Cloning and characterization of a putative human d-2-hydroxyacid dehydrogenase in chromosome 9q.

There is little information on d-isomer-specific dehydrogenases in humans. Identification of d-2-hydroxyglutaric aciduria, an inherited metabolic disorder associated with severe neurological dysfunction, highlights the role of d-isomers in human metabolism. The possibility of a defect in d-2-hydroxyglutarate dehydrogenation prompted us to employ E. coli d-2-hydroxyacid dehydrogenase cDNA to search the human expressed sequence tags database. Two human EST homologues were retrieved and sequenced. Analysis showed the two clones were identical with 1258 nucleotides encoding 248 amino acids of the putative human d-2-hydroxyacid dehydrogenase. It was highly homologous to bacterial d-2-hydroxyacid dehydrogenases (46%), d-phosphoglycerate dehydrogenase (38%), and formate dehydrogenase (36%) at the amino acid level. The gene is expressed ubiquitously in tissue, most abundantly in liver, and was mapped to chromosome 9q between markers WI-3028 and WI-93330. To our knowledge this is the first cloning and characterization of the cDNA for a human d-isomer specific NAD(+)-dependent 2-hydroxyacid dehydrogenase.

Efficacy and safety of preoperative lobar or segmental ablation via transarterial administration of ethiodol and ethanol mixture for treatment of hepatocellular carcinoma: clinical study.

Transarterial embolization (TAE) using various thrombotic substances for unresectable hepatocellular carcinoma (HCC) performed on many patients has resulted in a better survival rate. We evaluated the efficacy and clinical safety of using an Ethiodol-ethanol mixture as the embolizer for treatment of HCC and the possibility of a surgical approach for inoperable tumors after TAE. Twenty patients with HCC who underwent TAE and tumor resection were included in the study. Initially, eight had increased retention rate of indocyanine green dye via intravenous injection (0.5 mg/kg) at 15 minutes (ICGR15), and six had an insufficient residual volume that precluded them from undergoing tumor resection. TAE was performed by slowly infusing the mixture of Ethiodol and ethanol into the artery supplying the tumor until dual hepatic artery and portal vein embolization was achieved. Serum levels of alanine aminotransferase increased after embolization, but all biochemistry studies reverted to normal within 2 weeks. A decreased tumor size (n = 15), improved ICG (n = 8), and increased volume of the nonembolized lobe (n = 10) were noted. The operations performed were right lobectomy (n = 11), extended right lobectomy (n = 3), left lobectomy (n = 2), extended left lobectomy (n = 2), and wedge resection (n = 2), which included patients who did not want to undergo major hepatectomy. Complete tumor necrosis was found in seven cases. All patients survived with no associated complications. The 1-year survival rate was 95%. Transarterial Ethiodol and ethanol administration creating dual hepatic artery and portal vein embolization was a safe and efficacious method for treating HCC. It effectively decreases tumor size, causes compensatory hepatic hypertrophy, and improves the ICGR15, which allows a wider range of patients to undergo liver surgery and achieve better survival.

Reduction of lower palpebral bulge by plicating attenuated orbital septa: a technical modification in cosmetic blepharoplasty.

Bulging of the lower eyelid is regarded as a sign of aging. "Herniation" of the periorbital fat pads is traditionally regarded as the factor responsible for the change. Excision of fat pads, therefore, has been the mainstay of treatment in reducing the palpebral bulge in cosmetic blepharoplasty. The surgical excision of"excess" and "herniated" fat pads, however, causes problems such as lid ecchymosis, chemosis, lid contour irregularity, ectropion, and retrobulbar hematoma formation. The author proposes that the loss of fat pad support caused by the attenuation of the orbital septa, not herniation of the excess fat pads, is the major factor responsible for the bulge. The author further proposes that the functional integrity of the orbital septum can be restored by plicating the attenuated orbital septa with 5-0 absorbable sutures. This technique of invaginating the protruded fat pad was performed in 138 individuals (276 lower eyelids). The operation was technically simple, and the approach was "tissue friendly." The results obtained, with the exception of a mild degree of puffiness encountered soon after the surgery, were satisfactory. Morbidity was minimal.

A closed shell structured eyeball model with application to radial keratotomy.

A closed shell structured eyeball model was developed for predicting the displacements and curvatures in an eyeball due to radial keratotomy. Both the cornea and sclera are modeled as an ellipsoidal cap, and the two caps are connected at the limbus to form a closed shell. The analysis of the number of corneal collagen laminae required for the tissue to be theoretically transversely isotropic was presented. The cornea, as well as the limbus and sclera, is considered as macroscopically homogeneous and isotropic in this study. A procedure to obtain the principal curvature at a point on the exterior surface was established. In the basic formulation, large displacements are contemplated. However, the FORTRAN computer program that was prepared to implement the procedure considers small displacements, and the resulting equations are linear. Although the results from this shell structured eyeball model are fairly good quantitatively, they do show vividly the following qualitative corneal behavior after the operation is performed: The opening of an incision has a V-shape, the radial displacements through the corneal thickness are nearly the same, and the largest in-plane displacement is only one-tenth of the largest radial displacement.

Importance of holotoxin assembly in Ptl-mediated secretion of pertussis toxin from Bordetella pertussis.

We examined the structural components of pertussis toxin that are required for efficient export from Bordetella pertussis via the Ptl system, a member of the type IV family of macromolecular transporters. First, we constructed a strain of B. pertussis that contains a functional Ptl system but does not produce pertussis toxin. Plasmids which express either the S1 subunit or the B oligomer were then introduced into this strain. We found that the B oligomer of the toxin is not secreted in the absence of the S1 subunit. Conversely, the S1 subunit is also not secreted by a Ptl-mediated mechanism in the absence of the B oligomer. Thus, an assembled holotoxin is required for Ptl-mediated export of pertussis toxin from B. pertussis.

[Analysis of the design principle and mechanics nature of adjustable semi-circular planum external fixator].

This thesis presents our studies of an adjustable semi-circular planum external fixator and offers a relatively detailed introduction and analysis of the structure principle and mechanics nature. Because dual-direction joining pole with active joints was used, by adjusting the length of each pole, it can not only make the two semi-circular steel plates more parallely along the axis line to stabilize the fracture and lengthen the extremity, but it also can change the planum angle of the steel plate, and thus achieve the effect of orthopaedic function.

[Clinical study of shengxue mixture in treating aplastic anemia].

OBJECTIVE: To observe the clinical effect of Shengxue mixture (SXM) in treating aplastic anemia, and studied its mechanism. METHODS: Eighty-four patients of the treated group, those with Spleen-Kidney Yang deficiency Syndrome and Spleen-Kidney Yin deficiency Syndrome were treated with SXM-A and SXM-B respectively, and 30 patients of the control group were treated with Stanozolol. The clinical effect and several experimental parameters were also observed. RESULTS: The basic cure rate, remission rate, total effective rate of the treated and control group were 21.43%, 27.38%, 86.90% and 3.33%, 20.00%, 56.67% respectively, the difference between 2 groups was significant (P < 0.05, P < 0.01). The effect of the treated group was obviously better than that of the control group. While the patients' symptoms were alleviated, the peripheral blood cells increased, the ratio of T lymphocyte subsets tended to balance, the level of natural killer cells activity increased, interleukin 2 reduced, and the reproduction of the bone marrow were markedly improved in most of the patients treated by SXM. CONCLUSION: SXM is an effective and safe drug for aplastic anemia. Its mechanism might be regulating the immune function, which facilitated the recovery of the bone marrow hematopoiesis function.

[Germline LKB1 gene mutation screening in 4 Chinese Peutz-Jeghers syndrome pedigrees].

OBJECTIVE: To evaluate the frequency and nature of LKB1 gene germline mutations in 4 large Chinese Peutz-Jeghers syndrome pedigrees. METHODS: Four Chinese Peutz-Jeghers syndrome pedigrees were investigated. Two patients and 1 normal adult from each pedigree were selected, and genomic DNA from peripheral blood was extracted. The 9 exons of LKB1 gene were amplified by PCR. The products were tested by SSCP and abnormally shifted bands were sequenced. If there was no positive finding in any pedigree, the entire exons were sequenced. RESULTS: The same 842 C deletion of LKB1 gene frame-shift mutations was found in 2 pedigrees, which resulted in truncated protein. No exon variant was found in the left 2 pedigrees. CONCLUSIONS: LKB1 gene germline mutation is an important molecular pathogen of Peutz-Jeghers syndrome. 842 C deletion is a possible mutation hotspot and might be a common-ancestor mutation characteristic of Chinese.

[A clinical study on fresh amniotic membrane transplantation for treatment of severe ocular surface disorders at acute inflammatory and cicatricial stage].

OBJECTIVE: To investigate the safety and evaluate the effect of fresh amniotic membrane transplantation to reconstruct the ocular surface with severe inflammation and scarring. METHOD: Amniotic membrane transplantation (38 cases, 46 eyes), or combined with either limbal transplantation (9 cases, 9 eyes) or lamellar keratoplasty (8 cases, 8 eyes) was performed on ocular surface burns (5 cases, 6 eyes), recurrent Mooren's ulcer (8 cases, 8 eyes) and severe symblepharon (38 cases, 46 eyes). All the patients were followed-up for 6 - 18 months (mean, 11 months). Impression cytology was examined in 21 eyes with normal tear secretion post-operatively. RESULTS: No acute rejection was observed after fresh amniotic membrane transplantation. No progressive melt and perforation were seen in five out of six eyes with burns in reconstructed ocular surfaces. Neovascularization, pseudopterygium and iris atrophy were not found during the follow-up. Visual acuity was improved in various degrees. Corneal ulceration was not noted in 8 cases (8 eyes) with frequently recurrent Mooren's ulcer. Ocular surface reconstruction was successful in 46 of 49 eyes with severe symblepharon. Symblepharon occurred in the remaining three eyes but much less than pre-transplantation in degree. Amniotic epithelia have been examined in about three months on the transplanted eyes with normal tear secretion. CONCLUSION: Fresh amniotic membrane can be used as a graft to reconstruct ocular surface. Such transplantation can effectively reduce neovascularization, fibrosis and inflammation. Our data indicated that complete removal of pathological tissues and perfect fixation of amniotic membrane graft are crucial steps in the reconstruction of ocular surface.

[The recognition of the epidemic area of tsutsugamushi disease on Nan Peng Lie Island in China and the strategy of prevention].

OBJECTIVE: To investigate the region for the epidemic area of tsutsugamushi disease. METHODS: Epidemiological studies, Orientia tsutsugamushi isolation, and preventive measures were used. RESULTS: The region belonged to epidemic area of south subtropical zone. The main host was found Rattus norvegicu. The main biological vector was Leptotrombidium deliens. The seasonal trends of the quantity of Rattus norvegicu and Leptotrombidium deliens were consistent with the incidence change of the disease in the region. Orientia tsutsugamushi has been isolated from Rattus norvegicu and Leptotrombidium deliens. Data showed that Orientia tsutsugamushi isolated strains mainly belongs to Karp. Sero-epidemiological data showed that antibodies of Orientia tsutsugamushi are 100.0% positive in the residents of the region and 4.0% in army men. After taking comprehensive preventive measures, the incidence was descending. CONCLUSION: Nan Peng Lie island was proved an epidemic area of tsutsugamushi disease.

[Geographic information systems spatial analysis on transmission of schistosomiasis in China].

OBJECTIVE: To understand the epidemiologic status and geographical distribution of schistosomiasis in China. METHODS: Relevant detabases were set up after collection of data from two National Sampling Surveys on Schistosomiasis, in 1989 and 1995. Spatial analysis was undertaken after the database linked to the GIS software which was supported by Arc View 3.0a. Correlation analysis was performed to understand the relationship between rates of human infection and cattle infection. RESULTS: The epidemic areas of schistosomiasis with high risk are mainly distributed in the marshland along the Yangtze River and can be identified as five spatial distribution regions based on the results of spatial analysis. Both epidemic areas and positive rates of stool examination in cattle and water buffalo are much wider and higher than that in humans. The positive correlation was seen between infection rate in human and in cattle from data of both sampling surveys. CONCLUSION: The relevant strategy for schistosomiasis control in different spatial regions should be performed accordingly and the measures of control for cattle and water buffalo should be strengthened in the endemic areas.

[Analysis on the characteristic and trend of AIDS epidemics in Shandong].

OBJECTIVE: To analyse the data thru labs and field epidemiological investigation from 1987 to 1999, in order to find out the characteristics, sources of infections and its trend which would lead to the development of strategy, prevention and control of AIDS in Shandong. METHODS: Anti-HIV antibody was detected by ELISA and WB in the serum of high-risk groups. When Anti-HIV antibody was positive, personal and epidemiological history was investigated. The anticoagulated blood was repeatedly collected to verify the infected people. PBMCs were separated from the anticoagulated blood to collect previrus DNA. Gene sequence was then detected and subtype analysed by PCR using ABI company kit. RESULTS: Eight hundred five thousand eight hundred and one specimens were detected in the whole province from 1987 to 1999. 52 cases showed Anti-HIV antibody positive, with a rate of 0.06 per thousand. Of the 52 cases, 2 died. Sequence detection and subtype analysis of PBMC in 24 infected people showed that there were five HIV-1 subtypes M: A, B, B', C, E which were from 7 cities of the province. Blood and sexual transmission took the majority with proportions, 59.62% and 30.76% respectively. The subtypes from 13 blood donors were all B'. Among 10 labourers refurned from abroad and 1 spouse six cases were C, two were B, and another two were A and E subtypes respectively. Epidemiologic data showed that most of the infected people were mobile, making sexual transmission easy to occur. CONCLUSION: The moods of HIV/AIDS transmission in Shandong were mainly through blood and sexual contacts.

Alleviation of pre-exposure to low-dose 16O8+ ion on mouse testicular histological damage induced by subsequent high-dose irradiation.

The testes of the B6C3F1 hybrid strain mice were irradiated with 0.05 Gy of 16O8+ ion as the pre-exposure dose (D1), and were then irradiated with 2 Gy of 16O8+ ion as challenging radiation dose (D2) at 4 h after per-exposure. Testicular morphology was observed by light microscope at 35th day after radiation. The results showed that irradiation of mouse testes with 2 Gy of 16O8+ ion significantly impaired, mainly reduction of tubule diameter and decrease or loss of germ cells in various developing stages, especially spermatogenic elements. Pre-exposure to a low-dose (0.05 Gy) of 16O8+ ion significantly alleviated above mentioned damage on testicular morphology induced by subsequent a high-dose (2 Gy) radiation.

[Expression of inducible nitric oxide synthase in asthmatic rat lung].

OBJECTIVE: In order to study the role of nitric oxide (NO) during the airway inflammation of asthmatic rat, we investigated expression of inducible nitric oxide synthase (iNOS) in the lung tissue of asthmatic rats. METHODS: Animal model of asthmatic Wistar rats was developed by being sensitized and challenged with ovalbumin. Activities of iNOS were measured by using SP immunohistochemical method. RESULTS: In asthmatic rat group (n = 10), lung tissue showed stronger immunostaining (90%) for iNOS than control group (20%) (P < 0.001). Immunostaining was mainly present in the bronchial epithelium, alveolar epithelium, bronchial smooth muscle, vascular endothelial and smooth muscle cells and inflammatory cells such as macrophages and neutrophils. In glucocorticoid treated group (n = 10), there was down regulation of iNOS immunoreactivity (30% positive) compared with control asthmatic rat group (20%). CONCLUSIONS: The increased expression of iNOS in asthmatic rat lung tissue showed NO playing a critical role in asthmatic inflammation, otherwise expression of iNOS was inhibited by glucocorticoid.

Import of precursor proteins into mitochondria from soybean tissues during development.

Characterisation of the amount of protein import of the alternative oxidase (AOX) and the F(A)d precursor proteins (previously shown to use different import pathways) into mitochondria from developing soybean tissues indicated that they displayed different patterns. Import of the AOX declined in both cotyledon and root mitochondria with increasing age, whereas the import of the F(A)d into cotyledon mitochondria remained high throughout the same period. Using primary leaf mitochondria, it was evident that import of AOX remained high while it declined in cotyledon and root mitochondria. The amount of import of the AOX into mitochondria from different tissues closely matched the amount of the Tom 20 receptor.

Enhanced neuronal expression of the oxidoreductase--biliverdin reductase--after permanent focal cerebral ischemia.

This is the first report on increased neuronal levels of biliverdin reductase (BVR) in response to ischemic brain injury. BVR is an oxidoreductase, and is unique among all enzymes characterized to date in having dual pH/dual cofactor requirements--NADH and NADPH at 6.7 and 8.7, respectively. BVR catalyses the final step in the heme metabolic pathway and reduces the heme degradation product, biliverdin, to bilirubin. Bilirubin can be both a neurotoxicant and an antioxidant depending on its ratio to protein and concentration. Bilirubin also has immunomodulatory activity. Other biologically active heme degradation products are iron and CO. This study assessed time-dependent changes in the level of BVR, following permanent middle cerebral artery occlusion (MCAo). It also examined correlation of the change in BVR expression with display of indices of ischemic tissue injury. Under halothane anesthesia and normothermic conditions, 72 DNX inbred mice were subjected to MCAo. A time-dependent enlargement of an ischemic lesion over the course of 24 h was observed and measured 55 +/- 5 mm3 at 6 h, 63 +/- 6.7 mm3 at 12 h, and 73 +/- 5 mm3 at 24 h. Six hours after MCAo, increased immunoreactivity for BVR was noted in neurons in the peri-ischemic areas, intraischemic cortical layers 3 and 5, as well as in neurons in regions distant from the borders of vascular distribution of the MCA, such as those in substantia nigra, in the Purkinje layer of the cerebellum and in the central nucleus of inferior colliculus. Twenty-four hours after MCAo, immunoreactivity for BVR remained increased in the peri-ischemia areas. At all time points staining for BVR was decreased in the ischemic core. At the 24 h time point there was an increase in Fe staining in the perimeter of the lesion and an increase in Schiff's staining for lipid peroxidation at the rim of the lesion. In situ hybridization analysis demonstrated a time dependent increase in BVR mRNA labeling in neurons of the peri-ischemic area. In the ischemic hemisphere, when compared with the contralateral hemisphere, neither measurable decreases in BVR mRNA or total protein levels nor a decrease in NADH-dependent BVR activity at pH 6.7 were observed. As judged by Northern and Western blots and activity analysis, despite the apparent loss of BVR from the ischemic core, and its increase in the peri-ischemic region, when compared with the contralateral hemisphere, the overall capacity of the ischemic hemisphere to catalyze the reduction of biliverdin was unchanged throughout the experiment. Should, in the case of ischemia, the conditions favor the antioxidant activity of bilirubin, then we suggest that increase in BVR expression in ischemic penumbra may present a cellular defense mechanism against free radical-mediated neuronal damage. Furthermore, we interpret the apparent tightly regulated expression of BVR in the ischemic hemisphere as an important factor in protection against bilirubin neurotoxicity. Data suggest that pharmacological modulation of BVR expression is a possible new direction for protecting neurons against ischemic injury and oxidative stress.

Calpain inhibitors protect auditory sensory cells from hypoxia and neurotrophin-withdrawal induced apoptosis.

Inhibitors of calpain have been shown to protect nerve growth factor (NGF)-deprived ciliary ganglion neurons and hypoxic cortical neurons. Calpains have been identified in the cochlea and are active during ischemic injury. Since apoptosis can be initiated by loss of neurotrophic support, hypoxia, and ototoxins (e.g., cisplatin, CDDP), the role of calpain inhibitors under these conditions was examined in auditory hair cells and neurons. Dissociated spiral ganglion neuron (SGN) cell cultures and organ of Corti explants from P3 rats were used to test the efficacy of calpain inhibitors as otoprotective molecules. Our results indicate that calpain inhibitor I, calpain inhibitor II, and leupeptin all provided significant protection of SGNs against neurotrophin-withdrawal and hypoxia-induced apoptosis. The increase in neuronal survival ranged from 2.16 to 2.31 times greater than in untreated neurotrophin-withdrawn SGN cell cultures. BOC-Asp(Ome)-Fluoromethyl Ketone (B-D-FMK), a general caspase inhibitor, increased neuronal survival 2.16 times more. Neuronal survival rates were from 1.88 to 2.27 times greater than in untreated, hypoxic neurons and hair cell survival rates were from 1.98 to 2.03 times greater than untreated, hypoxic organ of Corti explants. However, protection of auditory hair cells and neurons from CDDP-induced damage (10 and 6 micrograms/ml, respectively) was limited with any of these calpain inhibitors. Apoptotic pathways initiated by neurotrophin-deprivation and ototoxic stress (e.g., CDDP) have been shown to be different. Our results agree with this finding, with neurotrophin-withdrawal and hypoxia, but not CDDP damage-induced apoptosis being calpain-dependent.
