Purification and characterization of two lectins from the sea cucumber Stichopus japonicus.

Two Ca(2+)-dependent lectins were purified from the sea cucumber Stichopus japonicus by affinity chromatography on lactosyl-Sepharose 4B and ion-exchange chromatography on Q-Sepharose. Their molecular masses were estimated to be 13kDa (SJL-I) and 15 kDa (SJL-II) on SDS-PAGE. SJL-I agglutinated rabbit erythrocytes as well as human A, B, and O-type erythrocytes, but SJL-II agglutinated only rabbit erythrocytes. Hemagglutination by SJL-I was competitively inhibited by N-acetyl-D-galactosamine and galactose-containing carbohydrates. On the other hand, only lactose, melibiose, and raffinose gave weak inhibition of hemagglutination by SJL-II, suggesting that SJL-II may have high specificity for particular complex carbohydrate(s) on the surface of rabbit erythrocytes. SJL-II was activated at ten times lower Ca2+ concentration than SJL-I. Both lectins lost activity in acidic pH, while SJL-I appeared more stable down to pH 4.5.

[Renal function of contralateral kidney after total or partial removal of kidney with ectopic ureter].

The effects of partial or total removal of the kidney with ectopic ureter on the contralateral kidney were evaluated using DMSA renal uptake rate on 8 patients. After partial nephrectomy on 6 cases, the affected residual kidneys showed significant decrease in DMSA renal uptake rate in 2 cases in which the extirpated kidneys had normal renal tissue in part but did not show significant change in DMSA uptake rate in 4 cases in which the extirpated kidneys were dominated mainly with dysplasia. Post-operative increase in DMSA uptake rate of the contralateral kidneys were significant in 3 of 4 patients who had the operation before they became 2 years old. In 2 of these 3 cases, pre-operative scintigram showed extremely low rate of DMSA renal uptake. Four cases aged 2 years old and over did not show post-operative increase in the uptake rate. As we have previously reported on the cases with hydronephrosis, these results suggest that 1) existence of obstruction caused by ectopic ureter inhibit the development of the contralateral kidney and 2) the growth of the kidney can resume the release of that obstruction, if the release is made while the kidneys are still in developing stage.

[Lomefloxacin concentration in human prostatic tissue following 3-day administration].

The penetration of Lomefloxacin (LFLX) into the prostatic tissue and the serum were examined. Forty four patients with benign prostatic hypertrophy treated with transurethral resection of the prostate (TUR-P) were entered in this study. LFLX was administered orally in a dose of 200 mg three times daily for three days preoperatively. The blood samples were taken simultaneously at the time of the tissue sampling. The patients were divided into groups 1 and 2. In group 1 (seventeen patients), the tissue sampling was done about 17 hours after the final drug administration. The mean concentration of LFLX was 3.31 +/- 1.16 micrograms/g in the prostatic tissue and 1.70 +/- 0.56 micrograms/ml in the serum. In group 2 (twenty seven patients), tissue sampling was done 5.5 hours after the final drug administration. The mean concentration of LFLX was 5.70 +/- 2.28 micrograms/g in the prostatic tissue and 2.93 +/- 1.14 micrograms/ml in the serum.

Inferior mesenteric and hypogastric artery reconstruction to prevent colonic ischaemia following abdominal aortic aneurysmectomy.

In 179 patients receiving prosthetic grafts for abdominal aortic aneurysmectomy, an attempt was made to preserve or restore continuity of at least two vessels, including the patent inferior mesenteric artery (IMA) and bilateral hypogastric arteries (HGAs), so as to prevent colorectal ischaemia. Adjunctive reconstruction of the IMA and/or HGAs was performed in 60 patients; a total of 40 HGAs and 31 IMAs were reconstructed. The hospital mortality rates of patients with elective and ruptured aneurysm repair were 3.9 and 22% respectively. There were no deaths related to colorectal ischaemia. The mean(s.d.) IMA stump blood pressure was 66(18) mmHg and the mean(s.d.) IMA stump pressure index (the ratio of mean peak systolic IMA pressure to mean systolic systemic pressure) was 0.58(0.15). The IMA was revascularized in all patients whose stump pressure was < 40 mmHg and whose index was < 0.4. Postoperative angiography revealed a 94% patency rate for the IMA and 92% for the HGAs. Colorectal ischaemia did not develop in any patient in whom aneurysmectomy was performed electively or in any who had colonoscopy because of watery diarrhoea after surgery. These results indicate the significance of adjunctive reconstruction of the IMA and HGA during abdominal aortic aneurysmectomy.

A microassay for proteases using succinylcasein as a substrate.

A photometric assay for proteases has been developed. A chemically modified casein whose amino groups were succinylated was used as a substrate. After incubation with trypsin, chymotrypsin, thermolysin, and subtilisin, the extent of hydrolysis of the substrate was determined with trinitrobenzene sulfonate (TNBS). The whole procedure of the assay was performed in the microtiter plate wells and the increase in the absorbance resulting from the reaction between TNBS and newly formed amino groups in the substrate was able to be determined with a high sensitivity by a microtiter plate reader, enabling the simultaneous measurement of a number of samples. Application of this method to the measurement of proteolytic activity contained in the protein extract of Tapes philippinarum is demonstrated.

Improvement in local cerebral blood flow measurement in gerbil brains by prevention of postmortem diffusion of [14C]iodoantipyrine.

Postmortem diffusion of [14C]iodoantipyrine, which distorts the image of cerebral blood flow, can occur in at least three steps; from decapitation until the brain is frozen, while frozen sections on coverslips are thawed, and when dried sections are applied to x-ray film. In the present study, the first two steps were modified to reduce the time during which brain tissue was wet. When the brains of gerbils were taken out of the skulls and immersed in chilled isopentane (-45 degrees C), 90-95 s elapsed between decapitation until the brain tissues were frozen. However, immersion of whole heads in liquid nitrogen after decapitation decreased the time to 25 s. The autoradiograms had better contrast after the freezing procedure with liquid nitrogen than after that with chilled isopentane. The drying time of the thawed sections was markedly reduced by blowing hot air across the sections on a hot plate, and this resulted in clearer images on autoradiograms. In most of the tissues with values of cerebral blood flow over 100 ml 100 g-1 min-1 as measured using the conventional drying method, the corresponding values were higher if the modified method was used. In contrast, in tissues with values less than 100 ml 100 g-1 min-1, the corresponding values were lower. Moreover, the differences between flows in adjacent small brain structures were less clear if the sections were dried by the conventional method. Reducing the time during which postmortem brains or sections are wet can help prevent [14C]iodoantipyrine diffusion artifacts.

Surgical management of petrous apex cholesteatoma: a therapeutic scheme.

From 1969 to 1990, 16 patients were treated for petrous apex cholesteatoma invading or extending beyond the internal auditory canal. This type of cholesteatoma poses various specific surgical problems and various surgical procedures are necessary for treatment. To attain maximal therapeutic effect and ensure postoperatively a satisfactory quality of life, the preoperative therapeutic scheme for each individual patient is very important, as is demonstrated by the case study presented here. The therapeutic scheme includes the following: (1) surgical approach; (2) management of a cavity following removal of the cholesteatoma; (3) handling of problems such as involvement of the dura and internal carotid artery; and (4) management of the facial nerve.

[Methicillin resistant Staphylococcus aureus (MRSA) infection in geriatric hospital--management for infected decubital ulcer].

Wide spread of MRSA infection in geriatric hospitals indicated two important problems which must be resolved. One is how we can eradicate MRSA in the elderly, and the other is how we can control cross infections of MRSA within the hospital. The presence of infected decubital ulcers in the elderly make it difficult to answer these problems. Usually, MRSA and Pseudomonas aeruginosa which is isolated from decubital ulcers act as colonizers on the surface of granulated wounds but do not contribute to clinical symptoms. However, these colonizers can be the source of cross infection toward other debilitated patients. The actual management for decubital ulcer infected by MRSA are discussed from the above viewpoints.

Primary structures of ribosomal proteins from the archaebacterium Halobacterium marismortui and the eubacterium Bacillus stearothermophilus.

Approximately 40 ribosomal proteins from each Halobacterium marismortui and Bacillus stearothermophilus have been sequenced either by direct protein sequence analysis or by DNA sequence analysis of the appropriate genes. The comparison of the amino acid sequences from the archaebacterium H marismortui with the available ribosomal proteins from the eubacterial and eukaryotic kingdoms revealed four different groups of proteins: 24 proteins are related to both eubacterial as well as eukaryotic proteins. Eleven proteins are exclusively related to eukaryotic counterparts. For three proteins only eubacterial relatives-and for another three proteins no counterpart-could be found. The similarities of the halobacterial ribosomal proteins are in general somewhat higher to their eukaryotic than to their eubacterial counterparts. The comparison of B stearothermophilus proteins with their E coli homologues showed that the proteins evolved at different rates. Some proteins are highly conserved with 64-76% identity, others are poorly conserved with only 25-34% identical amino acid residues.

Pediatric orbital eosinophilic granuloma with intra- and extracranial extension--case report.

A rare case of eosinophilic granuloma of the orbit in a 3-year-old boy presented as right upper eyelid swelling and proptosis. Computed tomographic scanning revealed a soft-tissue dense mass in the lateral wall of the right orbit. Magnetic resonance (MR) imaging demonstrated tumor extension into the orbit, anterior and middle cranial fossae, and extracranial region. The tumor was completely removed. Histological diagnosis was eosinophilic granuloma. No evidence of recurrence was found 14 months later. MR imaging is useful for diagnosis of the lesion, and particularly for surgical management.

Recurrence after incompletely resected acousticus neurinomas.

It is known that the recurrence rate is high when an acoustic neurinoma is incompletely excised, but the details of the process of recurrence are still unclear. We reviewed the recurrence of acoustic neurinomas, as evaluated by computed tomography (CT) or magnetic resonance imaging (MRI), in 51 consecutive patients over the past 10 years who underwent tumor resection and had postoperative follow-up by CT or MRI. The factors promoting recurrence related to incomplete excision of tumor were analyzed. Total resection of the tumor was performed in 22 patients (43%), nearly total resection in 17 patients (33%) and subtotal resection in 12 patients (24%). The recurrence rates were 29% (5 of 17 patients) and 25% (2 of 8 patients) for nearly total and subtotal resection, respectively, whereas no recurrence occurred following total resection. Of the patients having nearly total resection, all recurrences arose from residual tumor in the internal auditory meatus (5 of 12 patients). In contrast, there were no recurrences from residual tumor at the site of the brain stem (none of 4 patients). Acoustic neurinomas should be totally removed through neurosurgical and neuro-otological approaches whenever possible. If a small fragment of tumor is left in the internal auditory meatus due to fear of damaging the facial or cochlear nerves, it should be strictly followed up by CT or MRI.

[Ceftazidime concentration in human prostatic tissue and serum following intravenous injection].

The present study was undertaken to evaluate the penetration of Ceftazidime (CAZ) into prostatic tissue (P) and serum (S). Thirty-seven patients with benign prostatic hypertrophy took part in this study. CAZ was administered intravenously at a dose of 1g preoperatively. Blood samples were taken simultaneously at the time of tissue sampling by transurethral resection of the prostate (TUR-P). The patients were divided into 3 groups. In group 1 (twelve patients), intravenous injection of the drug was given 60 minutes before tissue sampling. The mean concentrations of CAZ were 23.4 +/- 7.4 micrograms/g in the prostatic tissue and 45.0 +/- 15.9 micrograms/ml in the serum. In group 2 (thirteen patients), the injections were given 120 minutes before tissue sampling. The mean concentrations of CAZ were 18.0 +/- 8.2 micrograms/g in the prostatic tissue and 39.8 +/- 21.3 micrograms/ml in the serum. In group 3 (twelve patients), the injections were given 240 minutes before tissue sampling. The mean concentrations of CAZ were 11.3 +/- 3.2 micrograms/g in the prostatic tissue and 22.3 +/- 9.6 micrograms/ml in the serum. These findings indicate that CAZ is useful for the treatment of prostatitis and preventive medication before TUR-P.

Amino acid sequences of the ribosomal proteins HL30 and HmaL5 from the archaebacterium Halobacterium marismortui.

The complete amino acid sequences of the ribosomal proteins HL30 and HmaL5 from the archaebacterium Halobacterium marismortui were determined. Protein HL30 was found to be acetylated at its N-terminal amino acid and shows homology to the eukaryotic ribosomal proteins YL34 from yeast and RL31 from rat. Protein HmaL5 was homologous to the protein L5 from Escherichia coli and Bacillus stearothermophilus as well as to YL16 from yeast. HmaL5 shows more similarities to its eukaryotic counterpart than to eubacterial ones.

Organization and nucleotide sequence of a gene cluster coding for eight ribosomal proteins in the archaebacterium Halobacterium marismortui.

A DNA fragment containing the genes for the eight ribosomal proteins HmaL3, HL6, HmaL23, HmaL2, HmaS19, HmaL22, HmaS3, and HmaL29 from Halobacterium marismortui has been cloned and sequenced. The organization of this gene cluster in general corresponds to the S10 operon of Escherichia coli although there exists some differences between them. The sequence analysis of the 5'- and 3'-region of the gene cluster revealed three open reading frames (orf1, orf2, and orf3) which do not code for any ribosomal protein whose structure is known. A putative promoter is located upstream of orf1. Out of the eight ribosomal proteins five have counterparts in eubacteria only, two in both eubacteria and eukaryotes, and one is exclusively related to an eukaryotic ribosomal protein.

Effects of polymyxin B on superoxide anion release and priming in human polymorphonuclear leukocytes.

We studied the effect of a potent inhibitor of protein kinase C, polymyxin B (PMXB), on superoxide anion (O2-) release by human polymorphonuclear leukocytes (PMNL). PMXB was compared with another inhibitor of protein kinase C, 1-(5-isoquinoline-sulfonyl)-2-methyl piperazine (H-7). Both PMXB and H-7 inhibited phorbol myristate acetate (PMA)-stimulated O2- release. Formyl-methionyl-leucyl-phenylalanine (FMLP)-stimulated O2- release by cytochalasin B-treated PMNL was not inhibited significantly by either PMXB or H-7. 1-Oleoyl-2-acetyl-glycerol (OAG,25-100 microM) stimulated PMNL to release O2- with a long lag-time (8-10 min). Although H-7 inhibited OAG-stimulated O2- release, PMXB augmented the OAG-stimulated response by increasing rate and reducing lag time. The augmenting effect of PMXB was evident only when added after stimulation by OAG, with maximum effect observed at 3 min after addition of OAG. The augmenting effect was also seen with PMXB immobilized on agarose beads. PMXB did not affect the respiratory burst response to 1,2-dioctanoylglycerol. PMXB-augmented, OAG-stimulated O2- release was inhibited by the addition of H-7 before OAG. In contrast to the effect on O2- release, OAG-stimulated protein phosphorylation was inhibited similarly by either PMXB or H-7, when these agents were added 3 min after stimulation by OAG. These results suggested that initial activation of protein kinase C by OAG is essential for O2- release, but that PMXB acts in a manner independent from protein kinase C to augment OAG-stimulated O2- release. When priming by OAG for enhanced O2- release (as opposed to direct stimulation of O2- release) in FMLP-stimulated PMNL was examined, PMXB inhibited O2- release in OAG-primed PMNL, suggesting that protein kinase C is involved in priming of PMNL by OAG.

Single photon emission computed tomography (SPECT) using N-isopropyl-p-(123I) iodoamphetamine (IMP) in the evaluation of patients with epileptic seizures.

To evaluate the clinical usefulness of IMP SPECT in the diagnosis of epilepsy, 6 normals and 52 patients in the interictal phase were studied. Thirty min after an intravenous injection of 111 MBq IMP, SPECT was performed using a rotating gamma camera. Of 21 patients with simple partial seizures, a localized decrease of uptake was shown in 16, and an increase in 3. Topologically, these findings corresponded well to the ictal symptoms. Nine of 13 patients with localized epileptic EEG had a good correspondence between the findings on EEG and IMP SPECT. In 20 of 23 with complex partial seizures, the coronal images showed laterality of uptake in the temporal lobes, whereas the CT was normal in 14. However, these findings on IMP SPECT agreed with the EEG in the temporal leads in only 5 cases. Of 8 patients with primary generalized seizures, a diffuse cerebral decrease was shown in 3 of 4 patients with convulsive seizures (grand mal), and a normal uptake in 3 of 4 patients with non-convulsive seizures (petit mal). However, 2 patients showed a localized decrease, therefore, we determined that they suffered from partial seizures evolving to secondarily generalization. From these data, we concluded that IMP SPECT could be a useful method in the diagnosis of epilepsy.

[Five cases of listeriosis in the elderly].

We reported five cases of listeriosis (sepsis and meningitis) in the elderly in our hospital during the last 4 years, where no cases of listeriosis had been found. These 5 cases had diabetes mellitus, lung cancer, chronic respiratory failure, gastric ulcer and aplastic anemia respectively as their underlying diseases. At the onset of listeriosis, 3 cases received corticosteroid and 3 cases received H2-blocker. 2 patients were cured and 3 patients died. Three autopsy cases had meningitis or meningoencephalitis and 2 cases of these autopsy cases had granulomatous changes in these spleens. In serotypes of Listeria monocytogenes (L. monocytogenes), 4 cases were 4b and 1 cases was 1b. All 5 strains were resistant to 3rd generation cephems. Wide uses of 3rd generation cephems and H2-blocker may be one of the reasons for the recent increase of listeriosis. Ingestion of contaminated food is the pathogenetic mechanism for initiating L. monocytogenes infections. And following the change of eating habits and the increase of imported foods, food-born listeriosis may increase. We suppose the increase of L. monocytogenes infections and must give attention to L. monocytogenes infections.
