[Malignant eccrine poroma].

We report on a 82-year-old woman suffering from malignant eccrine poroma on her back. The infiltrated surrounding tissue suggested lymphangiosis carcinomatosa; one axillary lymph node was enlarged. Both features, however, turned out to be only of inflammatory nature.

Early alcoholic liver injury: changes of the Disse space in acinar zone 3.

One hundred and twelve consecutive liver biopsies were studied by transmission electron microscopy. In 18 out of 23 biopsies from chronic alcoholics showing preserved architecture by light microscopy, basal laminas were found in acinar zone 3 applied to endothelial cells as well as to hepatocytes. Basal laminas occurred independently of Mallory bodies and lipogranulomas, and were observed even in cases where light microscopy showed no fibrosis in zone 3. In addition to basal laminas, deposition of collagen and defenestration of the endothelial cells were observed. Basal laminas were seen only rarely in biopsies from non-alcoholics. We suggest that basal lamina formation in acinar zone 3 is initiated by changes in the perisinusoidal space and that it may contribute to impaired liver cell function and possibly initiate liver cell necrosis.

Brown bowel syndrome.

The brown bowel syndrome (BBS) is a rare disease characterized by malabsorption and accumulation of lipofuscin in the smooth muscle cells of the muscularis externa of the small intestine. Recently, we incidentally observed a case of BBS in a colon operated on because of a neoplasm. Our ultrastructural investigation, which demonstrated changes in the mitochondria, further supports the significance of mitochondrial damage in BBS.

Enzymatic purification of chemically synthesized oligodeoxyribonucleotides prior to removal from a solid-support.

A novel combined solid-phase DNA synthesis and solid-phase DNA purification strategy is described. Removal of all N-, cap-, and phosphate protecting groups while maintaining a 5'-blocking group on the final product followed by a 5'- specific exonuclease digestion of failure sequences with the synthetic DNA still attached to the solid phase yields an essentially pure oligomer.

Base modification and cloning efficiency of oligodeoxyribonucleotides synthesized by the phosphoramidite method: methyl versus cyanoethyl phosphorus protection.

We have found large oligodeoxyribonucleotides (50-120 bases) synthesized with N,N-dialkylmethylphosphoramidites to have considerably lower cloning efficiencies than biological DNA. As evidenced by HPLC analysis, these large fragments contain as much as 30% thymidine conversion to 3-methylthymidine. If cyanoethyl- instead of methyl-phosphorous protection is employed, as anticipated, no thymidine methylation is noted. More importantly, large fragments produced by N,N-diisopropylcyanoethyl-phosphoramidite coupling show cloning efficiencies indistinguishable from biological DNA.

Gas gangrene of the omental bursa following acute pancreatitis.

Acute pancreatitis with necrosis was followed by infection with Clostridium perfringens and formation of gas in the omental bursa. This could be seen preoperatively on an abdominal roentgenogram and was verified at operation. The patient recovered after debridement, drainage and irrigation and antimicrobial therapy.

Mediastinal yolk sac tumor: ultrastructural and immunofluorescent studies.

Primary mediastinal yolk sac tumor is rare. In this report, a case of a 32-year-old male with a mediastinal yolk sac tumor was studied by light, electron, and immunofluorescent microscopy. A positive immunofluorescent reaction to alpha-fetoprotein (AFP) correlated with the PAS-positive hyaline globules and electron-dense deposits seen in intracellular and extracellular locations. Serum AFP is a useful tumor marker for monitoring the results of therapy. Review of the literature indicates that the prognosis for patients with yolk sac tumor is generally poor.

Large-cell calcifying Sertoli cell tumor of the testis: a case report with ultrastructural study.

Large-cell calcifying Sertoli cell tumor (LCCSCT) is a rare histologic variant of Sertoli cell tumor. Recently we observed a case of LCCSCT of the testis with no associated endocrine abnormality. Our ultrastructural findings of rows of tight junctions, numerous intracytoplasmic filaments, and abundant rough endoplasmic reticulum in whorled formations support the Sertoli cell origin of this neoplasm.

Patterns of IgA deposition in liver tissues in alcoholic liver disease.

Observations in 136 liver biopsies from patients with alcoholic and nonalcoholic liver diseases reveal that IgA deposition in liver tissues appears to have a high degree of morphologic specificity for alcohol injury. Using a direct immunofluorescence technic with fluorescein-conjugated anti-IgG, anti-IgA, anti-IgM, and anti-C1q, four different staining patterns are recognized. These are labelled as "continuous," "discontinuous," "granular," and "pericellular" types depending on their morphologic characteristics and distribution patterns. Fifty of 64 biopsies from alcoholics showed a "continuous" pattern of anti-IgA activity while only three of 72 biopsies from nonalcoholics showed a similar pattern (P less than 0.001). A "pericellular" pattern of anti-IgA activity appears to indicate a more aggressive behavior of alcoholic liver disease. "Continuous" and "pericellular" patterns are seen in "chronic active hepatitis of alcoholics" but not in chronic active hepatitis in nonalcoholics. Anti-IgM activity appears to indicate chronicity of the disease process but does not have any specificity.

Synthesis of oligonucleotides on cellulose by a phosphotriester method.

The synthesis of oligothymidilic acids, (dT)m (where m = 4, 7, 10, 13, 16, 19, 22, and 25), was carried out using a solid phase approach in combination with the modified phosphotriester methodology developed in solution. Cellulose was used as the solid support after its functionalization with a specially featured dinucleoside diphosphate, 5'-0-p-chlorophenylphospho-2'(3')-0-acetyluridilyl-[2'(3')-3']-5'-0-dimethoxytritylthymidine p-chlorophenylester. The fully protected trideoxynucleoside triphosphate containing only thymidine was repeatedly used to elongate the oligonucleotide chain in the 3'-5' direction. Individual coupling yields ranged from 45% to 75%. The total time needed to prepare (dT)25 was four days. Similarly, the tridecanucleotide d(AGAAGGTACTTTT) was synthesized in good yield. The results show that this approach can be used for a fast and economic way to synthesize oligodeoxynucleotides.

Synthesis of oligonucleotides on cellulose. Part II: Design and synthetic strategy to the synthesis of 22 oligodeoxynucleotides coding for gastric inhibitory polypeptide (GIP).

A solid-phase approach using trinucleotide building blocks and functionalized cellulose as solid support was used to synthesize the 22 oligodeoxynucleotides that constitute a totally synthetic gene coding for a 43 amino acid peptide. Gastric inhibitory Polypeptide. The overall strategy used for the design and synthesis of the oligomers is described.
