Metal hypersensitivity in dentists: a patch test study.

Patch test of dental alloys was conducted on dentists in the Department of Prosthetic Dentistry, Hiroshima University School of Dentistry. The positive rate of the first patch test in the test group was 47.1%, and that in the control group was 10.0%, showing a large difference between the two groups. Furthermore, a second patch test was conducted on those who showed a positive response together with pustular or follicular reaction. As neither positive response nor pustular or follicular reaction was observed in the second patch test, the results were regarded as negative. Only a very slight difference in the positive rate could be observed when the results of the two tests were combined. The results of the present study suggest that the possibility is small for dentists to develop metal hypersensitivity in normal clinical practice.

[Comparison of ultrasound and computed tomography (CT) for the diagnosis of paraaortic lymphnode metastasis in patients with gynecologic malignancies].

Fifty-one patients with gynecologic malignancies were tested for paraaortic lymphnode metastasis by ultrasound and computed tomography (CT). Eleven patients were found to have paraaortic lymphnode metastasis by ultrasound, and CT also showed the same metastatic image in the same patients. The site and size of the positive image found by both means were identical. The other forty patients diagnosed by ultrasound as free of metastasis were also proved to have no metastasis by CT. Ultrasound is as useful as CT in the evaluation of paraaortic nodes. However, ultrasound is more convenient and easier to use than CT.

[A study of the proliferative activity of DNA polymerase alpha immunoreactivity in breast cancer].

DNA polymerase alpha activity is associated with cell proliferation, independently of the cell cycle phase, and a simple and reproducible method for the immunohistochemical demonstration of DNA polymerase alpha has been developed. In an analysis of a total of 76 human breast cancer tissues using this procedure, the clinico-pathological findings revealed that only the estrogen receptor (ER) significantly correlated with the DNA polymerase alpha activity. Therefore, this suggests that ER-negative tumors had a higher proliferative activity.

Nevus lipomatosus cutaneus superficialis of the scalp.

A 12-year-old boy with nevus lipomatosus cutaneus superficialis of the scalp is described. Asymptomatic plaques made up of varying sized aggregates of elastic soft papules were observed on the occipital region. Histologic examination of a biopsy specimen revealed ectopic fat tissue in the dermis. Two types of nevus lipomatosus cutaneus superficialis can be differentiated clinically. The classic types, with multiple lesions in a zonal distribution, occur most often on the lumbar region, buttocks, and thigh. The other types, which involve a solitary papule or nodule, have no favored location. This is the first report of multiple lesions in a zonal distribution occurring characteristically on the scalp.

[Investigation of intracavernous injection of papaverine for erectile impotence].

Forty milligrams papaverine HCl was intracavernously injected to 28 patients with erectile dysfunction for diagnosis and treatment in 14 cases in which it was injected a few times. We designed a brief manual method for measurement of penile hardness by artificial erection. Twenty of the patients corresponding to 71.4%, reacted, but only 16 (57.1%) had efficient erection for possibility of coitus. It was useful for the diagnosis of vascular dysfunction with dorsal penile arterial pulse sound examination in impotence. It was useful for the cases of small vascular impediment with preservation of nerve supply after the operation of intrapelvic malignancies, and self injection might be possible, but on the other hand, it had no effect for the patients of advanced age with vascular impediment and diabetes mellitus neuropathy, and with arteriosclerosis.

[An experimental analysis of three-dimensional hip joint force after pelvic osteotomies].

A three-dimensional analysis of the resultant forces on the hip was carried out using a fresh cadaver of a 40 year old male. A cross sectional area of each of twenty-four muscles around the hip was measured. Markers were fixed at the proximal and the distal insertions of the muscles and the direction of muscular forces was calculated. From these data, three-dimensional resultant forces on the hip and muscular forces around the hip were calculated through the computer in the normal and the postoperative states of Salter pelvic osteotomy, Chiari pelvic osteotomy and rotational acetabular osteotomy. The results showed the resultant forces to be 3.38 times the body weight in the normal specimen, and 3.79 times for the Salter pelvic osteotomy, 2.74 times for the Chiari pelvic osteotomy, and 4.07 times for the rotational acetabular osteotomy. However, as the loaded area on the femoral head also had increased to 1.12 times for the Salter pelvic osteotomy, and 1.20 times for the rotational acetabular osteotomy, the resultant force in each square centimeter was 0.94 times for the Salter pelvic osteotomy, and 0.71 times for the rotational acetabular osteotomy.

[A case of extramedullary plasmacytoma of the spleen].

A case of extramedullary plasmacytoma of the spleen in a 39 years-old man was reported. The tumor occupied almost of the spleen and extended to the transverse colon. 67Ga scintigraphy and MRI, particularly T 2-weighted image were useful in diagnosis of the localization.

Leukemia cutis in acute monocytic leukemia.

We describe the case of a patient with acute monocytic leukemia who demonstrated striking leukemic infiltration of the abdomen. This cutaneous involvement occurred in association with systemic relapse. Histologic examination of a biopsy specimen of a cutaneous lesion showed a diffuse infiltrate of leukemic cells in the dermis. However, blood vessels and skin adnexa were well-preserved, despite the destructive nature of monocytic leukemia.

[A case of cardiac sarcoidosis with various electrocardiographic change].

The patient was a 61 year old woman of Japanese who was in good health until May 1983, when she came to our hospital because of the visual disturbance. The chest X-ray film showed bilateral hilar lymphadenopathy and a transbronchial lung biopsy revealed sarcoid granuloma. She received systemic steroids (10 to 40 mg/2 days of prednisolone) for visual disturbance from August 1983 to May 1986. Then steroid therapy was suspended temporarily. In August 1987, she was readmitted because of syncopal episodes. She was noted to have irregular bradycardia and the electrocardiogram revealed atrioventricular block and paroxysmal polymorphous ventricular tachycardia. Initially cardioversion to sinus rhythm was accomplished with direct current electric shock and temporary pacemaker was inserted and subsequently a permanent transvenous demand pacemaker implanted. The patient then became asymptomatic. Coronary arteriography revealed no significant organic stenosis and left ventriculogram showed a left ventricular aneurysm of the inferior wall of the left ventricle. A transvenous percutaneous right ventricular endomyocardial biopsy revealed non-specific myocardial degeneration with slight interstitial fibrosis. Thallium-201 myocardial perfusion scans revealed the inferior left ventricular defects. We conclude the syncopal episodes and electrocardiographic abnormalities of this case were due to massive myocardial sarcoidosis which caused left ventricular aneurysm.

[Cervical cancer after subtotal hysterectomy (so called "stump cancer")--clinical studies on 226 cases].

The Department of Gynecology of the Cancer Institute Hospital diagnosed and treated 226 cases of stump cancer between 1950 and 1983. We obtained the following results. 1) The age of the subjects ranged from 32 to 78 years. The highest frequency age group was 50 to 59 (44.7%) with 101 cases, giving an average age of 53.8 years. 2) Subtotal hysterectomy was performed at ages 23 to 64, the average age being 39.5. 105 patients (46.5%) underwent subtotal hysterectomy between the ages of 40 and 49. 3) The major reasons for subtotal hysterectomy were myoma in 181 cases (80.1%), benign ovary tumor in 12 cases (5.3%), ectopic pregnancy in 10 cases (4.4%), cystic mole in 6 cases (2.7%) and cesarean section in 5 cases (2.2%). 4) Major subjective symptoms of stump cancer were metrorrhagia in 164 cases (72.6%) and fluor in 30 cases (13.3%). 24 cases (10.6%) were asymptomatic. 5) The hystological types were squamous cell carcinoma in 208 cases (92.0%), adenocarcinoma in 17 cases (7.5%), and adenosquamous cell carcinoma in 1 case (0.4%). These ratios were similar to those of cervical cancer cases treated over the same period. 6) The stages of cancer advancement were stage 0 in 24 cases (10.6%), stage I in 51 cases (22.6%), stage II in 96 cases (42.5%), stage III in 50 cases (22.1%), and stage IV in 5 cases (2.2%). The percentage of stage 0 cases was lower than that for general cervical cancer of the uterus.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinicopathological study on clear cell carcinoma of the ovary].

The clinical and pathological findings in 23 cases of clear cell carcinoma of the ovary treated at Cancer Institute Hospital from January 1951 to September 1987 are discussed. Clear cell carcinoma accounted for 7.1% of all malignant tumors of the ovary. In the last 21 months of the study, the incidence of clear cell carcinoma increased to 23.7 at the institute. This tumor was frequently associated with endometriosis, and the ratio of nulliparous women was as high as 47.8%. Operative findings revealed adhesion of the tumor to surrounding tissues in more than half of the cases. Moreover, rupture of the tumor capsule during operation occurred in 42.1% of the cases. Incidences of clear cell carcinoma by clinical stage were 82.6% for stage I, 8.7% for stage II, 8.7% for stage III, and 0% for stage IV. The incidences of serous carcinoma were 20.8%, 24.2%, 45.8%, and 9.2%, respectively. Histologically, clear cell carcinoma could be divided into solid and tubular types, with the solid type being slightly more frequent. The three-year survival rate for stage I cases was 95.2% for serous carcinoma and 54.5% for clear cell carcinoma, showing a poor prognosis for clear cell carcinoma. When classified by the histologic type of clear cell carcinoma, the three-year survival rate was 50.0% for solid type and 60.0% for tubular type, with no significant difference. For clear cell carcinoma, the prognosis was poor despite even though most cases were stage I.

Non-specific adherence of Candida species to surface-modified glass.

The nature of the physicochemical forces involved in the adherence of Candida species to a solid surface was investigated using modified glass slides. It was found that hydrophobic yeasts adhered to hydrophobic surfaces mainly by hydrophobic interactions, whereas in less hydrophobic yeasts electrostatic interactions seemed to play a more important role in adherence to the solid phase.

[Diagnosis of early gastric cancer measuring 1 cm or less].

Rates of detection roentgenological and endoscopic examinations were investigated in 87 patients with 93 lesions of early gastric cancer measuring 1 cm or less. In the depressed type, the minimum size for detection was 5 mm roentgenologically and 3 mm endoscopically. Depressed lesions with ulcer scars could be detected roentgenologically, but depressed lesions without ulcer scars could not be detected, unless the depressions were more than 0.5 mm and were accompanied by mucosal elevations surrounding the lesions. The detection of undifferentiated cancer without ulcer scars was poor, because mucosal elevations surrounding the lesions were rare in such histological types. In the elevated type, the minimum size detectable roentgenologically was 7 mm in width, 0.9 mm in height, compared to 4 mm in width, 0.3 mm in height endoscopically. On diagnosis of the depth of invasion, the depressed type of cancers without ulcer scars and with granular depressions were mostly limited to the mucosa, on the other hand, clear depression and plateau like elevation were highly indicative of submucosal invasion. In cases of elevated cancers, a central depression was highly indicative of submucosal invasion.
