A case of heterotopic pancreas in lymph node.

A case of pancreatic heterotopia in a lymph node is described. Small pieces of pancreatic tissue were found incidentally in four lymph nodes located around the common hepatic artery in a 65-year-old man. Both exo- and endocrine elements of the pancreas, together with ductal structures were found to constitute the heterotopic tissue. No authentic case of heterotopic pancreas in lymph nodes has been previously reported in the literature.

Oat cell carcinoma (apudoma) of the esophagus: a case report.

An oat cell carcinoma occurring in the esophagus of a 62-year-old man is described. A strong argyrophilia of the oat cells together with the presence electron microscopically of neurosecretory type granules in their cytoplasm suggests a derivation of this tumor from the cells of the APUD series, and the tumor is diagnosed as apudoma. An aberrant columnar epithelium on the esophageal surface is presumed to be the source of its origin. A rapid postoperative course of the patient despite a rather circumscribed growth pattern of the primary tumor appears to characterize the clinical aspect of this case. An apparent accumulation of the cases with esophageal apudoma in Japan is discussed.

Reconstruction of mandibular defects with revascularized free rib grafts.

Two cases of hemimandibular reconstructions with revascularized free rib grafts are presented. The viability of the transplants was confirmed by bone scans and biopsy, even though the main nutrient vessels providing the intramedullary blood flow were not included in these grafts (and only the periosteal circulation was utilized). The removal of a rib graft without the nutrient vessel eliminates the need for a complicated posterior dissection, close to the costovertebral joint. Revascularized free bone grafts have a greater chance of survival, provide more rapid healing, offer less risk of absorption, and are more resistant to infection than conventional bone grafts.

The optimal design of MUMPS global file.

This paper presents the basic consideration of global file design on MUMPS language. The major goal of global design is to reduce the number of disc accesses made over the long-term use of a file. To attain this goal, the procedure for finding the global file which minimizes the average number of disc blocks accessed to obtain a data node is proposed. Furthermore, the total number of blocks in use with file design are demonstrated by simple examples.

[Brain abscess discovered 38 years after head injury (author's transl)].

We reported a case of delayed brain abscess following a penetrating gun-shot injury. The brain abscess surrounding retained intracranial fragments of bone and shell was found 38 years after head injury. The patient's only symptom had been minimal weakness of the right arm. This case was treated surgically with good results. The long period of silent infection was discussed.

Glutamate dehydrogenase from Bacillus subtilis PCI 219. I. Purification and properties.

Bacillus subtilis PCI 219 has a single glutamate dehydrogenase (GDH) [EC 1.4.1.3] with dual coenzyme specificity [for NAD(H) and NADP(H)]. The enzyme was purified 800-fold from crude extracts of B. subtilis from the post-exponential phase of growth and showed one significant protein band on gel electrophoresis. This band was determined, by activity staining, to have all the GDH nucleotide specificities. Its molecular weight was estimated to be 250,000+/-20,000 by gel filtration, and 270,000+/-30,000 by zone centrifugation in a sucrose density gradient. Polyacrylamide gel electrophoresis in sodium dodecyl sulfate showed that GDH has a subunit size of about 57,000. The pI of GDH was found to bepH 3.7 by isoelectric focusing. GDH exhibited nonlinear kinetics in the reduction of NAD+, and in the reverse direction, the substrate, NH4+, was strongly inhibitory at high concentrations. Purine nucleotides did not affect the activity. The oxidative demination of glutamate was significantly inhibited by the metabolites oxaloacetate and citrate, which acted as allosteric effectors of this enzyme, inhibiting the reaction in one direction. The pH optimum of each of the activities of GDH and the stability of GDH are also reported.

Protective action of estrogen in endotoxin shock.

Protective action of conjugated equine estrogen (CEE) against endotoxin shock was admitted in mice. Male DDN strain mice pretreated intraperitoneally with 2 mg of CEE showed various survival rates according to the interval between administration of CEE and endotoxin injection. Survival rates of the control were 20.4%, but in the group pretreated with CEE they were 0% at 0 hour, 26.3% at 1 hour, 33.3% at 12 hours, 55.7% at 24 hours (p less than 0.001), 10.5% at 36 hours, and 31.2% at 48 hours respectively. Mice pretreated with 2 mg of CEE before 24 hours exhibited excellent resistance to 0.5 mg endotoxin. On the other hand, the hemodynamic effect of CEE was examined in six mongrel dogs, which showed little hemodynamic changes following administration of 10 mg/kg CEE for three hours observation. We speculated that the anti-shock action of CEE was due to the modification of vascular response to vasoactive substances, on the one hand it augmented the vascular contractile response to vasopressors and on the other hand it lessened the hypersensitive vasoconstriction under shock state.

Use of the halo-pelvic apparatus for treatment of fracture-dislocations of the thoracic and lumbar spines accompanied by paraplegia.

Twenty-five cases of paraplegia due to fracture-dislocations of the thoracic and lumbar vertebrae were treated using the halo-pelvic apparatus. Thirteen of them only needed the apparatus, two underwent laminectomy, and ten had posterior spinal fusion in addition. The advantages of this method are that gentle reduction can be achieved without severe complications, and that posterior spinal fusion can be performed with this apparatus on, thus enabling an early start to the subsequent exercise program and minimal absence from work.

A new nail-plate for treatment of fracture of the neck of the femur.

Operative treatment of fractures of the neck and trochanter of the femur does not always produce a satisfactory result. This is usually due to biomechanical problems with the available internal fixation methods. We studied the anatomy of the neck of the femur by roentgenograms and sectional specimens from 70 cadavers. In addition, various nail-plates were subjected to buckling tests and, by simultaneously attaching a strain-gauge, stress distribution was calculated. The results of these preliminary studies were then used to design a new nail-plate better than those available at present. Testing of this new nail-plate confirmed that it had a strength equal to that of the Holt nail-plate (the strongest of the available nail-plates).
