Clinical usefulness of intraaortic balloon pumping in acute myocardial infarction complicated with cardiogenic shock, ventricular septal perforation and mitral regurgitation.

UNLABELLED: The effects of intraaortic balloon pumping (IABP) were studied in 91 patients with acute myocardial infarction complicated with cardiogenic shock (75 pts), ventricular septal perforation (VSP) (12 pts), and/or mitral regurgitation (MR) (4 pts). Out of 44 pts with cardiogenic shock in whom IABP was performed, 14 pts could not recover from cardiogenic shock, 6 pts became dependent on IABP and 13 pts survived (29.5%). In contrast, out of the remaining 31 pts with cardiogenic shock who did not undergo IABP because of inability to insert IABP catheter or other reasons and were treated medically, only 3 pts survived (9.7%, p less than 0.05). After the initiation of IABP, BPd, CI, SVI, SWI, TMG increased significantly, and HR, CVP, PCWP, TPR decreased significantly. Comparison of hemodynamic parameters after the initiation of IABP showed that SVI and SWI at 24 hours were higher and CVP lower in survivors. Out of 7 pts with VSP who underwent IABP 2 pts were operated and survived. IN CONCLUSION: short-term mortality in pts with cardiogenic shock was significantly lower in IABP-treated group, hemodynamic parameters improved after IABP, survivors from cardiogenic shock had higher SVI, SWI, BPd, and lower CVP than non-survivors, patients with VSP and MR had worse prognosis in spite of IABP.

Hemimegalencephaly with hemihypertrophy (Klippel-Trénaunay-Weber syndrome).

A 17-year-old Japanese girl had right hemihypertrophy, vascular nevus of the right side of the body, right hemimegalencephaly, multiple haemangiomatosis, varicosis and chronic proliferative glomerulonephritis. The hypertrophied side of the cerebrum showed no malformations. Quantitative studies showed no significant differences between the two cerebral hemispheres in size, form, density or DNA content of nerve and glial cells. The hemimegalencephaly seemed to be due to an increase in the absolute number of nerve and glial cells in the ipsilateral cerebral hemisphere. Eight cases of hemihypertrophy with hemimegalencephaly have been reported to date, in all of them hemihypertrophy and hemimegalencephaly were on the same side. This condition seems to be due to unilateral overproduction of neuroblasts and glioblasts or hindrance of the normal loss of excess neurons, which may be induced by hemihypertrophy of the mesenchyme surrounding the CNS.

Current status of benign biliary disorders in Japan and accuracy rates of preoperative diagnoses. Collective review of 14,654 patients.

Data on 14,654 patients preoperatively diagnosed for benign lesions who had undergone surgical treatment during a 2 year period (April 1979 to March 1981) were obtained from 145 institutions. A total of 18,891 biliary disorders were confirmed and classified into 18 groups: stones in the gallbladder, 12,272 patients (83.7 percent of all patients); stones in the extrahepatic bile duct, 3,605 patients (24.6 percent); stones in the intrahepatic bile duct, 570 patients (3.9 percent); acalculous cholecystitis, 463 patients (3.2 percent); idiopathic dilatation of bile ducts, 375 patients (2.6 percent); stenosing papillitis, 274 patients (1.9 percent); benign stricture of bile ducts, 133 patients (0.9 percent); and others. Malignant tumors were found in 243 patients, of whom 132 (54.3 percent) were not expected to have gallbladder cancer. Accuracy rates for preoperative diagnoses in the patients with gallstones of the intrahepatic or extrahepatic bile ducts or the gallbladder, idiopathic dilatation of the bile ducts, benign stricture of the bile ducts, or benign stenosis of the papilla of Vater were higher than 90 percent. However, the false-negative rates for these six disorders, except gallstones of the gallbladder, were higher than 16 percent. Therefore, despite advances in diagnostic procedures before surgery, intraoperative examinations remain indispensable for determining the most effective approach to surgical repair.

Scanning and transmission electron microscopic study on the cerebral arterial endothelium of experimentally hypertensive rats fed an atherogenic diet.

In order to clarify the roles of hypertension and hypercholesterolemia in atherogenesis, the cerebral arterial endothelium of experimentally hypertensive female rats fed an atherogenic diet were investigated by scanning and transmission electron microscopy. The hypertension was induced by constriction of the bilateral renal arteries. In both hypertensive groups, group I (hypertension + hypercholesterolemia) and group III (hypertension), many endothelial cells covered with numerous microvillous projections were observed. Pinocytotic vesicles and caveolae were also increased in these endothelial cells. On the other hand, in group II loaded with only hypercholesterolemia, microvillous projections on the endothelial cells tended to be increased, but its grade seemed to be lower than those of the hypertensive groups. These findings suggest that hypertension appears to be more effective than hypercholesterolemia in the cause of the increase of microvilli on the endothelial cell surfaces and the increase may be related to increased permeability in the cerebral arteries.

[Carcinoma in diverticulum of female urethra--a case report and review of the literature].

A case of carcinoma in urethral diverticulum is herein reported. The patient was a 58-year-old housewife presenting with the complaint of dysuria. Physical examination revealed a hen-egg-sized cystic mass at the anterior vaginal wall. Complete excision of the diverticulum was performed transvestibulary. Pathologically, it was described as papillary adenocarcinoma. The patient's postoperative course was uneventful and she was discharged. She was readmitted because of recurrence of tumor 9 months after the first operation. Forty-five cases including this case are reviewed and discussed.

Activation of platelets in patients with chronic proliferative glomerulonephritis and the nephrotic syndrome.

Platelet count, volume and aggregation and plasma levels of beta-thromboglobulin (beta-TG) and platelet factor 4 (PF-4) were measured in 54 patients with chronic glomerulonephritis (CGN). Platelet count and platelet aggregation induced by ADP, adrenaline and collagen were significantly higher in the patients than in normal subjects, and platelet aggregation was markedly increased in the cases with progressive glomerular lesions. Plasma levels of beta-TG and PF-4 were significantly higher in the patients than in the normal subjects. There was a significant inverse correlation between plasma beta-TG and creatinine clearance. Nephrotic patients showed significantly smaller platelet volume and markedly elevated plasma beta-TG levels when compared to the controls. Plasma beta-TG decreased remarkably in 3 out of 4 patients with markedly increased beta-TG levels when they were given antiplatelet drugs. The results suggest that platelet aggregation and the release reaction were increased in patients with CGN. Activated platelets may be an important factor in the genesis of the thrombotic tendency in the nephrotic syndrome.
