Changes in the FDP (fibrin and fibrinogen degradation products) value under mental and physical stress. II. Simultaneous observations of FDP and other indices of fatigue in persons under experimentally loaded stress.

We assumed the possibility of adopting FDP as an index of fatigue and carried out examinations upon various load conditions causing stress. For our observations other indices which are thought effective for fatigue evaluation, such as urine CA, flicker values, response time, Uchida's Kraepelin test, and examination of subjective symptoms recommended by the Japan Association of Industrial Health, were adopted at the same time. 1) Both mental and physical loads increased FDP values both in serum and in urine. 2) These changes in FDP values agreed comparatively well with the changes in CA by load. 3) The FDP value showed changes by load which was so light that it did not induce functional deterioration detectable by the response time or the flicker value which was considered to be one of effective physiological indices. 4) The extent or degree of subjective symptoms found by the survey agreed well with the changes in FDP values. Through above examinations, FDP values of serum and urine seem to be useful as an index of transient fatigue; especially they can well reflect mental fatigue which cannot be indicated well by other biochemical indices. Also FDP can well show individual characteristics, namely FDP indicates comparatively satisfactorily both individual sensitivity and specificity. Furthermore, this method does not require special techniques and examinations can be done simply.

Inhibitory effect of glucocorticoid on complement activation induced by lipopolysaccharide.

Endotoxic lipopolysaccharide is verified to activate complement, which seems to be one of the major pathogenic processes of septic shock, and glucocorticoid antagonizes this endotoxic manifestation in a dose-related manner. In this study, we tried to elucidate how lipopolysaccharide activates complement in vitro, and how glucocorticoid affects the interaction between them. First, we elucidated in vitro that complement was consumed in proportion to the dose of lipopolysaccharide added. Next we made clear that glucocorticoids inhibit the complement activation by lipopolysaccharide in a dose-related manner in vitro, and the dose that inhibits endotoxic complement activation by 50% was almost the same for hydrocortisone, prednisolone, and dexamethasone. We conclude that lipopolysaccharide activates complement in a dose-related manner and that glucocorticoids inhibit this activation in a dose-related manner, and this may be the main antiendotoxic property of glucocorticoids.

Neuroendocrine carcinoma (trabecular carcinoma) of the skin with ectopic ACTH production.

A case of ACTH-producing cutaneous neuroendocrine carcinoma occurring in the cheek of an 80-year-old woman is reported. The tumor was composed of undifferentiated cells which had a uniform round nucleus and scanty cytoplasm containing argyrophil granules. An ultrastructural study demonstrated neurosecretory-type granules in the cytoplasm of the tumor cells. Radioimmunoassay revealed an elevated level of ACTH in tumor tissue as well as in the plasma of the patient. This is believed to be the first documented case of ACTH-producing neuroendocrine carcinoma (trabecular carcinoma) of the skin.

Ovarian carcinoid composed of argyrophil and argentaffin cells.

A rare case of ovarian carcinoid tumor composed of both argyrophil and argentaffin cells is reported. The tumor cells were arranged in solid-trabecular or glandular fashion. Electron microscopically, each of the argyrophil and argentaffin tumor cells had intracytoplasmic neurosecretory granules possessing their own characteristics. Immunoflourescent and immunohistological procedures failed to demonstrate thyroglobulin, calcitonin, and CEA in the tumor. No structures suggestive of a struma ovarii were identifiable. Scattered in the tumor were mucinous cysts with APUD cells lying between the lining epithelial cells. These APUD cells are thought to be the origin of this pure ovarian carcinoid tumor.

An unusual case of localized multiple peptic ulcer of the gastric fundus producing a large exophytic fibrous pseudotumor.

We report an unusual case of fundic ulcer of the stomach, suspected radiologically of being an intramural malignant tumor. Grossly the lesion was a protruding large fibrous pseudotumor, reminiscent of fibromatosis or of a granuloma with secondary ulceration. Microscopical examination revealed that it was an unusually voluminous fibrosis produced in the base of long-standing multiple peptic ulcers in a limited area of the gastric fundus. The fundic gland area is a rare site of peptic ulceration, and the occurrence of such an exophytic tumor-forming type of peptic ulcer in this area even rarer, only one relevant case having been reported before.

[Histopathological findings of the ossification of the posterior longitudinal ligament of the cervical spine and their significance (author's transl)].

Characteristics features of the histopathological findings of the ossification of the posterior longitudinal ligament (OPLL) of the cervical spine are as follows: 1. In the segmental type of OPLL, the ossification of fibers usually initiates near the posterior border of the vertebral body and in those fibers which attach to the edge of the vertebral body. 2. At the axial ossifying point, degeneration of fibers occurs and enchondral ossification process follows, while in other part of the ossifying area, different types of ossification processes may also occur. 3. In association with OPLL, especially in the hyperostotic type, other paraspinal soft tissues may exhibit proliferation of the cartilage cells with or without ossification, implying the presence of ossifying tendency of the whole body. Of these tissues, the periosteum, the annulus fibrosus and/or the dura mater demonstrate proliferating changes in direct contact with OPLL, and the involvement of the periosteum precedes the other changes including OPLL, suggesting some influence from the vertebral side over the ossifying process of the paraspinal soft tissues.
