[Clinicopathologic study of patients with small cell carcinoma of the cervix].

OBJECTIVE: To study the clinicopathologic characteristics, diagnosis and treatment of small cell carcinoma of the cervix. METHODS: Eight patients with small cell carcinoma of the cervix were treated from May 1991 to December 1997 at Jiangxi Women's and Children's Hospital. Clinical data and follow-up records were retrospectively analyzed. RESULTS: Five patients had stage I(b) disease and 3 had stage II(b) disease. Seven patients underwent radical hysterectomy with pelvic lymph node dissection. One patient with stage II(b) disease was only treated with radiotherapy. Of the patients treated with operation, three showed pelvic lymph node metastases. Four patients who are alive free of the disease have been followed for 2, 13, 16 and 25 months, respectively. The survival intervals of 4 patients who died of the disease was 4, 5, 7 and 10 months, respectively. Of the death, one patient was only treated with radiotherapy, another had a very large cervical tumor and the two others complicated with pelvic lymph node metastases. CONCLUSIONS: Small cell carcinoma of the cervix is an aggressive tumor with a potentiality for early pelvic lymph node metastases and rapid recurrence, It is associated with higher mortality than any other cervical malignant neoplasms. It is able to make a right diagnosis with a combination of histopathologic and immunohistochemical characteristics.

[Comparison of mechanical properties of laser-welded wrought and cast titanium].

OBJECTIVE: This study was aimed to compare the mechanical properties of laser-welded wrought and cast titanium. METHODS: According to relevant reports and clinical demands, the specimens of TA2 were machine milled or cast into certain sizes using LZ centrifuge-vacuum-pressure titanium casting machine. This experiment involved four groups including the laser-welded wrought titanium group, the laser-welded cast titanium group, the wrought titanium group and the cast titanium group. There were 6 complete specimens in each group and the specimens were ultrasonically cleaned with ethyl and distilled water before they were welded with home-made Nd:YAG apparatus, and comparisons of mechanical properties were made among these groups. All tensile specimens were mounted on the Instron universal testing machine to test tensile loads, yield loads and absolute elongations were measured. The cross section of fractured surface was observed under S-2700 Hitachi SEM and Vickers' microhardness values in different areas were detected with AKASHI microhardness tester. RESULTS: It was showed that the tensile load of laser-welded wrought titanium made no difference from that of the original wrought titanium, but the yield load of laser-welded wrought titanium was bigger than that of the original wrought titanium. The tensile load and yield load of cast titanium were slightly bigger than those of wrought titanium. The tensile load and yield load of laser-welded wrought titanium made no statistic difference from those of laser-welded cast titanium. The absolute elongations of cast or welded titanium were lower than those of wrought or non-welded titanium. The fractured surface of cast titanium under SEM appeared lath-like structure and that of welded cast titanium honeycomb-like structure. The honeycomb-like structure of wrought titanium was more obvious than that of laser-welded wrought titanium. The microhardness value of laser-welded cast titanium was slightly bigger than that of laser-welded wrought titanium. CONCLUSION: The mechanical property of laser-welded wrought titanium might be not apparently different from that of laser-welded cast titanium.

[A surface reacted layer study of titanium-zirconium alloy after dental casting].

OBJECTIVE: To investigate the influence of the mold temperature on the surface reacted layer of Ti-Zr alloy castings. METHODS: Ti-Zr alloy was casted into a mold which was made of a zircon (ZrO2.SiO2) for inner coating and a phosphate-bonded material for outer investing with a casting machine (China) designed as vacuum, pressure and centrifuge. At three mold temperatures (room temperature, 300 degrees C, 600 degrees C) the Ti-Zr alloy was casted separately. The surface roughness of the castings was calculated by instrument of smooth finish (China). From the surface to the inner part the Knoop hardness and thickness in reacted layer of Ti-Zr alloy casting was measured. The structure of the surface reacted layer was analysed by SEM. Elemental analyses of the interfacial zone of the casting was made by element line scanning observation. RESULTS: The surface roughness of the castings was increased significantly with the mold temperature increasing. At a higher mold temperature the Knoop hardness of the reactive layer was increased. At the three mold temperature the outmost surface was very hard, and microhardness data decreased rapidly where they reached constant values. The thickness was about 85 microns for castings at room temperature and 300 degrees C, 105 microns for castings at 600 degrees C. From the SEM micrograph of the Ti-Zr alloy casting, the surface reacted layer could be divided into three different layers. The first layer was called non-structure layer, which thickness was about 10 microns for room temperature group, 20 microns for 300 degrees C and 25 microns for 600 degrees C. The second layer was characterized by coarse-grained acicular crystal, which thickness was about 50 microns for three mold temperatures. The third layer was Ti-Zr alloy. The element line scanning showed non-structure layer with higher level of element of O, Al, Si and Zr, The higher the mold temperature during casting, the deeper the Si permeating and in the second layer the element Si could also be found. CONCLUSION: The mold temperature is one of the major factors influencing to casting quality. In order to reduce the surface reacted layer of Ti-Zr alloy castings, the lower mold temperature and the investment without Si should be chosen.

Effect of SJAMP on ATP release of platelet.

The aggregation and ATP release of placelet of normal subjects were measured by platelet lumi-aggregometer. It was found that the aggregation curve induced by SJAMP at the concentration of 100 mg/L was a typical second phase aggregation. There existed a certain lag between platelet aggregation and secretion. The secretion actually began slightly after the second phase of aggregation, suggesting that the second phase aggregation induced by SJAMP is not dependent upon the release of contents of dense granule alone. If platelets were incubated with cyclo-oxygenase inhibitor, the second phase aggregation was inhibited and no ATP was released. The results indicated that the aggregation and release reaction induced by SJAMP were dependent upon the generation of prostaglandin endoperoxides and TXA2 in normal subjects. The amount of ATP release was 0.69 +/- 0.22 nmol/10(8) platelets as stimulated with SJAMP (100 mg/L). But the amount of ATP release were 1.60 +/- 0.25 and 1.37 +/- 0.15 nmol/10(8) platelets when platelets were stimulated with ADP (5 mumol/L) and collagen (5 mg/L). The amount of ATP release induced by SJAMP was significantly lower than that of ADP and collagen. These findings indicated that SJAMP was a weaker agonist than ADP in terms of platelets release reaction.

Dauricine inhibits redistribution of platelet membrane glycoprotein IV and release of intracellular alpha-granule thrombospondin induced by thrombin.

AIM: To study the possibility of dauricine (Dau) inhibiting redistribution of platelet membrane glycoprotein IV (GPIV) and release of intracellular alpha-granule thrombospondin (TSP) on platelet activation. METHODS: Using the flow cytometric assay of washed platelet to record expression of GPIV and release of TSP induced by thrombin. RESULTS: Dau did not affect GPIV and TSP on resting platelet membrane but inhibited redistribution of GPIV to the platelet surface and TSP release on activated platelet. There was a marked positive correlation between changes of GPIV and TSP (r = 0.511, P < 0.01). The inhibitory effect of Dau appeared not to be Ca2+ concentration-dependent. CONCLUSION: Dau inhibited redistribution of GPIV and release of intracellular alpha-granule thrombospondin induced by thrombin.

[The study on mechanical properties of laser welds of titanium in dentistry].

OBJECTIVE: To explore the mechanical properties of laser welds of titanium under different conditions. METHODS: Home-made Nd: YAG laser welding apparatus was used to weld TA2 and Ti-6Al-4V under different laser energy. Instron universal testing machine was adopted to measure the tensile load, 0.2% yield load and absolute elongation. The fractured surface was observed under SEM, Vickers microhardness values at welding area were measured and the mechanical properties after immersion in artificial saliva were detected. RESULTS: The mechanical strength of laser welds were related with material, voltage and the combination of them. At 700V-750V, there was no significant difference between tensile load of TA2 laser welds and that of the original material. For Ti-6Al-4V, At 750V. The mechanical strength of laser welds before and after immersion in artificial saliva made no statistic difference. CONCLUSION: It is suggested that the heat-affected zone of laser welding might be small and the corrosion resistance in artificial saliva might be high. The mechanical strength of titanium laser weld might meet the clinical demands at 19-20J/P.

Maximizing the hemodynamic benefit of enhanced external counterpulsation.

BACKGROUND: Enhanced external counterpulsation (EECP) has been demonstrated to be an effective treatment for angina and exertional ischemia in patients with coronary disease. HYPOTHESIS: It is hypothesized that the ability of EECP to enhance the recruitment or development of coronary collaterals in coronary artery disease may be determined by the relative magnitude of diastolic augmentation (DA) and systolic unloading (SU). This study examines the relation between the proposed EECP effectiveness ratio (DA/SU), as assessed by finger plethysmography, and changes in descending aortic flow as assessed by Doppler echocardiography in 15 patients during EECP. METHODS: Varying external cuff pressures (0-275 mmHg) were used to generate a range of DA/SU ratios. The effect on aortic antegrade systolic and retrograde diastolic flow was assessed by Doppler echocardiography to determine whether there was an optimal EECP effectiveness ratio that maximizes the hemodynamic effects of EECP. With increasing DA/SU there was an initial positive linear increase in both systolic and diastolic flow volume. Systolic flow maximized at an effectiveness ratio of 1.5 and diastolic flow at a ratio of 2.0 RESULT: Therefore, effectiveness ratios (DA/SU) in the range of 1.5-2.0 are optimal for maximizing the hemodynamic effects of EECP.

Prior revascularization increases the effectiveness of enhanced external counterpulsation.

BACKGROUND AND HYPOTHESIS: Enhanced external counterpulsation (EECP) is an effective noninvasive treatment for chronic angina. However, its usefulness has been felt to be limited in patients with angiographically demonstrated triple-vessel coronary artery disease (CAD), in accord with the hypothesis that a patent vessel is necessary for transmission of the EECP-augmented coronary artery pressure and volume to the distal coronary vasculature. METHODS: The effect of revascularization [coronary artery bypass grafting (CABG)] prior to EECP was examined in 60 patients with CAD and chronic angina (35 without and 25 with prior CABG). Patients were grouped by the extent of CAD (single-, double-, triple-vessel disease in the unrevascularized group) and by the extent of residual disease (number of stenotic native vessels unbypassed or supplied by a stenotic graft in the CABG group). Significant CAD or graft stenoses were defined as stenoses demonstrating > or = 70% luminal diameter narrowing. Benefit was assessed by improvement in post-EECP treatment over pretreatment radionuclide stress testing. RESULTS: Radionuclide stress testing demonstrated a comparable favorable response (80 vs. 71%; p = NS) in patients with prior CABG versus unrevascularized patients. Enhanced external counterpulsation was highly and comparably effective in patients with unrevascularized native single- and double-vessel CAD and in patients with CABG with residual single- and double-vessel CAD (88 vs. 80%; p = NS). Most notably, CABG significantly increased the beneficial response to EECP in those patients with triple-vessel CAD and stenotic grafts compared with unrevascularized patients with triple-vessel CAD (80 vs. 22%; p < 0.05 by chi-square test). CONCLUSION: The results suggest a new role for EECP as an effective treatment for post CABG ischemia, despite extensive CAD and even in the presence of stenotic grafts.

Effect of SJAMP on human platelet cytoplasmic Ca2+.

Using the method of dual-wavelength measurement of platelet [Ca2+]i and Fura-2 as the Ca2+ fluorophore probe, we measured the effect of acidic Mucopolysaccharide from Sticopus Japonicus Selenka (SJAMP) on platelet [Ca2+]i. The results showed that the most significant increase in platelets [Ca2+]i was seen when the concentration of SJAMP was 100 micrograms/ml and the elevation of normal platelet [Ca2+]i was 93.96 +/- 10.24 nmol/L (n = 10). In the presence of extracellular Ca2+ (1 mmol/L), the magnitude of platelet [Ca2+]i response to SJAMP was increased and the [Ca2+]i could reach 116.72 +/- 10.66 nmol/L (n = 10). On the other hand, the magnitude of increased platelet [Ca2+]i induced by SJAMP was smaller and the duration of [Ca2+]i reaching the highest level was longer when compared with other platelet aggregation agents. In the mean time, if platelets were first incubated with cyclooxygenase inhibitor, the rise of [Ca2+]i evoked by SJAMP was inhibited. The results indicated that the mechanism of the rise of [Ca2+]i induced by SJAMP might be dependent upon the generation of prostaglandin endoperoxides and(or) TXA2.

Synthesis and activity of bivalent FKBP12 ligands for the regulated dimerization of proteins.

The total synthesis and in vitro activities of a series of chemical inducers of dimerization (CIDs) is described. The use of small-molecule CIDs to control the dimerization of engineered FKBP12-containing fusion proteins has been demonstrated to have broad utility in biological research as well as potential medical applications in gene and cell therapies. The facility and flexibility of preparation make this new class of wholly synthetic compounds exceptionally versatile tools for the study of intracellular signaling events mediated by protein-protein interactions or protein localization. While some congeners possess potency comparable to or better than the first generation natural product-derived CID, FK1012, structure-activity relationships are complex and underscore the need for application-specific compound optimizations.
