[Postoperative rehabilitation in patients with sequels of tuberculous and nonspecific gonitis after total endoprosthetic repair of the knee joint].

To evaluate the effectiveness of rehabilitative measures, comprehensive studies have been conducted in 37 patients with sequels of tuberculous and nonspecific gonitis, who underwent primary total endoprosthetic repair of the knee joint with an Osteonics-7000 cement fixation prosthesis (USA) in 1996 to 2002. After discharge from hospital, 18 patients received a complete course of rehabilitation, 19 patients refused it for different reasons. The results were assessed just after completion of the course of rehabilitation (3 months after surgery), following 1 year and on the last examination (on the average 45.2 months later). Statistically significant advantages in the function of the knee joint and in the force of its extensors were observed in the group of patients undergoing a complete course of postoperative rehabilitation just after completion of the course and a year after surgery. The last examination revealed a great difference between the patients who had undergone a complete course of rehabilitation and those who had refused it.

Amiloride-sensitive sodium transport in lamprey red blood cells: evidence for two distinct transport pathways.

To determine Na+/H+ exchange in lamprey erythrocyte membranes, the cells were acidified to pH(i) 6.0 using the K+/H+ ionophore nigericin. Incubation of acidified erythrocytes in a NaCl medium at pH 8.0 caused a considerable rise in 22Na+ influx and H+ efflux during the first 1 min of exposure. In addition, exposure of acidified red cells to NaCl medium was associated with rapid elevation of intracellular Na+ content. The acid-induced changes in Na+ influx and H+ efflux were almost completely inhibited by amiloride and dimethylamiloride. In native lamprey erythrocytes, amiloride-sensitive Na+ influx progressively increased as the osmolality of incubation medium was increased by addition of 100, 200, or 300 mmol/l sucrose. Unexpectedly, the hypertonic stress induced a small, yet statistically significant decrease in intracellular Na+ content in these cells. The reduction in the cellular Na+ content increased with hypertonicity of the medium. The acid- and shrinkage-induced Na+ influxes were inhibited by both amiloride and 5-(N-ethyl-N-isopropyl)-amiloride (EIPA) in a dose-dependent manner. For both blockers, the half-maximal inhibitory values (IC50) were much greater for the shrinkage-induced (44 and 15 micromol/l for amiloride and EIPA, respectively) than for the acid-induced Na+ influx (5.1 and 3.3 micromol/l, respectively). The data obtained are the first demonstration of the presence of a Na+/H+ exchanger with high activity in acidified (pH(i) 6.0) lamprey red blood cells (on average, 512 +/- 56 mmol/l cells/h, n = 13). The amiloride-sensitive Na+ influxes produced by hypertonic cell shrinkage and acid load are likely to be mediated by distinct ion transporters in these cells.

Activation of Na+/H+ exchange by protein phosphatase inhibitors in red blood cells of the frog Rana ridibunda.

The present study was designed to evaluate the role of protein phosphatases in regulation of sodium transport in the marsh frog erythrocytes using 22Na as a tracer. For this purpose the cells were treated with several known inhibitors of protein phosphatases. In standard isotonic medium, exposure of the cells to 10 mmol l(-1) NaF, 20 nmol l(-1) calyculin A or 0.1 mmol l(-1) cantharidin resulted in a significant (1.7-fold) increase in unidirectional ouabain-insensitive Na+ influx. The Na+ influx in frog red cells was progressively activated as the medium osmolality was increased by addition of 100, 200 or 300 mmol l(-1) sucrose to standard isotonic medium. The stimulatory effect of protein phosphatase blockers on Na+ influx was much higher in hypertonic medium containing 100 or 200 mmol l(-1) sucrose than that in isotonic medium. Stimulation of Na+ transport enhanced with increasing concentrations of calyculin A, and half-maximal activation (EC50) was obtained at 16 nmol l(-1). However, Na+ influx induced by strong hypertonic treatment (+300 mmol l(-1) sucrose) was not altered further in the presence of protein phosphatase inhibitors. The changes in Na+ influx evoked by protein phosphatase inhibitors and hypertonic treatment were associated with a rise in the intracellular Na+, but not K+, content. Enhancement in Na+ influx after addition of protein phosphatase blockers to cell suspension in isotonic or hypertonic media was almost completely inhibited by Na+/H+ exchange inhibitors, amiloride and ethyl-isopropyl-amiloride. The basal Na+ influx in frog erythrocytes in isotonic medium was relatively low (1.7 mmol/l cells/h) and not affected by 1 mmol l(-1) amiloride. Thus, the data obtained clearly indicate that Na+/H+ exchanger in the marsh frog red blood cells is under tight regulatory control, in all likelihood via protein phosphatases of types PP-1 and PP-2A.

[External respiration function in patients with exudative pleurisy before and after video-thoracoscopic operations].

57 patients with exudative pleurisy were observed. Function of external respiration was examined with traditional spirometry and a new method--distant spirometry. Function of cardio-vascular system was examined with electrocardiography and echocardiography. The new method of monitoring permits to perform repeated and daily examinations of external respiration function. The method is highly informative, sabe, physiological, permits to evaluate general and isolated ventilation of the lungs simultaneously, and demonstrates dynamics of exudate accumulation that may help in choice of treatment policy.

[Correction of asthenovegetative disorders in the postoperative period in pulmonary tuberculosis patients].

UNLABELLED: 58 patients with asthenia were observed during postoperative period after surgery treatment of tuberculosis. All of the patients were divided into two groups: 34 patients taking anti tuberculosis drugs and Stimol, and 24 patients taking only anti tuberculosis drugs. Following method where used to appreciate states of the patients: scales of Symptoms Check List--90, acid-base and gas state of blood, EKG, ECHOKG, KIG, Contactles Spirometry. RESULTS: Treatment with using of Stimol leads to quicker regress of asthenia to improvement in both cardiovascular and respiratory systems and normalization of gas state of blood. Effect of this medicine can be connected with positive influence on cardiac and respiratory functions of the body due to normalization vegetative prevision and improvement of metabolism in cells of drain and cardiomyocyties.

Do quiescent human peripheral lymphocytes repair radiation-induced chromosome damage?

Peripheral blood samples from seven healthy donors were exposed to 2 Gy of (60)Co gamma rays. Lymphocyte cultures were prepared from the samples and stimulated to proliferate either immediately after the exposure or after 1, 2 or 4 h. No significant differences were found between the frequencies of chromosome aberrations (breaks or exchange) observed in lymphocytes stimulated after different postirradiation periods. We conclude that unstimulated lymphocytes do not undergo significant levels of recovery from potentially lethal damage at the chromosomal level.

Cytogenetic adaptive response in cultured human lymphocytes: dependence on the time of exposure to adapting and challenging doses of gamma-rays.

Human lymphocytes from 16 healthy donors were exposed in vitro to an adapting dose of gamma-rays (0.05 Gy) at G0, or G1, or G1/S stage of the cell cycle and subsequently to a challenging dose of gamma-rays at G1, or G1/S, or S (1 Gy), or G2 (0.5 Gy) stage. Frequencies and distributions of the induced chromosome aberrations were analyzed in first-division metaphases. The data averaged over the donors revealed the protective action of the adapting exposure under the irradiation schemes with the challenging dose delivered at S or G2 stage. The majority of aberrations induced at these stages belonged to the chromatid type, and their yield was significantly higher in G2-exposed cells than in S-exposed cells. However, the relative reduction of the challenging dose effect (about 34%) in the adapted cells did not depend on the magnitude of this effect, and its value remained the same (within the experimental error) if aberrations were subdivided into chromosome and chromatid types or grouped as total deletions and total fragments. The adaptive response was not revealed under the schemes with the challenging dose delivered at G1 or G1/S stage. Analysis of the individual results showed that, in one and the same donor, the adaptive response could be observed under one irradiation scheme and not observed under other schemes, the most effective schemes being those with the challenging dose delivered at G2 stage. Four donors, however, did not show the adaptive response even under such schemes. Data on aberration distributions suggested that different repair processes, rather than a unique one, may underlie the adaptive response.

Chromosome aberrations in cultured peripheral lymphocytes from persons with elevated skin radiosensitivity.

The purpose of this study was to elucidate whether an enhanced skin radiation reaction correlated with an enhanced chromosome radiation response. Twelve patients with late radiation skin ulcers formed after courses of radiation therapy were chosen as a group of individuals with elevated skin radiosensitivity. Half of the venous blood samples from each donor were irradiated with 2 Gy gamma-rays; the other half remained unexposed. Using standard cytogenetic technique, lymphocyte cultures were prepared with all samples. On the metaphase preparations, all chromosome aberrations detectable without karyotype identification were scored. The frequency of various aberrations in each patient were compared with relevant mean values in healthy unexposed donors. In several patients, the frequency of one aberration type or another exceeded the control value significantly. Comparison of aberration patterns in irradiated and nonirradiated cultures and consideration of elapsed time after therapeutic exposures suggested that the observed increased aberration levels reflected individual features of the patients' radiosensitivity, rather than lesions induced by previous in vivo exposures. Therefore, the question of a correlation between skin and chromosome radiosensitivity can be answered positively. Analysis of the peculiarities of cellular distribution of aberrations and of the relative contribution of different aberration types in patients and healthy donors indicates that the investigation of in vitro-induced aberrations is more suitable for the assessment of individual radiosensitivity than the study of aberrations observed in unexposed cultures.

[Oxidative phosphorylation in liver mitochondria in toxic form of experimental form of influenza].

The effect of free radical processes on the oxidation-phosphorylation activity in the liver of CBA-mice has been studied, using a model of toxic form viral infection. The EPR-spectroscopic and electrochemical methods applied in the study of animals infected with pathogenic form of the influenza virus made it possible to reveal a decrease in the activity of the respiratory chain in mitochondria. This seems likely to be due to accumulation of endogenic nitric oxide in the liver tissue.

[Generalized mastocytosis--hemoblastosis of mast cells].

A review of literature has been presented including current data on bone marrow genesis functional role of mast cells in the human body, as well as characteristics of the main clinical variants of mastocyte tumor proliferations (solitary mastocytoma, urticaria pigmentosa, generalized mastocytosis). Clinicomorphological picture and positive results of the treatment with the Soviet cytostatic spirobromine of two patients suffering from generalized mastocytosis are described.

[Effect of cadmium ions on the respiration, cation transport and morphology of the liver mitochondria in the rat and the lamprey].

The influence of Cd2+ on the function and structure of liver mitochondria of rats and lamprey (Lampetra fluviatilis L.) has been studied in vitro. It is shown that Cd2+ can penetrate into the mitochondrial matrix due to Ca2+-transport mechanism. Being stored in the mitochondria, Cd2+ inhibits respiration and an energy dependent transport of penetrating cations (Cs+-valinomycin), and disturbs passive permeability of the inner mitochondrial membrane for monovalent cations and H+. The effect of Cd2+ on the lamprey liver mitochondria is more pronounced than in the case of rats.
