Enhancement by recombinant human bone morphogenetic protein-2 of bone formation by means of porous hydroxyapatite in mandibular bone defects.

Hydroxyapatite is osteoconductive and can maintain an original biocompatible form. It is useful, in the reconstruction of bone defects, to enhance the osteoconduction of hydroxyapatite with an osteogenic protein. The aim of this study was to evaluate the bone formation in surgically created defects of rabbit mandibles by a combination of recombinant human bone morphogenetic protein-2 (rhBMP-2), with porous hydroxyapatite and atelopeptide type I collagen used as the carrier for rhBMP-2. A 10-microg rhBMP-2-implanted group (n = 15) and a control group (n = 15), in which only atelopeptide type I collagen and porous hydroxyapatite were implanted, were histologically examined 3, 7, and 21 days after implantation. The alkaline phosphatase activity was also quantitatively analyzed. No new bone formation was observed in either the tested or the control group after 3 days. At 7 days, immature bone tissue was observed in some pores of the rhBMP-2implanted group, while in the control group, immature mesenchymal cells were observed. At 21 days, trabecular bone lined some pore walls. In the central portion, the bone marrow, including angioid tissue, was observed. New trabecular bone formation was observed on portions of the external surface of the hydroxyapatite disk. On the other hand, the control group showed infiltration of immature mesenchymal cells into some pores. Marginal bone formation was found in the pores close to the surface of the disk which opposed mandibular bone. The control group showed a slow, small increase in alkaline phosphatase activity in this study, while the experimental group showed a marked increase at 21 days. This increase was significantly higher in the tested group than in the control group at both 7 and 21 days. The findings indicate that rhBMP-2 accelerated bone formation by osteoconduction from porous hydroxyapatite. The combination of rhBMP-2, atelopeptide type I collagen, and porous hydroxyapatite is suggested to be advantageous for clinical application in reconstructing mandibular bone defects.

The enhancing effect of excess retinol palmitate on induction of odontogenic tumors and inhibitory effect on squamous cell carcinoma of the gingiva in hamsters treated with N-methylnitrosourea.

The influence of excess retinol palmitate on induction of tumors in the oral region was examined histopathologically. Sixty-three weanling Syrian golden hamsters were divided into five groups and received either 0.2% N-methylnitrosourea (MNU) (1 mg/100 g body weight) or retinol palmitate (RP) (25,000 IU/100 g body weight) twice a week for 16 weeks, singly or in combination. Animals received RP intraperitoneally or intragastrically and then, 6 hours later, the animals received intragastric administration of MNU. To accelerate the cell activity of the incisal tooth buds, intentional disocclusion of the left upper and lower incisor of all hamsters was carried out by repeated cutting with cooled diamond disks to a level just above the inter-dental papilla twice a week for 12 weeks. The right incisors were left in occlusion. In all animals exposed to RP + MNU, while the induction of squamous cell carcinomas of the gingiva and forestomach were prevented, the notable findings were a significantly increased incidence of odontogenic tumors in cut incisal regions of the animals with intragastric administration of RP + MNU and an induction of maxillary neurogenic tumors. The incidence of MNU-induced disturbances in odontogenesis in the incisors was reduced but marked disturbances were increased. RP seemed to have opposite effects of prevention and enhancement for development of neoplastic changes in the oral region.

Morphologic changes in the perioral soft tissues in patients with mandibular hyperplasia using a laser system for three-dimensional surface measurement.

Soft tissue changes following correction of mandibular hyperplasia were studied using a laser system for three-dimensional (3-D) surface measurement. Twelve subjects (7 men and 5 women; age 17-48 years) were included in this study. Bilateral sagittal split ramus osteotomies were performed in ten patients and intraoral vertical ramus osteotomies in two patients. Pre- and postoperative images, obtained with the laser system, were superimposed to determine distribution of changes in the perioral soft tissue. Mean volume changes were 1032.3 mm3 in the Subnasale-UL area and 8700.9 mm3 in the LL-Menton area. Changes in both areas were symmetrical around the mid-sagittal plane in some patients and asymmetrical in others. This measurement system provided a simple method of determining 3-D changes in soft tissue following surgery and might be useful for clinical purposes.

Osteotomy site healing following mandibular sagittal split osteotomy and rigid fixation with polylactide biodegradable screws.

Follow-up of 47 patients, treated using mandibular bilateral sagittal split osteotomy and self-reinforced poly-L-lactide acid (SR-PLLA) screws for rigid internal fixation, is presented. The focus was on clinical and radiological osteotomy healing. The average follow-up time was 2.1 years (range 0.5-5 years). Clinical recovery and radiological osteotomy healing during follow-up were uneventful. Osteolytic changes were seen around the SR-PLLA screws in 27% of cases. The majority of the screw canals remained as radiolucent shadows without bony filling.

[A follow-up study on effects of ADL deterioration on QOL in patients with neurological intractable diseases].

OBJECTIVE: This follow-up study examined correlating factors, including change of activity of daily living (ADL), which are associated with subjective quality of life (QOL) in patients with neurological intractable diseases such as amyotrophic lateral sclerosis, spinocerebellar degeneration and Parkinson disease. METHODS: Baseline information of 159 patients with above mentioned three intractable diseases registered by two public health centers in Niigata and Fukuoka Prefectures was obtained in 1993-1994, and they were followed to 1995. At the end of follow-up, 124 subjects were interviewed by public health nurses using the 27 item-subjective QOL scale for patients with neurological intractable diseases, previously confirmed for reliability and validity. RESULTS: Low level of ADL at the time of registry and deterioration of ADL after the registry were independently related to low level of subjective QOL. CONCLUSIONS: It may be necessary to minimize the ADL deterioration, in order to maintain a high subjective QOL of patients with neurological intractable diseases.

[The characteristics of myocardial fatty acid metabolism in patients with left ventricular hypertrophy].

We evaluated the characteristics of myocardial fatty acid metabolism in patients with left ventricular hypertrophy (LVH). Myocardial imaging with 123I-beta-methyl iodophenyl pentadecanoic acid (BMIPP) was performed in 28 patients with hypertrophic cardiomyopathy (HCM), 15 patients with hypertensive heart disease (HHD), 13 patients with aortic stenosis (AS) and 8 normal controls (NC). The patients with HCM consisted of 13 patients of asymmetric septal hypertrophy (ASH), 7 patients of diffuse hypertrophy (Diffuse-HCM) and 8 patients of apical hypertrophy (APH). Planar and SPECT images of BMIPP were acquired 15 minutes and 4 hours after tracer injection. Resting 201Tl SPECT images and echocardiography were also performed on other days. We calculated heart/mediastinum count ratio and washout rate of BMIPP by using planar image. In patients with LVH, the incidence of reduced BMIPP uptake was more frequent than that of reduced 201Tl uptake. In delayed images, more than 60% of patients with LVH reduced BMIPP uptake, especially remarkable for patients with ASH and APH. The washout rate of all cardiac hypertrophic disorders was tended to be higher than that of normal subjects. Reduced BMIPP uptake was frequently found in septal portion of anterior and inferior wall in patients with ASH, in inferior wall in patients with Diffuse-HCM and HHD, in apex in patients with APH and AS. These results suggest that BMIPP scintigraphy can differentiate three types of cardiac hypertrophy.

Prediction of vertigo recurrences in Meniere's disease by the head-shaking test.

Researchers have observed that when head-shaking nystagmus (HSN) is provoked in patients with peripheral vestibular disorders, usually (in more than 75% of cases) it beats toward the normal or unaffected ear. The reverse of this pattern occurs commonly in patients with Meniere's disease. This finding presumably reflects the changeable pathophysiological state of the labyrinth of Meniere's disease. We retrospectively analyzed clinical records of eight patients who had unilateral Meniere's disease and came to Gunma University Hospital for consultation in the period from 1984 through 1989. All patients satisfied the following condition: In the period prior to the attacks of vertigo, for which a 10-day period preceding the attack was arbitrarily considered (the forerunning period), HSN reversed its direction, appeared, or disappeared. When HSN showed a biphasic pattern, only the first phase was considered in this present analysis. In the period before the attack, HSN reversed its direction from the normal to the morbid ear five times in four patients, appeared toward the morbid ear in three patients, and disappeared from one beating toward the normal ear before the forerunning period of vertigo attacks in one patient. These findings suggest that the occurrence of HSN directed to the morbid ear in the recuperation period in Meniere's disease might indicate the impending recurrence of a vertigo attack in a few days. In the present group of patients, vertigo attacks occurred from 6 hours to 8 days (average, 3.2 days) after the observation of HSN beating toward the morbid ear. In three of these patients, the immediate administration of isosorbide (a hyperosmotic diuretic) in this stage successfully suppressed the recurrence of vertigo attacks.

Isolation of myxobacteria from the marine environment.

In an attempt to isolate indigenous marine myxobacteria from coastal samples, we obtained two swarm forming bacteria. Both isolates formed cell aggregates which, at least in one isolate, developed to fruiting body-like structures consisting of a mass of myxospore-like cells. The optimum NaCl concentrations for their growth were between 2 and 3%, comparable to the NaCl concentration of seawater. This growth characteristic strongly suggests that the two isolates are specific marine bacteria. The 16S rDNA sequence studies indicated that the two isolates were related to the genus Nannocystis. Based on the phylogenetic distances between branches, we concluded that the isolates should be assigned to two new myxobacterial genera.

Characteristics of mutations in the p53 gene of oral squamous-cell carcinomas associated with betel-quid chewing in Sri Lanka.

Oral squamous-cell carcinoma (SCC) is the most common neoplasm in Sri Lanka, accounting for approximately 30% of all cancers in males. Epidemiologic evidence indicates that there is an unequivocal relationship between betel chewing and oral carcinogenesis, suggesting that there may be specific genetic targets of betel-quid ingredients. The p53 gene has been indicated to be a tumor-suppressor gene that is found in mutated form in common human cancers; however, there are few reports about "carcinogen-specific" p53 mutation. Because of this background, primary resected specimens from 23 oral SCCs, 7 leukoplakias and 2 oral submucous fibrosis were collected from oral SCC patients in Sri Lanka and were used for p53 mutation analysis. Exons 5 through 8 of the p53 gene were examined by polymerase chain reaction-single-strand conformation polymorphism (PCR-SSCP) and direct sequencing. Mutations in the p53 gene were frequent (10/23) in oral SCC specimens from Sri Lanka. Moreover, the mutations clustered significantly in exon 5 (7/10) of the p53 gene, and small deletions and inclusions other than point mutations were observed. These results indicate that 1) betel-quid chewing may cause specific genetic changes, including mutation in the p53 gene; 2) mutations in the p53 gene are not rare events in SCC patients who are betel-quid chewers, which contrasts with other reports; 3) exon 5 of the p53 gene could be one of the specific targets for some betel-quid ingredients; and 4) betel-quid chewing may be a critical environmental factor in the development of oral SCC.

Identification of the hydrophobic amino acid residues required for heme assembly in the rhizobial oxygen sensor protein FixL.

Rhizobial FixL is a novel heme protein, which senses environmental oxygen tension and directs signal transduction via protein phosphotransfer. To identify the essential residues for heme assembly in Rhizobium meliloti FixL, we individually replaced the 18 invariant hydrophobic amino acid residues (F, I, L, and V) in the heme-containing domain with alanine and histidine. Spectroscopic measurements of the soluble fractions from fixL recombinant Escherichia coli revealed that V152, F162, F170, I172, L185, F226, L230, and F243 as well as the proximal ligand H194 were indispensable for heme assembly. Autoxidation rates of purified I209H, I210A, and I210H were 65-fold, 15-fold, and 15-fold, respectively, faster than that of the wild type, although they retained heme in the protein. The absorption peak in the Soret region of the ferric I209H or I210H was red-shifted, suggesting that the ferric heme is a hexa-coordinate form in these mutants.

X-ray diffraction with a Bragg angle near pi/2 and its applications.

X-ray dynamical diffraction phenomena at a Bragg angle near pi/2 are studied. The X-ray transmissivity as well as the reflectivity from the (991) lattice plane of a silicon thin plate is observed. It agrees fairly well with the diffraction pattern calculated on the basis of the Darwin approach. The possibility is discussed whether a set of two crystal plates arranged face to face, in which the diffraction condition with a Bragg angle near pi/2 is satisfied, may be used as a very high resolution monochromator.

The effects of heme modification on reactivity, ligand binding properties and iron-coordination structures of cytochrome P450nor.

Artificial cytochrome P450nors (nitric oxide reductase) were prepared by replacing the native protoheme with various 2,4-substituted hemes: meso-, deutero-, and diacetyldeutero-hemes. For these samples, the ratio of low spin/high spin states of the ferric resting enzyme were varied, indicating that the coordination of the water molecule at the iron sixth site was affected by the electron withdrawing capacities of the heme 2,4-substituents. The binding of the water molecule reduces the rate of binding of nitric oxide (NO) to the ferric iron. In addition, the reduction reaction of the ferric-NO complex with NADH, which constitutes the second step in the NO reduction, was facilitated by the electron withdrawing capacity of 2,4-substituents. Consequently, proto- (native-) P450nor exhibited the highest overall enzymatic activity (NO reduction activity), while the enzymes containing diacetyl-, deutero-, and meso-hemes had considerably lower activities, since the NO reduction activity is determined by a balance of the reaction rates of the above two steps. The optical absorption spectra of the ferric-NO and the ferrous-CO complexes of the reconstituted enzymes show that the electron density on the heme in both states was modulated by the substituent groups. However, the resonance Raman spectral measurements showed that the Fe-NO and N-O stretching frequencies in the ferric-NO complex were insensitive to the electron density of the heme while the Fe-CO and C-O stretching frequencies in the ferrous-CO complex were sensitively varied by the electron withdrawing capacity of the 2,4-substituent. The differences are discussed in terms of the difference in the iron-ligand bond characters between the ferric-NO and the ferrous-CO complexes.

Cloning and characterization of the rat MAdCAM-1 cDNA and gene.

A cDNA clone for rat MAdCAM-1 homologue was isolated from mesenteric lymph nodes by RT-PCR using primers prepared from the exon sequences of the gene isolated from a genomic library of the WKAH rat, using mouse cDNA as a probe. A 1279 bp cDNA fragment contained an open reading frame (ORF) for a protein of 394 amino acids. Homology of nucleotide sequences between the mouse and rat MAdCAM-1 ORF was 85.1% with an amino acid identity of 80.5%. The rat MAdCAM-1 had two immunoglobulin-like domains, a mucinlike domain and the third immunoglobulin-like domain with a homology to alpha 3 domain of the rat MHC class I molecule. Northern blot analysis demonstrated transcripts in Peyer's patches and mesenteric lymph nodes but not in spleen. Organization of the gene in the rat was similar to that in the mouse, consisting of 5 exons located at about a 4 kbp genomic region.

Muscle afferent block for the treatment of oromandibular dystonia.

Oromandibular dystonia is a focal dystonia involving the masticatory and tongue muscles, causing difficulties in speech or mastication. We treated 13 patients with this condition by injecting diluted lidocaine and alcohol intramuscularly. This method is aimed at reducing muscle spindle afferent activity. The symptoms had been resistant to other therapies such as pharmacotherapy or dental treatment. All patients showed clinical improvement after this therapy with reduced EMG activities in the affected muscles, whereas control injection of normal saline gave no changes in EMG activities. The overall subjective improvement was 57.7 +/- 25.1% (mean +/- standard deviation) in a self-rating scale. The mean response of the jaw elevator muscles (70 +/- 13.1%) was significantly higher (p < 0.02, t test) than that of the depressor muscles (38 +/- 28.4%). Despite the precise mechanism being unknown, this difference might be related to the smaller number of muscle spindles in the depressor than the elevator muscles. This therapy is useful for the treatment of drug-resistant oromandibular dystonia.

Analysis of genetic information of an insect picorna-like virus, infectious flacherie virus of silkworm: evidence for evolutionary relationships among insect, mammalian and plant picorna(-like) viruses.

We synthesized the cDNAs of an insect picornavirus, infectious flacherie virus of silkworm (IFV), genomic RNA and inserted it into a bacterial plasmid (pUC119). The 9,650 nucleotides (nts) sequence except for the poly(A) tail was obtained from the cloned cDNAs, and the sequence integrity was confirmed by primer extension and direct RNA sequencing. The sequence has a large open reading frame (ORF) of 9,255 nts (3,085 codons) flanked by the short 5' non-coding region (156 nts) and by the rather long 3' non-coding (239 nts). The structural proteins VP3, 4, 1 and 2 were located at the N-terminus of the polyprotein in this order and were preceded by a tentative small peptide. Computer analysis identified the sequences similar to the consensus sequences of 2C (helicase?), 3C (protease), and 3D (RNA polymerase) conserved among mammalian and plant picorna(-like) viruses. In addition, the predicted genome organization of IFV was quite similar to those of picornaviruses. Further analyses of the characteristics of the genome structure and a tentative phylogenetic tree constructed on the basis of the amino acid sequence similarity emphasized the evolutionary relationships among the insect and plant viruses.

The use of a new bilayer artificial dermis for vestibular extension.

In this study we evaluated the usefulness of a new bilayer artificial dermis (TERUDERMIS; Terumo Corporation, Japan) for vestibular extension. It is composed of a collagen sponge with the silicone layer which is sloughed off as the mucous membrane heals. We have used it for vestibular extension in 50 patients. The material was effective in nearly all patients in promoting haemostasis, relieving pain, inducing granulation, promoting rapid epithelialization, and preventing contracture. There were no side-effects. The greatest problem with such materials in the past has been contracture at the vestibular extension area. However, we found that the degree of contracture was similar to that seen with autogenous mucosa or free skin graft. Our results indicate that the material might be useful for vestibular extension.
