Eosinophilia associated with chronic pancreatitis: an analysis of 122 patients with definite chronic pancreatitis.

Among 122 patients with chronic pancreatitis, marked eosinophilia (greater than 500 eosinophils/mm3 in the peripheral blood) was observed in 21 cases (17.2%). All of the affected patients were males, and there was no significant difference in the incidence of eosinophilia between patients with alcoholic and nonalcoholic pancreatitis. In the patients with eosinophilia, endocrine pancreatic function was maintained comparatively well, despite marked exocrine pancreatic dysfunction. The eosinophilia of chronic pancreatitis frequently developed in association with severe damage to neighboring organs (pleural effusion, pericarditis, and ascites), as well as in association with pancreatic pseudocyst. Our findings suggest that there is a close correlation between marked eosinophilia and severe tissue injury during acute exacerbations of chronic pancreatitis.

[Clinical use of the measurement of functional residual capacity during non-panting breathing--study on healthy subjects and respiratory patients].

The functional residual capacity (FRC) has been measured by gas dilution technique (GA) and body plethysmographic technique (BP) using the panting maneuver (PA). However, this maneuver is difficult to perform for patients who experience discomfort in breathing, and in cases of chronic obstructive pulmonary diseases, the FRC value measured by panting maneuver overestimates the true FRC value. Thus, in order to minimize these factors, we measured FRC during non-panting breathing (NP) using a BP device (BX-82, Minato Co., Osaka) and compared the results with the usual two methods. One hundred healthy subjects (Group I), 72 patients with restrictive ventilatory disorder (Group II, %VC = 62.7%) and 66 patients with pulmonary emphysema (Group III, FEV1.0% = 44.9%) were studied. All measurements were performed under 0.5 Hz respiratory frequency. The non-panting FRC measurement was performed by closing the mouth shutter of the BP for about 500 msec at the end of both the expiratory and inspiratory phase. From the box volume change and mouth pressure change, lisajous curves were formed and fitted by linear regression method. From this regression line, the volume of FRC was calculated. The obtained data had no significant differences among them, and there was a significant relationship between each technique. The correlation coefficient of non-panting breathing technique in Group III was slightly poor compared with the other techniques, and we therefore tried to re-classify Group III into small groups according to the severity of obstruction. It was subsequently found that this technique did not have the tendency to give a higher FRC value with increase in obstructive disorder.(ABSTRACT TRUNCATED AT 250 WORDS)

[A study on a postoperative change of beta-2-microglobulin in surgical patients].

Various renal tubular functions were monitored perioperatively in 42 surgical patients and the validity of beta-2-microglobulin (BMG) was evaluated. NAG index (urine NAG titer/urine creatinine), the absolute value of free water clearance and fractional excretion of sodium were worse at 3 to 5 hours after the beginning of operation, indicating the latent renal tubular damage took place during operation. On the other hand, fractional excretion of BMG (FE-BMG; BMG clearance/creatinine clearance) slowly elevated after operation and reached a peak on the 2nd postoperative day. The postoperative change of C-reactive protein (CRP), was similar with that of FE-BMG, showing a significant correlation (r = 0.716, p less than 0.001). The close relationship between CRP and FE BMG was confirmed in 26 patients with or without abnormal liver function who underwent abdominal operation. The peak value of FE BMG was significantly (p less than 0.01) lower in 5 patients with ICG 15' greater than 25% than in 21 patients with ICG 15' less than 20%. Similarly, the peak value of CRP tended to be lower in the former group. In conclusion, FE BMG can not be employed as an indicator of renal tubular function during postoperative period. It reacts as an acute phase reactant like CRP and is probably produced in the liver.

Time-dependent biphasic regulation of Na+/K+/Cl- cotransport in rat glomerular mesangial cells.

Time-dependent regulation of loop diuretic-sensitive Na+/K+/Cl- cotransport and [3H]bumetanide binding was investigated in cultured rat glomerular mesangial cells. Angiotensin II or epidermal growth factor induced stimulation of Na+/K+/Cl- cotransport within 5 min, with a return to the control values by 30 min. Treatment of cells with phorbol 12-myristate 13-acetate (0.1 microM) (PMA), the calcium ionophore A23187 (1 microM), or the combination of 5 mM NaF and 10 microM AlCl3 produced a transient stimulation of Na+/K+/Cl- cotransport in 5-10 min to 148, 135, and 163% of control, respectively, which was followed by a progressive decrease to 34, 64, and 20% of the base-line activity, respectively, by 60 min. Exposure to cyclic 8-bromo-AMP (0.1 mM) or to forskolin (1 microM) and isobutylmethylxanthine (0.1 mM) caused a maximal inhibition of the cotransport in 5 min to 79 and 60% of control, respectively, with a subsequent gradual increase to 137 and 164% of the base-line activity, respectively, by 60 min. The effects of PMA, forskolin, and cyclic 8-bromo-AMP were concentration-dependent. In order to characterize further the alterations in the cotransport activity, binding of [3H]bumetanide was determined. Saturation binding analyses showed that the late inhibition of the cotransport by PMA and stimulation by forskolin were associated with a significant decrease and increase, respectively, in Bmax, with no significant changes in binding affinity. Correlations between changes in the cotransport activity and [3H]bumetanide binding were also observed in cells treated with cyclic 8-bromo-AMP or with NaF and AlCl3. Incubation of cells in Cl- or Na+ free solution greater than or equal to 60 min resulted in an increase in both the cotransport activity and [3H]bumetanide binding. These observations indicate that, in glomerular mesangial cells, persistent stimulation of second messengers that regulate the cotransporter induces a time-dependent, biphasic regulation of Na+/K+/Cl- cotransport and that the regulation occurring after greater than or equal to 60 min of treatment is primarily due to changes in the number of the active cotransport sites. Because long term removal of the transported ions also increases the number of active cotransport sites, these results suggest that alterations in intracellular ionic homeostasis may also mediate cotransport activity.

The study of the mass screening of persons without symptoms and of the screening of outpatients with gastrointestinal complaints or icterus for pancreatic cancer in Japan, using CA19-9 and elastase-1 or ultrasonography.

To investigate the possibility of detecting carcinomas of the pancreas at an early stage by mass screening of persons without symptoms or by screening the outpatients with gastrointestinal complaints or jaundice, we performed a multicentral study of the mass screening of 10,162 persons at five local areas and of the outpatient screening of 4506 at 17 hospitals for pancreatic cancer in Japan for two years from 1984 to 1985, using serum CA19-9 and elastase-1 determinations or ultrasonography. Mass screening of 10,162 persons over 40 years old found only four (0.04%) cases of pancreatic cancer, including one case that was curatively treated. According to the screening of 4506 outpatients with gastrointestinal complaints or icterus, 85 (1.9%) patients were found to have pancreatic cancer. Of these 85 patients, 28 could undergo curative treatment. In addition to them, 73 (1.6%) patients with other digestive organ cancer were found. Our results suggest that the mass screening of persons without symptoms is not worthwhile in the early detection of pancreatic cancer, but outpatient screening is useful for detecting curative cancers of the pancreas.

Evaluation of the reticuloendothelial system function by the vascular clearance of chondroitin sulfate iron colloid.

Vascular clearance of chondroitin sulfate iron colloid (CSFe) was evaluated as a test for the reticuloendothelial system (RES) in rabbits. Injected CS59Fe was taken up by the RES in the liver (49%) and bone marrow (41%) after 60 min, suggesting its application for the RES function test. The clearance rate (K-value) of CSFe from the blood was calculated by measuring serum Fe concentrations after releasing iron from CSFe at certain intervals after injection. Depending upon different injected doses, K-values were varied and the phagocytic velocity, computed by multiplying K-values by corresponding injected doses, reached a plateau at high doses, indicating the phagocytosis of CSFe by the RES takes a saturation process. Double-reciprocal plotting of the dose and the phagocytic velocity showed a linear relationship, which provided the data on the maximum phagocytic velocity (Vmax), 0.122 mg/kg/min, and the CSFe concentration producing 1/2 Vmax (Kp), 0.426 mg/kg. Thus, this CSFe clearance test can be used for the evaluation of the RES function.

Elevated serum levels of Dupan-2 in pancreatic cancer patients negative for Lewis blood group phenotype.

CA19-9, a serum marker for pancreatic cancer, gives false-negative results in patients who are negative for the Lewis blood group phenotype. To determine whether other markers may compensate for this drawback, serum levels of CA50, Span-1, sialyl SSEA-1 and Dupan-2 were assayed and compared with those of CA19-9 in 207 normal subjects and in 200 patients with pancreatic carcinoma whose Lewis blood group phenotypes were confirmed. In normal subjects with the Lewis negative phenotype, the serum levels of CA50 and Span-1, as well as CA19-9, were significantly low, whereas those of sialyl SSEA-1 were independent of the Lewis blood group phenotype. Serum levels of Dupan-2 were significantly higher in normal subjects with the Le (a-b-) phenotype as compared with those with Le(a-b+). The sensitivity for pancreatic carcinoma was 81% for CA19-9, 84% for CA50, 82% for Span-1, 51% for sialyl SSEA-1 and 63% for Dupan-2. Among the 39 CA19-9 negative patients, 13 were determined as being Lewis negative by the serum dot-ELISA technique. Although the positive rates were essentially comparable when each marker was combined with CA19-9, a highly elevated serum level of Dupan-2, which strongly suggested the presence of malignancy, was most frequently encountered in 39 patients who were not diagnosed by CA19-9 assay, especially those with Lewis negative blood groups. With regard to the three other markers, we found few patients with a highly elevated serum level in either the Lewis-negative or -positive groups. We conclude that Dupan-2 tended to be elevated in patients with pancreatic cancer who were negative for the Lewis blood group phenotype.

Magnetic resonance imaging and histologic study of hypertrophic cervical posterior longitudinal ligament. A case report.

Magnetic resonance imaging and histologic features of hypertrophic cervical posterior longitudinal ligament were determined in a cervical myelopathy patient. On T1-weighted images, the thickened ligament and the prolapsed nucleus pulposus showed isointensity shadows, making it impossible to differentiate these tissues. On T2-weighted images, the thickened ligament and prolapsed nucleus pulposus were visualized at high and low intensity, respectively. Sagittal T1-weighted images after gadolinium-diethylene triamine pentaacetic acid injection showed obvious enhancement of the thickened ligament.

The Steffee variable screw placement system using different methods of bone grafting.

Two groups of patients with diseased lumbar spines treated by the Steffee variable screw placement system were studied, in order to compare concomitant posterolateral fusion and posterior lumbar interbody fusion. The use of posterolateral fusion resulted in less invasiveness than posterior lumbar interbody fusion, with regard to operating time and blood loss. With the posterolateral fusion method, correction of olisthesis or kyphotic deformity, if attained at all, was difficult to maintain. Posterior lumbar interbody fusion produced better correction and maintenance, even in osteoporotic cases. Bone union was achieved at an average of 11 months in only 60% of the posterolateral fusion group, whereas it was complete within a significantly shorter period (average; 6 months) in 91% of the posterior lumbar interbody fusion group. Breakage or loosening of screws occurred in 14% of the posterolateral fusion group. Deep wound infection and adhesive arachnoiditis, which were never seen in the posterolateral fusion group, developed in 3% of the posterior lumbar interbody fusion group. Posterior lumbar interbody fusion is considered to be a better method of bone grafting than posterolateral fusion, when used with the Steffee variable screw placement system.

Regulation of Na(+)-H+ exchange by ATP depletion and calmodulin antagonism in renal epithelial cells.

The present studies examined effects of ATP depletion and calmodulin antagonism on stimulation of Na(+)-H+ exchange by cytosolic acidification in renal epithelial cells (LLC-PK1). ATP depletion significantly inhibited both amiloride-sensitive 22Na+ uptake (P less than 0.001; n = 12) and Na(+)-dependent intracellular pH (pHi) recovery in 2',7'-bis (carboxyethyl)-5(6)-carboxyfluorescein acetoxymethylester (BCECF/AM)-loaded cells. Calmodulin antagonists, N-(6-aminohexyl)-5-chloro-1-naphthalene-sulfonamide (W-7) and calmidazolium, both caused a concentration-dependent inhibition of Na(+)-H+ exchange activity. The W-7-induced inhibition of Na(+)-H+ exchange occurred in cells incubated for 24 h with phorbol 12-myristate 13-acetate, indicating that the effect of W-7 was not mediated by protein kinase C inhibition. Both W-7 and ATP depletion shifted the pHi dependence of the antiporter, and ATP depletion also reduced the maximal activity. In LLC-PK1/CL4 cells grown on permeable filters, W-7 inhibited the cytosolic acidification-stimulated basolateral exchanger by 54 +/- 5% (P less than 0.005; n = 7) and, in contrast, stimulated the apical exchanger by 28 +/- 13% (P less than 0.05; n = 6). ATP depletion significantly inhibited apical Na(+)-H+ exchange. These results suggest that an ATP-Ca(2+)-calmodulin-dependent process is involved in regulation of Na(+)-H+ exchange in LLC-PK1 cells. A Ca(2+)-calmodulin-dependent process activated the amiloride-sensitive basolateral Na(+)-H+ exchanger and inhibited the amiloride-resistant apical antiporter. Phosphorylation of these two Na(+)-H+ exchangers or regulatory proteins by a Ca(2+)-calmodulin-dependent protein kinase may mediate this differential regulation.

Cytosolic acidification stimulates a calcium influx that activates Na(+)-H+ exchange in LLC-PK1.

We have previously shown that cytosolic acidification-stimulated Na(+)-H+ exchange in LLC-PK1 cells is inhibited by calmodulin antagonists. To investigate further the role of Ca(2+)-calmodulin-dependent processes in intracellular pH (pHi) regulation in these cells, we studied the effects of pHi changes on cytosolic Ca2+ concentration ([Ca2+]i). In fura-2/acetoxymethylester (fura-2/AM)-loaded cells maintained in isotonic Na(+)-free buffer containing 1.8 mM CaCl2, [Ca2+]i was 168 +/- 59 nM (n = 5). After NH4Cl-induced alkalinization, [Ca2+]i decreased to 83 +/- 28 nM and partially recovered to 126 +/- 42 nM. Cytosolic acidification, after NH4Cl washout, caused an increase in [Ca2+]i to 481 +/- 166 nM (P less than 0.05; n = 5) that was dependent on extracellular Ca2+. An increase in [Ca2+]i was also observed in cells acidified with KCl-nigericin, with a return of [Ca2+]i to baseline with cell alkalinization. No increase in 45Ca2+ efflux occurred in association with initial NH4Cl-induced [Ca2+]i decrease, suggesting Ca2+ flux into an intracellular store during alkalinization. Membrane depolarization did not alter [Ca2+]i. The acidification-induced [Ca2+]i increase was inhibited by preincubation with verapamil or the calmodulin antagonist N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7). Na(+)-dependent pHi recovery in 2,'7'-bis(carboxyethyl)-5(6)-carboxyfluorescein acetoxymethylester (BCECF/AM)-loaded LLC-PK1 cells and cytosolic acidification-stimulated basolateral Na(+)-H+ exchange activity in LLC-PK1/CL4 cells were both attenuated in the absence of extracellular Ca2+. The results indicate that cytosolic acidification activates an influx of extracellular Ca2+ in LLC-PK1 cells. Furthermore, in the absence of Ca2+ influx, Na(+)-H+ exchange is inhibited.(ABSTRACT TRUNCATED AT 250 WORDS)

Ramification pattern of intermetacarpal branches of the deep branch (ramus profundus) of the ulnar nerve in the human hand.

The branching pattern of the deep branch (ramus profundus) of the ulnar nerve and its relation with the target were analyzed in the human hand by an improved dissection method. After sending off branches to the hypothenar muscles, the r. profundus branched off an ulnar stem and a radial stem to the fourth, to the third and to the second intermetacarpal spaces, respectively, in this order, and an ulnar stem to the first to become terminal branches. The ulnar stems included an ulnar interosseous branch and a superficial articular branch in addition to a lumbrical branch in the third and fourth intermetacarpal spaces. The radial stems included only a radial interosseous branch. The branching pattern of the ulnar stems as well as its topographical relationship with the radial stems indicated a fundamental spatial arrangement of the branches in the intermetacarpal spaces: the lumbrical, superficial articular, ulnar interosseous and radial interosseous branches were arranged fundamentally in this order from ulnar to radial in each space; the first three branches may form a common trunk. The present observations demonstrate that individual nerves in the extremities may have a regular branching pattern, contrary to most of the previous observations.

Comparative study of CA-50 (time-resolved fluoroimmunoassay), Span-1, and CA19-9 in the diagnosis of pancreatic cancer.

The clinical diagnostic utility of CA-50 (time-resolved fluoroimmunoassay) and Span-1 was compared with that of CA19-9 by measuring their levels in sera from patients with pancreatic cancer and other diseases. In pancreatic cancer CA-50, Span-1, and CA19-9 showed similar positive rates (84%, 82%, and 81%, respectively). With regard to the ability to distinguish pancreatic cancer from chronic pancreatitis, however, the specificity of CA-50 and Span-1 was higher than that of CA19-9 (85%, 85%, and 79%, respectively). Despite the similar positive rates of CA-50 and Span-1 in pancreatic cancer, the correlation between these two markers was low. Thus, used in combination, they compensated for each other in the diagnosis of pancreatic cancer. In chronic liver diseases, serum levels of both CA-50 and Span-1 were correlated with that of biliary tract enzymes, alkaline phosphatase, and r-glutamyl transpeptidase. And these two markers were more affected by the biliary system than CA19-9, resulting in the significantly higher positive rates. In these diseases immunohistochemical study showed that all three markers were localized in the epithelial cells of the bile duct, with CA-50 and Span-1 showing a similar tissue distribution.

[Tumor marker--personal experience. Mass survey and outpatients screening of pancreatic cancer].

To investigate the possibility that to detect carcinoma of the pancreas at an early stage by mass survey or outpatients screening, we performed the multicentral study for pancreatic cancer in Japan for two years from 1984 to 1985, using serum CA 19-9 and elastase-1 determinations or ultrasonography. Only 4 cases of pancreatic cancer were found out by mass survey, and according to outpatients screening with gastrointestinal complaints or jaundice, 85 patients were found to have pancreatic cancer. These results suggest that mass screening of persons without symptom is not worthy of the early detection of pancreatic cancer, however, outpatients screening gis useful for detecting curative cancers of the pancreas.

[The usefulness of nucleolar organizer regions in diagnosis of colonic epithelial neoplasia].

The Ag-NORs technique, staining nucleolar organizer regions, was performed on 23 areas of colonic normal mucosa, 17 adenomas with low grade atypical and 8 adenocarcinoma invading submucosa, to examine its possibility of application to the pathological diagnosis of colonic epithelial neoplasia. In normal colonic gland, the average number of Ag-NORs per 1 nucleus in cells of lower two-thirds showed significantly higher value (p less than 0.01) than higher one-third, and the mean cross-section area per 1 Ag-NOR in lower two-thirds showed a tendency to be larger than higher one-third. This results suggested that there was a positive correlation between the proliferative activity of cells and the number and size of Ag-NORs. A comparison of normal gland, adenoma and adenocarcinoma showed that the mean cross-section area of Ag-NORs correlated positively to their histological grade of atypia, but the average number did not. However using these two parameters, these three lesions had a tendency to be discriminated respectively at the average Ag-NORs number of 2.8 dots and the area of 2 microns. These results suggested that the Ag-NORs technique was expected to be a useful method for histopathological diagnosis of colonic epithelial neoplasia.

Agonist stimulation of Na+/K+/Cl- cotransport in rat glomerular mesangial cells. Evidence for protein kinase C-dependent and Ca2+/calmodulin-dependent pathways.

Studies were performed to investigate regulatory pathways of loop diuretic-sensitive Na+/K+/Cl- cotransport in cultured rat glomerular mesangial cells. Angiotensin II, alpha-thrombin, and epidermal growth factor (EGF) all stimulated Na+/K+/Cl- cotransport in a concentration-dependent manner. Pertussis toxin pretreatment reduced the effects of angiotensin II and alpha-thrombin but not that of EGF. Addition of the protein kinase C inhibitor staurosporine or down-regulation of protein kinase C by prolonged incubation with phorbol 12-myristate 13-acetate partially reduced the effects of angiotensin II and alpha-thrombin and completely blunted the phorbol 12-myristate 13-acetate-induced stimulation of Na+/K+/Cl- cotransport but did not affect EGF-induced stimulation. Exposure of cells to a calcium ionophore, A23187, resulted in a concentration-dependent stimulation of Na+/K+/Cl- cotransport, which was not significantly inhibited by down-regulation of protein kinase C but was completely inhibited by the calmodulin antagonist, N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide (W-7). Stimulation of the cotransport by angiotensin II or alpha-thrombin was also partially inhibited by W-7. Inhibitory effects of protein kinase C down-regulation and W-7 were additive and, when combined, produced a complete inhibition of angiotensin II-induced stimulation of Na+/K+/Cl- cotransport. In saponin-permeabilized mesangial cells, phosphorylation of a synthetic decapeptide substrate for Ca2+/calmodulin-dependent kinase II, Pro-Leu-Ser-Arg-Thr-Leu-Ser-Val-Ser-Ser-NH3, was demonstrated. Maximal activation of the decapeptide substrate phosphorylation required the presence of Ca2+ and calmodulin and was dependent on Ca2+ concentration. These findings indicate that stimulation of Na+/K+/Cl- cotransport by angiotensin II and alpha-thrombin is mediated by protein kinase C and Ca2+/calmodulin-dependent kinases whereas the action of EGF is mediated by other pathways.
