A hyperpolarizing response induced by glutamate in mouse cerebellar Purkinje cells.

In the vertebrate nervous system, glutamate (Glu) receptors are generally known to cause depolarizing responses. We report here a novel type of Glu response in Purkinje neurons of mouse cerebellar slices, namely glutamate-induced hyperpolarization (GH). This response is not due to activation of inhibitory interneurons, because application of tetrodotoxin (TTX), bicuculline, or strychnine did not abolish GH. In addition, GH persisted in a Ca(2+)-free or a low-Cl- solution, which rules out the involvement of gK(Ca) or GABAA mechanisms. Quisqualate (Quis) and trans-1-amino-1,3-cyclopentanedicarboxylic acid (tACPD), which are potent and selective agonists, respectively, for the metabotropic Glu receptor (mGluR), failed to induce GH. L-2-Amino-4-phosphonobutyric acid (L-AP4) was also ineffective. Simultaneous recording of electrical activity and intracellular Ca2+ concentration ([Ca2+]i) showed that GH was not accompanied by [Ca2+]i changes. Voltage clamp experiments showed that GH is due to reduction of a tonically active conductance with a reversal potential around 0 mV. Two possible mechanisms are suggested for GH: (1) changes in the desensitized steady state of ionotropic Glu receptors, or (2) a novel Glu-mediated mechanism.

Properties of rhythmically active reticular neurons around the trigeminal motor nucleus during fictive mastication in the rat.

Response properties of the neurons in the reticular formation around the trigeminal motor nucleus (MoV) were examined during cortically-induced fictive mastication (CIFM) in anesthetized and immobilized rats. Forty-three neurons were rhythmically active (RA neurons) during CIFM, most of which were located in the supratrigeminal nucleus and the reticular formation medial to the oral spinal trigeminal nucleus. The firing frequency of 36 of the RA neurons was modulated in the same rhythm as that of masseteric or digastric nerve activities during CIFM. We divided these neurons into four groups according to the phase of activation: sixteen neurons fired mainly in the phase of masseteric activity (type 1), 11 fired in the transition phase from masseteric activity to digastric activity (type 2), 5 fired in the phase of digastric activity (type 3) and 4 fired in the transition phase from digastric activity to masseteric activity (type 4). Thirty-nine (91%) of the 43 RA neurons responded to at least one of the tested peripheral stimuli. The responses were mostly excitatory but inhibitory responses were sometimes obtained, especially for types-1 and 2 neurons. RA neurons in the reticular formation medial to the oral spinal trigeminal nucleus responded to stimulation of inferior alveolar nerve at a shorter latency than RA neurons in the supratrigeminal nucleus. Fifteen (48%) of 31 RA neurons responded to triple-pulse stimulation of the contralateral cortex. In contrast, only 5(26%) of the 19 RA neurons responded to the ipsilateral cortical stimulation. Stimulation of the ipsilateral MoV was performed on 24 RA neurons, of which 9 responded antidromically (A-RA neurons) at latencies of 0.4-1.4 ms. Eight (89%) of the 9 A-RA neurons received peripheral inputs. The spike triggered averaging method was applied to 4 of the 9 A-RA neurons, ad in all cases short latency field potentials were recorded in the MoV. We conclude that trigeminal premotor neurons receive convergence from central and peripheral inputs. This integration can adjust the appropriate level of motoneuronal excitability during mastication.

Profound vasoconstrictive effect of cigarette smoking in diabetics with autonomic neuropathy.

Changes in leg skin temperature during and after cigarette smoking were examined in 25 diabetics with autonomic neuropathy (au-neuropathy) (mean age 52 +/- 2 years) and 23 without (mean age 49 +/- 2 years), and 15 normal controls (mean age 49 +/- 2 years) using a Thermoviewer JTG 3300. All subjects were habitual smokers. Japanese Seven Star cigarettes (each containing 1.2 mg nicotine) were used for the test. Tender brand cigarettes (0.3 mg) were used additionally to compare the effect of nicotine content on the results. The smoking-stimulated thermographic pattern in the control group was rather flat, with a slight decrease in temperature followed by a slight increase before returning to the baseline level 15-20 min after smoking. The frequency of the typical temperature decreasing pattern, characterized by a profound decrease and gradual return to the baseline level in about 1 h, was significantly higher in the diabetic group than in the control group (33/48 vs 1/15; P less than 0.005). A comparison of the two diabetic subgroups revealed that the frequency was considerably higher in the subgroup with au-neuropathy (23/25 vs 10/23). These results suggest that au-neuropathy affects smoking-induced vasoconstriction in diabetic subjects. The maximum temperature decreases recorded were 1.5 +/- 2.0 and 2.69 +/- 0.24 degrees C in the controls and diabetics with au-neuropathy, respectively. The effect of Tender cigarettes with a low nicotine content was much weaker than that of Seven Star, which further suggests that smoking-induced vasoconstriction was also nicotine-dependent.

An immunopotentiator of beta-1,6;1,3 D-glucan prevents diabetes and insulitis in BB rats.

The preventive effect of an immunopotentiator, beta-1,6;1,3 D-glucan, on the development of diabetes and insulitis was studied in BB rats. The intravenous administration of 1 mg kg-1 week-1 of beta-1,6;1,3 D-glucan from the age of 4 weeks decreased the cumulative incidence of diabetes from 43.3% (13/30) to 6.7% (2/30) (P less than 0.005) and also the incidence of insulitis from 82.4% (14/17) to 26.3% (5/19) at the age of 20 weeks (P less than 0.002). Eight of nine rats were free from diabetes for 5 weeks after stopping beta-1,6;1,3 D-glucan at the age of 20 weeks. The total numbers of leukocytes in the peripheral blood and spleen of the rats were increased by beta-1,6;1,3 D-glucan treatment (P less than 0.05), whereas their T lymphocytes subsets were not changed. These data indicate that immunopotentiators could modulate the autoimmune mechanisms directed to pancreatic islets and inhibit the development of diabetes in BB rats.

Sex determination by discriminant function analysis of lateral cranial form.

To sex the cranium, morphological features of cranial specimens were quantified with a personal computer that automatically measures distance and gradient for 39 craniometric points in the lateral contour line of the skull, which were digitized by a tablet digitizer connected to the computer. Specimens used for discriminant analysis were 50 male and 50 female adult Japanese skulls. The lateral contour showed sex differences in the nasal bone, supraorbital ridge, forehead and vertex. The nasal bone and supraorbital ridge were more developed in male contour line, and the forehead was more rounded in female contour line. But compared with the supraorbital ridge and forehead, the vertex had a wide variety of contour lines in both sexes. The vertex seemed to be less reliable as the indicator of sex. The sex differences were better reflected by gradient than distance. From variables of the gradient and distance showing significant sex differences, the discriminant function was derived and tested in 21 other specimens (13 male and 8 female skulls). The mean ratio of correct sexing of the human skull by the discriminant function was 86%.

Direct correlation between sonography and histology of minute malignant hepatic nodule.

To improve the qualitative diagnosis of ultrasonography (US) of minute malignant hepatic nodules, we studied the accurate and direct correlations between US and histopathology of 33 lesions using formalin-fixed autopsied livers. The target nodules were cut on the same plane as their US-image. The solid nests of malignant tumor without degeneration were hypoechoic on US. Hepatocellular carcinoma showed a wide variety of echo levels, from hypoechoic to isoechoic, and its images were more heterogeneous than those of other malignant lesions. The squamous cell carcinoma ranged from hypoechoic to hyperechoic according to the degree of keratinization. US may be useful for evaluating the degree of differentiation in squamous cell carcinoma. Marked mucin-production and "comedo" pattern seem to be histological factors of the hyperechoic US image of adenocarcinoma. This study may contribute to improve the quality of US-diagnosis of minute hepatic nodular lesions.

Removal of micro-organisms by filtration through unwoven cloth coated with a pyridinium-type polymer.

Unwoven cloth coated with 32 mg/g of a copolymer of N-benzyl-4-vinyl-pyridinium chloride and styrene was found to be effective in removing micro-organisms from water. In experiments demonstrating removal of Escherichia coli by filtration through ten sheets of the unwoven cloth, the rate of removal was 99.99% at a filtration rate of 2.6 cm/h, and remained at 99% even at a high filtration rate of 300 cm/h and a low influent concentration of the bacterial cells such as 10(3) cells/ml. The rate of removal tended to increase with a decrease in the influent bacterial concentration. Seven other bacteria and two yeasts were effectively removed by filtration through the unwoven cloth. Filtration through the unwoven cloth was also effective in removing spores of fungi from water but was not very effective in removing bacteriophage T4 from aqueous solution.

Quantitative analyses of urinary pyridinoline and deoxypyridinoline excretion in patients with hyperthyroidism.

It is well established that bone turnover increases and bone resorption exceeds bone formation in patients with hyperthyroidism. Recently, urinary pyridinoline and deoxypyridinoline have been employed as indicators of bone resorption. In this study, urinary pyridinoline and deoxypyridinoline levels in fourteen female patients with untreated hyperthyroidism were measured and compared with levels in 134 healthy females. Values of urinary pyridinoline and deoxypyridinoline were significantly higher than in age-matched controls. Moreover, values of serum free triiodothyronine correlated with urinary pyridinoline (r = 0.769, p < 0.01), and urinary deoxypyridinoline (r = 0.799, p < 0.001) in patients with hyperthyroidism.

Adenosquamous carcinoma of the lung. Clinicopathologic and immunohistochemical features.

Adenosquamous carcinomas (ASC) of the lung comprise only 1.8% of all pulmonary cancers. Eleven patients with this disease (6 men and 5 women; mean age, 62 years [range, 51 to 78 years]) were treated. On the basis of pathologic and immunohistochemical evidence, the tumors were separated into three groups: five ASCs of predominantly glandular type, which may originate from squamous metaplasia in a preexisting adenocarcinoma; three mixed type ASCs, possibly derived from the transition of undifferentiated carcinoma to two different elements; and three predominantly squamous type ASCs resembling high-grade muco-epidermoid carcinoma. The 5-year survival rate of 35% for patients with ASCs was the same as that for patients with other nonsmall cell carcinomas. For two patients with ASCs of the predominantly glandular type and for one with the mixed type, resection was incomplete because of extensive local disease. All tumors of the predominantly squamous type could be excised completely. However, differences in the malignant potential of these three types of ASCs are still controversial.

Treatment of Langerhans cell histiocytosis in children with etoposide.

Ten children with Langerhans cell histiocytosis (LCH) were treated with etoposide. For five patients, this was the initial diagnosis. The other five had failed to respond to previous therapies. Etoposide (100 mg/m2) was given intravenously twice a week for 4 weeks, followed by maintenance therapy every 2 to 4 weeks for 2 years. All 10 patients responded to etoposide, and 6 of them (60%) have been in complete remission for 3 to 36 months without any side effects. One patient relapsed with diabetes insipidus, one with a soft tissue mass, and two others developed multiple bone lesions. Chemotherapy with etoposide appears to be effective and safe for the treatment of children with systemic LCH.

Transient unilateral reverse ventilation/perfusion mismatch in a patient with lung cancer.

The authors report a transient, unilateral reverse ventilation/perfusion (V/P) mismatch in a patient with squamous cell lung cancer. The right lung was well perfused but not ventilated (unilateral reverse V/P mismatch). However, lung imaging performed 3 days later demonstrated the absence of both perfusion and ventilation (V/P match). Reverse V/P mismatch may be a transient process.

The vasculature and clinical application of the posterior tibial perforator-based flap.

Use of the posterior tibial flap pedicled on the posterior tibial vessels has been described by several authors, but with it there is the major disadvantage of an unavoidable transection of the posterior tibial artery. To overcome this disadvantage, we anatomically studied the perforators from the posterior tibial artery and used posterior tibial perforator-based flaps clinically. Based on our anatomic study of 25 cadaveric legs, the cutaneous perforators were considered to be distributed from the distal to the proximal sides of the lower leg through the medial border of the tibia, and they were classified into three types: septocutaneous perforators mainly located in the distal third of the leg, muscle perforators located in the proximal half, and periosteal perforators in the proximal third of the leg. The average size and number of perforators was 0.8 mm and 3.1 in one leg, respectively. A considerable number were located at sites from 70 to 140 mm superior to the medial malleolus. Based on our clinical cases repaired with flaps, we consider this flap to be useful as a free flap for the repair of defects of the extremities and as an island flap for reconstruction of defects on the anteromedial aspect of the lower leg. The territory of the flap is relatively wide, being 19 x 13 cm. The long saphenous vein can be used safely as the venous drainage system in the case of free-flap transfer.

Genomic organization of the human WT1 gene.

We have analyzed the genomic structure of the human WT1 gene, one of the recessive oncogenes for Wilms' tumor at chromosome 11p13. By analyses of three cosmids covering the WT1 gene as well as products generated by polymerase chain reaction, cleavage sites for 10 restriction enzymes were mapped in a region of about 80 kb, and the positions of 10 exons were defined. We also mapped two polymorphic sites for TaqI. Our genomic map will be useful to analyze DNA abnormalities sometimes found in the tumors, as well as loss of heterozygosity.

Base Sequence

Increased Na+/H+ exchange activity in vascular smooth muscle cells of spontaneously hypertensive rats and possible involvement of protein kinase C.

1. Na+ influx into cultured vascular smooth muscle cells (VSMC) obtained from spontaneously hypertensive rats (SHR) and from Wistar-Kyoto rats (WKY) was measured. Na+ influx via the Na+/H+ exchange system was measured as the rate of 22Na+ influx into cultured VSMC sensitive to ethylisopropylamiloride (EIPA), a specific inhibitor of the exchange system. 2. The total 22Na+ influx rate in SHR was significantly higher than in WKY (6.08 +/- 0.16 vs 4.13 +/- 0.09 nmol/min per mg protein; P less than 0.001; n = 14). The EIPA (1 X 10(-4) mol/L)-sensitive 22Na+ influx rate in SHR was significantly higher than that in WKY (4.32 +/- 0.27 vs 2.17 +/- 0.14 nmol/min per mg protein; P less than 0.001; n = 14). There was no difference in EIPA-insensitive 22Na+ influx between SHR and WKY. The EIPA-sensitive 22Na+ influx rate into VSMC was significantly decreased in SHR but not in WKY by the addition of 1 X 10(-4) mol/L 1-(5-isoquinoline-sulfonyl)-methylpiperazine (H-7), an inhibitor of protein kinase C (PK-C). 3. These results suggest that the increase in Na+ influx in SHR may be due to elevation of the Na+/H+ exchange activity, and possible involvement of PK-C in the increased Na+/H+ exchange activity in VSMC from SHR.
