[Logical characteristics of specimens of the lung resected for tuberculosis].

The specimens of the lung resected for tuberculosis in 173 patients (10 with infiltrates, 133 with tuberculomas, and 30 with caverns) show its active forms with preponderance of decay, foci of dissemination, and a wide caseous and necrotic layer in the cavernous walls. Tissue responses reflect mainly immediate type hypersensitivity and they are followed by a slight development of epithelio-cellular granulomas.

[The morphological differences in tuberculous inflammation foci, which reflect the body's immunological responsiveness].

A comparative study of morphological reaction in the foci of tuberculous inflammation in decreased patients with caseous pneumonia (n = 2), fibrocavernous pulmonary tuberculosis (n = 19), disseminated tuberculosis (n = 5) and on the material of resected parts of the lung (infiltrate in a phase of separation (n = 2), tuberculomas (n = 22), caverns (n = 5)) has shown that exudative and necrotic reactions with minimum discriminating signs and no epitheliocytes, by accompanying by atrophy of the paracortical area of intrathoracic lymph nodes reflect immediate hypersensitivity. The decrease in the rate of exudative necrotic reactions, the increase in fibroplastic processes, and the appearance of epitheliocytes suggest that immediate hypersensitivity is added by delayed hypersensitivity. Pronounced fibroplastic reactions around the foci of caseosis and on the walls of tuberculous caverns, involvement of epitheliocytes in cellular reactions point to the fact that a tuberculous process develops in delayed hypersensitivity.

[Gastric tuberculosis in the past and present].

The paper presents a historical review of the literature on the rare form of extrapulmonary tuberculosis--gastric tuberculosis. It is more frequently associated with tuberculosis of other organs and observes as an isolated form. The disease has no specific clinical signs. It is diagnosed as gastric carcinoma or ulcer by X-ray and in most cases endoscopic studies. The diagnosis of gastric tuberculosis can be made only by histological study of the resected stomach or its biopsy specimen. Antibiotic treatment for tuberculosis yields a good effect.

[Tuberculosis thanatogenesis and pathoanatomy of caseous pneumonia].

Tuberculosis thanatogenesis of today has been studied on autopsy and operative material. Pathoanatomy and mechanism of development of caseous pneumonia have been investigated: types of tissue reactions and role of terminal and respiratory bronchioles in the development of caseous pneumonia are shown and early phases of pathological changes of the connective tissue and interstitium (acute mesenchymopathy) distinguished. Caseous pneumonia has been distinguished for the first time. Its reversible phase may be cured by modern treatment strategy, whereas in its irreversible form not only conservative, but urgent surgical treatment is indicated.

[The pathoanatomical,diagnosis of progressive forms of pulmonary tuberculosis in relation to the new clinical classification].

This communication is a letter of information that gives for postmortem diagnosis a brief account of tuberculous inflammation and major types of pulmonary tuberculosis during their progression to death and an approximate outline of pathoanatomical diagnosis. Terminal tuberculosis is shown to be now complicated by miliary and caseous pneumonias. Caseous pneumonia may appear as an independent nosological entity and as a complication of acute progression, more frequently, of fibrocavernous tuberculosis. Caseous pneumonia as a tuberculosis type is an irreversible process that calls for emergency surgical treatment. It has been found that there are primarily impairments in lung connective tissue function, acute mesenchymopathy with high blood barrier permeability in caseous pneumonia. Terminal bronchiolar lesion is a later stage in the pathogenesis of caseous pneumonia.

[Morphological features of tissue reactions in combined treatment of experimental tuberculosis induced by xenobiotics].

The trend to aggravated running and contribution of air pollution of large industrial centers with polycyclic aromatic hydrocarbons (PAH) to tuberculosis onset and progress dictate the necessity of the search for new treatment methods. The authors made an attempt to treat experimental tuberculosis with sodium glutamate and isoniazide under chronic exposure to PAH dust. Sodium glutamate especially in combination with isoniazide produces a good effect enhancing granulomatous reactions fibroplastic processes in the foci of specific inflammation. Sodium glutamate is thought an effective pathogenetic treatment of tuberculosis.

[Roentgeno-anatomical correlations in infiltrative pulmonary tuberculosis based on resection materials].

The material of 97 pulmonary resections for large caseous focuses of tuberculosis was compared to relevant x-ray and morphological evidence. Roentgenologically, caseous-necrotic foci may be stable for a long time (6-12 months) in spite of the absence of the fibrous capsule. In view of these findings, x-ray potentialities in the diagnosis of the process form and phase are considered.

[The causes of death in pulmonary tuberculosis patients living in the industrial areas of the Urals].

A comparative study of the causes of death of tuberculosis patients living in the industrial centres of the Ural for the period of 1953-1990 has shown that beginning in 1980's the frequency of progressive forms of tuberculosis increases, while the proportion of cardiopulmonary and chronic renal insufficiency as the cause of death diminishes (5.8 and 0.6%, respectively). Among those who died subjects of the able-bodied age and those with antisocial mode of life account for 40%. The middle-aged and elderly subjects (average age is 67 years) die mainly of silicotuberculosis among whom 40% die of cardiopulmonary insufficiency.

[Diagnosis of pleurisy of tuberculous etiology].

Among the 367 patients with the pleurisy syndrome examined in a specialized diagnostic department, 167 (45.5%) had tuberculous pleurisy and the active stage of pulmonary tuberculosis was diagnosed in 42 (25.1%); in 125 subjects pleurisy was an independent form of tuberculosis. The possibilities of the clinico-roentgenologic, bioptic and bacteriologic diagnosis of the disease etiology have been studied in different variants of the X-ray picture (unblocked, partially sacculated, sacculated and adhesive pleurisy).

[Differential diagnosis of pleurisy of tuberculous etiology].

A combined ++clinico-roentgenological+ examination of 367 patients with a pleurisy syndrome was aimed at the study of a potential establishment of the etiologic diagnosis of the disease. Examination comprised the routine tests and findings of various types of biopsy, including pleural biopsy performed within the first two months from disease onset. The etiologic diagnosis of tuberculous pleurisy was most difficult in 96% of the patients who exhibited prolonged and stable maintenance of exudation or, on the contrary, very rapid regression of the pleural exudate without residual changes. Simultaneous morphologic and bacteriological examination of a pleural biopsy specimen contributed to the most efficacious and reliable diagnosis of tuberculous etiology of pleurisy.

[Cavernoscopy in patients with disseminated fibrous-cavernous pulmonary tuberculosis].

A technique of a dynamic cavernoscopy permitting one to conduct diagnostic and therapeutic manipulations for several months has been suggested. The given procedure was conducted in 36 patients with disseminated fibrous-cavernous pulmonary tuberculosis. An important role in defining management tactics belongs to the direct bacteriological and histological examination of the cavern contents. Local treatment by means of antituberculous drug solution instillation was administered in 29 patients. A positive effect was registered in 26 cases. The above technique can be used for the process prognosis and the treatment modification.
