[Total proximal reconstruction of chronic aortic dissection in patients with Marfan syndrome].

Authors present two patients suffering from Marfan syndrome. Asymptomatically developed serious cardiovascular complications being characteristic of the syndrome itself were revealed at both cases. Type "A" aortic dissection was found at the first, and type "B" aortic dissection involving even the arch as well revealed at the other patient. They were operated on an elective base. Total proximal aortic reconstructions (aortic root-ascending aorta-arch) were performed under deep hypothermia using total circulatory arrest (35 and 87 min). Both patients are doing well without complaints 3-4 months following the surgical interventions. Control angiographic imagination has found normal anatomic conditions at one of the patients. Anastomosis stenosis at the borderline of the arch and descending aorta was revealed at the other case which should be necessary to operate at an later occasion.

Venous myogenic tone and its regulation through K+ channels depends on chronic intravascular pressure.

In this study, we compared the level of myogenic tone and its negative-feedback control through specific K+ channels in two types of human veins (saphenous [SV] and cephalic [CV] veins), which experience different ranges of pressure in vivo. We also investigated whether an experimental model of increased venous pressure in rats exposed to head-up tilt for 2 weeks produced changes similar to those observed in the human veins. Cylindrical vein segments were cannulated, their diameters were measured, and the intraluminal pressure was set at different levels (2 to 30 mm Hg) in vitro. Acetylcholine test showed that during the regular harvesting process 76% of the human SVs exposed for coronary bypass grafts had no functional endothelium. We found significant myogenic tone in the human SV, where the in vivo pressure is high, but it was not present in the human CV, where the in vivo pressure is low. The nonspecific K+ channel antagonist, tetraethylammonium (TEA), decreased the diameter of the human SV but not the CV. Iberiotoxin and 4-aminopyridine, blockers of the Ca(2+)-sensitive (KCa) and voltage-gated K+ (KV) channels, also decreased the diameter of the human SV by 10.2 +/- 4.8% and 19.5 +/- 4.7%, respectively. In the rat SV, significant myogenic tone was found, but TEA had no effect, even after 2 weeks of in vivo pressure increase in the hindlimb by head-up tilt. We conclude that (1) an increased venous myogenic tone correlates with higher chronic intraluminal pressure loads, (2) KCa and KV channels counterregulate the myogenic tone in human, but not in rat, saphenous vein, (3) the counterregulatory effect is more effective at high than at low intraluminal in vitro pressure levels, and (4) its development is probably a long-term process.

[Non-invasive and invasive assessment of the function of aortic valve homografts in infancy and childhood].

Study population involved 21 pts with complex congenital heart disease after corrective surgery using homografts between the years 1986 and 1992. Diagnoses included double outlet right ventricle, tetralogy of Fallot, transposition of great arteries, truncus arteriosus, pulmonary atresia with VSD, corrected transposition with pulmonary stenosis, and absent pulmonary valve. Pts age at surgery ranged from 18 days--to 15 yrs mean, 6.7 yrs. Time interval between surgery and diagnostic procedures was 8 days--6 yrs (mean 11 months). All pts were studied by Echo/Doppler. 2 pts by TEE and 3 pts by MRI. 6 pts had cardiac catheterisation. Distal (5) or proximal (1) stenosis was present in 6 cases. The severity and the type of stenosis was correctly identified by noninvasive technique compared to invasive findings. Homograft valve regurgitation was mild (4) or moderate (3). In two pts severe insufficiency was associated to homograft endocarditis. Postoperative residual shunts were found in 8 pts. Homograft stenosis can correctly be diagnosed using Echo/Doppler technique. Distal stenosis was more frequent than proximal. Homograft tends to become insufficient, but severe incompetence did not occur except in endocarditis.

[Preparation of human heart valve grafts in Hungary].

A method of preparation and preservation of valved aortic and pulmonary allografts is described, used in corrective procedures of congenital heart diseases. The method of sterilization, exclusion of bacterial and viral contaminations is enhanced. Complications did not occur after implantations of the grafts.

Stenosis of pulmonary veins with total left anomalous pulmonary venous return.

A case of congenital stenosis of the pulmonary veins and total left anomalous pulmonary venous return is reported. There was also obstruction between the left pulmonary veins and left vena cava superior. The diagnosis was based on 2-dimensional and Doppler echocardiography and was confirmed by angiocardiography. The 6-month-old baby with these anomalies was operated upon successfully. These cardiac malformations are very rare separately and their combination has not been previously reported.

[Surgical treatment of critical valvular aortic stenosis in neonates and infants based on echocardiographic diagnosis].

The results of surgical intervention are reported in 10 cases of critical valvular aorta stenosis diagnosed with echocardiography in neonatal age and infancy. The significance of echocardiography (2-dimensional, M-mode, Doppler) which does not mean stress for the patient in critical state and provides correct diagnosis is emphasized. Correct diagnosis contributes to the performance of operation in appropriate time as well as the use of immediately performable and correct surgical technique is considered necessary. These are the first cases in Hungary where open heart operations have been carried out on the basis of echocardiography in neonatal age and infancy. The good results proved the adequacy of the solution.

Thrombotic lesions of the tricuspid valve in a newborn: surgical management.

Thrombotic lesions on the leaflets of the tricuspid valve in a premature infant with an anatomically normal heart are described. The diagnosis was made by two-dimensional and Doppler echocardiography. No etiologic explanation could be obtained from the history and clinical findings. The newborn had no infection, no antithrombin deficiency, and no indwelling catheter through either a peripheral, central or umbilical vein. The multiple masses were successfully removed by surgery under deep hypothermic circulatory arrest at 1 month of age. The child is well, without neurological deficit, and is developing normally.

Congenital vascular rings: surgical management of 111 cases.

Between October 1974 and May 1987, 111 congenital vascular rings were submitted to surgical correction. There were 83 infants (age: 5 days-12 months, median: 7 months; weight: 1.9-8.5 kg, median: 7.1 kg), and 28 children (age: 1-13 years, median: 3.5 years; weight: 7.5-48 kg, median: 26.5 kg). Patients were divided into five groups: (1) double patent aortic arch (44 cases), (2) double aortic arch with atresia in different parts of the left arch (36 cases), (3) right aortic arch with left ligamentum arteriosum (21 cases), (4) left aortic arch with aberrant right subclavian artery and truncus caroticus (8 cases), and (5) pulmonary artery sling (2 cases). We had no intraoperative mortality but in the postoperative period, 2 neonates died of severe bacterial infections of the respiratory tract.
