Prognosis of thyroid carcinoma.

With 612 cases of malignant thyroid tumor, the follow-up observations were conducted from 7 to 11 years after initial examination, and 512 cases were follow up. It was found that 46 cases died of the primary cancer, while 13 died of other causes. The prognosis proved to be favorable with adenocarcinoma but with anaplastic carcinoma or squamous cell carcinoma the disease proved to be ultimately fatal. The survival rates 5 and 10 years after diagnosis were 97.4% and 95.8% for papillary adenocarcinoma, 95.6% and 92.5% for follicular adenocarcinoma, 7.7% and 7.7% for anaplastic carcinoma, 0% for squamous cell carcinoma, and 66.9% and 66.9% for malignant lymphoma. As to the causes of death, the obstruction due to the neck tumor ranks first, indicating that respiratory deaths are numerous. However, as there are histologically many deaths due to the transformation from adenocarcinoma to squamous cell carcinoma or anaplastic carcinoma, it is desirable to perform radical surgery at an early stage.

[Clinical studies of cefoxitin for the treatment of chronic respiratory tract infections (author's transl)].

1. Serum and sputum concentrations of cefoxitin were measured at fixed intervals following a one hour drip infusion of 2 g to a total of three patients with chronic respiratory tract diseases. The peak levels in serum were found to be 142.1-273.6 microgram/ml at the end of the infusion and those in sputum were found to be 0.92-2.30 microgram/ml at 2 to 4 hours after initiation of the infusion. 2. Cefoxitin was administered to a total of five patients with chronic respiratory tract infections who had failed to respond to the previous treatments with conventional antibiotics. The results were that in one case the therapeutic effect was judged as excellent, in three cases as good, and in one case it was difficult to evaluate. In all cases, there was no significant abnormality indicative of side effects regarding clinical symptoms and laboratory tests of renal and hepatic functions. In a view of the findings stated above, it is considered that cefoxitin is a new antibiotic which can be used for the treatment of chronic respiratory tract infections.

Molecular weight of the undegraded polypeptide chain of Pseudomonas amyloderamosa isoamylase.

Crystalline isoamylase of Pseudomonas amyloderamosa was found to be contaminated with a trace of proteolytic enzyme. This contaminant digested the isoamylase under neutral or alkaline conditions, especially in the presence of sodium dodecyl sulfate (SDS). A reliable molecular weight of the enzyme was obtained by SDS-polyacrylamide gel electrophoresis and by gel filtration on Sepharose-6B in 6 M guanidine-hydrochloride after heat inactivation of the contaminant. The molecular weight of the undergraded polypeptide chain of the isoamylase was about 90 000. The lower molecular weight and the subunit structure of the enzyme reported previously are incorrect.

Temporal bone histopathologic features in Fanconi's anemia syndrome.

A histopathologic study of the temporal bones from a 7-year-old girl with Fanconi's anemia syndrome demonstrated (1) hemorrhage in the submucosal layer and the cavity of the middle ear and mastoid, massive in the right ear; (2) hypocellularity of the bone marrow; (3) minor but multiple anomalies of the middle ear; and (4) hypodevelopment of the hook portion of the cochlea and reduced overall length of the cochlear duct. The histopathologic features of these temporal bones appears to suggest that congenital anomalies of the inner ear, as well as those of the external and middle ears, would be possible causes of the deafness that accompanies Fanconi's anemia syndrome.

Clinical evaluation of 201thallium chloride scan for thyroid nodule.

Thyroid scintigraphy with Tl in the evaluation of nodules was investigated. A Tl scan was performed on 55 patients with thyroid nodules previously scanned with Tc or I. In the case of 16 carcinomas, accumulation of Tl was shown in every case. In addition, some lymph node metastases were depicted. (Accumulation was noted also in some of the metastatic lymph nodes.) Cold defect was delineated in ten cases of colloid and fetal adenoma, although accumulation was shown in four cases of Hürthle and embryonal adenoma. Six cases of degenerative cysts were delineated as cold defect. As for colloid nodular goiter, the affinity to the individual nodule was not constant. In general, degenerative and cystic nodules showed less activity and hyperplastic solid nodules showed more activity than normal tissue. We concluded that although a Tl scan cannot differentiate between benign and malignant tumors satisfactorily, it is nevertheless useful for determining surgical indication, as the case showing Tl accumulation on the scan is malignant or benign tumor with poorly differentiated solid type and possiblility of further growth and malignant change.

Differential diagnosis of hypoparathyroid disorders during childhood.

Changes in the plasma and urinary adenosine 3', 5' monophosphate (cyclic AMP) levels on infusion of parathyroid hormone were studied in 10 normal children, 5 children with idiopathic hypoparathyroidism, and 3 children with pseudohypoparathyroidism. In normal children the plasma concentration of cyclic AMP ranges from 18.0 to 68.0 pmoles/ml under basal conditions, and increases to a peak of about 10-fold the basal level 5 min after infusion of parathyroid hormone. Infusion of parathyroid hormone caused no increase in plasma cyclic AMP in patients with pseudohypoparathyroidism but about 18-fold increase in those with idiopathic hypoparathyroidism. The measurement of plasma cyclic AMP before and 5 min after infusion of parathyroid hormone is concluded to be a single and effective method for differential diagnosis of hypoparathyroid disorders.

Complication of laparoscopy in Japan.

Trends of examinees and complications of laparoscopy were examined by a questionaire sent to all representative institutions in Japan. As a conclusion, the rate of complication by this examination was 10 times higher, while its mortality was 25 times higher, as compared with gastroenterological endoscopic examination. This article may indicate the present status of laparoscopy in Japan.

Peritoneoscopic findings and liver function in chronic liver disease.

In 100 cases with chronic liver disease, peritoneoscopy and liver biopsy were performed, and the association between peritoneoscopic findings and liver function was studied. With respect to liver function, a series of tests which seem to indicate residual liver function were selected as follows; dye excretion, serum levels of albumin and cholinesterase, and subfractions of serum proteins. In evaluating the periotoneoscopic view, new criteria for the classification of liver surface findings are proposed. All cases were divided into 5 types in accordance with these criteria. Analysis of the correlation between peritoneoscopic findings and liver function showed that the degree of liver dysfunction was clearly indicated by liver surface alterations. As a result, the residual liver function may be determined quite well by using this classification.

Binding of bullous pemphigoid antibodies to basal cells.

Eight out of 12 serum samples from patients with bullous pemphigoid having basement membrane zone antibodies gave positive binding not only to the basement membrane zone but also to the basal cell membrane and/or cytoplasm as observed by complement immunofluorescence. Reaction of fluorescein labeled pemphigus vulgaris gamma-globulins, binding mainly to the lower intercellular spaces of the epidermis, was greatly reduced by the prior incubation of high-titered bullous pemphigoid sera having the reactivity to the basal cells, while that of fluorescein labeled pemphigus foliaceus gamma-globulins binding mainly to the upper and middle intercellular spaces, was not influenced by the prior application of these bullous pemphigoid sera. These results indicate that bullous pemphigoid antibodies are heterogenous and can be classified into 2 types and that some cross reaction is present between pemphigus antibodies and bullous pemphigoid antibodies having the reactivity to the basal cells.

Functional pathology of aldosterone-producing adenoma.

In addition to the morphological examination, the measurement of the content of corticosteroids was done in aldosterone-producing adenoma. Histologically, the adenoma consists of four types of cells. The major component of the adenoma was clear-type cells. In this type of cells, the activities of 3 beta-hydroxysteroid dehydrogenase and glucose-6-phosphate dehydrogenase showed weakly positive. The fine structure was characterized by numerous lipid vacuoles and poor organellae. On the contrary, intermediate- and compact-type cells revealed higher than moderate activities of these enzymes. Moreover, marked development of smooth endoplasmic reticulum and mitochondria was noted. Zona glomerulosa-type cells were observed only in a small part. The content of aldosterone and corticosterone in aldosterone-producing adenoma was significantly larger than those of the other type of adenoma. A tendency of positive correlation between the increase of compact-type cells and content of aldosterone was found. The cell origin of this aldosterone-producing adenoma and functional role of clear- and compact-type cells were also discussed.

Formation and Purification of Serratia marcescens Arylsulfatase.

The effects of culture conditions on arylsulfatase production by six strains of the genus Serratia were studied. Synthesis of arylsulfatases in all six strains was repressed in media with inorganic sulfate or methionine as the sole source of sulfur and derepressed by the addition of tyramine. Serratia marcescens IFO 3046 grew most rapidly and produced a high level of arylsulfatase when cultured on mannitol with inorganic sulfate and tyramine. The derepressed synthesis of arylsulfatase in S. marcescens was not subject to strong catabolite repression. The molecular weight of purified arylsulfatase was determined to be between 46,000 and 49,000. Arylsulfatase from S. marcescens differed in K(m) and V(max) values, substrate specificities, fluoride inhibition, and electrophoretic mobility from the enzyme from K. aerogenes, but had the same molecular weight as the latter.
