Acute interstitial pneumonia induced by ONO-1078 (pranlukast), a leukotriene receptor antagonist.

A 62-year-old woman treated with pranlukast for 2 months developed interstitial pneumonitis with a high fever. A lymphocyte stimulation test was reactive to pranlukast. Her clinical symptoms improved with discontinuation of pranlukast and administration of systemic corticosteroid. To our knowledge, this is the first reported case of drug-induced lung disease involving a leukotriene. The steps that can be taken to promptly reach a diagnosis and to successfully treat this life-threatening condition are described.

Epstein-Barr virus-associated gastric cancer in a patient with dermatomyositis.

A 59-year-old man was admitted presenting systemic rash and muscle weakness. He was diagnosed to have dermatomyositis and a check was made for internal malignancy. Gastrointestinal endoscopy revealed a Borrmann type II tumor on the middle body of the stomach. Biopsy specimens showed a well differentiated adenocarcinoma, and total gastrectomy was performed. The final diagnosis was moderately differentiated adenocarcinoma invading into the proper muscular layer, with metastases to regional lymph nodes. Most of the neoplastic cells were shown to be positive for Epstein-Barr virus by means of EBV-encoded RNA in situ hybridization. The symptoms of dermatomyositis disappeared completely after surgery.

[Case of protein-losing gastroenteropathy associated with scleroderma in which central serous chorioretinopathy developed].

A 44-year-old woman noticed edema of the lower limbs in May 1999 and visited our hospital in September 1999 to undergo further examination. On admission, severe hypoalbuminemia (1.9 g/dl) was detected with a negative urinary protein level. Protein leakage into the gastrointestinal tract and deposition of immune complex in the colonic mucosa were shown by the fluorescent antibody method. In addition, anti-centromere antibody, sclerodactyly, and findings indicative of histological sclerotic changes on a skin biopsy were observed. These findings supported a diagnosis of protein-losing gastroenteropathy complicated by scleroderma. Administration of oral corticosteroids was begun one month after admission and the patient experienced diminished visual acuity immediately after steroid pulse therapy in November. Central serous chorioretinopathy (CSC) was diagnosed at the Department of Ophthalmology of our hospital, and the administration of corticosteroids was suspected as a possible cause of CSC. Considering the severity of hypoproteinemia, the corticosteroid treatment was continued despite corticosteroids being strogly suspected as the primary cause of CSC. A complete disappearance of CSC was achieved in 30 days after the onset of symptoms despite continuation of the steroid therapy, and her serum albumin and complement levels both normalized. We concluded that damage to the retinal pigment epithelium secondary to the vascular lesion at the choroidal level plays a causative role in CSC. In the present case, the findings suggested that the deposition of immune complex in choroidal tissues as well as the gastrointestinal tract caused hyperpermeability of choroidal vessels and led to the development of CSC.

Gas-filled intradural cyst of the lumbar spine. Case report.

The authors describe the case of a gas-filled intradural cyst of the lumbar spine that caused radicular pain. The cyst was similar to an intervertebral vacuum disc phenomenon and the cystic gas collection resembled the features of gas-containing pseudocyst, herniated intradiscal gas, or free gas, all of which have been found in the epidural space.

[Murine IgE antibody to recognize human major allergen, Mal f4].

To elucidate the specificity of murine IgE antibody against Malassezia furfur, the immunoblot patterns of IgE in sera obtained from mice inoculated repeatedly in the nasal cavity with M. f. cells, or injected intraperitoneally with M. f. cells mixed with Al (OH)3 gel were compared with those of IgE antibody detected in sera from patients with AD. Most of the murine IgE anti-Malassezia antibodies shared some antigenic bands with IgE in sera from patients with AD. The IgE antibody of murine also recognized Mal f4, one of the major allergens detected in patients with AD.

[Two cases of canine histoplasmosis in Japan].

Histoplasmosis is distributed in tropical, subtropical and temperate zones of the world. The disease is one of the imported mycoses in Japan. To date, although more than 30 human and one canine case of histoplasmosis have been reported in Japan, some including that of the canine might have been infected domestically, since the patients have no history of going abroad. The pathogen of histoplasmosis is thus believed to be present in our country. We examined skin biopsies from two dogs in Tokyo and Kumamoto, and found fungal elements 1-2 or 2-4 microEm in diameter in the macrophages. The homology of DNA sequences for the ITS rRNA gene were correspondent to Ajellomyces capsulatus at a rate of more than 97.4%. Therefore, the two dogs were diagnosed as having been infected with Histoplasma capsulatum which is the anamorph of A. capsulatus. Since the dogs had no history of having been outside Japan and had not been brought from an endemic area, they might have been infected domestically. Further epidemiological surveys on canine histoplasmosis may be able to estimate autochthonous human cases in Japan.

Germline mutation of the PTEN gene in a Japanese patient with Cowden's disease.

Cowden's disease (CD) is an autosomal dominant disorder which confers a high susceptibility to diverse benign and malignant tumors. The PTEN (phosphatase and tensin homologue deleted in chromosome ten) gene has been identified as a tumor suppressor gene responsible for cancers of the endometrium, ovary, prostate, and glioblastomas. Recently, germline mutations of this gene were also found in patients with CD, and it is now recognized as a gene responsible for this disease. We identified a germline nonsense mutation at codon 130 in exon 5 of PTEN in a 56-year-old Japanese woman with CD. The patient had adenoid facies and mucocutaneous lesions including multiple facial papules, acral keratoses on neck and shoulders, palmoplantar keratoses, multiple oral papillomas, scrotal tongue, mucosal and cutaneous hemangiomas, and a sclerotic fibroma on the arm. She also had benign and malignant polypoid neoplasms throughout the entire digestive tract, including adenocarcinoma of the colon and submucosal lipomas of the rectum, as well as bilateral breast carcinomas, multinodular goiters, an ovarian cyst with a fibroma-like nodule, hepatic hemangiomas, and abdominal hernia. We searched CD cases with the same genotypic PTEN mutation as the present case and compared their phenotypes. Further studies will disclose a better understanding of the role of mutation in the PTEN gene in the course of tumorigenesis of both benign and malignant tumors developed in patients with CD.

A comparison of the effects of 3-hydroxy-3-methylglutaryl-coenzyme a (HMG-CoA) reductase inhibitors on the CYP3A4-dependent oxidation of mexazolam in vitro.

HMG-CoA reductase inhibitors can be divided into two groups: those administered as the prodrug, i.e., the lactone form (e.g., simvastatin and lovastatin), and those administered in the active form, i.e., the acid form (e.g., pravastatin, fluvastatin, atorvastatin, and cerivastatin). In this study, the influence of the lactone and acid forms of various HMG-CoA reductase inhibitors on metabolism by CYP3A4, a major cytochrome P450 isoform in human liver, was investigated by determining the in vitro inhibition constant (K(i) value) using an antianxiety agent, mexazolam, as a probe substrate. In human liver microsomes, all the lactone forms tested inhibited the oxidative metabolism of mexazolam more strongly than did the acid forms, which have lower partition coefficient (logD(7.0)) values. In addition, the degree of inhibition of mexazolam metabolism tended to increase with an increasing logD(7.0) value of the HMG-CoA reductase inhibitors among the lactone and acid forms. In particular, pravastatin (acid form), which has the lowest logD(7.0) value, failed to inhibit CYP3A4 activity. Taking account of the lipophilicity of the inhibitors, in conjunction with the CYP3A4-inhibitory activity, could be very useful in predicting drug interactions between substrates of CYP3A4 and HMG-CoA reductase inhibitors.

Fipronil modulation of gamma-aminobutyric acid(A) receptors in rat dorsal root ganglion neurons.

The gamma-aminobutyric acid (GABA) receptor is an important site of action of a variety of chemicals, including barbiturates, benzodiazepines, picrotoxin, bicuculline, general anesthetics, alcohols, and certain insecticides. Fipronil is the first phenylpyrazole insecticide introduced for pest control. It is effective against some insects that have become resistant to the existing insecticides. To elucidate the mechanism of fipronil interaction with the mammalian GABA system, whole-cell patch-clamp experiments were performed using rat dorsal root ganglion neurons in primary culture. Fipronil suppressed the GABA-induced whole-cell currents reversibly in both closed and activated states. The IC(50) values and Hill coefficients for fipronil block of the GABA(A) receptor were estimated to be 1.66 +/- 0.18 microM and 1.23 +/- 0.14 for the closed receptor, respectively, and 1.61 +/- 0.14 microM and 0.96 +/- 0.06 for the activated receptor, respectively. The association rate and dissociation rate constants of fipronil effect were estimated to be 673 +/- 220 M(-1) s(-1) and 0.018 +/- 0.0035 s(-1) for the closed GABA(A) receptor, respectively, and 6600 +/- 380 M(-1) s(-1) and 0.11 +/- 0.0054 s(-1) for the activated GABA(A) receptor, respectively. Thus, both the association and dissociation rate constants of fipronil for the activated GABA(A) receptor are approximately 10 times as large as those for the closed receptor. Experiments with coapplication of fipronil and picrotoxinin indicated that they did not compete for the same binding site to block the receptor. It is concluded that although fipronil binds to the GABA(A) receptor without activation, channel opening facilitates fipronil binding to and unbinding from the receptor.

[Carinal resection of bronchogenic carcinoma].

Carinal resection was performed in 10 cases of bronchogenic carcinoma during 12 year-period. The mean age of patients was 58 years, with a range of 42 to 68 years. There were 7 male and 3 female. The tumor was located on the right side in 7 cases, on the left side in 1 case on the carina in 2 cases. The histological examination showed squamous cell carcinoma in 5 cases, adenocarcinoma in 4 cases and adenoid cystic carcinoma in 1 case. The staging revealed T3N2 M0 Stage IIIA in 2 cases, T4N0M0 Stage IIIB in 1 case, T4N1M0 Stage IIIB in 1 case, T4N2M0 Stage IIIB in 5 cases and T4N3M0 Stage IIIB in 1 case. The surgical methods were as follows; sleeve pneumonectomy in 5 cases, wedge carinal resection with pulmonary resection in 3 cases (right sleeve upper lobectomy in 2 cases and right pneumonectomy in 1 case), carinal resection in 2 cases. The site of bronchial anastomosis was overlapped by thymus in 6 cases. The 30-day mortality rate in tracheo-carinal resection was 10% (one patient). Eight patients died and remaining 2 patients are still alive without any evidence of recurrence. 5-year survival rate was 36%. These outcomes were almost equal to those of surgical case in the same stage.

[The ratios of apoC-III/apoC-II in normal full-term maternal bloods and their umbilical cord bloods].

In this paper, serum concentrations of CII and CIII in MB(n = 12) represented 158% and 237% of those in 20 specimens of normolipidemic venous blood from healthy fasting non-pregnant females(Control), and the CIII/CII ratio of MB(mean +/- SD = 3.6 +/- 0.4) was significantly higher than that of the Control(2.38 +/- 0.42). While the difference between the serum CII level in MB and that of 14 specimens of postmenopausal patients lipid-matched to MB(postM HL) was not significant, serum CIII concentration of MB was significantly higher than that of postM HL. Furthermore, while the difference between the CIII level in apoB-containing lipoproteins of MB and that of postM HL was not significant, the CIII concentration in HDL(HDL-CIII) of MB was significantly higher than that of postM HL. It was suggested that the effect of pregnancy on apoC levels which change in parallel with the TGs level might reflect the selective increase in HDL-CIII concentration. In addition, the positive correlation between HDL-CIII and apoA-I concentration in MB was significant. However, it has been reported that the CIII/CII ratio of patients with type III hyperlipidemia treated with estrogens had been successively increased, and it was also reported that estrogens stimulate the synthesis of CIII in hepatocytes. It was thus suggested that maternal CIII as well as apoA-I might be selectively overproduced in the liver by stimulation from increased endogenous estrogens during normal pregnancy. On the contrary, the serum concentrations of CII and CIII in UCB represented 71% and 55% of those in Control, and the CIII/CII ratio of UCB(1.87 +/- 0.5) was significantly lower than those of the Control. In UCB, the positive correlation between CIII and TGs was weak(p < 0.1) and CII did not correlate with markedly decreased TGs. Serum concentrations of CII, CIII and the ratio of CIII/CII in MB did not correlate with those in UCB, respectively. Moreover, birth weight and birth length of neonates positively correlated with concentrations of TGs, CII and CIII in MB, respectively, but not in UCB. In conclusion, the increased maternal CIII/CII ratio is due to the estrogen-induced raise of CIII while the reduced neonatal CIII/CII is related to the decrease in TGs. These observations suggest that fetal TGs-rich lipoprotein metabolism is not affected by maternal lipoprotein metabolism.

Expression of transduced HSP70 gene protects chondrocytes from stress.

OBJECTIVE: To investigate the efficacy of adenovirus vector mediated transduction of heat shock protein 70 (HSP70) gene to human chondrocyte-like cell (HCS-2/8) against heat stress. METHODS: Two adenovirus vectors that contain wild-type (AxSHEwt) or mutant-type (AxSHEmt) HSP70 gene, and that are regulated by SRalpha promoter, were constructed. The mutant-type lacks the area that expresses stress durability. One of the 2 adenovirus vectors was added to the cultures of human chondrocyte-like cells (HCS-2/8). Heat stress (48 degrees C) was applied to the transduced cells for 2 h, and the efficacy of adenovirus vector mediated transduction of HSP70 gene against heat stress in the chondrocytes was investigated using alamar blue assay and MTT assay. RESULTS: Absorbance levels at 48 degrees C were 300.3 +/- 51.9 and 1.173 +/- 0.011 in the controls, 278.5 + 33.8 and 1.217 +/- 0.018 in the AxSHEmt transduced cells, and 349 +/- 14.7 and 1.371 +/- 0.033 in the AxSHEwt transduced cells. The level in the AxSHEwt transduced cells was significantly higher than in the other 2 groups (p < 0.05). With 37 degrees C treatment, no significant difference was observed. CONCLUSION: Chondrocytes to which HSP70 gene was transduced had a significantly higher metabolic activity and viability under heat stress.

Correlation of early CT signs in the deep middle cerebral artery territories with angiographically confirmed site of arterial occlusion.

BACKGROUND AND PURPOSE: Early CT signs in the deep middle cerebral artery (MCA) territories have been reported to be seen at the initial period of ischemia. We attempted to investigate the incidence of parenchymal hypodensity within 3 hours after ischemic onset among patients with angiographically proved embolic MCA occlusion and to assess the correlation of subtle hypodensity in the deep MCA territories with involvement of the lenticulostriate arteries in the presence of ischemia. METHODS: Fifty CT images obtained within 3 hours after onset of embolic MCA occlusion were retrospectively reviewed by three neurosurgeons who were aware of clinical features. Early CT signs in the deep MCA territories were divided into three grades according to their anatomic location: grade I, normal basal ganglia with hypodensity localized to the insula; grade II, partial obscuration of the posterolateral part of the putamen; and grade III, hypodensity of the entire lentiform nucleus. A grade I CT sign was considered to be a negative finding for lenticulostriate artery involvement, whereas grade II and III CT signs were considered to be positive findings. Site of occlusion and involvement of the lenticulostriate arteries were confirmed by angiography. RESULTS: Thirty-eight (76%) of 50 patients had early CT signs in the deep MCA territories. Sensitivity and specificity of a grade I CT sign indicating absence of lenticulostriate artery involvement in ischemia were 65% and 87%, respectively. On the other hand, sensitivity and specificity of grade II and grade III CT signs for presence of lenticulostriate artery involvement in ischemia were 77% and 100%, respectively. Grade II CT signs resulted from various sites of occlusion, whereas grade III was unequivocally predictive of proximal occlusion to all of the lenticulostriate arteries. CONCLUSION: Involvement of the lenticulostriate arteries may be presumed by precise evaluation of subtle, CT-revealed hypodensity in the deep MCA territories, even within 3 hours of ischemic onset.

Traumatic rupture of the bronchus.

We experienced a case of incomplete traumatic rupture of the bronchus which was operated on two months after a traffic accident. Traumatic rupture of the bronchus is rare after blunt chest injury but it can have serious consequences. Early surgical repair is recommended without repeated attempts to remove granulation tissue if the initial attempt at non-surgical repair results in failure, even in the case of incomplete rupture.

[A case of recurrent gastric cancer successfully treated with TS-1].

A patient with recurrent gastric cancer which infiltrated the pelvic muscle after the treatment of paraaortic lymph node and ovarian metastases was successfully managed by a novel oral anticancer drug, TS-1. TS-1 was administered at a dose of 80 mg/day. One course consisted of two repetitions of consecutive administration of TS-1 for 14 days and withdrawal of TS-1 for 14 days. Adverse reactions were mild and the patient did not request hospitalization after two courses had been completed. Subjective symptoms such as difficulty in walking improved after one week and a partial response was obtained after 2 weeks of treatment. At the end of 4 courses we could remove an indwelling pyelocatheter for the ureter stricture. As of 14 months after the beginning of administration of TS-1, the patient is being treated as an outpatient and has attained a better QOL than before.

Human liver-specific organic anion transporter, LST-1, mediates uptake of pravastatin by human hepatocytes.

Involvement of LST-1 (a human liver-specific transporter, also called OATP2) as the major transporter in the uptake of pravastatin, a 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitor, by human liver was demonstrated. The hepatic uptake of pravastatin evaluated using human hepatocytes was Na(+)-independent and reached saturation with a Michaelis constant (K(m)) of 11.5 +/- 2.2 microM. The uptake of pravastatin was temperature-dependent and was inhibited by estradiol-17beta-D-glucuronide, taurocholic acid, bromosulfophthalein, and simvastatin acid, but not by p-aminohippurate. Estradiol-17beta-D-glucuronide competitively inhibited pravastatin uptake with an inhibition constant comparable to the K(m) value for estradiol-17beta-D-glucuronide transport, indicating that a common transporter mediates the transport of pravastatin and estradiol-17beta-D-glucuronide in human hepatocytes. The results obtained with human hepatocytes agreed with those obtained with LST-1 expressing Xenopus oocytes. Oocytes microinjected with human liver polyadenylated mRNA showed Na(+)-independent uptake of pravastatin and estradiol-17beta-D-glucuronide. A simultaneous injection of LST-1 antisense oligonucleotides completely abolished this uptake. Expression of LST-1 was immunohistochemically demonstrated in the human hepatocytes, but not in Hep G2 cells, which showed very low uptake of pravastatin. Therefore, LST-1 was regarded as a key molecule for pravastatin in liver-specific inhibition of cholesterol synthesis, making pravastatin accessible to the target enzyme, which would otherwise not be inhibited by this hydrophilic drug.

[Effect of caffeine on microcirculation of human ocular fundus].

PURPOSE: To determine the effect of caffeine on the microcirculation in the human ocular fundus. SUBJECTS AND METHODS: The microcirculation of the ocular fundus in 10 eyes of 10 healthy volunteers was studied using a laser speckle tissue circulation analyser. 100 mg of caffeine or placebo was given orally in a double-blind test. Square blur rate(SBR), a quantitative index of blood flow velocity, was measured in the temporal site of the optic nerve head(ONH) free of surface vessels and in the middle site of the choroid-retina between the ONH and the macula. Intraocular pressure(IOP), blood pressure(BP), pulse rate(PR), and central critical fusion frequency(CFF) were also measured. These parameters were measured before and for 2 hours after administration. The area under curve(AUC), of the SBR was calculated for each area. Ocular perfusion pressure(OPP) was also obtained from blood pressure and intraocular pressure. RESULTS: The time-course change of SBR value showed much individual difference. Caffeine decreased the AUC of the SBR in the ONH as well as in the choroid-retina significantly. IOP, mean BP, PR, OPP and central CFF did not change significantly. CONCLUSIONS: These results suggest that caffeine may increase blood vessel resistance and decrease blood flow in the ONH and choroid-retina in humans.
