with primary colorectal carcinoma.

The usefulness of FDG positron emission tomography for the detection of mediastinal lymph node metastases in patients with non-small cell lung cancer: a comparative study with X-ray computed tomography.

We evaluated the usefulness of fluorine-18-fluoro-2-deoxy-d-glucose positron emission tomography (FDG PET) in the detection of mediastinal lymph node metastases in patients with non-small cell lung cancer and then compared the findings with the results of X-ray CT by region based on the histological diagnoses. We examined 29 patients with non-small cell lung cancer. One hundred and thirty-two mediastinal lymph nodes were surgically removed and the histological diagnoses were confirmed. FDG PET images, including 146 mediastinal regions, were visually analysed and the mediastinal lymph nodes were scored as positive when the FDG uptake was higher than that in the other mediastinal structures. On the X-ray CT scans, any mediastinal lymph nodes with a diameter of 10 mm or larger were scored as positive. All three examinations were successfully performed on 71 regions. For FDG PET, we found a sensitivity of 76%, a specificity of 98% and an accuracy of 93%. On the other hand, for X-ray CT a sensitivity of 65%, a specificity of 87% and an accuracy of 82% were observed. A significant difference was observed in respect of both specificity and accuracy (P<0.05). Based on the above findings, FDG PET is suggested to be superior to X-ray CT when used for the detection of mediastinal lymph node metastases in patients with non-small cell lung cancer.

Comparison of insertion rate of L1 retroposon into intron 30 of the neurofibromatosis type 1 gene in seven Asian and Pacific populations.

The allele frequency of a L1 retroposon insertion into intron 30 of the neurofibromatosis type 1 (NF1) gene was determined by analyzing amplified fragment lengths in seven Asian or Pacific population; namely, Japanese, Chinese. Indian, Malay, Filipino, Indonesian and New Guinean. Nearly 100 chromosomes from each group were analyzed. The presence of the L1 insertion was identified by the appearance of an abnormally large PCR-amplified product. The insertion frequency varied from 0.45 to 0.75, depending on the population group. Malay and Indonesian populations were found to have the highest insertion frequencies (0.75 and 0.72, respectively), while the wild-type genotype was more prevalent in Indians. The lowest insertion frequency (0.45), observed in Indians, was nearest to that reported in Westerners (0.35). The different L1 insertion frequencies found in Asian and Pacific groups reflect a major divergence in these human populations. Japanese and Chinese populations showed the highest heterozygosity (0.50), suggesting the usefulness of this polymorphism in linkage analysis in these populations.

Quantitative analysis of geometric-pattern features of interstitial infiltrates in digital chest radiographs: preliminary results.

We are developing a computerized method for detection and characterization of interstitial diseases based on a quantitative analysis of geometric features of various infiltrate patterns in digital chest radiographs. In our approach, regions of interest (ROIs) with 128 x 128 matrix size (22.4 mm x 22.4 mm) are automatically selected, covering peripheral lung regions. Next, nodular and linear opacities, which are the basic components of interstitial infiltrates, are identified from two processed images obtained by use of a multiple-level thresholding technique and a line enhancement filter, respectively. Finally, the total area of nodular opacities and the total length of linear opacities in each ROI are determined as measures of geometric pattern features. We have applied this computer analysis to 72 ROIs with normal and abnormal patterns that were classified in advance by six chest radiologists. Preliminary results indicate that the distribution of measures of geometric-pattern features correlate well with radiologists' classification. These early results are encouraging, and further evaluation hopes to establish that this computerized method might prove useful to radiologists in their assessment of interstitial diseases.

MR arthrography of elbow: evaluation of the ulnar collateral ligament of elbow.

OBJECTIVE: The purpose of this study was to evaluate ulnar collateral ligament (UCL) injury of the elbow in throwing athletes by MRI and MR arthrography. DESIGN: Ten elbows of throwing athletes were examined on both plain MRI and MR saline arthrography and the injuries subsequently surgically proven. Spin-echo (SE) T1-weighted and fast SE T2-weighted coronal images were obtained. RESULTS: The UCL was unclear in all ten cases on T1-weighted MRI. In five cases an avulsion fracture was also found on T1-weighted MRI. On T2-weighted MRI, abnormal high-intensity areas were identified in or around the UCL. On T2-weighted MR arthrography images, extracapsular high-intensity areas, which represent extracapsular leakage, were found in four of five cases with avulsion fracture. At surgery, all these four cases showed avulsion fractures with instability; the other case had a fracture but it was stable and adherent to the humerus. On T2-weighted MR arthrography images, an extracapsular high-intensity area was found in one of the five cases without avulsion fracture. At surgery this patient had a complete tear of the UCL itself. CONCLUSION: MR arthrography provided additional information for evaluating the degree of UCL injury.

Intra-articular calcifying synovial sarcoma mimicking synovial chondromatosis.

A case of intra-articular calcifying synovial sarcoma of the right knee joint in a 27-year-old man is reported. Prior to admission the patient had been suffering pain in the right knee for 5 years. Plain radiographs showed multiple calcified densities in the anterior aspect of the right knee closely mimicking synovial chondromatosis. Histological examination revealed that the lesion was a synovial sarcoma with extensive calcification subjacent to the synovial surface. Although intra-articular calcifying synovial sarcoma is very rare, it is important to be aware of the fact that synovial sarcoma, as well as synovial chondromatosis, can be present as a calcified lesion in the joint cavity.

A coplanar PCB induces a selenium binding protein as a major cytosolic protein in rat liver.

We obtained evidence that a toxic coplanar polychlorinated biphenyl (PCB) induces a counterpart of murine 56kDa selenium binding protein in rat liver cytosol. A 54kDa protein in the liver cytosol was significantly induced by 3,3',4,4',5-pentachlorobiphenyl and proved to be a major cytosolic protein in the rat liver. The protein exhibited pI of 6.8 on two-dimensional gel electrophoresis. The amino acid sequence of peptide fragments from the protein digested in situ, was highly similar to a 56kDa selenium binding protein and similar to an acetaminophen binding protein in mice.

Genetic polymorphisms of the major histocompatibility complex-encoded antigen-processing genes TAP and LMP in sarcoidosis.

Sarcoidosis is a granulomatous disease showing a significant increase in the HLA-DR5, -DR6, and -DR8 associated alleles in Japanese. To investigate whether the class I antigen-processing genes, encoded within the MHC class II region between the HLA-DP and -DQ loci, are involved in determining the susceptibility to sarcoidosis, TAP1, TAP2, and LMP2 alleles were analyzed by the PCR-RFLP method in 85 Japanese patients with sarcoidosis and 91 healthy controls. There were no significant differences in the distribution of TAP1 and LMP2 alleles between the subgroups of the patients and controls positive or negative for DR5, DR6, and DR8. A significant decrease in the frequency of TAP2*0201 was found among the patients negative for DR5, DR6, and DR8 as compared to the DR-matched controls (p < 0.05), but this could be explained by its linkage disequilibrium to the negatively associated allele DR1. These findings suggest that the TAP or LMP2 gene is not primarily involved in the susceptibility to sarcoidosis. In the course of this study, a linkage disequilibrium was observed in the Japanese population between TAP1 and TAP2 alleles, TAP1*0201 and TAP2*0102.

Significant induction of a 54-kDa protein in rat liver with homologous alignment to mouse selenium binding protein by a coplanar polychlorinated biphenyl, 3,4,5,3',4'-pentachlorobiphenyl and 3-methylcholanthrene.

A 54-kDa protein in rat liver cytosol was significantly induced by treatment with 3,4,5,3',4'-pentachlorobiphenyl (25 mg/kg, single i.p.) and 3-methylcholanthrene (20 mg/kg, once a day for 3 days, i.p.). The protein exhibited pI of 6.8 on two-dimensional gel electrophoresis. The amino acid sequences of peptide fragments from the protein digested in situ were highly similar to a selenium binding protein in mice and to the isoform acetaminophen binding protein in mice. The present result clearly demonstrates that a coplanar polychlorinated biphenyl and 3-methylcholanthrene are responsible for induction of selenium binding protein homologues. The physiological role of the mouse proteins, however, is not yet elucidated.

An artificial CuII complex with intriguing oxygen radical-quenching profile. X-ray structure, cytochrome c assay, and ESR study.

A novel artificial peptide named HPH-Pep, comprising a pyridine and two histidine units, was synthesized. The HPH-Pep-CuII complex had unique pentacoordinated structure as shown by X-ray crystallography and exhibited superoxide-scavenging activity as indicated by ESR spectroscopy. The superoxide-quenching profile of HPH-Pep-CuII was studied in detail by cytochrome c assay and ESR spin trapping and it was found that (1) HPH-Pep-CuII did not scavenge hydrogen peroxide or hydroxyl radical and hence the scavenging activity was specific to superoxide, and (2) HPH-Pep-CuII did not generate hydrogen peroxide upon scavenging superoxide.

Expression of activation-related molecules on regional lymph node lymphocytes in human lung cancer.

Using two- and three-color flow cytometry, we investigated the surface makers of regional lymph node lymphocytes (RLNL) in 54 patients with primary lung cancer in order to determine whether or not RLNL are in an activation state when compared with the corresponding peripheral blood lymphocytes (PBL) for the purpose of clarifying the characteristics of RLNL. RLNL showed a cell composition that was different from the corresponding PBL with a high proportion of CD3+ cells, CD4+ cells and CD20+ (B) cells as well as a low proportion of CD8+ cells and CD16+ (NK) cells. RLNL also contained a significantly higher proportion of CD45RO+ T cells and a lower proportion of CD45RA+ T cells in comparison to the corresponding PBL. Furthermore, we explored the activation-related molecules such as the interleukin-2 receptor (IL-2R) alpha chain, the IL-2R beta chain, HLA-DR and leukocyte function-associated antigen 1 (LFA-1) on CD4+ and CD8+ cells. The data showed that the expression of CD45RO, the IL-2R alpha chain, HLA-DR on CD4+ cells, and those of CD45RO and HLA-DR on CD8+ cells were significantly higher in RLNL than in PBL. On the other hand, PBL showed a higher expression of the IL-2R beta chain and LFA-1 only on CD8+ cells, which are thought to include CD8+ NK cells. When these activation related molecules were analyzed on CD45RO+ T cells, which are thought to be memory T cells, then the expression of the IL-2R alpha chain, HLA-DR on CD4+CD45RO+ cells and HLA-DR on CD8+CD45RO+ cells were significantly higher in RLNL than in PBL. Lastly, we analyzed the surface molecules according to such clinical factors as metastasis to the lymph nodes and the stage of lung cancer. Interestingly, RLNL with lymph node metastasis showed an increase in the percentage of CD20+ cells and a decrease in the percentage of CD4+ cells in comparison to those with benign lung disease, while these differences were not observed in PBL. The expression of ++CD45RO and LFA-1 on CD4+ and LFA-1 on CD8+ cells on RLNL in stage I + II is significantly higher in comparison with benign lung disease. Furthermore, RLNL in stage III + IV revealed the reduced expression of CD45RO, HLA-DR and LFA-1 on both CD4+ and CD8+ cells compared with those in stage I + II. These results thus demonstrated that RLNL were in a more activated state, particularly in stages I + II, than PBL. Whether these activation states in RLNL are directed against autologous tumor cells or not can hopefully be elucidated after the completion of further ongoing studies.

LMP7 polymorphism in Japanese patients with sarcoidosis and Behçet's disease.

To evaluate the influence of the MHC-linked LMP7 gene on disease susceptibility in HLA class I and class II-associated diseases, the distribution of LMP7 alleles was determined using the PCR-RFLP method in 69 Japanese patients with Behçet's disease, 65 patients with sarcoidosis, and 100 unrelated healthy controls. No differences were found between either of the patient groups and the healthy control group, indicating that LMP7 allelic variation may not contribute to the pathogenesis of either Behçet's disease or sarcoidosis. We also analyzed linkage disequilibria between LMP7 and HLA class II alleles in Japanese populations.

Synergistic effect between size and cholesterol content in the enhanced hepatic uptake clearance of liposomes through complement activation in rats.

PURPOSE: The effect of liposome size and cholesterol (CH) content on the pharmacokinetics of liposomes was investigated in rats. METHODS: The pharmacokinetics of liposomes was examined using 5(6)-carboxyfluorescein (CF) as an aqueous phase marker. The extent of complement activation (ECA) was also measured by the release of CF from liposomes in serum. RESULTS: Both the size and the CH content influenced the mean residence time, total body clearance, and the hepatic uptake clearance (CLh) of liposomes. The increase of the size of liposomes increased the CLh at each CH content. There was no CH dependency of CLh in small liposomes (200 nm in diameter), although the CLh increased with the increase in the CH content in large (800 nm) and medium (400 nm) liposomes. A significant interaction effect was observed between liposome size and the CH content on CLh according to the analysis of variance. The good correlation between CLh and ECA indicated the role of complements as opsonins in enhancing the hepatic uptake of liposomes. The interaction effect between the size and CH content on CLh was explained principally by the product of the size and CH content. CONCLUSIONS: A synergistic effect was observed between the size and the CH content on CLh. An underlying hypothesis of the synergistic effect was postulated based on the size dependent recognition of liposomes by complement system.

A novel biologically active eel calcitonin analogue with carboxyl terminal Hse32-amide: carboxyl terminal Pro32-amide in calcitonin is not essential for biological activity.

Eel calcitonin (CT) analogues having C-terminal homoserine (Hse)-amide at position 31, 32, or 33 were synthesized, and in vivo hypocalcemic activity of the analogues were determined. The present study showed that: (i) An eel CT analogue having Hse-amide at position 32 was more active than native eel CT, and the duration of hypocalcemic action of the analogue was equivalent to that of native eel CT. (ii) Either curtailment or elongation of [Hse32-amide]-eel CT peptide chain by one amino acid resulted in a great loss of hypocalcemic activity. The results of the present study indicate that Pro-amide at the C-terminus of CT is not essential for its biological activity in vivo. Replacement of C-terminal Pro32-amide by Hse32-amide in eel CT molecule produced an analogue with a hypocalcemic activity greater than that of native eel CT.

A gene for a dominant form of non-syndromic sensorineural deafness (DFNA11) maps within the region containing the DFNB2 recessive deafness gene.

Hereditary hearing loss is divided into two groups, syndromic and non-syndromic, the latter being more common and highly heterogeneous. Linkage analyses were performed on a Japanese family showing a dominant form of non-syndromic progressive sensorineural hearing loss. This gene (DFNA11) was localized within the region of chromosome 11q which contains the second gene for a recessive form of non-syndromic sensorineural hearing loss (DFNB2). Since it has been reported that another gene for dominant non-syndromic hearing loss (DFNA3) has been mapped to the same region as the first gene for recessive hearing loss (DFNB1), it is possible that different mutations in the DFNB2 gene may result in either dominant or recessive hearing loss.

Crystallization and preliminary X-ray diffraction study of recombinant human eukaryotic initiation factor-4E.

Recombinant human eukaryotic initiation factor-4E (eIF-4E), purified by m7GTP-Sepharose 4B affinity chromatography, was used for crystallization. After concentration of the eIF-4E protein (7 mg/ml), the solution was subjected to crystallization by the hanging-drop method. Transparent needle crystals complexed with m7GTP were obtained from 50 mM 2-(N-morpholino)ethanesulfonic acid-KOH buffer (pH 6.5) containing 25% (w/v) polyethylene glycol 6000 and 0.2 M (NH4)2SO4. The crystals belong to tetragonal space group P4(1) or P4(3), of Z = 4, with unit-cell dimensions of a = 89.26, b = 89.26, and c = 38.51 angstrum, and diffract beyond 2.1 angstrum resolution. The Vm value was calculated to be 3.07 angstrum 3/Da, which indicates a solvent content of 59.9%.
