In an attempt to delineate the differential characteristics of serum from hypertensives on the proliferation of vascular smooth muscle cells (VSMC), we compare the effect of serum from spontaneously hypertensive rats (SHR) with that from normotensive Wistar-Kyoto rats (WKY) in its ability to stimulate proliferation in VSMC. Cell number determination showed that 10% serum from either of the two strains substantially stimulated proliferation of serially passed cultured VSMC, the effect being significantly greater with SHR serum than with WKY serum. 3H-thymidine incorporation into newly synthetized DNA was also tested after treatment with 10% SHR or WKY serum. A substantial increase in the uptake was noted in response to the rat serum, the effect tending to be greater with SHR serum than with WKY serum. No difference was found between the two strains in cell size determined as intracellular water volume. These data provide the evidence of an increased growth stimulating activity of SHR serum. This abnormality may be superimposed on the already-known innate hyperproliferative capacity of VSMC of SHR, leading to an increase in the deterioration of vascular complications seen in this model.
Two cases of traumatic testicular luxation are described. Patients were a 21 year-old man and a 19 year-old man who were involved in a traffic accident. They were admitted to our hospital with the complaint of right groin mass after the traffic accident. Surgical replacement of the testis, which was dislocated at the right inguinal region, was successfully carried out in both cases. These cases are the 57th and 58th cases of traumatic testicular luxation reported in the Japanese literature.
The usefulness of MRI in diagnosis of a basal encephalomeningocele case is reported. A 72-year-old woman with a several-year history of occasional watery rhinorrhea and previous histories of meningitis at the ages of 55 and 65 is the subject of this report initially. After referring to the department of otorhinolaryngology, she was admitted to our department for close examination. CT scan showed only a mass forming a cyst in the nasal cavity, but MRI demonstrated part of the left rectal gyrus invaginating into the ethmoid sinus and nasal cavity. The mass was concluded to be a basal encephalomeningocele of a transethmoidal type, and dural plasty by bifrontal craniotomy was performed for the CSF rhinorrhea. MRI has numerous advantages over CT, e.g. the multiplanar capability without artificial dilution of the image of surrounding bones makes it the most valuable image technique for skull base lesions, making the diagnosis of basal encephalomeningocele much easier than it would be using conventional methods.
Behçet's disease is a systemic inflammatory disorder capable of inducing thrombophlebitis. Clinicopathologically, it is characterized by neutrophil and platelet hyperfunction. Lesions are formed presumably because neutrophils infiltrating the affected tissue release active oxygen and lysosomal enzymes in large amounts. The mechanism of neutrophil hyperfunction was obscure, but in recent years attention has focused on the effects of cytokines released by mononuclear cells. As more has been learned about the etiopathogenesis of Behçet's disease, novel anti-inflammatory drugs, immunosuppressants, anti-thrombotic agents, and anticoagulants have helped to achieve remarkable progress in treatment. Particularly notable is the effect on severe uveitis of cyclosporine, which is more effective than colchicine and conventional immunosuppressants. With the advent of cyclosporine, the number of patients with Behçet's disease who lose their eyesight has clearly decreased.
To elucidate the usefulness in prediction of hypersensitive reactions of L-asparaginase (L-asp), anti-L-asp IgG antibody was measured by two kinds of enzyme-linked immunosolvent assay (ELISA) with improved microplate method and original immunobeads method. The correlation coefficient of both methods matched very well (R = 0.949). Nine out of the 10 patients who were treated with L-asp for the first time showed the elevation of the anti L-asp IgG antibody and returned to normal range within 9 months thereafter. Twenty-two out of 26 patients (84.6%) who had ever developed hypersensitive reactions showed the positive anti-L-asp antibody. There was no positive correlation between the anti-L-asp titer and the severity of the hypersensitive reactions. We concluded that the ELISA with microplate was simple and useful methods to determined the anti-L-asp IgG antibody.
Among 329 cases with comatose state caused by severe head injury, 32 had primary intraventricular hemorrhage as revealed on initial CT scan (13.4%). We divided traumatic intraventricular hemorrhage into 4 types. Type 1: massive hemorrhage (9 cases). Type 2: subependymal hemorrhage (8 cases). Type 3: damage of fornix or septum pellucidum (8 cases). Type 4: Nieveau (7 cases). Most of these cases were caused by traffic accident. Shearing injury may be the most accurate mechanism to produce the intraventricular hemorrhage. Shear strain in severe head injury should cause tears of the subependymal vein, fornix, septum pellucidum and choroid plexus. 8 cases out of 32 ventricular hemorrhage showed tear of the subependymal vein. 8 of our cases showed damage of the fornix of septum pellucidum. These results suggested that the anatomical structure of the fornix and septum pellucidum were weak points for shearing force. 20 cases in 32 with ventricular hemorrhage died. These cases were frequently associated with other traumatic lesions, namely contusion of white matter and grey matter, brain stem lesions and cerebellar contusion, caused by shearing injury. Therefore, the prognosis of severe head injury with intraventricular hemorrhage is poor.
The author analyzed codon 347 of the rhodopsin gene using PCR (polymerase chain reaction) amplification and restriction enzymes in 19 unrelated Japanese families including 28 patients with autosomal dominant retinitis pigmentosa (ADRP). An allele of codon 347 mutation was found in a family (father and daughter). Sequence analysis shows that the mutation is from CCG to CTG. This mutation appears to be the cause of one form of ADRP, since it was also found in Japanese cases of ADRP which have a different racial background from families reported by Dryja et al.
Peroxisomes in higher animals had been considered as a fossil organelle. But, the discovery of peroxisomal fatty acid beta-oxidation system in 1976 revived a great deal of interest. In the last decade, peroxisomes have been shown to have an important role in the lipid metabolism. In accord with this progress, the presence of peroxisome diseases has been established. The peroxisomal disorders recognized at present comprise 12 different diseases, with neurological involvements in 10 of them. Recent studies have identified at least eight complementation groups in peroxisome-deficiency disorders, and indicated that currently used clinical categories do not represent distinct genotypes.
Peroxisomes contain hydrogen peroxide-forming oxidase(s) and catalase. Mammalian peroxisomes had been considered as a fossil organelle, since other important metabolism was not found. The discovery of a new fatty acid beta-oxidation system in rat liver peroxisomes in 1976 revived a great deal of interest of this organelle. Biochemical features and molecular aspects of individual enzymes of this system are summarized. It has also been elucidated that peroxisomes have important roles in biosynthesis of cholesterol, bile acids, and plasmalogens. These metabolisms together with some oxidases are summarized briefly.
Hypertension-hypervolemia therapy (HHT) is widely employed for treatment against vasospasm after subarachnoid hemorrhage (SAH). A few investigations have been reported to establish the fact that HHT results in a high incidence of congestive heart failure and pulmonary edema as well as deterioration of brain edema. From the point of view that the cerebral circulation is not independent of the systemic circulation, the authors investigated the effect of HHT on the systemic circulation of patients with SAH. In 72 patients, intracranial pressure (ICP), pulmonary catheter wedge pressure (PCWP), pulmonary arterial pressure (PA), central venous pressure (CVP), arterial pressure (AP), cardiac index (CI), arterial blood gas (ABGS), electrocardiogram (ECG), serum and urine electrolytes were monitored postoperatively. Furthermore, among these patients, the flow (Flow), volume (Volume) and velocity (Velocity) of the cortical vessels were monitored by means of a Laser Doppler in 25 patients. A cisternal or spinal drain was placed in all of the patients. Elevation of PCWP and CVP and Flow were observed when 300ml of 10% glycerol was administered within a period of 30 minutes, whereas administration of the same dose of glycerol over a period of 60 or 120 minutes caused no significant changes on these parameters. Elevation of PCWP and CVP and decrease of CI and Flow, occasionally associated with premature ventricular contraction (PVC), were observed in some patients when 100ml of 25% albumin was administered. However, administration of the same dose of albumin over a period of 120 or 240 minutes did not cause deterioration of the cardiac function. These facts could be explained by Guyton's law in which massive transfusion causes cardiac dysfunction.(ABSTRACT TRUNCATED AT 250 WORDS)
The clinical usefulness of an angiotensin-converting enzyme inhibitor on the posturally induced fall in creatinine clearance (ClCr) was studied in patients with biopsy-proved chronic glomerulonephritis (CGN). After a change from supine to upright position, ClCr fell significantly in all patients. This fall was significantly ameliorated by the pretreatment with captopril (fall in ClCr 28.8 +/- 14.3% for nontreated vs 16.5 +/- 11.5% for treated patients; n = 12; p less than 0.05). The ameliorating effect was more marked in patients with mesangial cell proliferation (MP) than those without it (fall in ClCr 7.2 +/- 6.3% for MP vs 17.1 +/- 12.3% for no MP; n = 6; p less than 0.05). The results suggest that captopril apparently had a renoprotective effect on the orthostatic acute fall in ClCr in patients with CGN, especially those with MP.

Complication of secondary amyloidosis is becoming an important problem in the management of rheumatoid arthritis in Japan, because patients with rheumatoid arthritis, complicated by amyloidosis, have been increasing recently and the prognosis is poor in most cases. Renal and gastrointestinal symptoms are cardinal features of amyloidosis in rheumatoid arthritis, and most of these patients die of renal failure or cachexia, based on gastrointestinal involvement, within several years. Diagnosis in the early stage is recommended for good management of amyloidosis in rheumatoid arthritis, and gastroscopic biopsy appears to be the most available method for early diagnosis of amyloidosis.

Although many urodynamic techniques have been used to evaluate the function of the lower urinary tract, the principles of these measurements are only based on a relationship between flow rate and pressure. In addition to these measurements, the intravesical urine stream would provide useful information on urodynamics. We have experimentally attempted to visualize the urine stream in a bladder using transrectal ultrasound examination. Since an air-particle is a good target for ultrasound visualization, we instillated a 10% glycerine solution into the bladder for the maintenance of air-particles. Before instillation, this glycerine solution was strongly shaken so that air-particles were kept in the solution. The urine stream was successfully observed throughout the whole process of voiding. In video-recording analysis, the velocity of urine stream was measured by tracing some air-particles. The velocity of urine stream was approximately 28.4 cm/sec in the body and increased to 116.6 cm/sec at the bladder neck. Thus, the urine stream was accelerated toward the urethra during voiding. Therefore, it seems that visualization of the urine stream offers a new parameter for evaluation of urodynamic studies.
