An autopsy case report of diffuse esophageal intramural pseudodiverticulosis.

A case report of diffuse esophageal intramural pseudodiverticulosis (EIPD) we encountered at autopsy is described. The pseudodiverticula represented dilated excretory ducts of esophageal submucosal glands with squamous metaplasia of the epithelium. They contained keratin flakes, mucin and/or inflammatory cells which were mainly neutrophils. Submucosal chronic inflammation surrounding the glands was prominent. The findings suggested that the ductal dilatation resulted from obstruction of the ducts by inflammatory material, mucus and desquamated epithelium. To the best of our knowledge, this is the first autopsy report of diffuse EIPD in Japan.

Cerebellar form of progressive multifocal leukoencephalopathy in a patient with chronic renal failure.

Progressive multifocal leukoencephalopathy (PML) developed in a 64-year-old woman who had undergone hemodialysis treatment due to chronic renal failure (CRF) for 6 yr. Her initial symptom was ataxia, and computed tomographic (CT) scan and magnetic resonance imaging (MRI) suggested a demyelinating disease of the cerebellum. Her cell-mediated immunity was impaired. At autopsy, the cerebellar disease was confirmed as PML by ultrastructural and immunohistochemical studies. Moreover, the JC type of papova virus infection was verified by Southern blot analysis.

Analysis of left ventricular motion after mitral valve replacement with a technique of preservation of all chordae tendineae. Comparison with conventional mitral valve replacement or mitral valve repair.

The postoperative regional left ventricular motion of 22 patients with a diagnosis of mitral regurgitation, and who underwent mitral valve replacement with preservation of chordae tendineae, were retrospectively analyzed by cineangiography in the early postoperative period and by multiple-gated cardiac blood pool scintigraphy in the mid-to-late postoperative period. The operation consisted of the division of the anterior leaflet into anterior and posterior segments, the shifting and reattachment of the divided segments to the mitral ring of the respective commissural areas, and the use of a low-profile bileaflet prosthetic valve. Control groups consisted of 28 patients with mitral regurgitation who underwent mitral valve replacement with a conventional technique and 16 patients who underwent mitral valve repair. Compared with the conventional mitral valve replacement group, the radial shortening of the left ventricle of the chordae-preserved mitral valve replacement group was greater at the apical septal, inferoapical, anterobasal, and anterolateral portions, whereas the radial shortening of the repair group was greater than that of the chordae-preserved group only at the inferolateral portion. The ejection fraction of the whole left ventricle was statistically greater in the chordae-preserved group, and also regional ejection fraction of the chordae-preserved mitral valve replacement group was greater at the apical septal, inferoapical, inferolateral, anterobasal, and anterolateral portions than that of the conventional mitral valve replacement group at these portions. On the other hand, the postoperative regional and global motion was identical to that of the mitral valve repair group except at the inferolateral portion. The result of this study supports a concept that maintenance of continuity between the mitral anulus and the papillary muscle has a beneficial effect on postoperative left ventricular performance.

[Clinical course of bone metastases by prostatic cancer studies with conventional X-ray].

The clinical courses of bone metastases in 81 cases of prostatic cancer were examined using conventional X-ray. All patients received endocrine therapy as the initial treatment; castration and estrogen were given to 46 cases, castration and antiandrogen to 19, LH-RH analog to 11, flutamide to 5. The response to endocrine therapy was evaluated every 6 months after the start of the treatment according to National Prostatic Cancer Project criteria. The prognosis was calculated by cause-specific survival. The types of untreated bone metastases on X-ray were classified into five: sclerotic in 12 cases (15%), mixed but mainly sclerotic in 25 (31%), mixed but mainly lytic in 14 (17%), lytic in 8 (10%) and undetermined with positive scintigraphy in 22 (27%). Two mixed types showed more spread metastases and tendency of elevated prostatic acid phosphatase when compared with other types. Temporary enlargement of sclerotic area within 6 months after start of the treatment did not correlate with deterioration of disease. Remodeling of metastatic areas occurred 6-36 months after start of the treatment showed curative course and the prognosis of these groups were favorable. Most of lytic areas tended to sclerotic finding following endocrine therapy regardless of effects by endocrine therapy. Sclerotic and two mixed types showed better and worse tendencies, respectively. On viewpoint of remodeling, change of metastatic area and newly appearance of metastases, an evaluation criteria of response to the therapy was proposed, which showed good correlation with prognosis. From these observations, it was concluded that bone metastases tended from lytic to sclerotic changes and that lytic-sclerotic cycles occurred repeatedly along with disease progression regardless of effect to therapy. Remodeling seemed to be curative signs of metastatic area.

Investigating the response of prostatic cancer to endocrine therapy.

The presence of androphilic protein identified histochemically with R 1881-CMO-BSA-FITC correlated with response to endocrine therapy and prognosis. Ras P21 was not a useful indicator of the subsequent course of the disease. Conventional X ray assessment of boney metastases still seems of use in monitoring overall progress of the disease.

Increased accumulation of N-isopropyl-(I-123)p-iodoamphetamine in bronchial carcinoid tumor.

Radionuclide imaging with N-isopropyl-(I-123)p-iodoamphetamine ([123I]IMP) was performed in a patient with recurrent bronchial carcinoid tumor. Increased accumulation of [123I]IMP was observed in the known lesions of the brain and neck on both tomographic and planar images. Scintigraphy also revealed unknown metastatic lesions of the paraaortic lymph nodes, which were later confirmed on x-ray CT. Iodine-123-IMP may have a potential role in evaluating carcinoid tumors.

Substance P-like immunoreactivity in human cerebrospinal fluid.

In a study to elucidate molecular mechanisms in pain, substance P-like immunoreactivity (SP-LI) was measured in lumbar CSF from 75 patients with lower back pain. Two samples--one before and one after lidocaine treatment--were obtained from each patient, and total SP-LI was measured in unfractionated (no HPLC) samples. SP-LI data were separated into three categories: placebo responders, pharmacological responders, and pharmacological non-responders. A significant difference was observed between the total SP-LI measurement of first and second samples of pharmacological non-responders. Distribution of SP-LI immunoreactive molecular species in two CSF patient samples (no ODS) was analyzed with a combination of reversed phase (RP) HPLC and RIA. Immunoreactivity in collected HPLC fractions was measured at calibrated retention times of synthetic SP-sulfoxide (SP-O), SP, and SP. Qualitative and quantitative differences in those HPLC-RIA metabolic profiles were observed within and between those two patients' samples. These data indicate that the type and amount of SP metabolism and SP precursor-processing differs in CSF between these two representative patients and within the short amount of time elapsed between acquiring these two samples.

Optimizing recovery of peptides from an octadecylsilyl (ODS) cartridge.

An analytical procedure has been studied in detail to optimize the recovery of the radiolabeled and unlabeled synthetic peptides substance P and methionine-enkephalin from a reversed phase octadecylsilyl disposable cartridge, which is one of the first steps to preferentially enrich peptides from a biologic matrix. Our previous research on biologic peptides indicated the need for the present study. The column flow-rate was found to be one of the most important experimental parameters; and its effect on the recovery of those two synthetic peptides was determined by the measurement of recovered radioactivity and by high performance liquid chromatography u.v. detection. The accuracy and reproducibility of the flow-rate affected recovery, and it was necessary to regulate the flow-rate with a syringe infusion pump. Recovery was optimum using a slow rate for both sample application and elution. Furthermore, a slow elution was more important than a slow rate of sample application. Such basic recovery studies on individual synthetic peptides are required before undertaking peptide extractions from biologic matrices.

Chemical degradation of 3H-labeled substance P in tris buffer solution.

Substance P (SP) is an important neuropeptide that has been implicated in several physiological processes, and it is necessary to devise an analytical procedure to measure endogenous SP with a combination of high sensitivity and maximum molecular specificity. However, the unique chemical nature of SP (polarity, chemical stability, ease of oxidation, peptide bond lability) plays a significant role in its analysis, such as in receptor assays, immunoassays, chromatography, and mass spectrometry. In this study, we evaluated in polypropylene and glass assay tubes the effects on the recovery and stability of tritiated SP ([3H]SP) of several pertinent experimental parameters such as buffer, pH, multiple freeze-thaw cycles, and incubation temperature and time. Bovine serum albumin (BSA) effectively reduced the absorption of [3H]SP to polypropylene and glass tube surfaces. Following multiple (6X) freeze-thaw cycles of solutions in BSA-precoated tubes, the recovery of radioactive [3H]SP remained high (greater than 75%) after the last cycle, whereas recovery was minimal in uncoated or siliconized glass tubes. A high level of radioactivity recovery was maintained for 14 days of storage of [3H]SP in triethylamine formate (TEAF) solution in BSA-precoated tubes at 4 and -20 degrees C, but decreased at 37 degrees C to less than 80% in only 3 h. Following storage in Tris-HCl (pH 7.4) buffer, a combination of HPLC and mass spectrometric analyses revealed that a significant amount of peptide bond cleavage occurred to produce the two peptides ArgProLys (RPK) and ArgProLysProGlnGln (RPKPQQ), with only a small amount of remaining intact SP. That decomposition was not observed in triethylamine formate TEAF (pH 3.14) buffer solutions.

Aortic atresia with a solitary and indeterminate ventricle.

A male infant having aortic atresia and double inlet to a solitary and indeterminate ventricle, presented a clinical picture of "hypoplastic left heart syndrome". Haemodynamic consequences are described and comparison made to other types of aortic atresia. Cross-sectional echocardiography was found to be reliable in determining both atrioventricular and ventriculo-arterial connections in the presence of aortic atresia and a solitary ventricle.

[Basic and clinical studies on patients who have undergone autologous or allogeneic bone marrow transplantation].

Bone marrow transplantation (BMT) is now increasingly applied not only to hematologic disorders such as leukemias and aplastic anemia but also to some of solid tumors. We experienced ten cases of bone marrow harvest and six cases of autologous or allogeneic BMT during the period from June, 1986 to August, 1987. For autologous BMT cases, bone marrow cells were harvested and cryopreserved at -196 degrees C in liquid nitrogen tank and colony forming assays were examined to ascertain the colony forming ability of harvested bone marrow stem cells. Recovery of colony forming ability after cryopreservation in colony forming unit in culture and burst forming unit in erythrocyte is about 63 to 73% and well preserved. Among 6 cases undergone BMT, five cases are patients with advanced solid tumors including breast cancer, seminoma, lung cancer, malignant lymphoma and brain tumor and these 5 cases have undergone autologous BMT. Conditioning regimen for each cancer in autologous BMT differs based upon conventional chemotherapy regimen and two to four fold dose of conventionally used chemotherapy regimen were administered and after two or three days of high dose chemotherapy, cryopreserved bone marrow cells were thawed and infused. Peripheral white blood cells and platelets were recovered to more than 1000/microliters and 5 x 10(4)/microliters, respectively, within 21 days after bone marrow cell infusion. Disappearance or decrease in tumor size was observed in all cases except brain tumor even though those cases were advanced ones. After autologous BMT, recurrence occurred within three to five months in three cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Prophylaxis of superficial bladder cancer with instillation of adriamycin or mitomycin C.

A multicenter trial for postoperative prophylaxis of superficial Ta-T1, G1-G2 bladder cancer was performed. Intravesical instillation using either 20-30 mg adriamycin or 20 mg mitomycin C per dose was carried out for 4 weeks or 2 years. Patients without instillation served as controls. A total of 259 patients was considered eligible for the evaluation. The instillation group showed a better disease free survival rate than the control group. Better prophylactic effects of instillation therapy were observed when one of following factors was present: multiple tumors, large tumors, T1 and G2 bladder cancer. The total dose of drug instilled seemed to correlate with the effects, but there were no differences between adriamycin and mitomycin C. The side effects were minimal and temporary.

Plasma concentration of human atrial natriuretic polypeptide in patients with impaired renal function.

Using direct radioimmunoassay, the plasma concentration of human atrial natriuretic polypeptide (hANP) was measured in patients with impaired renal function. Patients on maintenance hemodialysis (HD) and those still on medical management (non-HD) were examined. In 13 non-HD patients with serum creatinine values from 2.0 to 8.3 mg/dl, mean plasma hANP (+/- SE) was 404 +/- 23 pg/ml, while it was 236 +/- 11 pg/ml in the normal control group (n = 15) and the difference was significant (p less than 0.001). In all patients as a whole, there was a positive correlation between plasma hANP and mean blood pressure (r = 0.56, p less than 0.05) but no correlation was present between plasma hANP and renal function. Fifty-six HD patients were divided into 2 groups depending on blood pressure level. Plasma levels of hANP in the hypertensive (BP greater than or equal to 150/90 mmHg, n = 21) and in the normotensive (BP less than 150/90 mmHg, n = 35) HD group were 588 +/- 58 pg/ml and 364 +/- 29 pg/ml, respectively, with plasma hANP in both HD groups significantly higher than in the controls (p less than 0.001). There was also a significant difference of plasma hANP between hypertensive and normotensive HD patients (p less than 0.01). However, when the normotensive HD group without cardiomegaly (cardiothoracic ratio less than 50%, n = 17) was compared with the control, the value of plasma hANP was not statistically different from that of the control group. These results suggest that plasma hANP in patients with impaired renal function is influenced by blood pressure and/or cardiac condition.

Structural composition of central pulmonary arteries. Growth potential after surgical shunts.

Failure to achieve adequate pulmonary artery growth in patients with cyanotic congenital heart disease is a major obstacle to surgical correction. To assess whether differences in structural composition of central pulmonary arteries influence their growth potential after surgically created shunts, we obtained full-thickness biopsy specimens from the hilar pulmonary arteries of eight patients with pulmonary atresia or tetralogy of Fallot undergoing modified Blalock-Taussig shunts under 1 year of age. Tissue was processed for electron microscopic studies and a morphometric assessment was made of the volume proportions of smooth muscle, collagen, ground substance, and elastin. Initial pulmonary artery size was determined angiographically during the diagnostic cardiac catheterization. Pulmonary artery size was determined by cross-sectional echocardiograms 7 to 27 months later (mean 19 months). Pulmonary artery growth did not correlate with the interval between examinations but did correlate with the volume proportion of elastin (r = 0.73, standard error of the estimate = 1.45, p less than 0.05). Thus the structural composition of pulmonary arteries may influence their potential for growth after surgical shunts. In particular, an inadequate proportion of elastin may be a hindrance to growth.
