[Adjuvant therapy in resectable non-small cell lung cancer].

In order to improve the management of lung cancer at various stages, we analyzed results of treatment in 928 of 1024 patients who were registered at our Hospital Tumor Registry of 1952-1983 with a pathological diagnosis of TNM for carcinoma of the lung after pulmonary resection. The 5-year-survival rate was 43% in 928 patients excluded the cases who were lost follow-up or succumbed within post-operative 1 month. The 5-year-survival rate was 77% for the stage I, 54.7% for the stage II, 17% for the stage III and 4% for the stage IV. The 5-year-survival rate by therapeutic modality was as follows: 52% for the group with chemotherapy, 35% for the one without adjuvant therapies, 29% for the one with irradiation and 15% for the one with radiochemotherapy. Patients with adenocarcinoma who underwent curative surgery showed improvement of survival by postoperative chemotherapy. No increase in survival time was noticed in the irradiated group with N2.

Varying expression of opioid receptors and GTP-binding proteins in human pheochromocytomas.

The expression of opioid receptors and GTP-binding proteins was studied in 14 pheochromocytomas. The amounts of [3H]diprenorphine bound to membranes varied from 13 to 62 fmole/mg protein, but significantly higher in adrenaline-secreting tumors than in noradrenaline-secreting tumors. None of [3H]DADLE, [125I]beta-endorphin or [3H]ethylketocyclazocine binding was correlated with [3H]diprenorphine binding. Gpp(NH)p inhibition of [3H]DADLE binding was evident in all four normal human adrenal medullae but in only 8 out of 14 pheochromocytomas. The extent of Gpp(NH)p inhibition was not correlated with the amount of pertussis toxin (PT)-sensitive GTP-binding proteins as measured by PT-catalyzed [32P]ADP-ribosylation. The present findings suggest that opioid receptors and PT-sensitive GTP-binding proteins are variously expressed in transformed chromaffin cells, pheochromocytoma.

Effects of diltiazem hydrochloride in diabetics.

The effects of diltiazem hydrochloride on insulin secretion and glucose tolerance were studied in diabetic subjects treated with dietary management alone for their glycemic control. Sixteen diabetic subjects with heart complaints and/or abnormal electrocardiographic findings which were compatible with diagnosis of ischemic heart disease were given diltiazem hydrochloride 90 mg daily for a mean of 9.3 +/- 1.0 (SE) months and oral 100 g glucose tolerance tests (o-GTT) were performed before and after the administration of diltiazem hydrochloride. As a control group, 14 diabetic patients with roughly the same level of fasting blood glucose and the same degree of glucose intolerance were selected, and in these patients oral 100 g glucose tolerance tests were also performed before and after a mean follow up period of 12.6 +/- 1.5 months. Although improvement in glucose tolerance was observed at the end of the trial in the group of patients with diltiazem hydrochloride, control group also showed improved glucose tolerance and there was no significant difference in the change of glucose tolerance between both groups. No significant change of IRI response in oral 100 g glucose tolerance test was observed in both groups. We could, therefore, conclude from these observations that diltiazem hydrochloride with usual clinical dosage dose not exert unfavourable effects on glucose tolerance and insulin secretion in the mild diabetic patients.

[Study on extensive lymph node dissection: metastatic lymph nodes in papillary carcinoma of the thyroid].

Bilateral cervical lymph node dissection was performed in 71 cases of papillary thyroid carcinoma, considered to be relatively early cases because of mobility, irrespective of the size of tumor or presence of node enlargement. Of these, 33 cases received additional node dissection of the anterosuperior mediastinum through longitudinal sternotomy. The number of lymph nodes examined per subject averaged 89.9, the number of metastatic nodes was 13.8, and metastasis was noted in 88.7% of all cases. Lymph node metastasis tended to be more frequent on the affected side, but was simultaneously scattered over the whole cervical area. As to sites, metastasis of paratracheal nodes on the affected side occurred at a frequency of 66.2%, inferior and superior jugular nodes at 62.0% and 59.0% respectively, pretracheal nodes at 50.7%, and tracheoesophageal nodes at 47.9%. The high incidence of para- and pretracheal nodes suggests that the lymph flow in this direction is of great importance in metastasis. In fact, lymph nodes in the mediastinum, which were directly continuous with these nodes, showed as high as 39.4% metastasis in cases of anterosuperior mediastinal extirpation. This extensive node dissection is considered to be very preferable as at least the agony of survival with carcinoma can be lessened.

[Results and prevention of complications in bronchoplastic surgery in lung cancer].

Fifty-eight patients have undergone bronchoplastic surgery for lung cancer and were reviewed in an attempt to evaluate its postoperative survival rate and to establish a certain method to prevent postoperative complications. Five-year survival rates of the patients with the reconstructive surgery were 36.9, 66.6, 7.9 per cent in over-all patients, stage I and II patients, and stage III patients, respectively. These results can compare favorable with the results obtained in patients who underwent standard operation for lung cancer. Seven of 9 patients with sleeve lobectomy and pulmonary artery reconstruction died within 2 years and 7 months postoperatively, and 4 of them showed distant metastasis. This type of operation may be alternative to pneumonectomy only when pneumonectomy would not be tolerated because of low cardiopulmonary reserve. Six of 8 patients died within 3 years after carina reconstruction. In this type of operation 4 cases showed tumor relapse around anastomotic site. Including 4 patients with carina reconstruction, 8 cases revealed postoperative local tumor recurrence despite the tumor free bronchial stump by frozen sections. These results indicate that prophylactic radiation therapy may be necessary to prevent local tumor relapse after the reconstructive surgery for the advanced lung cancer patients.

FAD-dependent malate dehydrogenase from Mycobacterium smegmatis: activation of the lipid-depleted enzyme by incorporation into cardiolipin liposome.

The lipid-depleted, enzymatically inactive malate dehydrogenase isolated from Mycobacterium smegmatis membrane was found to be incorporated spontaneously into cardiolipin liposome, but not into phosphatidylcholine liposome, as was revealed by electron spin resonance spectra with the use of 5-doxylstearic acid as a spin probe in the phospholipid liposomes. In addition, sucrose density gradient centrifugation in 0.5 M KCl showed hydrophobic interaction of the enzyme with cardiolipin liposome and further proved that the enzyme thus interacted hydrophobically with cardiolipin liposome became enzymatically active. From the results obtained above, it was concluded that the lipid-depleted, enzymatically inactive malate dehydrogenase isolated from M. smegmatis membrane was found to be activated by incorporating into the hydrophobic region of cardiolipin liposome.

Comparison of kinetic study of the photochemical changes of (ZZ)-bilirubin IX alpha bound to human serum albumin with that bound to rat serum albumin.

It has been stated by McDonagh, Palma & Lightner [(1982) J. Am. Chem. Soc. 104, 6867-6871] that complexing of bilirubin with serum albumin has a marked species-dependent influence on bilirubin photoisomerization in vitro and in vivo. Therefore the kinetics for the quantitatively important reaction: (Formula: see text) of the photochemical interconversion between bilirubin and its photoisomers bound to human or rat serum albumin in aqueous solution, assayed by h.p.l.c., was used to elucidate the observed species-dependent difference. The relative rate constants for bilirubin bound to human serum albumin, except for k4, the rate of interconversion from (ZZ)-bilirubin into (EZ)-bilirubin, proved to be considerably larger than those for bilirubin bound to rat serum albumin. In accordance with these rate constants, the formation of photoisomers of bilirubin bound to human serum albumin, except for (EZ)-bilirubin, is very rapid and much greater than that for bilirubin bound to rat serum albumin.

Improvement of dissolution and suppository release characteristics of flurbiprofen by inclusion complexation with heptakis(2,6-di-O-methyl)-beta-cyclodextrin.

The inclusion behavior of methylated beta-cyclodextrins, heptakis(2,6-di-O-methyl)-beta-cyclodextrin and heptakis-(2,3,6-tri-O-methyl)-beta-cyclodextrin in solution and the solid state was compared with that of natural beta-cyclodextrin using an anti-inflammatory drug, flurbiprofen, as a guest molecule. Stability constants were determined by the solubility method at various temperatures, and the thermodynamic parameters were calculated for inclusion complex formation in aqueous solution. The solid complexes were obtained in a molar ratio of 1:1, and their dissolution behavior and release from suppository bases were examined. The data suggest that the inclusion mode of the complex with 3 is somewhat different from that of the complexes with 1 and 2. From a practical point of view, 2 seems to be particularly useful for improving the pharmaceutical properties of flurbiprofen in various dosage forms.

Suppression of IgE antibody production after exposure to ozone in mice.

The effect of ozone exposure on IgE antibody production with aerosolized ovalbumin (OA) administration was investigated in Balb/c mice. Mice were continuously exposed to 0.8 ppm ozone for 1, 2, or 4 weeks, respectively, and subsequently aerosolized OA was administered through the respiratory tract for 6 min with a nebulizer. The mice were then immunized intraperitoneally 1 week later with OA. IgE antibody production was suppressed in ozone-exposed mice. However, no significant difference in the primary IgE antibody production by intraperitoneal immunization alone with OA was observed between ozone-exposed mice and nonexposed mice. In order to elucidate the suppressive mechanism of IgE antibody production, hapten-carrier antigenic system was used. It is shown that the induction of helper T cell function was suppressed if the aerosolized carrier protein was administered before intraperitoneal immunization in the mice exposed to ozone. These results suggest that ozone exposure has the effect on the stage of administration of inhaled antigen and the quite insignificant effect on the IgE antibody production after intraperitoneal immunization with OA.
