[Identification of effective region of the pons in response to inaperisone which facilitates urine storage].

To identify the effective region of the pons in response to inaperisone which facilitates urine storage, inaperisone (100 mM, 0.2 microliters) was injected into the nucleus locus coeruleus alpha (LCa, the pontine micturition center), the nucleus locus subcoeruleus (LSC, the pontine urine storage center) and the nucleus reticularis pontis oralis (PoO, micturition inhibitory region) of the decerebrate cats. On reflex micturition, inaperisone injection into the LSC decreased voiding volume, and increased residual volume and bladder capacity, significantly. However, there was no difference in the maximum bladder pressure before and after inaperisone injection into the LSC. Inaperisone injection into the LCa or the PoO had no influence on reflex micturition. These results suggest that effective region of the pons in response to inaperisone is the LSC, and that inaperisone facilitates the urine storage neural mechanism in the LSC.

[A case of Fahr's disease associated with juvenile rheumatoid arthritis].

We studied a case of Fahr's disease type idiopathic intracerebral calcification (Fahr's disease) associated with juvenile rheumatoid arthritis. The patient was a 15-year-old male with a chief complaint of gait disturbance. His family members had no similar signs and symptoms. His parents had no consanguinity. He was born with the normal perinatal course at 1967. He had repeated episodes of convulsive attacks during fever elevation from 2 years and 8 months to 9 years of age. Morning stiffness of bilateral hands, and pernio in the auricles, fingers, planta, and toes had occurred in every winter, since 6 years old. Swelling and pain of the bilateral knee and foot joints appeared, making ambulation difficult in 1983 (15 years old), and the patient was admitted to our hospital in July, the same year. On admission, congenital anomalies such as epicanthus and high-arched palate were noted, and swelling, deformation and contracture of limb joints, and Raynaud phenomenon were shown. His ocular fundus showed no arteriosclerotic change. He didn't have Albright's sign. Mild mental retardation and bilateral pyramidal tract signs were noted, but extrapyramidal tract and cerebellar signs, and sensory disturbance were absent. Laboratory findings exhibited markedly elevated ESR, positive CRP, RA, and antinuclear antibody. The levels of serum Ca, P, alkaline phosphatase and parathyroid hormone were normal. Peripheral blood study showed microcytic and hypochromic anemia. Anti-DNA antibody was negative. Ellsworth-Howard test was positive. Elevated antibody titer to toxoplasma, rubella virus, herpes simplex virus and cytomegalovirus were not proven. He had no chromosomal change.(ABSTRACT TRUNCATED AT 250 WORDS)

[Computerized tomography immediately after surgery in the neurosurgical operating theater].

A TCT-300 scanner (manufactured by the Toshiba Co., Tokyo) has been installed in the operating room of Shinshu University Hospital since 1986. This neurosurgical operating CT scanner system was developed for obtaining intra- and postoperative CT images in the operating room. We have carried out immediate postoperative CT scanning in 206 cases: 170 were major and 36 were minor operations. A mobile CT scanner gantry has been used in 125 cases since June, 1988. We obtained CT images immediately after surgery on the digitalized operating table, the motion of which can be controlled as with the conventional CT scanner table. Immediate postoperative CT scans showed the extent of removed tumors or hematomas, position of the tip of ventricular or cisternal tubes, injury to the surrounding normal brain caused during the removal of lesions, and postoperative complications such as hemorrhage, brain swelling and surgical patties which had been inadvertently left in the wound. This CT scanner system in the operating room proved to be useful in the postoperative care of neurosurgical patients.

[Clinical study of vitamin E status in patients with chronic liver diseases].

Vitamin E status was evaluated in patients with chronic liver diseases (chronic hepatitis, CH; n = 27 and liver cirrhosis, LC; n = 32) by the ratio of plasma alpha-tocopherol to plasma lipids (P-toc/lipid) and red blood cell alpha-tocopherol (RBC-toc). P-toc/lipid and RBC-Toc were significantly reduced in LC patients compared to controls (n = 31). There was a significant correlation between P-Toc/lipid and RBC-Toc values. Low vitamin E status was found in 3 patients with CH (11.1%) and 12 patients with LC (37.5%), but their P-Toc values were not necessarily reduced. Serum beta-lipoprotein concentration (beta-lipo) did not differ significantly between the low vitamin E group and the normal vitamin E group, but the ratio of P-Toc to beta-lipo was significantly reduced in the low vitamin E group. No significant differences were found in the clinical features or liver functions tests including parameters of hepatic synthetic functions and cholestasis between the two groups. Factors other than a cholestasis or an impaired synthesis of carrier protein may also be responsible for the frequent occurrence of low vitamin E status in patients with LC.

[Non-invasive imaging diagnosis of left renal vein compression causing hematuria. Part 1. Ultrasonography].

Left renal veins of 100 out-patients were examined by transabdominal ultrasonography to evaluate its usefulness in determining left renal vein compression which is causing renal bleeding. Ultrasonography revealed the left renal vein in 86 patients. In 61 of the 86 cases, the internal cavity of left renal vein was opened at least in diastolic phase, but in 23 cases, the internal cavity was closed between the abdominal aorta and the superior mesenteric artery in both systolic and diastolic phases. In the remaining 2 cases, left renal vein was compressed at a point where it intersected the right renal artery. Left renal vein compression was observed in 18 (69%) of the 26 cases which had been classified as idiopathic renal bleeding and in 7 (26%) of the 27 cases which had urinary tract diseases causing hematuria. In 33 cases which did not have hematuria, left renal vein compression was not observed. These results suggest that diagnosis of left renal vein compression causing renal bleeding is possible by transabdominal ultrasonography.

[Non-invasive imaging diagnosis of left renal vein compression causing hematuria. Part 2. CT].

Left renal veins of 77 patients were examined by computed tomography (CT) to evaluate its usefulness in determining the left renal vein compression which is causing renal bleeding. From CT image, left renal vein compression was observed in 6 (86%) of the 7 cases which had been classified as idiopathic renal bleeding, in 9 (21%) of the 42 cases which had urinary tract diseases causing hematuria, and in 3 (11%) of the 28 cases which did not have hematuria. In 15 of the 18 cases of left renal vein compression, left renal vein was compressed between the superior mesenteric artery and the abdominal aorta, showing so-called nutcracker phenomenon. In the remaining 3 cases, however, the superior mesenteric artery provided sharp delineation from the abdominal aorta. The superior mesenteric artery and the abdominal aorta made the mean angle of 35.5 degree in patients with normal left renal vein, the mean angle of 45.4 degrees in those with left renal vein compression without nutcracker phenomenon, and the mean angle of 11.9 degrees in those with nutcracker phenomenon. CT was superior to ultrasonography, in revealing left renal vein compression.

The effect of bovine brain gangliosides on essential tremor.

We reported the case of a 74-year-old woman suffering from essential tremor for 20 years which was treated effectively with purified bovine brain gangliosides containing GM1, GD1a, GD1b and GT1b gangliosides. The trials of the treatment were conducted twice, 40 mg and 20 mg gangliosides per day respectively administered intramuscularly. Essential tremor dramatically improved with gangliosides on the second day of treatment, suggesting that the effect of gangliosides was not induced by neuronal sprouting or regeneration. It is speculated that a certain type of essential tremor is a reversible disease of membrane disorder.

[A case of uretero-vaginal fistula with ureteral foreign body stone originated from the suture thread].

A 32-year-old female was admitted to our hospital with the chief complaint of vaginal discharge of urine. She had undergone radical hysterectomy due to uterine cancer at another hospital by a gynecologic surgeon 5 years earlier. X-ray examination showed a stone-like shadow at the left ureter without hydronephrosis. She was diagnosed with ureterovaginal fistula with the left ureteral stone. Left ureterolithotomy and ureterovesiconeostomy was performed. The stone revealed a foreign body stone originating from the silk worm-gut which had penetrated accidentally the ureter when the vaginal wall was sutured at the previous surgery. Including our case, 15 cases of foreign body stones in the upper urinary tract were found in the Japanese literature and none of them were associated with ureterovaginal fistula.

[A case of renal oncocytoma with multilocular cyst].

A 63-year-old woman was admitted to our hospital for a left renal tumor with multiple cystic lesions. Computed tomography and renal angiography revealed a hypovascular tumor 9 cm in diameter and multilocular cyst with a thick wall. Renal cell carcinoma and multilocular cystic nephroma were suspected, and left nephrectomy was performed. Histopathological diagnosis of this tumor was renal oncocytoma.

[Three cases of catamenial pneumothorax].

Three cases of catamenial pneumothorax were presented. A 42-year-old woman with intrapelvic endometriosis had recurrent right-sided pneumothorax with menstruation more than six times. At the operation only a bleb was noted at the apex of the right lung. In a 16-year-old woman who already had been operated on for recurrent right-sided pneumothorax one year ago, left-sided pneumothorax occurred on the day of the beginning of each menstruation four times. A small bleb was noted at the apex. A 29-year-old woman with cesarean section at the age of 25 had recurrent right-sided pneumothorax occurring on the day before the beginning of menstruation three times. Though the operation was performed during her menstrual period, neither air leakage nor any lesions were found. In all cases each lesion was resected and did not prove endometriosis, and partial parietal pleurectomy was performed for pleurodesis. No pneumothorax has recurred.

[A case of disproportionately large communicating fourth ventricle (DLCFV) combined with syringomyelia and Chiari malformation].

We report a rare case of disproportionately large communicating fourth ventricle (DLCFV) combined with syringomyelia and Chiari malformation. The case was a 27-year-old male who underwent ventriculoperitoneal (V-P) shunt on the right side for hydrocephalus caused by traumatic intracerebral and intraventricular hemorrhage. One month later, he became somnolent with posterior fossa symptoms (nausea, vomiting and nystagmus). CT scan demonstrated enlarged fourth ventricle, which was diagnosed as DLCFV because the ventriculogram revealed patency of the aqueduct. One and half month later a second V-P shunt was made on the left side to increase the shunt flow. He became ambulatory with a cane, although the fourth ventricle remained moderately dilated on CT scan. Two months after the additional V-P shunt, he slipped and hit the occiput and immediately became tetraparetic. The patient was treated conservatively under the diagnosis of central spinal cord injury. The MRI taken 2 months after the accident revealed Chiari malformation (type 1), syringomyelia and a dilated fourth ventricle which was compressing the brainstem. After the fourth ventriculoperitoneal (FV-P) shunt, the tetraparesis transiently improved but then again worsened. On the CT scan the syrinx did non change in size, while the size of the fourth ventricle became normal. After syringoperitoneal (S-P) shunt the patient showed a moderate improvement of tetraparesis. Unfortunately he suffered appendicitis complicated with peritonitis and all the shunts were immediately changed to external drainage. However, the patient developed meningitis and became paraplegic. The motor function of the upper extremities slightly improved by aspiration of fluid via the external drainage system from the syrinx.(ABSTRACT TRUNCATED AT 250 WORDS)

Esophagocutaneous fistula after anterior cervical spine surgery and successful treatment using a sternocleidomastoid muscle flap. A case report.

An esophagocutaneous fistula following anterior cervical fusion is rare. A 61-year-old man had cervical myelopathy because of ossification of the posterior longitudinal ligament of the cervical spine. Anterior decompression of the cervical spine and anterior fusion with strut bone grafting were performed. A second anterior fusion was done because the graft was dislodged after the patient fell out of bed one month after surgery. An esophagocutaneous fistula occurred three months after the second anterior surgery. One of the causes of this esophagocutaneous fistula was considered to be a pressure necrosis of the esophagus because of to projection of the bone graft. Conservative treatment, which consisted of wound drainage and intravenous administration of antibiotics, was tried but was unsuccessful. A good result was achieved by cancellous bone grafting, closure of the esophageal fistula, and transposition of a sternocleidomastoid muscle flap to the interspace between the esophagus and the cervical spine.

[MR imaging of aortic arch anomalies--an analysis of MR visualization of ductus arteriosus].

MR imaging of 10 patients with aortic arch anomaly was performed on a 1.0 T system. Coronal and sagittal images with a 5 to 6-mm-thick slice were taken in each patient and the visibility of the ductus arteriosus (ductus ligament) was detected on the sagittal images including its anatomical connection between the pulmonary trunk and arch vessel. The result indicates that MR imaging is of great help for detection of the ductus in case of aortic arch anomaly.

Surgical techniques and reattachment rates in retinal detachment due to macular hole.

We evaluated reattachment rates of the transvitreal and transscleral techniques for treating 250 eyes with retinal detachment due to macular hole. The initial success rate of the transvitreal approach was 56% (53/94). There was no difference between the results of gas tamponade alone and vitrectomy and gas tamponade. The initial success rate of the transscleral approach was 83% (130/156). Macular diathermy and macular buckling showed a higher reattachment rate than macular diathermy alone. The final success rates were the same (95%), regardless of which approach was selected as an initial technique. The patients whose initial transvitreal reattachment failed and who underwent additional transvitreal procedures (including macular laser photocoagulation) showed a success rate of 93%. We believe that gas tamponade, which has possibility of better visual prognosis, should be selected as an initial technique because the final success rate was found to be the same regardless of the initial surgical techniques.

Lysozyme levels in middle ear effusion and serum in otitis media.

Lysozyme concentrations in middle ear effusion and serum were determined in patients with otitis media with effusion. Lysozyme concentrations in middle ear effusion were significantly higher than in serum. Children with mucoid otitis media showed significantly higher levels of lysozyme in middle ear effusion than children with serous otitis media and adults with otitis media with effusion. Higher levels of lysozyme were observed in the group of children younger than 5 years old compared with the age group of 6- to 10-year-olds. Lysozyme concentrations of middle ear effusion in adults were significantly lower than those of mucoid otitis media in children. These results indicate that lysozyme plays an important role in the disease process of otitis media.
