Prospective study of relations between cortical Lewy bodies, poor eyesight, and hallucinations in Alzheimer's disease.

The presence of hallucinations is included in some, but not all, of the sets of clinical diagnostic criteria that have been proposed for dementia associated with cortical Lewy bodies. These criteria were developed from retrospective casenote analyses. This prospective, longitudinal study suggests that, in patients with Alzheimer's disease, cortical Lewy bodies are associated with more persistent and severe hallucinations, independently of any association with severity of cognitive decline. Poor eyesight contributes to the severity but not the persistence of the hallucinations.

Ethics of treatment and research in psychiatry.

Most of the publications in the year under review have centered around dilemmas, particularly ethical problems in clinical practice. It has not, however, been a period of systematic theoretical work. In this review, a range of dilemmas which arise in clinical practice are discussed.

The Oxford Practice Skills Project: teaching ethics, law and communication skills to clinical medical students.

We describe the teaching programme in ethics, law and communication skills for clinical medical students which is being developed as part of the Oxford Practice Skills Project. These three elements of practice are approached in an integrated teaching programme which aims to address everyday clinical practice. The role of a central value of patient-centred health care in guiding the teaching is described. Although the final aim of the teaching is to improve actual practice, we have found three 'sub-aims' helpful in the development of the programme. These sub-aims are: increasing students' awareness of ethical issues; enhancing their analytical thinking skills, and teaching specific knowledge.

"Not clinically indicated": patients' interests or resource allocation?

The decision that a particular intervention is not clinically indicated may conceal two quite different ethical assumptions. The first assumption is that the intervention is not of overall benefit to this patient. The second is that limited resources should not be used for this patient. These issues are discussed with reference to cardiac surgery in elderly patients with reference to the main theories of allocation: QALYs, needs theories, the sanctity of life theory, the lottery theory, and market forces.

Crying following stroke. A report on 30 cases.

Following stroke, many patients experience a heightened tendency to cry. The aim of the present study was to provide a detailed description of this behavior and its accompaniments. Thirty stroke patients who had cried in the previous month were studied using a detailed structured clinical interview. There was a wide range of frequency of crying. The components of crying combine in a more complex and varied way than commonly held stereotyped views suggest. No distinct subtypes were identified. Further research is required to determine the relationship between emotionalism and the frequency and severity of symptoms of depression.

The Present Behavioural Examination (PBE): the development of an interview to measure current behavioural abnormalities.

Behavioural problems are an important feature of dementia and other neuropsychiatric conditions. This paper describes the development and performance of an investigator-based interview (the PBE) for the detailed assessment of the behaviour of subjects over the preceding four weeks. The interview is designed to be administered to carers. A test-retest reliability study was carried out on the penultimate edition of this interview; and the inter-rater reliability of the final edition was measured. These studies show that the reliability of the PBE is comparable with that of other investigator-based interviews.

Primary non-Hodgkin's lymphoma of the central nervous system: phase II study of chemotherapy (BVAM) prior to radiotherapy.

Ten patients were diagnosed as having primary non-Hodgkin's lymphoma of the central nervous system at University Hospital, Nottingham, between September 1986 and April 1989. None had clinical evidence of HIV-1 infection. All the patients started treatment with chemotherapy (BVAM), designed to cross the blood-brain barrier, followed by radiotherapy. Seven patients completed both chemotherapy and radiotherapy. Dose reduction during chemotherapy was necessary in three patients because of neutropenia. In two of the six patients with solitary tumours, complete resection was achieved surgically prior to treatment. Five of the remaining eight patients (63%) had radiological evidence of a complete response with chemotherapy. The other three patients had no response to chemotherapy but one had a complete response after radiotherapy. The two-year cause-specific survival of the 10 patients was 37%. Two of the three patients who had a postoperative performance status of 0 or 1 (ECOG/WHO) are alive and disease-free at 26 and 46 months from diagnosis. The median survival of the seven patients with a performance status of 2-4 was 10 months with two patients alive and disease-free at 19 and 26 months. The two-year cause-specific survival of these seven patients was 22%.

Giant intracranial aneurysms: diagnosis with special reference to computerised tomography.

The clinical presentation, radiological investigations and surgical management of 11 patients with giant intracranial aneurysm (greater than 2.5 cm) have been reviewed. Most patients had signs and symptomd haemorrhage. The most helpful plain film finding was intracranial calcification. This varied from a mere fleck to the classical curvilinear variety. Computerised tomography (CT) scans were available on all 11 patients. All showed a space-occupying lesion of high density. Two types of enhancement were seen following intravenous contrast medium, (a) homogeneous and (b) rim enhancement with or without a patchy increase in density centrally. The CT appearances in some of the cases resembled those shown by other lesions such as neoplasm. Arteriography in almost all cases demonstrated the aneurysm but often under-estimated its size. On two occasions the aneurysm did not fill and the nature of the lesion demonstrated on the plain CT remained unconfirmed. Direct surgical attack rather than carotid ligation was the operation of choice. Most patients responded very well to this treatment.

Results of 10 years of supervised fluoride toothbrushing in Rygge, Norway.

At the annual examination in 1978 of 141 patients born in 1962, DMFS, sound teeth and the number of erupted permanent teeth were registered. The number of filled tooth surfaces per child per year was compared for the period 1967--78. The average number of erupted teeth in 1978 was 27.3 of which 69 were sound (mean = 18.9). The average DMFS score was 14.3 and decayed surfaces equaled 2.0. The mean number of filled tooth surfaces per year dropped from 6.1 in 1968 to 1.2 in 1978. The need for annual recall and examination routines at the dental clinics are questioned. More attention should be paid to preventive care.
