Comparison of estrogen-induced hyperplasia to endometrial carcinoma.

Use of exogenous estrogens has been associated with endometrial cancer in a number of case-control studies. This observed association could be biased as a result of frequent misclassification of estrogen-induced hyperplasia as endometrial cancer. To evaluate this possibility, pathology slides from 233 patients with a hospital diagnosis of endometrial cancer were reviewed independently by two pathologists. The hospital diagnosis was confirmed by both pathologists in 86% of the cases, indicating that the misclassification of hyperplasia as carcinoma is uncommon.

Infection of the minor vestibular gland.

Infection of the minor vestibular gland has been a poorly identified entity. Recognition of these glands and the contribution they may make, if chronically infected, to vulvar pain and dyspareunia are presented.

Hysterosalpingography in cynomolgus monkeys.

This paper describes recent success in performing hysterosalpingography in cynomolgus monkeys. The animals were anesthesized, and a needle was manipulated through the cervical canal into the endometrial cavity for injection of contrast material. Difficulties in threading the needle through the canal were due to several blind pouches formed by folds in the cervical mucosa. The uterine lumen and both tubes could be visualized in over 75% of the cases. The study demonstrates that the hysterosalpingography technique can be performed satisfactorily in cynomolgus monkeys.

Microinvasive cervical cancer after five negative Papanicolaou smears.

In view of the controversy regarding the appropriate frequency for cytologic screening, a 28-year-old woman who developed microinvasive carcinoma of the cervix less than one year after her fifth negative Papanicolaou smear is described. The lesion was found not high in the endocervical canal, but at the squamocolumnar junction well out on the portio of the cervix. This case documents that the risk of less frequent screening is not zero.

Tubal anastomosis following unipolar cautery.

Twenty-five of 48 women (52%) sterilized by unipolar cautery techniques conceived following tubal anastomosis, of whom 17 (36%) had a living child. The overall cumulative probability of conception at the end of follow-up as determined by life-table analysis was 76%. Increasing age, parity, and the duration of the interval from sterilization to reversal did not influence pregnancy success. A decreased pregnancy rate was associated with ampullary-isthmic anastomosis; however, a pregnancy was least likely to occur in women with shortened oviducts of less than or equal to 4 cm (P less than 0.01). A decreased pregnancy rate in cautery-sterilized patients undergoing reversal may be related to the destruction of a larger segment of the fallopian tube. Interestingly, 71% of the cautery-sterilized patients were noted to have associated tubal disease such as endometriosis and/or proximal hydrosalpinx. The influence of these findings on subsequent pregnancy success remains to be established.

Pharmacokinetic studies on quinacrine following intrauterine administration to cynomolgus monkeys.

Recent efforts have been made to develop a chemical oviductal occluding agent. Intrauterine quinacrine has been used in certain areas of the world with moderate success in effected tubal closure. This report presents the pharmacokinetics of a quinacrine solution (30 mg) as administered to cynomolgus monkeys via the intrauterine route, compared with intravascular injection. The data show rapid transfer of the drug from the uterine to the vascular compartment and uptake by almost all tissues examined. Although plasma concentrations disappear within 24 hours, levels can be detected in most tissues for at least 1 week following intrauterine injection. After 28 days, however, tissue levels of the drug are absent or near the limit of detection.

"Aging" pigments in term human placenta.

First-, second-, and third-trimester placentas were examined with fluorescent microscopy for the presence of lipofuscin pigments. Prior to 32 weeks' gestation these were not seen, but they were routinely found in the trophoblast of the term placenta. These pigments may or may not represent "aging" of the placenta.

The diagnosis and management of cervical intraepithelial neoplasia in the patient under the age of twenty-one.

The diagnosis and management of 260 patients between the ages of 15 and 21 with cervical intraepithelial neoplasia is discussed. These patients represented 22% of all new patients seen in the Colposcopy Clinic of The Johns Hopkins Hospital between January 1975 and December 1979. Sixty-six percent of the patients were referred for evaluation of two Papanicolaou smears which were compatible with the diagnosis of CIN I while only 1.5% of the patients were referred with the cytologic diagnosis of CIN III. The colposcopic impression in the majority of patients was CIN I. There was excellent correlation between the colposcopic impression and histopathology. Thirteen percent of the patients evaluated had surgical procedures, 11% (29 patients) under-went conization and 2% (five patients) hysterectomy. The majority of patients under the age of 21 had minimal degree of cervical intraepithelial neoplasia. The importance of conservative management is discussed.

Endometriosis and the development of tuboperitoneal fistulas after tubal ligation.

The present study details gross and histologic findings of 79 previously ligated fallopian tubes from 3 groups of patients. Of 20 oviducts removed after documented sterilization failure (group I), 6 revealed a process compatible with endometriosis. Four of nine previously ligated fallopian tubes removed at the Johns Hopkins Hospital (group II) were successfully injected with India ink. In two patients histologic examination demonstrated the India ink in epithelium-lined spaces that lay beyond the muscle of the tubal wall extending from the tubal lumen to the serosal surface. Fifty oviducts were studied in twenty-five patients requesting reversal of their sterilizations (group III). A higher percentage of fistulas was demonstrated in patients with less than 4 cm of remaining proximal tubal segment. Furthermore, most of these fistulas were demonstrated in patients for whom 3 years had elapsed since the original sterilization procedure. Patients sterilzed by laparoscopic cautery methods were observed to have a higher percentage of fistula formation and histologic documentation of endometriosis at the sterilization site as compared with patients sterilized by other methods. Our observations suggest that ligation of the oviduct within 4 cm of the uterine cornu may predispose to the development of endometriosis and subsequent fistula formation in the tip of the ligated oviduct.

Effect of silver nitrate on pregnancy termination in cynomolgus monkeys.

The effect of intrauterine injection of 1% silver nitrate on pregnancy was studied in 13 cynomolgus monkeys. The monkeys received, in a random manner, intrauterine injections of 1 ml of either 1% silver nitrate (n = 7) or 0.9% NaCl solution (n = 6) between days 27 and 43 of pregnancy. Silver nitrate injection resulted in vaginal bleeding, beginning 1 or 2 days after treatment, that lasted for an average of 5.3 days. In all of these cases, pregnancy was terminated. Injection of normal saline had variable sequelae, but four of six monkeys were delivered of healthy offspring. All of the silver nitrate-treated animals recycled and, after remating, two of the seven subsequently became pregnant again and were delivered of normal healthy infants. This study demonstrates the efficacy of intrauterine injection of 1% silver nitrate in terminating early pregnancy.

Leiomyomatosis in pelvic lymph nodes.

A decidual reaction may occur in the pelvic lymph nodes during normal pregnancy. This reaction may be replaced by the same type of benign metaplastic reaction that gives rise to leiomyomatosis peritonealis disseminata. The occurrence of this apparent smooth muscle in the pelvic lymph nodes may result in an erroneous diagnosis of metastatic leiomyosarcoma, or of benign metastasizing leiomyoma.

Primary, papillary peritoneal neoplasia.

Subsequent to the recognition of the intraperitoneal tumors of low malignant potential, clinicians have repeatedly faced the ambiguities inherent in a disease that seems aggressive on the basis of its wide distribution in the peritoneal cavity but benign on the basis of its histopathology and clinical course. Whereas the occasional case has been associated with extensive local reaction and ascites, except for a rare exception these tumors result in prolonged survival and in an absence of extraabdominal extension. The current review of 154 cases followed from 2 to 40 years, performed in an attempt to understand this perplexing disease, leads to the following conclusions: 1) Whereas frequently beginning on the ovary and showing a predilection for the pelvis, there are examples of widely disseminated peritoneal disease with minimal, if any, ovarian involvement; 2) the outcome without adjunctive therapy is excellent and thus such therapy is contraindicated in view of the death of only 2 of the 154 patients with disease, 1 of whom had had adjunctive intraperitoneal isotope therapy; and 3) this disease is best understood as a diffuse primary peritoneal tumor probably developing on the basis of irritating agents' reaching the abdominal cavity from the lower genital canal, a process similar to that proposed for the genesis of endometriosis. Such a low-grade primary in situ tumor that may involve the entire peritoneal cavity is compatible with prolonged survival.

Cutaneous metastasis of endometrial carcinoma.

Five cases of endometrial carcinoma with cutaneous metastases are reported. Cutaneous metastases of endometrial carcinoma are uncommon. There are no other cases reported in the literature. In 91 autopsy cases only one example (1%) could be found. The appearance of subcutaneous nodules is evidence of widespread dissemination and a harbinger of early demise.

Tumor cell origin of histaminase activity in ascites fluid from patients with ovarian carcinoma.

We have demonstrated by tissue assay and immunohistochemical techniques that tumor cells appear to be the source of the increased histaminase activity found in 60% of ascitic fluids from patients with ovarian carcinoma. The histaminase activity in 64% of tumor tissues studied was greater than 100 U/g of tissue, a value higher than that for normal tissues other than kidney and intestine. The presence of elevated ascitic fluid histaminase activity (greater than 3.5 U/ml) correlates with the presence of high histaminase activity in tumor tissue. Histaminase activity is higher in ascitic fluid from the patients than it is in their plasma, probably reflecting the direct entry of histaminase into the peritoneal cavity.

The histopathology of the developing tubal ectopic pregnancy.

Examination of histopathologic sections taken of tubal ectopic pregnancies and gross dissections of these specimens suggest that, in the majority of cases, the growth of the developing trophoblast takes place in a largely extratubal site and that the common clinical impression that the mass associated with the tubal ectopic pregnancy is a dilated fallopian tube is largely erroneous.
