Nutritional assessment in gynecology patients.

A nutritional assessment was performed upon 72 patients admitted for major elective gynecologic surgery for nonmalignant diseases. This nutritional assessment included body weight, triceps skinfold, arm muscle circumference, creatinine-height index, serum albumin and transferrin, total lymphocyte count, and dermal antigen testing. We found that 29 patients (42%) had at least one test result indicating a nutritional deficiency. Of the 72 patients, 19 (26%) experienced a postoperative complication. The occurrence of a complication did not correlate with any nutritional parameter other than albumin (p = 0.013), but even then, the positive predictive value of albumin was only 40%. Age also correlated with complication status (p = 0.002).

Methyl-CCNU, doxorubicin, and cis-diamminedichloroplatinum II in the management of recurrent and metastatic squamous carcinoma of the cervix.

Twenty-three patients with recurrent unresectable carcinoma of the cervix or distant metastasis at initial presentation were treated with methyl-CCNU (175 mg/m2) and doxorubicin (45 mg/m2) on day 1 and cis- diamminedichloroplatinum II (90 mg/m2) on day 22 of a 42-day treatment cycle. Twenty-two patients had squamous carcinoma and 1 had adenosquamous carcinoma. There were two complete responses (CR), five partial responses (PR) (greater than 50% tumor reduction, greater than 3-month duration), four patients with stable disease (less than 50% reduction, greater than 3-month duration), and 12 patients who had tumor progression. One CR has been maintained greater than 28 months, and the other greater than 8 months. Total CR and PR was 7 of 23 (30.4%). Three 23 responses occurred among 15 patients (20%) who had cancer primarily confined to the pelvis, while 4 of 8 patients (50%) with distant metastasis responded. During the initial 2 cycles of chemotherapy, 12 patients had myelosuppression, defined as a leukocytes less than 3000/mm3, granulocytes less than 1,000/mm3, or platelets less than 100,000/mm3. There were no treatment-related deaths.

Tamoxifen and endometrial carcinoma: alterations in estrogen and progesterone receptors in untreated patients and combination hormonal therapy in advanced neoplasia.

Estrogen receptors and progesterone receptors were measured in tumors from patients with previously untreated endometrial carcinoma before and after a 5-day course of tamoxifen citrate. On initial biopsy, 13 of 25 tumors (52%) were progesterone receptor-positive, whereas 21 of 25 tumors (84%) were progesterone receptor-positive after tamoxifen. Grades 1 and 2 tumors were more likely to demonstrate this increased incidence of measurable progesterone receptors. Considering these results, and the work of others who have shown that progesterone receptor-positive metastatic endometrial cancer is more responsive to progestin therapy than are progesterone receptor-negative tumors, we instituted a phase II clinical trial of tamoxifen plus progestin for patients with recurrent endometrial carcinoma. Thus far, however, the 33% total response rate achieved with the combination therapy has not been superior to standard progestin therapy.

Platinum tissue concentrations following intra-arterial and intravenous cis-diamminedichloroplatinum II in New Zealand white rabbits.

Intra-arterial chemotherapy is supported by the rationale that regional infusion more than systemic administration can produce higher regional concentrations of cytotoxic drugs, thereby enhancing tumor kill. The hypothesis that intra-arterial infusion of cis-diamminedichloroplatinum II (DDP) enhances regional drug concentrations in tissue has not been studied. Therefore, 30 New Zealand white rabbits were treated with DDP. Fifteen animals were given DDP intravenously, and the other 15 received DDP by intra-arterial infusion into the terminal aorta. Necropsy was performed at 2, 6, 24, and 48 hours after infusion. Samples of tissue were analyzed for platinum content by flameless atomic absorption spectrophotometry. Uterine and ovarian platinum content was significantly greater after intra-arterial infusion than after intravenous infusion (P less than 0.001 and P less than 0.01, respectively). This substantiates the rationale that regional arterial infusion of DDP can achieve higher tissue platinum levels than the intravenous route, at least, in this animal model.

Endoarterial pulmonary metastasis of malignant trophoblast associated with a term intrauterine pregnancy.

A previously healthy gravida 4, para 3, developed preclampsia and progressive dyspnea at the 37th gestational week and had bilateral pulmonary infiltrates on chest roentgenogram. She delivered a healthy, term, male infant with a normal appearing placenta. Postpartum, her respiratory status gradually worsened. A lung biopsy on the 20th postpartum day revealed intravascular trophoblasts, diffuse arteriolar thrombosis with pulmonary infarction, and subacute interstitial pneumonitis. Combination chemotherapy was instituted, but the patient died from respiratory insufficiency.

Pubic osteomyelitis due to anaerobic bacteria.

Osteomyelitis of the pubic bone due to anaerobic bacteria has been reported infrequently, although an entity known as "sterile" osteitis pubis is common to the literature. We have described two cases of pubic osteomyelitis due to anaerobic bacteria, discussed two previously reported cases, and suggested that most cases of what has previously been termed sterile osteitis pubis may actually have been due to anaerobic bacteria that were not isolated because of deficiencies in collection, transport, and culture of clinical specimens. Included are the pathogenesis and an approach to the treatment of this entity.

Rewiew of ovarian cancer at the University of Texas Systems Cancer Center, M.D. Anderson Hospital and Tumor Institute.

The records of 2,115 patients with ovarian cancer who were treated at the M.D. Anderson Hospital and Tumor Institute in Houston, Texas, during the 30 year period from 1944 to 1973 were reviewed. Ninety percent of the patients had an epithelial cancer of the ovary. The important prognostic factors include stage and grade of tumor and the presence or absence of ascites. Probably the most important prognostic factor, however, was the size of the largest tumor mass that remained after initial surgery. The patient's age and socioeconomic level were also influencing factors in the survival rate in this series of patients. Most of the patients had advanced disease when first examined and received some type of adjunctive postoperative treatment. The survival of patients who received postoperative irradiation, when compared by stage and size of the largest residual tumor mass, was improved over those who received chemotherapy.

Occurrence of a thermostable antigen of ovarian carcinoma in normal tissues and secretions.

Thermostable antigen (TA) occurs in ovarian carcinoma and in certain specialized tissues. High titers of TA were found in nineteen of twenty endocervical extracts but not in myometrium, endometrium, or exocervix. TA was present in all of sixteen cervical mucus samples. Antibodies to perchloric acid extract of bronchus showed cross-reaticity with TA from ovarian carcinoma and cervix. Immunofluorescence with frozen sections revealed TA to be present in the columnar epithelium of ovarian neoplasms, and endocervical glands, in bronchial seromucous glands and in hepatic bile canaliculi. Immunofluorescence and immunodiffusion were used to test for cancer patient antibodies against TA. All sera tested negative. An immunofluorescence inhibition test for circulating TA also tested negative in sera of ovarian and cervical cancer patients.

Gonadal dysgenesis with vulvar carcinoma.

An unusual case of vulvar carcinoma in a gonadal dysgenesis patient is described. In addition to clinical manifestations, certain laboratory data related to endocrine status were gathered from serum and urine analyses. Finally, receptors for estrogens, progestins, and glucocorticoids were determined on biopsies of the vulvar carcinoma.

Chemotherapy for squamous carcinoma of the cervix: doxorubicin--methyl CCNU.

Treatment was initiated for metastatic or recurrent squamous carcinoma of the cervix with a combination of doxorubicin and methyl CCNU in 37 patients. Results after at least one full course of chemotherapy were fully evaluated in 31 of these patients. The objective response rate of 45.1 per cent obtained with this therapy compared favorably with those obtained by other authors using single or multiple agents. This chemotherapy regimen may need to be modified to improve patients' tolerance. The improved survival rate and high number of complete remissions demonstrated in this pilot study show promise for this type of chemotherapy in the future.

The use of laparoscopy to determine the results of chemotherapy for ovarian cancer.

The chemotherapeutic management of patients with epithelial tumors of the ovary is hampered by a lack of diagnostic tests that are sufficiently sensitive for detecting residual or recurrent tumor. At the completion of a chemotherapy program, direct visualization of the peritoneal cavity is necessary to accurately assess the amount and location of residual tumor. Laparoscopy, with visually directed biopsies of residual tumor masses, can spare some patients a laparotomy and yet provide a safe and easy route for assessment of intraabdominal disease. If residual tumor is not seen at laparoscopy, than laparotomy with multiple peritoneal biopsies must be done so that residual tumor will not be missed and chemotherapy prematurely stopped. Laparoscopy is not an alternative to "second look" laparotomy but is a useful adjunct for determining the presence of resectable, unresectable or diffuse disease after chemotherapy.

Preirradiation celiotomy and extended field irradiation for invasive carcinoma of the cervix.

Epidermoid carcinoma of the cervix was treated in 120 patients by means of exploratory celiotomy (with semitherapeutic excision of involved lymph nodes) followed by irradiation. The size of the field used for irradiation was determined by the presence and site of lymph involvement. Of 64 patients, metastatic cancer in pelvic nodes was found in 40, and in common iliac or aortic nodes in 24. Of the 2 groups, 8 and 3 patients, respectively, survived for 2 years or more. Irradiation to extended fields (using 5500 rads at 850 rads per week) controlled the cancer satisfactorily within the treated area, but the incidence of bowel complications was high. Recurrent carcinoma usually appeared as distant metastases outside the treatment area which suggests that patients with bulky primary lesions and positive nodes actually already have systemic disease as treatment is started. A safe yet effective dosage level for radiation therapy to extended fields has not yet been established.

Vulvovaginoplasty in gynecologic oncology.

Eight patients from M.D. Anderson Hospital underwent Williams' vulvovaginoplasty for sexual rehabilitation following pelvic exenteration. In 7 of the 8 patients the functional results were excellent, and none of the patients experienced morbidity. This surgical procedure is less extensive than other reconstructive methods and skin grafting or mechanical dilatation are rarely required.

Epithelial carcinoma of the ovary:prognostic importance of histologic grade.

Although this report contains only a few cases of epithelial carcinoma of the ovary and is not complete, it does suggest several new points and confirms other points reported in the literature: 1) increasing stage of cancer correlates well with decreased survival; 2) higher histologic grade of cancer correlates with decreases survival; 3) patients with a given stage of ovarian cancer having higher grades of malignancy have poorer survival; and 4) lower grade tumors are most frequently associated with lower stages of cancer, and higher grade tumors are most frequently associated with advanced stages.

Diagnosis and staging of ovarian carcinoma.

The early diagnosis of ovarian carcinoma will await the development of adequate and accurate screening tests. In the interval prior to development of these tests, ovarian cancers will frequently be discovered in an advanced stage. Improved methods of treatment must be based on an adequate trial of existing treatment methods founded on an understanding of factors influencing survival. FIGO stage grouping identifies many of these factors and should be used to identify patients with similar characteristics. Other prognostic factors such as tumor grade, volume of residual disease and the presence of ascites must be recorded and considered in evaluating therapeutic trials. The adoption of a routine method for patient evaluation and exploration may enhance the amount of information available for each patient and assure that adequate information is available with which to place the pateint in FIGO stage and substage.
