[Effect of combination therapy with disodium cromoglycate (DSCG) and Ozagrel on non-atopic asthmatics].

Ozagrel (OKY-046), a selective inhibitor of thromboxane biosynthesis has been reported to reduce airway hyperresponsiveness in asthmatics. DSCG has been widely used for atopic, but not for non-atopic asthmatics. We evaluated the additive effect of combination therapy with DSCG and Ozagrel on nonatopic asthmatics. Fourteen asthmatics were divided into two groups. The patients in the O-D group were initially treated with Ozagrel (400 mg/day) for 4 weeks, and then DSCG (8 mg/day) was added for another 4 weeks. The patients in D-O group were initially treated with DSCG (8 mg/day), and then Ozagrel (400 mg/day) was added for 4 more weeks. TXB2, PGF2 alpha, PGE2, 6-keto-PGF1 alpha in sputum and 11-dehydro TXB2 in urine were measured before and after 4th and 8th weeks of therapy. Ozagrel decreased the metabolites of thromboxane and PGF2 alpha in sputum and urine. PGE2 in sputum tended to be increased by Ozagrel. DSCG did not affect mediators in sputum. Combination therapy with Ozagrel and DSCG showed the additive effect to single use on attack score and PF in non-atopic asthmatics.

Multiple systems for the biliary excretion of organic anions in rats: liquiritigenin conjugates as model compounds.

Liquiritigenin (LG), 2, 3-dihydro-7-hydroxy-2-(4-hydroxyphenyl)-(S)-4H-1-benzopyran-4-1, is metabolized to five kinds of conjugates (glucuronides and sulfates) that are excreted predominantly into the bile (Shimamura et al., 1993). Using LG as a model compound, we studied the multiplicity for the biliary excretion of conjugates in vivo. LG was administered i.v. as a bolus to Sprague-Dawley rats (SD rats) that received i.v. infusions of the inhibitors [glycyrrhizin (GR), dibromosulfophthalein (DBSP) and indocyanine green (ICG)] at their maximal transport rates. The infusion of GR and that of DBSP reduced significantly the biliary excretion of all the conjugates except LG 4',7-O-disulfate (M3), whereas infusion of ICG did not affect the excretion of conjugates. To minimize the effect of the other tissues on the disposition of the conjugates, the apparent biliary excretion clearance (CLbile,app) was calculated. The CLbile,app values of LG 4'-O-glucuronide (M1), LG 7-O-glucuronide (M2), LG 4'-O-glucuronide 7-O-sulfate (M4) and LG 7-O-glucuronide 4'-O-sulfate (M5) were significantly reduced by GR and DBSP infusion, whereas the CLbile, app value of M3 was not affected by these inhibitors. The CLbile, app values of all the conjugates except M2 were not reduced by ICG infusion. In Eisai hyperbilirubinemic rats, which have a hereditary defect in the canalicular transport system for several organic anions, the biliary excretion clearance of M1, M2, M4 and M5 was markedly reduced, whereas that of M3 was comparable with that in control rats.(ABSTRACT TRUNCATED AT 250 WORDS)

[Tracheal rupture in a patient intubated with a double-lumen endobronchial tube].

We report a case of tracheal rupture associated with use of a double-lumen endobronchial tube. The patient was a 58-year-old woman with metastatic carcinoma of the right upper lung lobe. Her trachea was intubated easily with a left-sided double-lumen endobronchial tube (Broncho-Cath, #35Fr). She underwent right upper lobectomy under general anesthesia using isoflurane (0.4-1.0%) and nitrous oxide (50%) in oxygen. When the lungs were inflated before closure of the chest, an air leak at the membranous part of the trachea was detected. A tracheal rupture, beginning 3 cm above the carina and 7 cm long, was noted at the membranous part of the trachea, and the cuff protruded partially from the ruptured trachea. The trachea was sutured, and a tracheostomy was carried out. Mechanisms of tracheal rupture related to double-lumen endobronchial tubes are discussed.

[The role of parasympathetic nerve on hyperventilation-induced bronchoconstriction in sensitized rabbits].

We have demonstrated that hyperventilation-induced bronchoconstriction (HIB) is observed in sensitized rabbits, and that histamine may play a role in HIB. To investigate the role of parasympathetic neurotransmission in HIB, the effects of atropine and vagotomy on HIB were examined. Japanese white rabbits were sensitized with ovalbumin. They were then anesthetized and paralyzed with pancuronium, and then mechanically ventilated (TV: 7 ml/kg, RR: 40/min). The rabbits were divided into the following three groups: I) control group (n = 7), II) atropine-treated group (n = 6), III) bilateral vagotomy group (n = 6). Hyperventilation was performed at TV: 7 ml/kg, RR: 120/min for 15 min in dry room air containing 5% CO2. Total lung resistance (RL) and dynamic compliance (Cdyn) were measured before and at 0, 5, 15 and 30 minutes after hyperventilation. RL increases significantly at 5 minutes in group 1, but this change was significantly less marked in groups 2 and 3. However Cdyn was not significant change in each group. We conclude that parasympathetic neurotransmission may play an important role in HIB of sensitized rabbits.

[Heerfordt syndrome].

The Heerfordt syndrome is characterized by fever, uveitis, swelling of parotid gland and facial nerve palsy, and 39 cases have been reported in Japan until 1993'. In the present review, we mainly focused on those clinical cases reported. Most patients were between 20 and 40 years of age, and females appeared to have greater risk than males. The definite diagnosis of this syndrome is established histologically according to sarcoidosis. In addition 67 gallium scan is helpful for diagnosis, and it shows increased uptake of Ga to the ophthal lesions, parotid glands and hilar lesions. Therapeutic trial with prednisolone is sometimes required especially for facial palsy.

[A case of giant bulla which ruptured and disappeared on chest roentgenogram].

We experienced a case of progressive giant bulla which ruptured and disappeared on chest roentgenogram. The patient was a 60-year-old male who had been treated with home oxygen therapy for chronic pulmonary emphysema. One year after initiating home oxygen, emphysematous bulla occurred and expanded to become giant bulla which occupied 3/4 of the right hemithorax. Although we attempted to persuade him to undergo surgery for bullectomy, he refused. While being followed as an outpatient, sudden right anterior chest pain occurred, and dyspnea was markedly alleviated at the same time. Chest roentgenogram revealed right pneumothorax and pleural effusion, and the giant bulla subsequently receded. The patient has been stable for the approximately one year period since, without evidence of recurrence. It is rare for giant bullae to cause a pneumothorax. In addition, there are no previous reports in the literature with a clinical course such as that experienced by our patient.

[Analysis of factors for the development of pulmonary tuberculosis in persons with collagenosis].

Recently, the annual reduction in the incidence rate of tuberculosis has slowed in Japan. One reason for this trend is believed to be due to an increase in the number of immunocompromised hosts. In our department, we discovered 10 cases of pulmonary tuberculosis among 962 cases of collagenosis, and have analyzed the factors for the development of pulmonary tuberculosis in these patients. A total of 29 patients wer involved in the study: 22 with systemic lupus erythematosus (SLE) (active disease, 10; inactive disease, 12) and having no pulmonary complications, and seven with pulmonary tuberculosis and no concomitant diseases. Our findings were as follows: 1. Seven of the 10 patients with pulmonary tuberculosis also suffered from SLE. 2. Nine of the 10 patients had been treated with a corticosteroid or immunosuppressant. 3. Serum CH50 and erythrocyte sedimentation rate (ESR) were valuable indicators for diagnosing pulmonary tuberculosis in the patients with SLE. 4. Patients with SLE and pulmonary tuberculosis tended to show lymphopenia in peripheral blood, which was further enhanced by prolonged use of steroids. 5. Miliary tuberculosis rather than pulmonary tuberculosis tended to develop in elderly patients, and required longer hospitalization.

[Inhibition by sedatives of compensatory vasoconstriction during lumbar epidural anesthesia].

Compensatory vasoconstriction in an upper extremity during lumbar epidural anesthesia contributes to the stability of hemodynamics. We measured blood flow of an index fingertip (FBF) with laser-Doppler flowmetry, surface temperature of a middle fingertip (FT), deep body temperature of the forehead (HDT) and a big toe (BDT) by an insulated thermistor probe of zero thermal flow between skin and subcutaneous tissue, and mean arterial pressure (MAP) in 16 patients undergoing elective lower abdominal surgery under lumbar epidural anesthesia. Epidural puncture was done at L1-2 (n = 2), L2-3 (n = 8) or L3-4 (n = 6), and the initial dose of 13 +/- 1 ml (mean +/- SD) of 2% plain mepivacaine (n = 14) or 2% plain lidocaine (n = 2) was injected five minutes after the test dose of 3 ml. Five minutes after the initial injection, the hypesthesia region of cold sensation examined by alcohol absorbent cotton was T9 +/- 3 (range, T2-T12) at the upper limit and S2 +/- 2 (range, L2-S3) at the lower limit. After determination of the hypesthesia region, a combination of butorphanol 1.0 +/- 0.1 mg and midazolam 2.0 +/- 0.9 mg or diazepam 7.5 +/- 2.5 mg alone was administered intravenously. The difference between HDT and BDT decreased significantly from 6.4 degrees C (P < 0.01) at the time of test dose injection to 0.4 degrees C (P < 0.001) 20 minutes after the initial dose injection of local anesthetics. FBF showed a twofold significant increase (P < 0.01) 20 minutes after the initial dose injection.(ABSTRACT TRUNCATED AT 250 WORDS)

Determination of a novel beta-lactam antibiotic (E-1100) in rat plasma by high-performance liquid chromatography.

A fast, simple and accurate method to determine the concentration of E-1100, a novel beta-lactam antibiotic, in rat plasma has been developed. This method involves deproteinization by methanol with 0.2% perchloric acid and reversed-phase high-performance liquid chromatography with ultraviolet detection. Regression analysis showed that the method was linear over the standard-curve range from 0.191 to 191.0 micrograms/ml. This newly developed method has been applied for the analysis of plasma samples in a preliminary pharmacokinetic study in rats.

In vivo proton magnetic resonance spectroscopy study on premature aging in adult Down's syndrome.

Proton magnetic resonance spectroscopy (1H-MRS) was performed in a group of 18 adult patients with Down's syndrome (DS) aged 20-46 years, and the peak area ratios (NAA/Cr, Cho/Cr, NAA/Cho) of N-acetylaspartate (NAA), total creatine (Cr), and choline-containing compounds (Cho) calculated separately in the patients in their 20's, 30's, and 40's. In age-matched healthy control groups, there were no significant age-related changes in any of the peak area ratios. In contrast, in the DS group, although the relative amount of NAA (NAA/Cr) showed no significant change with increasing age, the relative amount of Cho (Cho/Cr and NAA/Cho) was significantly increased in the 40's group. At least as judged by MRI, few age-related general morphological changes such as brain atrophy were apparent in the third, fourth, and fifth decade groups. However, the MRI findings considered together with the age-related changes in the peak area ratios suggest that in DS patients in the fifth decade metabolic abnormalities such as degradation and/or rapid synthesis of brain cell membrane may occur prior to neuronal loss and degeneration.

Lipid peroxidation in rat liver microsomes during naproxen metabolism.

Naproxen, a non-steroidal anti-inflammatory drug, is known to be highly effective and relatively safe, but some side-effects in the liver have been reported. In the present study, the effect of naproxen metabolism on rat liver microsomes was studied by determining lipid peroxidation in terms of thiobarbituric acid reactive substances (TBA-RS), high molecular weight protein aggregates and fluorescent substances formed in the microsomal suspension containing naproxen, NADPH and MgCl2. Lipid peroxidation was found to occur at 10 mM naproxen. Production of chemiluminescence from the microsomal suspension was observed during naproxen metabolism. The time course of 6-demethyl-naproxen formation by O-demethylation of naproxen appeared to be comparable to that of the chemiluminescence production in their initial periods of production. These results suggest that the lipid peroxidation was provoked through the reactive oxygen species generated during the oxidative metabolism of naproxen.

Purification and characterization of two forms of hepatic microsomal cytochrome P450 from untreated cynomolgus monkeys.

Two forms of cytochrome P450, referred to as P450 CMLb and P450 CMLc, were purified and characterized from hepatic microsomes of untreated cynomolgus monkeys (Macaca irus). The final preparations were apparently homogeneous as judged by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The minimum molecular weights of P450 CMLb and P450 CMLc estimated from the mobilities on the gel were 48 and 50 kDa, respectively. The N-terminal amino acid sequences of P450 CMLb and P450 CMLc (first 13 residues) were identical, respectively, with those of P450 FI isolated from baboons and P450 MK-2, which has been characterized as a P450 3A enzyme in cynomolgus monkeys. P450 CMLb showed coumarin 7-hydroxylation and 7-ethoxycoumarin O-deethylation activities. This P450 enzyme reacted with anti-P450 2A6 antibody and acts as a coumarin 7-hydroxylase in hepatic microsomes. On the basis of these results P450 CMLb was classified into the 2A subfamily. P450 CMLb is expressed constitutively as one of the minor forms of P450 in liver microsomes of untreated cynomolgus monkeys and is inducible by phenobarbital. P450 CMLc reacted with anti-P450 3A4 antibody and hydroxylated testosterone at the 6 beta-position. The testosterone 6 beta-hydroxylation activities in hepatic microsomes of cynomolgus monkeys, common squirrel monkeys, and humans were strongly inhibited by the anti-P450 CMLc antibody. These results demonstrate that this P450 enzyme is in the P450 3A subfamily.

Effect of vitamin A on methotrexate cytotoxicity in L1210 murine leukemia cells in culture.

Vitamin A (VA) protects the small intestine from methotrexate (MTX)-induced damage. However, before VA can be used as a remedy to protect cancer patients from MTX-induced damage to the intestine, it is essential to clarify whether or not it disturbs the antitumor activity of MTX. This study investigated the effect of VA on the antitumor activity of MTX in vitro in L1210 murine leukemia cells. The incorporation of [6-3H]-thymidine and [6-3H]-uridine, [5-3H]-uridine, and [4,5-3H]-leucine into DNA, RNA, and proteins, respectively, was examined to evaluate this effect. The incorporation of thymidine, the uridines, and leucine decreased dose-dependently in MTX-treated L1210 cells and profoundly in the MTX plus VA-treated L1210 cells, since VA itself had a cell-killing activity. Thus, MTX depressed the growth of L1210 cells dose-dependently and this depression was not affected by the presence of VA. The present study proved in L1210 murine leukemia cells in vitro that VA did not disturb the antitumor activity of MTX.

Effect of coexisting cells on the NGF-inducing neurite growth from PC12h-R.

Possible roles of coexisting cells in inducing neurite growth from a nerve cell were studied. Nerve growth factor (NGF)-inducing neurite growth from PC12h-R (a cell line derived from cultured nerve cells) was investigated at various cell densities. At the cell density 10(2)-10(4) cells/ml neurites appeared even without NGF. In contrast, no neurite appeared without NGF in single cell culture. The neurite growth observed in plural cell culture without NGF was only partially inhibited by antibody to NGF receptor (Ab-NGFR). However, the effect of the used medium alone was mostly inhibited by Ab-NGFR. These results suggest that the neurite inducing potency of coexisting cells is via different sites than the NGF receptor.

Vitamin A, a useful biochemical modulator capable of preventing intestinal damage during methotrexate treatment.

The effects of vitamin A on methotrexate treatment of mice inoculated with L1210 leukaemia cells or sarcoma 180 cells were studied. The small intestine of tumour transplanted mice was severely damaged after methotrexate treatment. Coadministration of vitamin A with methotrexate protected the small intestine from methotrexate-induced damage. The protective effect of vitamin A was histologically and biochemically demonstrated. Further, coadministration of vitamin A did not inhibit in vivo antitumour activity of methotrexate. Thus, biochemical modulation used to prevent methotrexate-induced malabsorption by vitamin A coadministration will be of great use in methotrexate cancer chemotherapy.

Interstitial pneumonitis possibly due to mitoxantrone.

A 41-year-old patient with chronic myelogenous leukemia in the accelerated phase was treated with mitoxantrone. She developed pyrexia 7 days after receiving the third administration of mitoxantrone. After 3 more days, she experienced dry cough and dyspnea. Bilateral fine crackles were audible, but no signs of heart failure were found. A chest X-ray film revealed diffuse reticulogranular infiltrates bilaterally. An increase in the prednisolone dosage led to an improvement. Specimens of the bronchoalveolar lavage revealed an increase in CD4-/CD8- lymphocytes. The peripheral lymphocytes also expressed neither CD4 nor CD8. Specimens of a transbronchial lung biopsy disclosed thickening of the alveolar wall with infiltration of lymphoid cells.
