[Systematic diagnosis and treatment method for temporomandibular disorders--new chart for primary diagnosis and treatment].

In this study, the classification of temporomandibular disorders of Japanese Society for Temporomandibular Joint was subdivided into ten types for the purpose of connecting directly with treatment, then a new chart for primary diagnosis and treatment were expressed. Ten types, i.e. type I (masticatory muscle disorders), type II (micro traumatic synovitis), type IIIc- (disk disorders except internal derangement; open lock, eminense click and crepitus), type IIIc+ (internal derangement; disc anterior displacement with reduction), type III L (internal derangement; disc anterior displacement without reduction, type IVo (osteoarthrosis with no TMJ symptoms), type IVp (osteoarthrosis with TMJ pain and no disc disorders), type IVc- (osteoarthrosis with crepitus, eminense click and open lock), type IVc+ (osteoarthrosis with internal derangement; disc anterior displacement with reduction), type IV L (osteoarthrosis with internal derangement; disc anterior displacement without reduction) 605 patients who visited our clinic from January 1994 to June 1994 were classified by the new chart for primary diagnosis; type I: 37 cases, type II: 142 cases, type IIIc-:56 cases, type III c+: 215 cases, type III L: 82 cases, type IVo: 8 cases, type IVp: 11 cases, type IVc-: 10 cases, type IV+: 19 cases, type IVL: 25 cases.

[Urinary bladder carcinoma was induced in a patient treated with cyclophosphamide for malignant lymphoma].

A case of urinary bladder carcinoma in a patient with malignant lymphoma (ML) involving the thyroid gland, gastrointestinal tract and the gallbladder is reported. The histological type of ML from the biopsy specimens of the thyroid gland and the stomach was non-Hodgkin's, diffuse large cell lymphoma of B cell origin. The patient had been treated for ML with cyclophosphamide (CPM), doxorubicin, vincristine and prednisolone effectively then maintained with low dose CPM. Two and a half years after the initial therapy, urinary bladder tumor diagnosed transitional cell carcinoma histologically was found. The patient had received 55 gr. of CPM totally. The dose of CPM was relatively smaller than the one in the other second cancer cases reported induced by CPM previously. It may be suggested that the urinary bladder carcinoma induced by CPM is related not only total dose of CPM but also the other factors. Thus usage of CPM must be cautious because of the secondary disease.

Clinical evaluation of four cases of acute viral hepatitis complicated by pure red cell aplasia.

Four patients with acute viral hepatitis complicated by pure red cell aplasia were clinically evaluated. Two patients with hepatitis type A, one with hepatitis type B, and one with posttransfusion non-A non-B hepatitis had pure red cell aplasia. Only the patient with posttransfusion hepatitis was female, and the mean age of the four patients was 37.8 yr (31-51 yr.) The mean interval from the onset of hepatitis to the appearance of pure red cell aplasia was 30.5 days (27-37 days), and the development of hematopoietic disorders coincided with decreases in transaminase levels. Laboratory findings included peak serum glutamate pyruvate transaminase levels of 1305-3160 KU and decreases in the prothrombin time to about 50% in the two patients with hepatitis type A. Pure red cell aplasia was successfully treated by prednisolone or transfusion in all patients.

Characterization of a Cl(-)-channel from rabbit transverse tubules in the planar lipid bilayer system.

A Cl(-)-channel of the transverse tubule(TT) was incorporated into artificial planar lipid bilayers and its properties were investigated. The channel was found to have the following properties. (1) The single-channel conductance is 40 pS in choline-Cl solution. (2) The rate of gating of the channel does not depend on the membrane voltage. (3) The channel is blocked by stilbene derivatives (DIDS and SITS), which are known as inhibitors of voltage-dependent Cl(-)-channels of the Torpedo electric organ, from both sides of the membrane. (4) An inhibitor of voltage-dependent Cl(-)-channels of skeletal muscle, 9-anthracene carboxylic acid(9-AC), inhibits Cl(-)-current from the cis-side to which the TT vesicles were added.

DNA-DNA subtractive cDNA cloning using oligo(dT)30-Latex and PCR: identification of cellular genes which are overexpressed in senescent human diploid fibroblasts.

We previously reported an efficient method for subtractive cDNA cloning using oligo(dT)30-Latex and polymerase chain reaction (PCR) (E. Hara et al., Nucleic Acids Res. 19, 7097-7104, 1991). The subtraction was performed by hybridization between mRNA of cell type B and the cDNA made from mRNA of cell type A using an oligo(dT)30 primer covalently linked to Latex particles in an Eppendorf tube. The mRNA common to both types of cells could be removed by a brief centrifugation. In the present paper, the method was improved by using the sense strand DNA instead of mRNA for hybridization to cDNA covalently linked to the particles to minimize mRNA degradation and by optimizing the hybridization condition. The sense strand DNA was made from cDNA-oligo(dT)30-Latex by asymmetric PCR. Using the improved method, a subtractive cDNA library with longer cDNA inserts was successfully constructed with higher probability than the original method.

Diaphragmatic activity during isoflurane anaesthesia in dogs.

The effect of isoflurane administration on diaphragmatic activity was investigated in six anaesthetized mechanically ventilated dogs. Diaphragmatic strength was assessed by measuring the transdiaphragmatic pressure (Pdi) generated during supramaximal stimulation of both cervical phrenic nerves at frequencies of 0.5, 10, 20, 50 and 100 Hz under partially isometric conditions at 1, 1.5 and 2 minimum alveolar anaesthetic concentrations (MAC), after maintaining 1 h of stable conditions. Pdi measurements were made at the start of the stimulation (initial) and at the end of a 2-s period (2-s). The force-frequency relationship was compared at each anaesthetic level. For single twitch (0.5 Hz) stimulation, the time constant of diaphragmatic relaxation was also assessed. The sequence of changing anaesthetic depth was altered in random fashion between animals. Pdi amplitude at single twitch stimulation was unchanged at the three anaesthetic concentrations. There was no significant difference in initial Pdi at various stimulus frequencies with increasing depth of isoflurane anaesthesia. In addition, no change in 2-s Pdi during low frequency stimulation (10 and 20 Hz) was noted during any of the three levels of anaesthesia. By contrast, 2-s Pdi with 50 Hz stimulation during 2 MAC isoflurane exposure decreased significantly below Pdi levels seen at 1 and 1.5 MAC (P < 0.01). Furthermore, 2-s Pdi at 100 Hz stimulation decreased significantly in a dose-dependent fashion. From these results, we conclude that isoflurane reduces diaphragmatic activity at higher stimulation frequencies of 50 and 100 Hz.

A study of erythrocyte C3b receptors (E-CR1) in patients with acute viral hepatitis.

Erythrocyte C3b receptors (E-CR1) were assayed by ELISA in 16 patients with acute viral hepatitis. In 13 patients with hepatitis type A, type B, or type non-A non-B, the E-CR1 level was significantly lower in the acute phase than in healthy controls, and recovered in the convalescent phase. The E-CR1 levels in 3 patients with non-A, non-B hepatitis remained below the mean level of the healthy controls. These findings suggest that the E-CR1 level fluctuates in acute viral hepatitis, and that it continues to be reduced in some patients with non-A non-B acute hepatitis.

Dietary diacylglycerol-dependent reduction in serum triacylglycerol concentration in rats.

The effects of dietary diacylglycerol consisting of 1,3 (65.2%) and 1,2 species (32.6%) and triacylglycerol (rapeseed oil) on the serum and hepatic lipid profiles were compared in the rat. The fatty acid composition was similar between these dietary lipids. The dietary acylglycerols were added to the experimental diets so as to provide the same amounts of fatty acids (9.39%). Dietary diacylglycerol compared with triacylglycerol significantly reduced concentrations of serum triacylglycerol at 17 and 34 days of the feeding periods without influencing those of phospholipid and cholesterol. There were no significant differences in the concentrations of hepatic triacylglycerol, cholesterol and phospholipid between the two groups of rats at 34 days of the feeding period. In the second trial, triacylglycerol in the experimental diet was replaced by varying amounts of diacylglycerol while maintaining the fatty acid contents (9.39%). After 14 days of the feeding period, significant reductions in serum triacylglycerol levels were confirmed in the groups of rats fed the diets in which diacylglycerol fatty acids supplied more than 50% (50, 75 and 100%) of total dietary fatty acids. Thus, it was confirmed that dietary diacylglycerol compared with triacylglycerol exerts a potent serum triacylglycerol-lowering effect in the rat.

Evaluation of first- and second-generation assays for detection of antibody to hepatitis C virus in non-A, non-B chronic liver diseases--evaluation of 1st and 2nd-generation assays in NANBH.

The positive rates of the second-generation enzyme-linked immunoassay (2nd-generation assay), and two first-generation immunoassays (C100-3 and KCL-163 assay) to test for anti-HCV antibodies in the serum of patients with non-A, non-B chronic liver disease were determined. The clinical usefulness of these assays was also evaluated. The group positive for the 2nd-generation assay alone was compared with that positive for the 2nd-generation and C100-3 assays with respect to the serum GPT levels determined simultaneously with the antibodies. The latter group showed slightly higher GPT levels. These findings suggest that the 2nd-generation assay is useful for the diagnosis of hepatitis C, and that C100-3 and KCL-163 assays are useful indicators of the activity of hepatic disorders.

Automatic suspension device for gait training.

The automatic suspension device (REHABOT) suspends the patient's body in a standing position allowing the patient to walk around the circular handrail without forward propulsion. Reduction of body weight is accurately maintained automatically while safely supporting the patient. The device was used for 23 patients with orthopaedic disorders or central nervous system disorders who were chosen because of their initial difficulties with gait training in parallel bars. Its advantages are that (1) it may be used for patients with open wounds or cardiac problems, or patients using prostheses or orthoses, (2) preparation and walking practice are simpler both for patients and staff than the therapeutic pool and walking trolley, (3) running costs are lower than the therapeutic pool. Its drawbacks are that the initial cost is relatively high, only one patient can be trained at a time, and the effect of warm water is missing. The automatic suspension device will become one of the new and fundamental pieces of equipment for gait training, especially for hospitals where there are many elderly patients and also severely and multiple disabled persons.

Thigh pain in cementless total hip replacement.

The incidence of thigh pain was investigated in 51 joints implanted with model Y or YII cementless total hip system. The 51 patients were followed for one or more years after surgery. Twenty-six out of 51 implanted with the model Y, and the other 25 with the model YII. Thigh pain appeared in 11 out of 26 patients (43%) replaced by model Y and in only 2 out of 25 (8%) replaced by model Y II. The difference in the incidence of thigh pain was attributed to the difference in the extent of the press fit of the stem, stress distribution and postoperative activity. To prevent thigh pain, a favorable press fit must be achieved during surgery, and the excessive weight-bearing must be restricted in postoperative course. In our improved stem design, stress is distributed uniformly on the interface between the bone and the stem resulting in reduction of thigh pain.

Clinical significance of serum HBVDNA detected by the PCR method.

Changes in serum HBVDNA and their relationship to the prognosis following interferon (IFN) therapy were examined by the polymerase chain reaction (PCR) in 4 patients with chronic hepatitis B infection undergoing IFN therapy. The disappearance of HBVDNA was confirmed by the PCR methods in 3 out of 4 patients who showed favorable subsequent courses. The remaining patient was positive for HBVDNA despite the disappearance of the HBe antigen. In this patient, the hepatitis recurred, and was accompanied by a persistent hepatic disorder. The PCR assay was also carried out to clarify the involvement of the hepatitis B virus in 12 patients who were positive for the HBe antibody and who showed fluctuating transaminase levels. The PCR method revealed that all 12 patients were positive for HBVDNA. These findings indicate the clinical usefulness of the PCR assay for hepatitis B virus infection.

Determination of cholesterol in sub-nanomolar quantities in biological fluids by high-performance liquid chromatography.

A highly sensitive method for the determination of cholesterol in biological fluids is described. Unsaponifiable lipids from rat serum and thoracic duct lymph chylomicron samples were treated with cholesterol oxidase. The product of the enzymatic reaction, delta 4-cholestenone, was analysed by normal-phase high-performance liquid chromatography (HPLC) using hexane-isopropanol (95:5, v/v) as a mobile phase and detected with a UV spectrophotometer at 240 nm. When the standard samples containing varying amounts of cholesterol (0.15-3 nmol) were treated with cholesterol oxidase and analysed by HPLC (injected amounts 0.09-1.8 nmol of cholesterol), the peak areas increased proportionally with the amounts of authentic cholesterol with a correlation coefficient of 0.996. The values in these biological fluids determined by the HPLC method were identical to those obtained by enzymatic-colorimetric or gas chromatographic methods. Moreover, the detection limit (0.09 nmol) of the present method (0.15 nmol are required for the sample preparation) is lower than those of conventional methods (approximately 30 nmol). Because of the excellent sensitivity and reproducibility, this method is well suited for the determination of cholesterol in biological fluids where cholesterol concentration is low.

Soybean phospholipid dependent reductions in triacylglycerol concentration and synthesis in the liver of fasted-refed rats.

The effect of dietary soybean phospholipid on the activities of hepatic triacylglycerol-synthesizing enzymes was compared with soybean oil in fasted-refed rats. Soybean oil at the dietary level corresponding to 20% but not at 5% fatty acid level (21.2 and 5.3% on weight bases, respectively) significantly decreased liver microsomal diacylglycerol acyltransferase activities measured with the endogenous diacylglycerol substrate. Dietary soybean phospholipid even at the dietary level corresponding to 2% fatty acids (3.4% on weight base) significantly decreased the acyltransferase activities measured with endogenous substrate. The dietary phospholipid further decreased the parameter as the dietary level increased, and at the 5% fatty acid level, it was lower than that obtained with soybean oil at 20% fatty acid level. Soybean oil and phospholipid decreased the diacylglycerol acyltransferase activities measured with the saturating concentration of exogenous dioleoylglycerol substrate only when the activities were expressed in terms of total activity (mumol/min per liver) but to much lesser extents. Dietary phospholipid compared to the oil profoundly decreased not only hepatic triacylglycerol but also microsomal diacylglycerol levels. It was indicated that the availability of microsomal diacylglycerol as the substrate for diacylglycerol transferase is the critical determinant in regulating hepatic triacylglycerol synthesis and concentration in this experimental situation. Alterations in the activities of microsomal glycerol 3-phosphate acyltransferase and of the enzymes in fatty acid synthesis could account for the phospholipid-dependent decrease in the microsomal concentration of this intermediate in triacylglycerol synthesis.
