[Clinical studies against chronic prostatitis and prostatitis-like syndrome (2). Evaluation of therapeutic effect].

We treated 229 cases of chronic prostatitis (bacterial in 29, non-bacterial in 200) and 238 cases of prostatitis-like syndrome and evaluated the clinical efficacy. Clinical efficacy was seen in 77.3% of the chronic prostatitis cases (82.8% bacterial, 76.5% non-bacterial) and 53.8% of the prostatitis-like syndrome cases, with a statistical difference between these two types of disease (p less than 0.01). Treatment included chemotherapy, anti-inflammation agents and minor tranquilizers. None of them were effective against the prostatitis-like syndrome. On the other hand, combination usage (chemotherapies and anti-inflammation agent) was satisfactory on chronic bacterial prostatitis. Also on chronic non-bacterial prostatitis, chemotherapy was significantly effective (p less than 0.05%). Chemotherapeutic effectiveness against chronic prostatitis (both bacterial and non-bacterial) was high with sulfamethoxazole-trimethoprim, tetracyclines, cephalosporins, nalidixic acid and penicillins in this order. There was no difference among these drugs. Prostatic massage was generally effective especially for chronic non-bacterial prostatitis (p less than 0.05) and prostatitis-like syndrome (p less than 0.01). Light psychological treatment was significantly effective toward some cases of prostatitis-like syndrome with strong psychosomatic factors (p less than 0.01).

[Reevaluation of clinical efficacy of piperacillin against complicated urinary tract infections].

Forty-six inpatients with complicated urinary tract infections (UTI) were treated with 2 g piperacillin (PIPC) twice a day through drip intravenous infusion. After 5 days, evaluation was made using the criteria proposed by the UTI Committee in Japan. The overall clinical efficacy was rated "excellent" in 4 (8.9%), "moderate" in 22 (47.8%), and "poor" in 20 (43.5%) cases with a total efficacy rate of 56.5% (26/46). We previously presented the clinical efficacy of PIPC in 74 cases with complicated UTI. Herein, a total of 120 cases were analyzed and these cases were divided into two groups. The overall clinical efficacy, bacteriological response, type of infection, background of patients and its safety were evaluated. Almost no change was seen for four years. Also this study revealed the usefulness against Enterococcus infections. In conclusion, PIPC is still a useful antimicrobial agent against complicated UTI even after the appearance of many new Cephems.

[Clinical studies against chronic prostatitis and prostatitis-like syndrome (1). Clinico-statistical observation].

We clinically evaluated 605 cases of chronic prostatis and other resembling disorders. The incidence was 4.34% of all out-patients (7.54% of male patients). The average age was 38.1 years, and many were adolescents. A greater number of out-patients were seen in October, November, and December (p less than 0.01) and June (p less than 0.05). The type of disease was chronic prostatitis in 48.1% (bacterial in 5.3%, non-bacterial in 42.8%) and prostatitis-like syndrome in 51.9%. The incidence among male patients was 3.63% for chronic prostatitis (bacterial in 0.40%, non-bacterial in 3.23%), and 3.91% for prostatitis-like syndrome. In the case of chronic prostatis, the abnormal finding rate (p less than 0.01), tenderness (p less than 0.01), induration (p less than 0.05) on prostatic palpation, non-pretreated rate (p less than 0.01), urological past history (p less than 0.05) except prostatitis and urological complications (p less than 0.01) were significantly higher. On the other hand, the negative rate on urine culture (p less than 0.01), no abnormal findings on palpation (p less than 0.01), discomfort (p less than 0.01), discomfort with tenderness (p less than 0.01) and past prostatitis episodes (p less than 0.01) were seen significantly more in the case of prostatitis-like syndrome. In conclusion, chronic prostatitis with inflammatory change on prostate and prostatitis-like syndrome without inflammation have similar clinical features, but should be diagnosed as totally different types of disease.

Purification and characterization of the spoOA protein of Bacillus subtilis from an overproducing strain of Escherichia coli.

The spoOA gene of Bacillus subtilis is essential for the earliest stage of sporulation. To purify and characterize the product of the spoOA gene, we constructed a fusion plasmid in which the spoOA coding region was placed under the control of the Ptac promoter. When expression of the spoOA gene was induced in Escherichia coli cells by derepression of Ptac, the SpoOA protein constituted 15% of total cellular protein. The SpoOA protein was purified to homogeneity from these cells. We found that the NH2-terminal amino acid sequence of the purified protein was essentially the same as that of the SpoOA protein (spoOA-cat protein) from B. subtilis, and that the NH2-terminal methionine of the SpoOA protein from E. coli was formylated presumably because of insufficient amounts of the deformylating enzyme. The T signal [Ganoza, M. C., Marliere, P., Kofoid, E. C. and Louis, B. G. (1985) Proc. Natl Acad. Sci. USA 82, 4587-4591], in addition to the Shine-Dalgarno signal to determine the initiation codon of the spoOA gene, is considered to function in E. coli as well as in B. subtilis. We also found that the purified SpoOA protein had a DNA-binding activity. It was preferentially bound to the 175-bp BclI fragment of phi 105 DNA, and was released in the presence of 0.3 M KCl.

Dermatoglyphic development on the volar pads of rats with chromosome abnormalities.

Dermatoglyphic abnormalities are often observed in patients with chromosome aberrations, but no similar observations have been made in animals. In the present study, palmar dermatoglyphics were examined in 4 rats with chromosome anomalies. Reciprocal translocations were induced by gamma-irradiation; the animals used were obtained from among offspring with abnormal karyotypes that were derived from the original mutant rats. As the epidermal surface of the volar pad of the rat is flat, dermatoglyphic characteristics were observed on the dermal surface following staining with toluidine blue. Unusual ridge configurations were found in some of them, suggesting that dermatoglyphic development in the rat reflects, to some extent, an abnormal chromosome constitution.

Lack of association and linkage between HLA and familial polyposis coli.

We investigated possible association of and linkage between HLA and familial polyposis coli (FPC). In 182 individuals from 66 pedigrees of FPC and 108 individuals from a normal population, HLA-A, -B, and -C antigens were determined. When the frequencies of HLA antigens in 66 unrelated patients and in normal controls were compared, no association of FPC with HLA was observed. For the linkage analysis, HLA haplotypes of 17 affected sib pairs were investigated by the affected sib pair method. The number of pairs which shared two, one, and no haplotypes identical by descent was not significantly different from the number expected with random occurrence (P greater than 0.95). Finally, seven families were analyzed using Morton's sequential test. A maximum lod score of -0.056 at a recombination fraction of 0.4, and a lod of -3.089 at a recombination fraction of 0.05 were obtained. Therefore, there is neither an association of nor linkage between FPC and HLA.

Localization at a subband level of polymorphic 13q14 DNA probes for diagnosis of hereditary retinoblastoma and Wilson disease.

Two single-copy DNA sequences, pG24E6.8 (D13S21) detecting a low-frequency MspI RFLP and pG14E1.9 (D13S22) detecting a high-frequency Dra I RFLP, have been isolated and cloned from a human chromosome 13-specific phage library and localized at 13q14. Their subband localization was described using a panel of cell lines from patients with different chromosome 13 deletions. A quantitative analysis of hybridization signals was carried out, taking for reference a single-copy DNA sequence from another chromosome. D13S21 and D13S22 were both assigned to q14.1-14.2, which also harbors the genes responsible for retinoblastoma and Wilson disease. The Dra I polymorphism detected by pG14E1.9 is a very suitable one for linkage studies in families with either disease.

Activity of red cell esterase D in 50 cases of retinoblastoma patients and their family members.

Esterase D was qualitatively and quantitatively analyzed in the red blood cells from 50 retinoblastoma patients who had been followed-up at Juntendo University and their family. In only one case with deletion of chromosome No. 13 (q12.3-q21.2), was the esterase D activity (type 2) 1.47 Unit/gHb which was approximately 50% of the level in normals. Additional retinoblastoma patients and their family members, 77 parents and 25 siblings, showed a normal range of esterase D level in their own phenotypes. These facts confirmed that the deletion of an esterase D locus was infrequent (approximately 2% in our series) in retinoblastoma patients. It was also compatible with the published data that the range of deletion of chromosome was No. 13 (q12.3-q21.2) involving 13q14.

An alpha-amylase inhibitor from cranberry bean (Phaseolus vulgaris): its specificity in inhibition of mammalian pancreatic alpha-amylases and formation of a complex with the porcine enzyme.

A proteinaceous inhibitor that inhibits mammalian alpha-amylases was prepared from cranberry bean and examined for its reactivity with alpha-amylases from various origins. The cranberry bean alpha-amylase inhibitor (CBAI) exhibited inhibitory effects on pancreatic alpha-amylases from the following mammals: pig, dog, cat, horse, sheep, cow, rabbit, guinea pig, rat, and mouse. CBAI showed a maximal inhibition at pH 5.5 against porcine pancreatic alpha-amylase (PPA). It was confirmed by gel filtration that a complex was formed in the 1:1 ratio between CBAI and PPA when they were incubated at 37 degrees C for 30 min at pH 5.5. A similar inhibition pattern was also observed at pH 6.9 that is optimal for the amylase reaction, but much higher concentrations of CABI were required to give 50% inhibition at pH 6.9 than at pH 5.5. Especially, both bovine and rat alpha-amylases were virtually unreactive to CBAI at pH 6.9.

Modification of the cysteamine side chain of thienamycin. III.

Thienamycin derivatives (4) having a cyclic amidine moiety at the C-2 position were prepared. The susceptibility to renal dehydropeptidase-1 and the antimicrobial activity of these compounds were determined. Their structure-activity relationships are also discussed.

Modification of the cysteamine side chain of thienamycin. II.

A new type of thienamycin derivatives (3a-3j, 4a, 4b), having a monothioacetal or a thioacetal side chain at the C-2 position was prepared, and the susceptibility to renal dehydropeptidase-1 (DHP-1) and the antimicrobial activity of these compounds were determined. The structure-activity relationships of these derivatives are also discussed.

[The analysis of prognostic factors on postsurgical pyuria of benign prostatic hypertrophy].

The outcome of postsurgical pyuria in benign prostatic hypertrophy was studied in 87 patients, and the factors that might affect the outcome were determined. No significant differences were found between operation method and duration until normalization of pyuria, which was 75.5 +/- 46.0 days for transurethral resection of the prostate, 72.7 +/- 30.6 days for suprapubic prostatectomy and 69.3 +/- 32.7 days for retropubic prostatectomy. Prognostic factors were statistically analyzed preoperatively, at operation, and postoperatively. The definite prognostic factors were preoperative diabetes mellitus, preoperative pyuria, preoperative bacteriuria, and postoperative hypoproteinemia. The probable prognostic factors were old-age, preoperative indwelling catheters, heavy prostate tissue, postoperative bacteriuria, postoperative anemia and postoperative complications.

Loss of genes on chromosome 22 in medullary thyroid carcinoma and pheochromocytoma.

Using polymorphic DNA markers, we compared the constitutional and tumor genotypes of patients with multiple endocrine neoplasia type 2A (MEN2 A). We found loss of constitutional heterozygosity at the D22S9 locus in one out of 9 medullary thyroid carcinomas (MTCs). No loss of heterozygosity was detected at 12 other loci in any of the MTCs tested. Loss of heterozygosity at D22S9 and/or D22S1 was also demonstrated in 2 out of 5 pheochromocytomas tested. These results suggest that loss or mutation of a gene on chromosome 22 may play an important role in tumorigenesis in MEN2A.
