Median raphe cyst of the penis.

A case of median raphe cyst of the penis in a 7-year-old boy is reported. Although the lesion is rare, characteristic clinical features makes it easy to diagnose median raphe cyst of the penis, which has been mistakenly reported as apocrine hidrocystoma. Surgical excision must be performed to prevent recurrence.

[A case of ovarian carcinoma preceded by intracranial metastasis].

A 28-year-old woman with primary ovarian carcinoma which was found with intracranial metastasis is reported. The patient was operated on because of metastatic brain tumor with unknown primary lesion. Subsequently, left ovarian tumor was found, and left salpingo-oophorectomy was performed. The histological diagnosis was endometrioid carcinoma. A review of the literature shows that intracranial metastasis by ovarian carcinoma is very rare. The incidence is lower than 2.1%. The present case in which intracranial metastasis manifested itself before primary ovarian carcinoma seems to be extremely rare.

[Clinical studies on multiple lacunar state].

Sixty-five cases of clinically diagnosed multiple lacunar state have been analysed. The clinical course of multiple lacunar state is usually progressive in nature, simulating degenerative diseases, in contrast with the mode of acute onset seen in the other vascular diseases. Average age at the initial visit was 63.8 year-old, and sex ratio showed marked male preponderance, being 12 for males and 1 for females. Gait disturbance and speech disturbance are the most frequent initial symptoms, followed by slow motion, emotional lability and swallowing difficulty. Neurological manifestations are dysarthria, short-stepped and apraxic gait, hyperreflexia, positive Babinski and Chaddock reflexes, minimal spasticity, dementia, positive palmo-mental reflex, emotional lability, fixed face, rigidity, bradykinesia, foot grasping, dysphagia, positive Myerson's sign and tremor, in the order described. The important point is that the rigidity is paratonic and the tremor is action or postural, not the cogwheel rigidity or resting tremor like Parkinson's disease. The appearance of pathological reflexes (Babinski and Chaddock reflexes) are quite important, especially Chaddock reflex, which can frequently become positive despite negative or equivocal Babinski reflex. Hypertension, especially longstanding in nature, is the major contributing factor in this disorder. CT scan showed the presence of one or more lacunes in 52 out of 65 cases (80.0%). The detection of lacune can be influenced by the quality of CT scan, and the high resolution CT scanner is greatly useful for that purpose. It is stressed that the detailed neurological and computed tomographic evaluations will make it possible to reach the clinical diagnosis and appropriate treatment of multiple lacunar state.

Binocular and monocular optokinetic nystagmus in cats to textured visual patterns.

Binocular and monocular optokinetic nystagmus (OKN) to the motion of textured visual patterns have been studied in alert cats. In the temporonasal monocular stimulation at a velocity above 10 deg/sec and in the nasotemporal monocular stimulation, the motion of random noise or a checkerboard elicited larger slow phase eye velocities than the motion of stripes with regularly or randomly arranged vertical bars. To temporonasal stimulation at a velocity below 10 deg/sec, there were no differences in slow phase eye velocities according to the types of the textures. Binocular OKN showed similar texture preferences as temporonasal monocular OKN.

Alteration in directional specificity of interdental dimension discrimination with the degree of mouth opening.

The interdental dimension discrimination (IDD) ability was tested at seven different degrees of mouth opening: 40 mm, 30 mm, 20 mm and 10 mm openings for the dentate subjects, and 10 mm opening, intercuspal position (ICP) and the ICP minus 5 mm for the edentulous subjects. The subjects were asked to discriminate between pairs of sticks held alternately between the upper and lower incisors. The IDD ability, evaluated in terms of the 50% difference limen (DL), did not differ significantly between any two of the tests conducted at seven different openings, although the number of misjudgments of the dentate subjects tended to increase with the degree of mouth opening. On the other hand, the directional specificity of the IDD, which is a tendency to overestimate the dimension of the first stick of each pair, developed almost linearly with a decrease in the opening degree. These findings indicate that the behaviour of the receptors concerned varies with the intermaxillary distance. Some edentulous subjects could not perceive the presence of a small test stick at the overclosed position (ICP -5 mm), but vibration applied to the mandible enabled the subjects to detect the stick at the same opening. These results support the assumption that the muscle spindles of the jaw-closing muscles are the receptors responsible for the IDD.

Treatment of onychomycosis by ODT therapy with 20% urea ointment and 2% tolnaftate ointment.

20 patients with distal onychomycosis were given daily application of an ointment containing 2% tolnaftate and an ointment containing 20% urea under ODT. Following this, 17 of 20 patients developed onychomalacia and seven of these developed onycholysis 1 or 2 weeks later. The separated nails were cut as short as possible. Similarly to those patients with onychomalacia alone, occlusive dressing technique was continuously performed until the newly developed nails became macroscopically normal and no fungi were observed microscopically (responders). Following treatment, out of 20 patients, 14 responded. 5 patients who had a short course of treatment did not respond. Side-effects such as pain, hemorrhage and infection did not occur.
