Histiocytic hemophagocytosis in the bone marrow in children with sepsis and disseminated intravascular coagulation.

Two children with systemic E. coli and candidial infections developed disseminated intravascular coagulation (DIC). Bone marrow examination in both cases showed histiocytic hemophagocytosis, consistent with the diagnosis of the hemophagocytic syndrome. Histiocytic hemophagocytosis in the bone marrow, one of the markers of the activated mononuclear phagocyte system, might be common in patients with severe sepsis and DIC, especially in immunodeficiency.

Isolation and characterization of mouse FM3A cell mutants which are devoid of Newcastle disease virus receptors.

A method was developed to select host cell mutants which did not permit the replication of Newcastle disease virus (NDV), and 14 isolates of NDV-nonpermissive mutants of mouse FM3A cells were obtained. All these isolates were judged to be deficient in NDV receptors, since their ability to adsorb 3H-labeled NDV virions was markedly decreased. They were tested for genetic complementation in pairs by cell fusion and shown to fall into a single recessive complementation group, which was designated as Had-1. Vesicular stomatitis virus was able to replicate in this mutant to produce infectious progeny, but the glycoprotein of the released virion was abnormal in size, suggesting a defective processing of the asparagine-linked carbohydrate chains in the mutant cell. The Had-1 mutant was resistant to wheat germ agglutinin, but sensitive to a Griffonia simplicifolia lectin, GS-II, which recognizes terminal N-acetylglucosamine residues. The altered sensitivity to these plant lectins compared with that of the parental FM3A cells indicates that sialylated sugar chains on the cell surface are almost absent from the Had-1 cells, thereby rendering the cells NDV receptor deficient.

[Histochemical investigation on glycogen in the epithelium of mucosa under denture base].

Accumulation of glycogen in the epithelium of the oral mucosa was evaluated in relation to the existence of wound and inflammation of the oral mucosa histochemically and histopathologically. Antemolar rhombus (the anterior part of the intermolar palate) of adult Wistar rat was employed as experimental area. Experimental wound of palatal mucosa was made by excision of crestal one third of the palatine ruga. Inflammation of the connective tissue was artificially induced by the insertion of silk thread or silk thread immersed in 5% formalin in the connective tissue parallel to the crest line of palatine ruga. The experimental area was examined histochemically and histopathologically up to 72 hours after experimental operation. In excision wound the accumulation of glycogen was observed in prickle cell layer of the epithelium after 24 hours. Although the inflammatory reaction was observed more prominently in the thread inserted connective tissue, the accumulation of glycogen in the epithelium could not be detected. The findings of this study suggested that inflammation itself does not induce the accumulation of glycogen in the epithelium of the oral mucosa and the accumulation could be an indication of wound and/or its healing process of oral epithelium.

Malignant histiocytosis involving pancreas at initial presentation.

A 10-year-old boy with malignant histiocytosis presented with fever, hepatosplenomegaly, and diffuse pancreatic enlargement, mimicking acute pancreatitis. Although malignant histiocytosis involving pancreas at initial presentation is exceptional, this entity should be included in the differential diagnosis of acute pancreatitis, especially when hepatosplenomegaly and pancytopenia are present.

[An alpha-fetoprotein producing Sertoli-Leydig cell tumor--a case report].

A case of an ovarian, poorly differentiated, Sertoli-Leydig cell tumor in a 55-year-old woman is reported. The patient showed no virilization, but did show elevated serum alpha-fetoprotein (AFP) levels. The tumor, measuring 70 x 45 x 50 mm, consisted mainly of a diffuse proliferation of spindle cells. Among this proliferation, some tubules of Sertoli cells and groups of leydig cells were found. Immunoperoxidase studies, using anti-AFP, localized the AFP in the leydig cells.

[Electron histochemical alteration on the plasma membrane of biliary epithelial cells in obstructive jaundice].

The plasma membrane of rat's biliary epithelial cells were histochemically examined in experimentally induced obstructive jaundice. Ultrastructurally, atrophy of microvilli and bleb formation were observed in bile canaliculi and ductular cells. These morphological alteration and microfilament aggregation were still remained even 2 weeks after the biliary drainage of 2 weeks obstruction. Histochemically, the reaction products of ruthenium red (RR), alkaline phosphatase (ALPase) and Na+ K+ ATPase were localized on the luminal membrane of the ductular cells and bile canaliculi in the control. In the obstructive jaundice, the reaction products of RR and Na+ K+ ATPase were decreased, and ALPase were localized on the sinusoidal, lateral and bile canalicular membranes of hepatocytes. In the biliary drainage after 2 weeks obstruction, the reaction products on the ductular cells were decreased even 2 weeks later. However RR, ALPase and Na+ K+ ATPase were rearranged on the bile canalicular membrane compared with the ductular cell membrane. These findings suggest that the molecular changes of the plasma membrane of biliary epithelial cells in long term obstructive jaundice would be prolonged after the biliary drainage and the membrane vulnerability of the ductular cells would be different from that of the bile canaliculi.

[Studies on adhesion of MMA (TBB-O) resin to dentin. The efficacy of the primer treatment].

In this study the efficacy of the primer on adhesion of MMA (TBB-O) resin to dentin was investigated. The primer consists of two parts. One is 6% MTYA and 70% HEMA aqueous solution, and the other is 2% glutaraldehyde aqueous solution. As an etching agents, 10% citric acid-3% FeCl3 (10-3), 40% phosphoric acid, 10% citric acid, and 0.5 M EDTA were used. After etching, the two parts of the primer were mixed well and applied on the etched surface. Then MMA (TBB-O) resin or p-MBA/MMA (TBB-O) resin was applied on the treated surface. The tensile bond strengths after storage in 37 degrees C water were measured. It revealed that the primer treatment increased the bond strength despite the kind of etching agents. Though the thickness of the resin reinforced dentin was the same as that of demineralized dentin, it was not proportional to the bond strength. Then the durability of the adhesion to dentin treated with primer was examined by the thermal cycling tests. After 2000 thermocycles between 5 degrees C and 55 degrees C, the decrease in the bond strength was very small.

[Effects of nitroglycerin on the graft flow during aortocoronary bypass surgery].

The effects of an intravenous infusion of nitroglycerin on the hemodynamics and bypass graft flow to left coronary artery were studied in sixteen patients during coronary artery surgery. Systolic blood pressure, cardiac output and left ventricular minute work were not affected by 0.2 microgram/kg/min of nitroglycerin but graft flow volume especially diastolic graft flow increased significantly and the characteristics of flow velocity waveform in the graft became more diastolic predominant. By 1 microgram/kg/min of nitroglycerin a significant decrease in systolic blood pressure, cardiac output and graft flow were recognized, however diastolic graft flow was preserved despite of a decrease in left ventricular minute work. These findings demonstrate that nitroglycerin can be safely administered intravenously during aortocoronary bypass surgery, and suggest that nitroglycerin improves the balance of myocardial oxygen demand and supply, and relieves myocardial ischemia.

Effects of glycyrrhizin and glycyrrhetinic acid on (Na+ + K+)-ATPase of renal basolateral membranes in vitro.

Studies were made on the direct effects of glycyrrhizin and its aglycone, glycyrrhetinic acid on the activities of (Na+ + K+)-ATPase and (Ca2+ + Mg2+)-ATPase, a membrane bound Na+ and Ca2+-extrusion pump enzyme of the basolateral membranes (BLM) of canine kidney. Glycyrrhetinic acid inhibited the activity of the Na+-pump enzyme dose-dependently (IC50 = 1.5 x 10(-4) M), but had no effect on that of the Ca2+-pump enzyme of kidney BLM and homogenates. Glycyrrhizin also inhibited the Na+-pump enzyme activity but had less effect (IC50 = 2 x 10(-3) M). The effects of these compounds were due to competitive inhibition with ATP binding to the enzyme (Ki = 12 microM) and so were different from that of ouabain, which inhibits the Na+-pump by binding to its extracellular K+-binding site. The direct effect of glycyrrhetinic acid on the membrane may be important role in the multiple actions of licorice.

[Circulating mature NK (Leu11+) cells positive for IL2 receptor and HLA-DR in patients with various liver diseases and asymptomatic HBsAg carriers].

IL2R+ Leu11+ cells (A) and HLADR+ Leu11+ cells (B) of peripheral blood in patients with various liver diseases and ASCs were measured using double colour immunofluorescence assay of MoAb with FACS flow cytometry. 1) The mean % of IL2R+ Leu11+ cells which was 0.5 +/- 0.2 in healthy controls, decreased significantly in HCC in comparison with CALD, ASC and healthy controls, and in ASC rather than in healthy controls. They (A) were less than 0.1% in eight of thirteen cases with HCC, in one of twenty cases with CALD and of eleven ASCs, respectively. 2) In the mean % of HLADR+ Leu11+ cells which was 2.9 +/- 1.8 in healthy controls there was not a significant difference among HCC, CALD, ASC, AH and healthy controls. They (B) were less than 0.1% only in one case with HCC. 3) The value of fluorescence intensity of IL2R on IL2R+ Leu11+ cells reduced in B-CALD, ASC, AH and HCC, and one of HLADR on HLADR+ Leu11+ cells increased in CALD. These results suggested that the decrease of IL2R+ Leu11+ cells was due to the existence of HCC.

Glanzmann's thrombasthenia associated with a transient deficiency of factor XIII.

A three-year-old girl suffering from ecchymoses developed severe epistaxis. The diagnosis of thrombasthenia was made on the basis of platelet aggregation studies, flow cytometric analysis with monoclonal antibodies and gel electrophoretic analysis. In addition, coagulation studies at the time of epistaxis repeatedly showed a transient deficiency of factor XIII activity and antigen.

Virus-associated hemophagocytic syndrome: the diagnostic usefulness of immature histiocytes with benign features in the bone marrow.

A 2-year-old girl developed fever, hepatosplenomegaly, jaundice, lymphadenopathy and pancytopenia. Bone marrow examination revealed increased immature histiocytes (5.5%) and mature histiocytes with hemophagocytosis. All the abnormalities were normalized in one month without any chemotherapy. It was suggested that the presence of immature histiocytes with benign features, even if their number exceeds that of mature histiocytes, does not favor the diagnosis of malignant histiocytosis.

Antimicrobial activity of dactimicin in vitro compared with that of dibekacin, netilmicin, sisomicin and micronomicin.

Antimicrobial activity of dactimicin, a pseudo-disaccharide aminoglycoside antibiotic, was compared with those of dibekacin, netilmicin, sisomicin and micronomicin using clinical isolates of four Gram-positive and sixteen Gram-negative bacteria. Dactimicin was more active than the reference amino-glycosides against Serratia marcescens, especially gentamicin-resistant Serratia sp., Proteus vulgaris, P. rettgeri and Klebsiella oxytoca, but less active against Pseudomonas aeruginosa and P. mirabilis. Dactimicin was equally active as the references excepting netilmicin against Gram-positive bacteria and some Gram-negative bacteria including Escherichia coli, K. pneumoniae, Morganella morganii, Haemophilus influenzae, Citrobacter freundii, Enterobacter aerogens, E. cloacae, Acinetobacter calcoaceticus and Campylobacter jejunii. Dactimicin was active against resistant strains possessing various aminoglycoside-modifying enzymes including AAC(3)-1, by which dactimicin was acetylated.

Comparisons of maltase activities in kidney brush border membranes from normal, diabetic, glucose-infused and maltose-infused rabbits.

The specific activities of membrane-bound maltase (alpha-D-glucoside glucohydrolase, EC 3.2.1.20) in isolated brush border membranes (BBMs) of alloxan-induced diabetic, glucose-infused and maltose-infused rabbits were 30%, 140% and 160%, respectively, of those of control rabbits. Differences in the relative activities of trehalase (EC 3.2.1.28), another disaccharidase, in these groups were similar but less marked. However, the activities of two other marker enzymes of the brush border, alkaline-phosphatase and gamma-glutamyl transpeptidase, were similar in the 4 groups of rabbits. The decreases in the activities of the two disaccharidases were due to changes in the Vmax values of the enzymes without change in their Km values for maltose and trehalose. The maltase activities in the 4 groups showed similar dependences on Tris-HCl, KCl and NaCl. The electrophoretic profiles of the BBMs of the 4 groups on SDS-polyacrylamide gel showed slight differences. From these results, we conclude that diabetes, glucose infusion and maltose infusion probably change the concentrations of active enzymes in the BBM of the kidney in rabbits.

Deep sclerectomy with trabeculotomy ab externo: one-stage procedure (DS 1).

Our new full-thickness filtering procedure was performed on 20 eyes with narrow-angle glaucomas (15 patients, mean age 69 +/- 12 years) with high intraocular pressure (IOP) despite initial peripheral iridectomy. This procedure was intended to maintain anterior chamber depth completely during intraoperative and postoperative periods. Under conjunctival and thin scleral flaps, the deep (4 X 1.5 mm) scleral block containing the outer wall of Schlemm's canal was removed. After closing the scleral flap with eight interrupted sutures and the conjunctival flap with a running suture, a 1-mm spatula was inserted beneath the scleral flap, and the trabeculum was punctured. The mean preoperative and final postoperative IOPs were 34.8 +/- 17.2 mm Hg and 14.9 +/- 4.9 mm Hg, respectively. Follow-up time was at least 12 months. Final postoperative IOP was less than 21 mm Hg without oral medication in 16 eyes (80.0%). The only complication was slight bleeding from the punctured trabeculum. This bleeding was completely absorbed within a week.
