Nuclear magnetic resonance studies of sugar metabolism in the human infant lens.

We investigated sugar metabolism in lenses isolated from infants with stage-5 retinopathy of prematurity (ROP). The lenses were incubated in 13C-labeled glucose (35.5 mM) or galactose (30 mM) for 24 h at 37.5 degrees C. Six to 8 lenses were pooled for each experiment. 13C-nuclear magnetic resonance spectroscopy was then performed on perchloric acid extracts of these lenses. Active aldose metabolism was indicated by the production of lactate. However, neither sorbitol nor dulcitol was detected. On the other hand, an experimental aldose reductase inhibitor, sorbinil, caused a decrease in the hexose monophosphate shunt activity suggesting an active polyol pathway. Both polyols therefore appeared to have been metabolized further. In addition, galactose metabolism was unique in that it not only involved a galactose-->glucose epimerization, but also a direct galactose-->ribose conversion, and a production of galactose-3-phosphate. Although it is possible that the ROP lenses have adapted to oxidative stress during the postnatal high-oxygen incubation for premature infants, we believe the observed sugar metabolism is representative of the developing human lens.

Difficulties in excluding the diagnosis of retinoblastoma in cases of advanced Coats' disease: a clinicopathologic report.

We highlight the difficulty in differentiating Coats' disease from retinoblastoma. Two patients with bullous exudative retinal detachment and abnormal vessels are discussed. Histologic evaluation of the 5-year-old patient revealed retinoblastoma. The 1-year-old patient had Coats' disease by histologic identification. With this in mind, we suggest enucleation of such eyes if diagnostic uncertainty exists, since failure to treat retinoblastoma is a more serious error than enucleation of an eye with minimal function.

Role of tachykinins in bronchoconstriction induced by intravenous administration of bradykinin in guinea-pigs.

To elucidate the role of tachykinins in bronchoconstriction induced by intravenous administration of bradykinin (Bk), we studied the effects of FK224, a neurokinin-1 (NK1) and neurokinin-2 (NK2) receptor antagonist, on the bronchoconstriction induced by intravenous (i.v.) administration of Bk (5-100 micrograms.kg-1) in guinea-pigs. Total pulmonary resistance -(RL) was measured using a pressure-volume sensitive body plethysmograph in anaesthetized artificially ventilated guinea-pigs pretreated with atropine (1 mg.kg-1) and propranolol (1 mg.kg-1). In the control group, i.v. administration of Bk produced a dose-dependent increase in RL. In animals pretreated with FK224, bronchoconstriction induced by higher doses of Bk (10, 50 and 100 micrograms.kg-1) was significantly reduced, whilst the bronchoconstriction caused by lower doses of Bk (5 and 7.5 micrograms.kg-1) was not. Pretreatment with a combination of FK224 and indomethacin markedly inhibited the bronchoconstriction induced by each dose of Bk compared with the groups pretreated with FK224 alone. Although pretreatment with indomethacin alone significantly reduced RL at a high dose of Bk (50 micrograms.kg-1), the reduction was significantly lower than that produced by a combination of FK224 and indomethacin. These results suggest that intravenous administration of a high dose of bradykinin causes bronchoconstriction both by cyclo-oxygenase products and by release of tachykinins.

Change in osmolarity of disodium cromoglycate solution and protection against exercise-induced bronchospasm in children with asthma.

It has been suggested that osmolarity and/or nebulizer output may affect the protective effects of disodium cromoglycate (DSCG) in asthma. The aim of this study was to evaluate the influence of osmolarity of the DSCG solution on exercise-induced bronchospasm (EIB) in children with bronchial asthma. A jet nebulizer was used for DSCG inhalation in Study 1 and an ultrasonic nebulizer in Study 2. Thirteen asthmatic children (7 males and 6 females, aged 6-14 yrs) were enrolled in Study 1, and nine asthmatic children (5 males and 4 females, aged 9-13 yrs) in Study 2. After pretreatment with saline (control), hypotonic DSCG or isotonic DSCG, children underwent exercise challenge with a cycle ergometer. The percentage fall in forced expiratory volume in one second (FEV1) was measured at 5 and 15 min postexercise. The data were compared by analysis of variance (ANOVA). Both in Study 1 and Study 2, there were no significant differences in minute ventilation volume or maximum heart rate during exercise between the different treatment groups. Both hypotonic and isotonic DSCG significantly reduced the maximum percentage fall in FEV1. There were no significant differences in protective effects between hypotonic and isotonic DSCG in either study. We conclude that the efficacy of hypotonic and isotonic disodium cromoglycate solutions is similar for protection against exercise-induced bronchospasm. Hypotonic disodium cromoglycate seems to be clinically effective for prevention of exercise-induced bronchospasm and treatment of asthmatic children.

Contrast-enhanced MRI with gadodiamide injection in rat disease models.

The present study was designed to confirm the usefulness of contrast-enhanced magnetic resonance imaging (MRI) in diagnosing strokes of stroke-prone spontaneously hypertensive rats (SHRSP) and middle cerebral artery (MCA) occlusion, hepatocellular carcinoma and hydronephrosis of each experimental rat model. Male Sprague-Dawley rats (250-500 g), male SHRSP (ca. 250 g) and male F344 rats (ca. 300 g) were used for the investigation. Gadodiamide injection (Omniscan, Daiichi Pharmaceutical Co., Ltd. and Nycomed AS, Norway) was administered intravenously as the contrast agent at a dose of 0.1 mmol/kg except in hydronephrosis, where a dose of 0.05 mmol/kg was used. Magnetic resonance (MR) images were obtained with a 1.5T or a 2.0T magnetic field strength MRI unit. The signal intensity of the stroke lesions was increased after administration of gadodiamide injection in SHRSP and MCA-occluded rats. Hepatocellular carcinoma was undetectable without the use of the contrast agent, but the signal intensity of the tumor increased after administration of the gadodiamide injection, allowing the lesions to be detected. The signal intensity of the renal medulla increased in the non-ligated kidney, but not in the hydronephrotic kidney. The information given by the post-contrast images were superior to those obtained from the pre-contrast images in all the models. Contrast effects in SHRSP and MCA-occluded rats were related to differences in capillary permeability, those in rats with hepatocellular carcinoma depended on differences in vascularity, and those in hydronephrotic rats depended on blood flow and permeability.

Contrast-enhanced MRI with gadodiamide injection in rabbit carcinoma models.

This study was designed to demonstrate the broad utility of contrast-enhanced magnetic resonance imaging (MRI) for diagnosis of carcinoma. Twenty-six New Zealand White rabbits of either sex (1.7-3.4 kg) were used for the investigation. VX2 carcinoma (squamous cell carcinoma) was implanted in either the brain, lung, ovary, bone or muscle of rabbits. Contrast agent, Gadodiamide Injection, was administered intravenously at a dose of 0.1 mmol/kg. MR images were obtained by a 1.5 T, or a 2.0 T magnetic field strength super-conductive MRI unit. The intensity of the signal for the carcinoma was increased after administration of contrast agent, and the detectability or diagnostic information of post-contrast images was superior to that of pre-contrast images in all models. In addition, no significant side effects were observed during the MRI examination. After diagnosis using MRI, morphological damage in each model was assessed by gross and histopathological examinations. In contrast-enhanced MRI, though there were variations between the models employed, contrast effects in brain and muscle carcinoma models were generally related to differences in capillary permeability, while imaging in lung, ovary and bone carcinoma models was dependent on differences in blood flow rate and the size of interstitial spaces. Overall, our results demonstrate that contrast-enhanced MRI is a useful and safe method for diagnosing tumors.

BAY u3405, a thromboxane A2 antagonist, reduces bronchial hyperresponsiveness in asthmatics.

OBJECTIVE: Thromboxane A2 (TXA2) is reported to induce bronchial hyperresponsiveness along with the well-documented bronchoconstrictor action on smooth muscles. We examined the effect of the TXA2 antagonist, BAY u3405, on bronchial hyperresponsiveness to methacholine (MCh) in asthmatics. PATIENTS: Twelve adult asthmatics were studied in a randomized, double-blind, placebo-controlled, crossover fashion. DESIGN: Following a 2-week run-in period, the subjects were administered 75 mg of BAY u3405 or placebo orally, twice a day for 2 weeks each in a crossover design, interposing a 2-week washout period. Bronchial hyperresponsiveness was measured by the astograph method. Briefly, the respiratory resistance (Rrs) was measured by the forced oscillation method during continuous inhalation of MCh in stepwise incremental concentrations, until Rrs reached twice the baseline value. Bronchial hyperresponsiveness was evaluated as the minimum cumulative dose (Dmin) of MCh that induced an increase in Rrs. Dmin was calculated so that 1 U of Dmin equals to 1 min of inhalation of aerosol solution at 1.0 mg/mL during quiet breathing. RESULTS: Three subjects were withdrawn from the evaluation because they had asthmatic attacks or wheezing during the study. The Dmin value of 0.533 U (GSEM 1.675) after the BAY u3405 treatment was significantly greater than that of 0.135 U (GSEM 1.969) after the placebo treatment (p = 0.0139). There were no safety concerns in either treatment group. CONCLUSION: We conclude that BAY u3405 may be a useful drug for attenuating bronchial hyperresponsiveness in bronchial asthma.

Osteoporosis-pseudoglioma syndrome, a disorder affecting skeletal strength and vision, is assigned to chromosome region 11q12-13.

Osteoporosis-pseudoglioma syndrome (OPS) is an autosomal recessive disorder characterized by severe juvenile-onset osteoporosis and congenital or juvenile-onset blindness. The pathogenic mechanism is not known. Clinical, biochemical, and microscopic analyses suggest that OPS may be a disorder of matrix homeostasis rather than a disorder of matrix structure. Consequently, identification of the OPS gene and its protein product could provide insights regarding common osteoporotic conditions, such as postmenopausal and senile osteoporosis. As a first step toward determining the cause of OPS, we utilized a combination of traditional linkage analysis and homozygosity mapping to assign the OPS locus to chromosome region 11q12-13. Mapping was accomplished by analyzing 16 DNA samples (seven affected individuals) from three different consanguineous kindreds. Studies in 10 additional families narrowed the candidate region, supported locus homogeneity, and did not detect founder effects. The OPS locus maps to a 13-cM interval between D11S1298 and D11S971 and most likely lies in a 3-cM region between GSTP1 and D11S1296. At present, no strong candidate genes colocalize with OPS.

[Diffuse tracheobronchial amyloidosis with airflow limitation and repeated pneumonias].

A 67-year-old man was first admitted to our hospital complaining of a productive cough, and repeated episodes of pneumonia in different sites. Physical examination revealed expiratory wheezing and airflow limitation wlas documented with lung function tests. A chest X-ray film showed patchy pneumonic infiltrates. Chest computed tomography revealed a marked thickening of the tracheal and bronchial wall with linear calcification. Fiberoptic bronchoscopy revealed a diffuse infiltrative process in the tracheobronchial tree, which uniformly reduced the bronchial lumen. Bronchial biopsy specimens showed amyloid deposits with focal calcification, which was confirmed by Congo red and Dylon staining. Extensive examinations including rectal biopsy were negatie for systemic amyloidosis. We suggest that diffuse tracheobronchial amyloidosis should be included in the differential diagnosis of repeated pulmonary infections with airflow obstruction.

[A special form of Stargardt's disease/fundus flavimaculatus].

Stargardt's disease is a bull's-eye macular dystrophy with the characteristic appearance of a perifoveal lesion. We have observed four cases of an autosomal recessive disease with fundus features undistinguishable from Stargardt's disease, but with a different nature and much poorer prognosis. One case typical of Stargardt's disease characterized with macular degeneration showed a bull's-eye with dark choroid. Sixteen years later, the same eye developed bone corpuscle pigmentation in the periphery with non-recordable electroretinogram. The other three cases were members of a single family. They showed typical macular degeneration consistent with Stargardt's disease but had peripheral degeneration as well. Taking into account that there is a great diversity of retinal functions in Stargardt's disease, these features are different from those reported previously. Stargardt's disease should not be considered as a single clinical entity with a fair prognosis.

[Three cases of tuberculous mediastinal lymphadenitis].

Mediastinal lymph node involvement is uncommon in intrathoracic tuberculosis. We report three cases of this disease, each of which had a different clinical course. Chest CT scans showed preferential involvement of right paratracheal nodes, central areas of relatively low density with peripheral rim enhancement after injection of contrast medium. Specimens obtained by mediastinoscopy and fiberoptic bronchoscopy revealed acid-fast bacilli in all cases. In view of its relative frequency, tuberculous mediastinal lymphadenitis in adults must be distinguished from other causes of mediastinal masses.

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from patients with urinary tract infections (1994). II. Background of patients].

Clinical background was investigated on 628 bacterial strains isolated from patients with urinary tract infections (UTIs) in 10 hospitals during period from June, 1994 to May, 1995. 1. Distributions of sex, age and urinary tract infections Among over sixties males, the majority was taken by complicated urinary tract infections. Among females, the uncomplicated urinary tract infections was most frequent without a relation of age. As for over 40 females, the increase of complicated UTI was admitted. 2. Distribution of sex, age and pathogens isolated from UTIs In uncomplicated UTIs, Escherichia coli was most frequently isolated without a relation of age, and next Enterococcus faecalis and CNS. In complicated UTIs without indwelling catheter, E. coli, Pseudomonas aeruginosa and E. faecalis were isolated the most frequent. In complicated UTIs with indwelling catheter, P. aeruginosa was most frequently isolated, and next E. faecalis and Staphylococcus aureus. 3. Administration of antimicrobial agents and pathogens isolated from UTIs In uncomplicated UTIs, pathogens, after administration of antibiotics, isolated from patients have obviously decreased from 216 to 32 isolates. And also, pathogens of complicated UTIs without indwelling catheter, have decreased from 127 to 50 isolates. However, in complicated UTIs with indwelling catheter, pathogens after or before administration of antibiotics, were not revealed an obvious change. 4. Pathogens and UTIs with or without factor and operation In uncomplicated UTIs with or without factor and operation, E. coli was mainly detected. In complicated UTIs without indwelling catheter and with factor and operation E. faecalis was mainly detected, and next E. coli, S. aureus and P. aeruginosa. Without factor and operation, E. coli was mainly detected. In complicated UTIs with indwelling catheter and with factor and operation, P. aeruginosa, E. faecalis and S. aureus were mainly detected at 23.5%, 15.0%, 15.0%, respectively. Without factor and operation, Proteus spp. 22%, and next E. faecalis and P. aeruginosa 10% and 12%, were detected, respectively. The distribution of pathogens, in the case of Proteus spp. and P. aeruginosa has been obviously varied by the influence of factor and operation.

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from patients with urinary tract infections (1994). III. Secular changes in susceptibility].

Susceptibilities of Enterococcus faecalis, Staphylococcus aureus, Citrobacter spp., Enterobacter spp., Escherichia coli, Klebsiella spp., Proteus mirabilis, Pseudomonas aeruginosa and Serratia spp. isolated from patients with urinary tract infections (UTIs) in 10 hospitals during June 1994 to May 1995 to various antimicrobial agents were compared with those in the same period of previous years according to a classification, uncomplicated UTIs, complicated UTIs without indwelling catheter, and complicated UTIs with indwelling catheter. No remarkable changes were found in susceptibilities of Citrobacter spp., Enterobacter spp. and Serratia spp. The susceptibilities of E. faecalis to amikacin and quinolones were better than those in 1993. As for S. aureus, susceptible strains to all drugs increased in uncomplicated UTIs. Against E. coli in 1993, the antimicrobial activities of piperacillin, cefotiam and aminoglycosides have decreased, however, in 1994, these activities have turned to the better state. As for Klebsiella spp. susceptible strains to ABPC decreased. The susceptibilities of P. mirabilis to all drugs except minocycline were good. Against P. aeruginosa in 1993, the activities of aminoglycosides have decreased, but, in 1994, these activities have turned to the better state. These data should be considered in clinical treatment of various urinary tract infections.
