Left latissimus dorsi muscle cardiomyoplasty is a surgical procedure in which the muscle is mobilized as a pedicled flap, moved into the left pleural and mediastinal cavities, and used for surgical repair and/or mechanical augmentation of a failing left ventricle. Chest radiographs in four patients who had undergone this procedure were evaluated. In all four patients, there was bandlike or diffuse increase in density in the left midhemithorax with loss of the left heart border. These findings, which could be misinterpreted as lung disease, represent the transposed muscle flap. CT showed the exact anatomic position of the flap in relation to the intrathoracic structures. Familiarity with the postoperative radiographic appearance of this type of myoplasty is important to avoid misinterpretation.
Bit-mapped color imaging of scalp potential fields evoked by sensory stimulation in humans disclosed significant features not identified by mere inspection of multichannel traces. Methodological problems are considered in detail for early cortical SEPs which include several components with sharp rise times occurring at spatially distinct scalp locations. A manageable yet efficient imaging system requires recording electrodes in adequate number and scalp locations, bandpass fidelity to resolve slow and fast components, consistency of bioelectric input data, optimal interpolation and mapping algorithms, and consistent color scaling. Critical steps in these procedures were investigated in conjunction with new evidence on the scalp topography and neural generators of the N20, P20, P22, P27 and N30 SEP components. It is concluded that N20-P20 reflect a tangential equivalent dipole in parietal area 3b while P22 reflects a radial equivalent dipole in motor area 4.

The hydrolysis of poly(ortho-ester)s and a monomeric model compound, 3,9-dibenzyloxy-3,9-diethyl-2,4,8,10-tetraoxaspiro[5,5]un decane, was carried out in dioxane-d8-dioxane and followed by 1H-NMR and HPLC, respectively. Experimental results suggested that the polymer degradation proceeds to a large extent via random scission. The hydrolysis was catalyzed by the acid; the catalytic rate constant increased predictably with decreasing aqueous pKa of the acid. The reaction is first order with respect to the catalyst concentration and the number of ortho-ester linkages present, and it is independent of water in the concentration range studied. Strain at the ortho-ester bond may be a factor influencing the hydrolysis rate.
Early somatosensory evoked potentials (SEPs) to median or ulnar nerve stimulation were recorded with non-cephalic reference from neck, oesophagus and scalp in normal young adults. Transit times and durations were estimated for different components. SEP fields were mapped with up to 24 skin electrodes around the neck or along the midline neck and scalp, and projected onto a 2-dimensional plane for bit-mapped colour imaging. The posterior neck N11 is a near-field potential propagated caudorostrally in the dorsal column, associated with a positive P11 far field beyond termination of the cuneate bundle. A true phase reversal of the posterior neck N13 into an anterior neck P13 is substantiated, identifying a segmental generator with horizontal axis in dorsal horn. The N13-P13 represents postsynaptic excitatory potentials in interneurones of layers IV-V of the dorsal horn. It is not reflected in any scalp far field. The duration and onset latency of N13-P13 are in line with this interpretation. A new montage of posterior-to-anterior neck can enhance this component without introducing extraneous potentials. The P14 far field does not extend below the inion and presents distinct features. Neck-to-front scalp montages confound the SEP components generated in the spinal cord and above the foramen magnum respectively, but may serve to estimate N11 onset latency.
Sulfhydryl groups in the apoproteins of the major lipoprotein (density 0.95 g/ml) of hen's egg yolk have been studied by colorimetry, analysis for carboxymethylcysteine, and labelling with [14C]iodo-compounds and their distribution amongst the individual apoproteins (i.e. apovitellenins I-VI) has been determined. In the intact lipoprotein, reaction of sulfhydryl groups with aqueous reagents was slower than in the apoprotein mixture and there were large differences amongst different reagents in the extent of reaction. If the lipoprotein was heated for a short time in 1 mM EDTA above about 70 degrees C and then cooled, more sulfhydryl groups were accessible although the total number did not change. This increase in accessibility was accompanied by the disruption of the lipoprotein structure and an increase in the viscosity of its solutions. Furthermore, the isolated apoprotein mixture was less soluble in urea solutions and formed a gel, probably as a result of cross-linking by inter-protein disulfides. There was also a small change in the distribution of sulfhydryl groups so it is likely that sulfhydryl-disulfide interchanges occurred on heating. It is suggested that such interchanges help determine the physical properties of heated yolk and that in the intact lipoprotein sulfhydryl groups are concealed to prevent sulfhydryl-disulfide interchanges in the egg.
Early somatosensory evoked potential (SEP) components to median nerve stimulation were recorded with non-cephalic reference from scalp, neck and oesophagus in normal young adults. The nerve volley was recorded from the arm and Erb's point. The latencies of the stationary P9 far field were increased significantly by supporting the shoulder in a high position. This manoeuvre was shown in X-rays to change the axis of the nerve at levels between lateral and middle parts of the clavicle. This suggests that the far field is not related to inhomogeneities of the medium surrounding the nerve whereby abrupt changes in extracellular current flow might be produced as the volley arrives at a certain level. Rather, the axial orientation of the propagated dipole appears as a major factor determining the features of the stationary P9 far field. The reversible effect described does not significantly influence the conduction of the volley itself nor the latencies of the subsequent SEP components.
Between 1969 and 1975, postoperative radiotherapy was performed at the J. Bordet Institute on 787 stage I-III operable breast cancer patients. Crude 10 years survival rates are respectively 61% for stage I, 59% for stage II and 30% for stage III. The results have been analysed according to clinical characteristics of significant prognostic value and according to the technical conditions of treatment. The status of the axillary nodes appears to be the most important prognostic factor. Radiotherapy to the thoracic wall and lymph drainage areas has varied little throughout the whole period with the dose to the internal mammary chain being the only parameter which varied significantly. Loco-regional recurrences occurred only in 5, 2% of patients without distant metastases appearing previously or during the following year (2.4 for stage I patients). Severe sequelae were seen in only a very few cases. Survival appears significantly correlated with an adequate dose of irradiation to mammary node chain.
An unusual case of pheochromocytoma, which presented as a primary lung tumor, is described. The tumor was multifocal, arising in both the adrenal glands and in the chromaffin tissue around the thoracic aorta; it had invaded the lung and metastasized to the brain. Histologically the tumor had an unusual sarcomatous appearance, and the final diagnosis depended on the ultrastructural finding of scanty amounts of neurosecretory granules, typical of epinephrine and norepinephrine. This is the first reported case of pheochromocytoma presenting clinically as a lung tumor.
Intact and cecectomized adult male, Single Comb White Leghorns were fasted for 24 hr prior to the initiation of the collection period. Excreta were quantitatively collected from each of the unfed control roosters for a 24 to 48 hr fasting period. The amino acids excretion values (mg/bird/24 hr) were not significantly different (P greater than .05) between the intact and cecectomized roosters for histidine or methionine. However, lysine and glutamic acid were significantly different (P less than .05) and all other reported amino acids were significantly different (P less than .01). The results of this preliminary experiment indicate the possible role of the ceca in altering the amino acid excretion values. Additional research is required in the elucidation of cecal involvement, as well as the selection of a proper control for this method of analysis for amino acid availability.
Immunoglobulins were quantitatively determined on 21 serum samples by radial immunodiffusion, nephelometry and immunofluorescence. Good correlations were obtained with all three different methods in regard to IgG and IgA. IgM values were about 40% higher with radial immunodiffusion than with the other two methods.
A granular cell myoblastoma of the bronchus occurring in a 20-year-old man was successfully treated by right lower lobectomy. His symptoms of chest pain and cough, the presence of an obstructing mass in a young patient, and the location of the lesion in a large bronchus on the right side are characteristic of this tumor. Electron microscopic examination of the tumor confirmed the diagnosis.
One hundred and four consecutive cases of carcinoid among 194 APUD-omas (cerebral and gynecological tumours expected) are presented. Localization is as follows: bronchopulmonary (21%), duodenum (5,5%), small intestine (19%), Meckel's diverticulum (5,5%), appendix (39%), colorectal (5,5%), other (4,5%). Average age at diagnosis is very different for each localization of carcinoid tumour, as is prognosis: in our material, 33% of cases had an infiltrative tumour, 17% had lymph nodes metastases and 16% hepatic metastases. All 15 patients with carcinoidosis died but their survival varied considerably in length (from a few weeks to 21 years after the beginning of carcinoidosis). In addition, 3 patients presented with the Zollinger-Ellison syndrome, 2 of them having an atypical or probable form of the MEN I syndrome.
A case of atypical Turner's syndrome with unusual karyotype is reported. The chromosome complements of the patient, studied with different banding techniques, is 45,XO/46,X,dic(X)(Xqter leads to p22::p22 leads to qter). In the literature 8 similar cases have been reported. Short stature and amenorrhea are the most constant findings. The mechanisms by which the observed chromosomal "rearrangement" can be produced are briefly discussed.
Three mean particle sizes of oxfendazole raw material (1.65 micron, lot A; 3.2 micron, 10t B; 12.0 micron, lot C) were prepared and identically formulated as corresponding (A, B, and C) suspensions at 2.26% (W/V) concentration. Studies involving microscopic examination, scanning electron microscope analysis, particle size distribution, and surface area measurement were carried out on raw materials. In vitro dissolution profiles were obtained for the suspensions. A comparative bioavailability study of these 3 suspensions was performed in 12 sheep with each sheep given each formulation in a Latin square crossover study design; oxfendazole was dosed at rate of 5 mg/kg of body weight. Plasma-value measurements were made followed by an analysis of various bioavailability studies. Plasma area values indicated that suspension C (dw = 12.0 micron) was significantly (P less than 0.05) less bioavailable than was suspension A (dw = 1.65 micron); there was no difference between suspension A and suspension B. Significant differences were not seen in biological half-life and maximum plasma concentrations. The term dw refers to that particle diameter (determined by Coulter counting) at which 50% of the oxfendazole mass was in the form of particles having a lesser diameter and 50% was in the form of particles having a greater diameter. In a separate study involving 20 Merino weaner sheep infected with benzimidazole-resistant Haemonchus contortus larvae, oxfendazole's anthelmintic efficacy was demonstrated in the 2.26% suspension dosage form (90% particles less than 10 micron) at a dosing rate of 5 mg/kg. A correlation was found between its anthelmintic activity and plasma area values when compared in individual sheep. Data demonstrated that substantial differences in particle size distribution of oxfendazole could influence its dissolution rate, plasma concentrations, and absorption characteristics, thus indicating that oxfendazole's absorption could be dissolution-rate limited.
