Role of secondary hyperparathyroidism and liver function in hyperamylasemia in chronic renal failure.

Elevated values of pancreatic-type amylase activity in serum were found in 59% of patients with liver cirrhosis not complicated with renal failure, in 67% of patients with chronic renal failure not complicated with hepatopathy and in 95% of patients with chronic renal failure complicated with hepatopathy. In all the three groups, a significant positive correlation was found between the pancreatic-type amylase and intestinal isoenzyme of serum alkaline phosphatase which is an asialoglycoprotein. However, in pancreatitis a prevalence of an increase in pancreatic-type amylase with respect to intestinal alkaline phosphatase was found. A multivariate analysis showed that in chronic renal failure not complicated with hepatopathy, and in chronic renal failure complicated with chronic liver disease, the changes in calcium homeostasis and also the liver disorder, respectively, contribute significantly to the above-normal values for pancreatic-type amylase.

Age and sex dependency of the biochemical indices of bone remodelling.

The values for the bone isoenzyme of serum alkaline phosphatase peak in the first two years of age, between 6 and 7 years of age, before the end of puberty and in the postmenopause. A population between the ages of 29 and 45 provides a reference population to which all other age groupings can be compared. A significant positive correlation was found between bone isoenzyme of serum alkaline phosphatase and urinary hydroxyproline excretion in children as well as after puberty. However, in the children the urinary hydroxyproline excretion was significantly higher when compared with the bone isoenzyme of alkaline phosphatase. A significant positive correlation was found between the bone isoenzyme of alkaline phosphatase and plasma tartrate-resistant acid phosphatase, irrespective of age and sex. The biochemical indices of bone remodelling correlated significantly with the growth rate in children and adolescents. The results are in good agreement with the concept of the coupling of bone formation to bone resorption.

Metabolic implications in the elevation of serum activity of intestinal alkaline phosphatase in chronic renal failure.

The activity of intestinal isoenzyme of serum alkaline phosphatase was evaluated in 21 non-dialyzed patients with advanced renal failure and in 52 patients on regular hemodialysis. In patients without hepatopathy, a significant inverse correlation was found between the enzyme activity and serum calcium levels. Hepatopathy was the most significant variable influencing the enzyme activity in patients on dialysis. Secondary hyperparathyroidism and a decreased rate in enzyme elimination should be assessed for the above-normal activities of intestinal ALP in serum in chronic renal failure.

Sequential dependences of perceptual interpretations of a repetitively illuminated reversible figure.

The Necker cube pattern was illuminated by xenon flashes repetitively at different but always regular interstimulus intervals. Subjects pushed one of two buttons corresponding to the subjective perceptual interpretation of the pattern. Analysis of the resulting binary sequences showed that none of the sequences was random. The order of Markovian dependence was higher for sequences with longer (3.6 and 2.4 s) interstimulus intervals than for shorter (1.2 s) ones. This result points to the significance of perceptual sets related to short-term memory mechanism upon perception.

Relationship of plasma tartrate resistant acid phosphatase to the bone isoenzyme of serum alkaline phosphatase in hyperparathyroidism.

In 46 patients with primary hyperparathyroidism, in 21 non-dialysed patients with advanced renal failure, and in 52 patients on hemodialysis, a significant positive correlation was found between bone isoenzyme of serum alkaline phosphatase and plasma tartrate resistant acid phosphatase. In primary hyperparathyroidism, a significant positive correlation was found between the radiological degree of osteodystrophy and the biochemical parameters of bone remodelling. After removal of the parathyroid adenoma, only the tartrate-resistant acid phosphatase decreased to normal limits. Plasma tartrate resistant acid phosphatase was most significantly influenced by serum immunoreactive parathyroid hormone levels. In chronic renal failure, bone isoenzyme of serum alkaline phosphatase was most significantly influenced by serum immunoreactive parathyroid hormone levels, by hypocalcemia and by duration of hemodialysis. The results confirm that in hyperparathyroidism the extent of the whole-body rates of bone resorption and formation are approximately equal. The biochemical parameters can be used for serial assessment of the course of the disease but are not specific for diagnosis.

Serum digoxin level - determined or estimated?

In 108 patients on maintenance digoxin therapy the serum digoxin levels were determined by means of homogeneous enzymoimmunoanalysis and compared with those estimated by means of a commonly accepted nomogram and with those estimated by means of the equation derived from multiple regression analysis of variables. By using stepwise discriminant analysis and a jacknifed classification matrix, based on serum digoxin and potassium levels, 96% and 90% of the classification of patients with and without clinical signs of digoxin intoxication, respectively, was correct. Based on serum creatinine and potassium levels and the daily digoxin dose/kg only 83% and 78% of the classification, respectively, was correct. The direct determination of serum digoxin level provides, in real time, information of higher reliability than does the estimate of the serum digoxin level. It is of basic importance in patients with dysrhythmias, in persons with an implanted pacemaker and in persons in whom the estimated levels of serum digoxin are around 2.6 nmol/l.

Epidemiology and prevention of ischaemic heart disease in automobile industry workers.

Following on the epidemiological study of cardiovascular diseases carried out in the rural population of Northern Bohemia in 1970 a long-term ischaemic heart disease prevention programme was launched in a group of 2582 men aged between 30-59 years, worked of two automobile plants in the district of Mladá Boleslav. Under der programme "Protected Your Heart", sponsored by the Automobile Works National Enterprise (AZNP), workers were urged to adjust their food intake in keeping with the principles of rational nutrition, to stop smoking and to take regular after-work cross-country 4 km/1 hr walks. No such programme was carried out at the automobile plant LIAZ serving as control.

Plasma 25-hydroxycholecalciferol in oral sulfonylurea treated diabetes mellitus.

In oral sulfonylurea treated diabetics, decreased plasma 25-hydroxycholecalciferol and increased activity of bone isoenzyme of serum alkaline phosphatase and urinary hydroxyproline excretion was found when compared with control subjects. In stepwise regression analysis significant relationships were found between bone isoenzyme, urinary hydroxyproline excretion, serum calcium levels and blood glucose levels. The partial correlation and multivariate regression analysis showed that in oral sulfonylurea treated patients, but not in insulin treated patients, the activity of bone isoenzyme of serum alkaline phosphatase was significantly inverse dependent on the 25-hydroxycholecalciferol levels in plasma. Oral sulfonylurea agents which are known to induce the hepatic microsomal system seem to be an additional factor to poor control of diabetes, leading to osteopathy.

Discrimination analysis as a tool for classifying the value of biochemical and immunological tests in lung cancer.

The data obtained with routine biochemical, immunological and haematological tests in healthy blood donors and in patients with lung cancer were subjected to discrimination analysis with the aim to select the minimum possible combination of methods with the highest probability of distinguishing between the two groups. Transformation of lymphocytes (TR.LY) after PHA stimulation, formation of E-rosettes (E-ROS), and blood glycoproteins (GP) were shown to permit the most complete differentiation between the group of patients and the blood donors. On the basis of the above-mentioned tests the discrimination rule could be defined: IF 52,73249 - 0.45913 X E-ROS - 0.40752 X TR.LY + 0.02790 X GP greater than or equal to 0, then the investigated person is classified as a cancer patient while at values less than 0 the person is classified as a healthy individual.
