Maternal body burden of organochlorine pesticides and dioxins.

To investigate the body burden of organochlorine pesticides and dioxins in Japanese women, 125 milk samples were collected from 41 mothers in 1994, 42 in 1995, and 42 in 1996. Of the 125 samples, 82 were from primipara mothers (first delivery) and 43 were from multipara mothers (second or later delivery). By using capillary gas chromatography with electron capture detection, beta-HCH and p,p'-DDE were detected as the major chlorine pesticides in human milk. Average levels of beta-HCH and p,p'-DDE were 475 and 368 ng/g lipid, respectively, in primipara breast milk, 314 and 259 ng/g lipid in multipara breast milk, and 420 and 330 ng/g lipid in total breast milk. Dieldrin, heptachor epoxide, oxychlordane, trans-chlordane, and cis-chlordane were detected at lower average levels of 3, 4, 34, 41, and 5 ng/g lipid, respectively. By using high-resolution gas chromatography with mass spectrometric detection, dioxins were detected in all samples. Average levels of total polychlorinated dibenzo-p-dioxin (PCDD), total polychlorinated dibenzofuran (PCDF), total PCDD + PCDF, total coplanar polychlorinatedbiphenyl (CoPCB), and total dioxin were 10.0, 7.8, 17.7, 9.9, and 27.5 TEQ (toxic equivalent) pg/g lipid, respectively, in primipara breast milk; 7.0, 5.8, 12.8, 7.3, and 20.1 TEQ pg/g lipid in multipara breast milk; and 8.9, 7.1, 16.1, 8.9, and 25.0 TEQ pg/g lipid in total breast milk. In primipara breast milk, significant correlations were found among levels of beta-HCH, p,p'-DDE, total PCDD-TEQ, total PCDF-TEQ, total CoPCB-TEQ, and total TEQ except for less correlation between p,p'-DDE and total PCDF-TEQ. Levels of these analytes also significantly increased depending on mother's age, except for total Co-PCB-TEQ. For the correlation with food habit, the only positive correlation was between total PCDF-TEQs and fish intake.

Multi-fractionated wide-field radiation therapy for palliation of multiple symptomatic bone metastases from solid tumors.

PURPOSE: This was a pilot study to explore the toxicity and response of multi-fractionated wide-field radiation therapy (MF-WFRT) in patients with multiple symptomatic osseous metastases. PATIENTS AND METHODS: From February 1997 to April 1998, a total of nine patients (5 lung cancer, 3 breast cancer, 1 prostate cancer) were treated with MF-WFRT. The patients received 1.5 Gy per fraction (twice a day) to a total dose of 7 Gy in 3 days for the upper body and a total dose of 9 Gy in 3 days for the lower body. Ten treatments in nine patients were carried out with this technique (2 upper half-body, 5 lower half-body, 3 mid-body; one patient had both upper mid-body and lower-half body treatments). RESULTS: Pain relief was complete in two patients (20%) and eight (80%) achieved better than 50% pain relief. Seventy-five percent of pain responders achieved pain relief within one week of MF-WFRT. The pain relief was long-lasting and continued without need of reirradiation for 45% of the remainder of the patients' lives. The incidence of gastrointestinal complications was low, basically Grade 1-2 toxicity. Four patients (40%) experienced Grade 3-4 hematological toxicity. Hematological toxicity was treated with blood transfusion or G-CSF. General tolerance was excellent, and no pneumonitis or radiation-related deaths occurred. CONCLUSION: This treatment modality appears to be well tolerated and effective. The optimal indications, dose, and fractionation for MF-WFRT should be further explored in randomized studies.

Clinical study on hepatocellular carcinoma with extrahepatic malignancies.

In a consecutive series of 146 patients with hepatocellular carcinoma (HCC), 10 patients (6.8%) were found to have one or two extrahepatic malignancies (EHM). Of these, eight had double cancers and two, triple cancers. The associated malignancies included eight cases of gastric cancer and four cases of colon cancer. Among the 12 lesions, eight were in the early stage. All the 10 patients were hepatitis B surface antigen negative. The incidence of coexisting liver cirrhosis and the retention rate of indocyanin green in 15 minutes among HCCs with EHM were significantly lower than those among HCC alone. These results suggest that the etiology of HCC with EHM is different from the etiology of HCC alone in Japan.
