Susceptibility of mammalian cell cultures to infection with arbo-togaviruses.

Six alphaviruses and four flaviviruses were inoculated intracerebrally into 1 to 3 days old suckling mice. All mice developed CNS diseases and died 2-5 days post-infection. It appeared that alphaviruses were more virulent than flaviviruses since they brought death to the injected mice earlier than the flaviviruses. The susceptibilities of nine different culture cells to those viruses were also investigated using plaque assay. BHK-21 cells produced clearer plaques and were more sensitive than other cells. Plaque reduction tests using antibodies against various viruses revealed no cross-reactions between alphavirus and flavivirus. However, minor cross-reactions were found within groups. The results of this study suggest that BHK-21 cells can be used for the large scale screening of viruses in domestic animal specimens and for detecting serum neutralizing antibodies against arboviruses.

Watery diarrhea syndrome caused by multihormonal malignant pancreatic islet cell tumor secreting somatostatin, vasoactive intestinal peptide, serotonin, and prostaglandin E--a clinicopathological, biochemical, immunohistochemical, and ultrastructural study.

The pathophysiological, biochemical, histological, ultrastructural, and immunohistochemical characters of a case of malignant pancreatic islet cell tumor with watery diarrhea syndrome were carefully investigated. Four hormones or mediators--somatostatin (SST), vasoactive intestinal peptide (VIP), serotonin, and prostaglandin E--were markedly elevated in the circulation. The diagnosis was further confirmed by exploratory laparotomy and autopsy. The contents of SST and VIP in tumor tissues were very high. Gel chromatography of tumor extract revealed single peaks for both SST and VIP. Immunohistochemical studies of tumor tissues showed numerous immunoreactive cells to anti-SST, moderate amount of VIP-positive cells, and a few hCG-, insulin-, and glucagon-positive cells. In conclusion, this is an unusual case of Verner-Morrison syndrome in which three kinds of bioactive hormones or mediators were simultaneously secreted; peptides, amine, and prostaglandin.

Insulinoma. An immunocytochemical and morphologic analysis of 95 cases.

One hundred twenty-seven insulinomas from 95 cases (1 malignant and 94 benign) were studied pathologically. Thirty-six tumors (35 cases) were examined by electron microscopy. Typical beta-cell secretory granules of crystalloid-form cores and/or atypical secretory granules were discerned in all tumors examined. A new type of secretory granule, with high electron-dense crystalloid-form cores and moderate electron-dense granular substance filling the space between the core and the limiting membrane, were observed in two cases. Among 68 insulinomas (67 cases) subjected to immunocytochemical investigations with ten peptide hormones (insulin, glucagon, somatostatin, pancreatic polypeptide (PP), gastrin, motilin, secretin, vasoactive intestinal polypeptide (VIP), gastric inhibitory polypeptide (GIP), and neurotensin), 42 were found to be multihormonal, varying from two to four peptides secreted. The hormones contained were insulin, glucagon, PP, somatostatin, and gastrin in different combinations. One patient had hyperinsulinemia and hypergastrinemia concurrently, and two islet tumors were excised at an interval of 10 months. Both electron microscopy and immunocytochemistry confirmed the presence of beta- and alpha-cells in the first tumor, whereas the second tumor revealed only G-cells by electron microscopy, and G- and beta-cells on immunocytochemical staining. The morphologic and immunocytochemical characteristics of the insulinomas in this series are discussed.

Clinical and pathological characteristics of carcinosarcoma of the esophagus: report of four cases.

Carcinosarcoma of the esophagus is a rare malignant neoplasm composed of both carcinomatous and sarcomatous elements. Four out of 850 patients with carcinoma of the esophagus or the cardiac portion of the stomach undergoing operation at Capital Hospital, Beijing, had carcinosarcoma of the esophagus. They ranged from 50 to 58 years old, but only 45% of all 850 patients with carcinoma were in the age group from 51 through 60 years old. All 4 patients with carcinosarcoma of the esophagus were men. On barium swallow esophagogram, the 4 patients had a characteristic intraluminal polypoid lesion in the middle third of the esophagus. Such lesions of the esophagus also can occur in patients with benign fibrovascular polyps, leiomyoma, and the polypoid form of esophageal carcinoma and esophageal melanoma. The last two can be differentiated from carcinosarcoma only by pathological examination after excision. After radical excision and esophagogastrostomy, the 4 patients were followed up for 19, 4, 4, and 3 years. They were in good condition without recurrence.

Clinico-pathological and ultrastructural characteristics of pheochromocytoma. An analysis of 55 cases.

58 tumors of pheochromocytoma from 55 cases were studied clinico-pathologically and electron-microscopically. In this series, males were predominant and the number of left adrenal pheochromocytomas was 1.6 times that of right ones. Extraadrenal pheochromocytomas amounted to 41.4%. All 5 malignant pheochromocytomas in this series arose from extraadrenal paraganglia. Two types of secretory granules corresponding to the epinephrine and norepinephrine granules in the normal adrenal medulla were discerned in all 13 tumors examined under electron microscope. The contents of different types of granules were compared with urinary catecholamine excretion, and no apparent correlation was disclosed. Large amount of "E" granules in tumor cells of some cases might be associated with very low level of urinary epinephrine, while in the other cases, the reverse was true. The amount of catecholamine excreted in urine also bore no relation to the size of the tumor.
