Multiple sclerosis in Hokkaido, the northernmost island of Japan: prospective analyses of clinical features.

We analyzed the clinical features of multiple sclerosis (MS) prospectively seen between July 1986 and October 1989 on Hokkaido island, the northernmost part of Japan. Clinical features were generally considered to be intermediate between the previous Japanese reports and those of Western countries. Devic's disease was rare and simultaneous bilateral visual loss at on set was not too common this series, differing from that previously reported of Japanese MS. The high incidence of acute transverse myelopathy and lesser involvement of the cerebellum, however, support the previous view. Further clinical and epidemiological studies will be necessary on this island.

[Evaluation of subjective happiness in the elderly using a visual analogue scale of happiness in correlation with depression scale].

A Visual analogue scale of happiness (VAS-H) was applied to elderly population in a community to evaluate their subjective quality of life. The study population consisted of 313 elderly people aged over 75. Using a 20 cm visual analogue scale (VAS), VAS-H was measured by asking subjects to place a single vertical mark on a horizontal line at a point corresponding to their current subjective degree of happiness judged from health condition and psycho-social background as a whole; The VAS-H was anchored on the left and with the words "-100%; unhappiest" and on the right end with the words "+100%; happiest". The scores of VAS-H (%) were obtained by multiplying the distance (cm) from the zero point to the vertical line by 10. To determine the characteristics of VAS from the standpoint of the quality of life, we compared VAS with scores of two kinds of established depression scale, because the mood which was assessed by these scale was supposed to be an important factor of the quality of life. The results of the simultaneously assessed Geriatric Depression Scale (GDS), and Zung's depression scale (SDS) which was obtained 1 year before for the same subjects were compared with that of VAS-H. The score of VAS-H had a significantly negative correlation with both GDS and SDS. In conclusion, VAS-H was a simple and useful method to evaluate degree of subjective happiness which was one factor of quality of life in the elderly people.

[Evaluation of subjective happiness in the elderly using a visual analogue scale of happiness to analyze the effect of life style and neurobehavioral function on subjective happiness].

The effects of activity of daily living (ADL), physical conditions, social environmental factors and neurobehavioral functions on subjective happiness were investigated in 313 elderly subjects. The degree of subjective happiness was measured using visual analogue scale of happiness (VAS-H). There was significant relationship between VAS-H and ADL. The VAS-H score had weak but significant correlation with physical conditions and neurobehavioral functions. The social environmental factors, especially economical conditions, family relationships and group behavior had a significant correlation with VAS-H. In conclusion, in apparent healthy elderly people, the subjective degree of happiness was not related to ADLs, but to social environment and information physical conditions.

[Application of visual analogue scale of happiness to elderly Himalayan highlanders].

Visual analogue scale of happiness (VAS-H) was applied to elderly Himalayan highlanders to investigate the influence of life-style and socio-cultural background on subjective happiness. The score of "VAS-H" and neuropsychological function tests in 733 Himalayan highlanders who belonged to 5 separate villages were compared with those in 313 Japanese elderly subjects. The places of study consisted of 4 villages (Khalimabad; K, Gulmit; G, Pasu; P, Shimshal; S) in Hunza area in Pakistan, and Namche Bazar; N in Nepal. Degree of modern civilization was highest in K, followed by N, G, P and lowest was that in S by historical and sociological aspects. The mean VAS-H sore in Himalayan highlanders and the degree of modern civilization in the area was in inverse proportion. Himalayan elderly women showed a significant lower score on VAS-H compared with elderly Himalayan men or elderly Japanese subjects. The VAS-H score in the Japanese elderly correlated significantly with the stability of posture and walk, and that of Himalayan highlanders correlated significantly with manual skill. In conclusion, the life style or socio-cultural background strongly affected subjective happiness in the elderly. The VAS-H was useful in comparing subjective happiness in different cultures.

[Studies on the spiders as folk medicines (II). Species of the medicinal spiders in Japan].

In old Japanese literature of natural history, there were some descriptions on the medicinal spiders. We investigated these descriptions and could determine 14 groups as follows: 1. Latouchia typica, 2. Atypus karschi, 3. Loxosceles rufescens, 4. Achaearanea tepidariorum, 5. Araneus ventricosus, 6. Argiope amoena, 7. A bruennichii, 8. Nephila clavata, 9. Tetragnatha spp., 10. Uroctea compactilis, 11. Agelena limbata, 12. Tegenaria domestica, 13. Heteropoda venatoria, 14. Menemerus confusus and some species belonging to the family Salticidae.

[Studies on the medicinal plant in the "Sambutsu-cho" of Bungo Province possessed by the Kumamoto Clan (I) Studies on the medicinal trees].

The Samabutsu-chou, a list of the natural products, of the Kumamoto clan in Bungo Province was dedicated to the Tokugawa shogunate in 1735. This book had many names of the plants, animals and minerals. In the plant kingdom, there were many names of cereals, vegetables, fruits, trees, bamboos, herbs, seaweeds and mushrooms. In this report, I studied the species of the trees which were listed by the Chinese and local names. I identified them and found 106 species. Among them, 68 species were medicinal plants. For example, the names of gardeniae fructus, forsythiae fructus, phellodendri radix and magnoliae radix were found as the famous medicinal plants. The ratio of the medicinal plants was 64%.

[Studies on the medicinal plant in the "Sambutsu-cho" of Bungo Province possessed by the Kumamoto Clan (II); studies on the medicinal herbs].

In the previous report, I studied the medicinal trees contained in the Sambutsu-cho of the Kumamoto clan in Bungo Province. In this report, I studied the medicinal herbs contained in the same book. There were 259 names in the herbal part. I identified 156 species and found 103 species as the medicinal plants. The ratio of the medicinal plants was 66%. The famous medicinal plants were as follows: Atractilodes japonica, Platycodon gradiflorum, Mentha arvensis var. piperascens, Lisospermum erythrorizon, Pharbitis nil, Gentiana scabra var. buergeri, Cnidium officianal, Angelica acutiloba, Bupleurum falcatum, Pueraria lobata, Paeonia lactiflora, Paeonia suffruticosa, Achyranthes fauriei, Asarum dimidiatum, Ophiopogon japonica, Pinellia ternata and Cyperus rotundus.

[Medicine in the "Nisuke-banashi"].

In the Edo period, the "Nisuke-banashi" was written in Yabe district in Higo, Kumamoto Prefecture. This book described the lives of the farmers who were very poor and lived under very difficult conditions. In the book a doctor had an important role in explaining the truth. He also advised on some medical affairs. The author of this book was presumed to be a doctor named Watanabe Tadasu who lived in Hama-machi, Yabe. I studied the medical explanations in this book and found that these stories were based on old Chinese medical texts such as Somon, Shokanron and Kinkiyoryaku.

[Observation of the posterior urethra by transrectal linear ultrasonography].

Transrectal linear ultrasonography was performed on 59 male patients with dysuria, and voiding movement of the posterior urethra was observed with a videomonitor. Uroflowmetry was performed simultaneously, and the results were compared with those obtained in a previous study. On the image synchronized with maximum flow rate, the diameter of the bladder neck, and the center of the prostatic urethra were measured as parameters and the correlation with maximum flow rate, average flow rate, and residual urine rate, was examined. The forward movement of the ventral side of the posterior urethra was found to play a more important role in the opening of the posterior urethra, than the opening of the bladder neck, and the width of the prostatic urethra to play a more important role in the efficiency of urination than the width of the bladder neck.

[Two cases of recurrent optic neuritis (OPN) and acute transverse myelopathy (ATM) with associated anticardiolipin antibodies].

We investigated anticardiolipin antibodies (aCL) by enzyme linked immunosorbent assay with adding aCL-cofactor in two cases of recurrent OPN and ATM patients. These two patients had similar clinical features with ATM and OPN during their clinical courses. They were supposed to be suffered with multiple sclerosis (MS), although cranial MRI was normal and oligoclonal IgG band (OCB) was consistently absent in the cerebrospinal fluid. Positive aCL is suggestive that this disease may be a disorder associated with aCL with different etiology and pathogenesis from other MS patients. Serologic testing for aCL with aCL-cofactor should be warranted for MS patients, especially for those showing OPN and ATM during the clinical course, because in orientals the incidence of ATM and OPN is relatively high among MS.

[Selection of graft materials in case of emergency coronary bypass surgery following failed angioplasty].

The clinical characteristics of selection of graft materials were analysed for patients undergoing emergency coronary artery bypass surgery (CABG) following failed coronary angioplasty (PTCA). Ten emergency CABGs were performed from January 1983 to December 1991. Perioperative variables and follow-up were compared to 18 patients undergoing elective CABG after PTCA. The emergency group had shorter operative time (p less than 0.01) and shorter bypass time (p less than 0.05). Moreover the emergency group had decreased use of the internal thoracic artery (ITA) (40% vs 94.4%, p less than 0.01). There was no use of bilateral ITAs in the emergency group. There was not significant difference in hospital mortality and medium term follow-up between two groups. In conclusion, emergency CABG carries a significantly less use of ITA graft than elective CABG although ITAs are superior to SVGs about long term patency rate. So it is desirable that arterial graft should be used under an appropriate selection at emergency operation.

[Epidemiological study of motor neuron disease in Hokkaido island--its incidence, prevalence and regional distributions--ALS Study Group].

The incidence, prevalence and regional distributions of sporadic motor neuron disease (MND) from 1980 through 1989 were evaluated in collaboration with multiple neurological institutes in Hokkaido island. Patients with sporadic MND were collected from three sources: 1) neurologist practicing in Hokkaido island, 2) sending inquiries to 620 major hospitals, 3) notification file of MND provided by Japanese Ministry of Welfare and Health. Three hundred and eighty-nine patients with sporadic MND were ascertained for this study. Of 389 patients, 238 patients were men and 151 patients were women, and the ratio of men to women was 1.6:1. The mean age of onset was 58.2 +/- 10.3 years old, 57.7 +/- 10.4 for men and 58.9 +/- 10.0 for women. Their clinical presentations were 303 patients with ALS, 52 patients were PBP and 34 patients were SPMA. The crude incidence rate for both sexes combined for 1980 through 1989 was estimated as 0.69 per 100,000 person-year. The age- and sex-adjusted incidence for men was 0.86 per 100,000 person-year (95% CI, 0.75 to 0.97) and that is higher than 0.53 per 100,000 person-year (95% CI, 0.45 to 0.61) for women. The average, crude prevalence rate from 1985 through 1989 was estimated as 2.25 per 100,000 person-year. There are no overall trends of changing the pattern in incidence and prevalence of MND in Hokkaido island, however the geographic distributions of the incidence of MND according to towns and cities disclosed the presence of some relative clustering areas.(ABSTRACT TRUNCATED AT 250 WORDS)

[Surgical repair of discrete subaortic stenosis complicated with prosthetic valve endocarditis--a case report].

A 46-year-old man was referred to our hospital because of prosthetic valve regurgitation. Eight years previously he had undergone aortic valve replacement because of aortic regurgitation due to infective endocarditis. At reoperation, we found prosthetic valve endocarditis and discrete subaortic stenosis. The obstructing fibrous tissue was resected and the aortic valve was replaced. Because discrete subaortic stenosis is usually located just below the aortic valve, the aortic valve cusps are liable to become thickened by the jet through the discrete stenosis and thus are vulnerable to infective endocarditis. It is pointed out that care must be taken not to overlook discrete subaortic stenosis in the presence of other associated cardiac disorders.

[A case of benign intracranial hypertension with fluctuated symptoms and CSF pressure synchronized with menstrual cycle].

A 39-year-old woman presented with a 2-month history of repeated severe headache, nausea and diplopia. On admission she was obese with bilateral papilledma and abducens weakness. Mass lesion and sinus thrombosis were ruled out by brain CT and angiography. CSF pressure was normal initially. CSF pressure fluctuated with menstrual cycle, sometimes showing over 600 mmH2O with worsening of the symptoms. She was diagnosed as benign intracranial hypertension (BIH). Diuretics did not improve the symptoms, and visual disturbances ensued and deteriorated. A spinal subarachnoid space-peritoneal shunt was inserted to control CSF pressure, showing rapid improvement of headache and diplopia but visual disturbances remained almost unchanged. Optic nerve sheath fenestration was performed without improvement of visual deterioration. We postulated multiple factors such as obesity, menstrual abnormality, iron deficiency anemia and analgesic drugs played important roles to produce BIH in this case. Careful quantitative perimetry should be done to decide a suitable time for surgical treatment in BIH.

Keratoacanthoma ensuing from bone marrow transplantation for chronic myeloid leukemia.

We encountered a 36-year-old woman, who developed two keratoacanthomas at rather a short interval after bone marrow transplantation for chronic myeloid leukemia. This skin disease seems to be associated with various factors, but its etiology still remains obscure. The fact that the patient was under postoperative immunosuppressive treatment suggests that, among other factors, a deterioration or an impairment of immunocapacity may play some role in the pathogenesis of keratoacanthoma. It is intriguing that two lesions appeared successively in one patient.

[A case of unilateral pulmonary edema associated rupture of mitral chordae tendineae].

A 57-year-old man was admitted with dyspnea and bloody sputum. The chest X-ray showed unilateral alveolar infiltration, and alveolar cell carcinoma was suspected. Physical examination showed orthopnea and a loud systolic murmur, and the echocardiogram showed mitral valve prolapse. A chest X-ray 4 days later revealed bilateral infiltration. The cardiac catheterization showed pulmonary congestion and the capillary wedge pressure revealed a prominent V wave. Papanicolaou's test of sputum was negative. These findings suggested heart failure due to mitral regurgitation rather than lung carcinoma. The patient underwent mitral valve replacement because of his refractoriness to the medical treatment. During the operation, the chordae tendineae of the anterior mitral leaflet was found to be completely ruptured. The mechanisms of unilateral pulmonary edema could not be ascertained, but the effect of posture and gravity was thought to be a possible mechanism.
