[Expression of c-H-ras, c-erb B1 and c-erb B2 gene products in human bladder cancer].

To investigate the expression of c-H-ras (p21), c-erb B1 (EGFR) and c-erb B2 (p185) gene products in human bladder cancer, immunohistochemical studies using monoclonal antibodies to these proteins were performed on formaline fixed (within 15 hours)-paraffin sections of tumor tissues from 20 patients with bladder cancer, normal appearing adjacent bladder (non-tumor) tissues from 11 of the 20 patients, and normal bladder tissues from 3 patients who died of non-cancerous diseases as control. p21 Positive staining was demonstrated in the superficial cells of urothelium in 1 of 3 controls, also in 5 of 20 tumor tissues compact cells without vacuole in cells which have an increased nuclear/cytoplasmic ratio. Seven of 11 non-tumor tissues indicated positive staining either in superficial layer only or in whole layers of urothelium, and 1 of the latter group reacted with the monoclonal antibody to human bladder cancer produced in our laboratory. EGFR was found in 5 of 20 tumor tissues and 7 of 11 non-tumor tissues, but not in controls. Most EGFR positive tissues also indicated p21 positivity except in 1 of the tumor tissues. p185 Positive staining was demonstrated in 9 of 20 tumor tissues and 5 of 11 non-tumor tissues, but not in the controls. Furthermore, 5 of 6 tumor tissues from the patients with lymph node metastasis indicated p185 positivity. These results suggest that both p21 and EGFR may have a role in transformation and that p185 has a role in the development of metastasis in some urothelial malignancies.

[Combination therapy with natural type human tumor necrosis factor (MHR-24) and human lymphoblastoid interferon-alpha (MOR-22) against renal cell carcinoma--a multiclinic cooperative, early phase II study. Subcommittee on Urogenital Malignancy, Committee on MHR-24 against Tumors].

The combination therapy with natural type human tumor necrosis factor (n-TNF; MHR-24) and human lymphoblastoid interferon-alpha (n-IFN-alpha; MOR-22) was investigated for antitumor effect against renal cell carcinoma in a multiclinic cooperative study throughout Japan. The "Response criteria of Japan Society for Cancer Therapy" were followed for the handling of subjects and the evaluation of antitumor effect. MHR-24 was administered at a daily dosage of 5,000-10,000 JRU by intravenous drip and MOR-22 at a dosage of 5,000,000 IU daily was administered intramuscularly at the same time. Both drugs were administered for 4 weeks or longer. A total of 36 patients were enrolled as subjects in the study. None of them were classified as ineligible. Five patients, were classified as imperfectly evaluable, and 31, as evaluable for the results of treatment. The responses in the evaluable patients were partial response (PR) in 4 patients, minor response (MR) in 3 patients, no change (NC) in 14 patients and progressive disease (PD) in 10 patients, with a response rate of 12.9%. Adverse reactions to the therapy were investigated in all 36 patients. The frequent subjective and objective reactions that occurred were fever, rigors and shivering, anorexia, and generalized malaise, and the frequent abnormal laboratory findings were leukopenia, thrombocytopenia, elevation of GOT, and elevation of GPT.

[Urinary prostaglandin E2 and kallikrein-like activity excretion in renal transplant recipients].

We investigated the relationship between urinary prostaglandin E2 (UPGE2), kallikrein (UKal), graft function and complications after renal transplantation in 11 patients. Grafts of 9 patients were from living-related donors (LRD), and other 2 patients were from a cadaveric donor (CAD). UPGE2 was measured by the radio immunoassay, and UKal was measured by the amidolytic method using Pro-Phe-Arg-MCA. The results were as follows. 1. In 5 of 6 patients from LRD without acute rejection episode (ARE), both UPGE2 and UKAL were within normal and/or slightly less than normal. UKal values of the other patient were high in his donor. 2. In 2 of 3 recipients from LRD who experienced ARE, UKal increased prior to ARE. UPGE2 also increased at the time of ARE, but it showed a periodic rise in the stable condition. 3. In 1 of 2 recipients from CAD, UKal exhibited a transient elevation at the time of acute tubular necrosis (ATN) and pyelonephritis while UPGE2 was low. In another recipient, UKal was almost within normal range at the time of ATN, and UPGE2 showed a periodic rise. 4. A significant correlation was seen between UKal, UPGE2 and UAld in the recipients from LRD without ARE (except 1 patient who showed high UKal values). However, the correlation was blurred inclusive of values in the patients who experienced ARE or other complications. There was no relationship between UKal, UPGE2, creatinine clearance, urine volume and urinary sodium. 5. [Urinary kallikrein-like activity in renal transplant recipients].

Koolen, M.I. et al. (1984) measured urinary kallikrein (UKal) in renal transplant recipients using 2 methods, S-2266 amidolytic assay and radioimmunoassay, and reported that the values by the former method showed an increase in amount at the time of acute rejection episode (ARE) despite of no variation in those by the latter. To clear the discrepancy, 24 hours excretion of UKal was measured by the amidolytic assay using Pro-Phe-Arg-MCA under 2 conditions, with and without soybean trypsin inhibitor (STI), in 10 living related kidney recipients (LR), their donors (LD), and 8 cadaver kidney recipients (CR), Furthermore, gel filtration chromatography (Sephacryl S-200) was performed in some recipients and LD to confirm whether the amidolytic activity (ALA) indicates exclusively UKal or not. The results were as follows. In LD, there were no statistical differences in ALA between not only before and after uninephrectomy but also with and without STI. On the post-operative course, some recipients showed a tendency of decrease in ALA with STI, and some kept a constant level. There occasionally appeared a difference between ALA with and without STI just after the transplantation, and at the time of ARE, acute tubular necrosis (ATN) or urinary tract infection (UTI). There was no fixed characteristic variation in ALA, both with and without STI, at the time of ARE. ALA with STI was lower in LR than in LD, and the lowest in CR, From the second week of transplantation and afterward, ALA with STI showed a positive correlation not with creatinine clearance but with urinary aldosterone.(ABSTRACT TRUNCATED AT 250 WORDS)

[Total replacement of the urinary bladder using an ileal pouch bladder].

We have used the continent ileal bladder as a bladder replacement after radical cystectomy. The ileal bladder is an ileal pouch which is anastomosed to the urethral stumps. The ureters are implanted by a free end ureteroileostomy. The long term results with 26 patients who underwent this procedure are reported. In the early postoperative period, urodynamic and radiographic studies revealed small capacity and high intravesical pressure of the ileal bladder. However, it became a low pressure reservoir with increased capacity gradually. The average bladder capacity was about 250 ml and average residual urine was 30 ml. Most of the patients were continent in the daytime if the voiding intervals were less than 3 hours at night, some patients were incontinent. Urinary leakage was the most frequent complication. VUR and hydronephrosis were still the problems to be solved. The ileal pouch bladder is a valuable procedure in properly selected cases.

[Plasma normetanephrine and metanephrine levels and their relationship to norepinephrine and epinephrine].

To study the relation of normetanephrine (NM) and metanephrine (M) to norepinephrine (NE) and epinephrine (E), plasma free NM (f-NM), free M (f-M), total NM (t-NM) and total M (t-M) were measured in normal subjects and patients with pheochromocytoma (PHEO), neuroblastoma, Cushing's syndrome, primary aldosteronism and chronic renal failure (CRF) by radioimmunoassay. Plasma f-NE and E were measured by radioenzymatic assay. Both f- and t-NM were high in PHEO, neuroblastoma and CRF. f- and t-M were also high in some patients with PHEO and CRF. Positive correlation was observed not only in f-NE with f-NM and t-NM, but also in f-E with f-M and t-M except for CRF. Although upright posture induced an elevation in f-NE and f-NM, t-NM was unchanged in normal subjects. In patients with PHEO, metoclopramide induced a prompt elevation in f-NE and E but no-change in t-NM and M levels. f-NE, f-E, f-NM, t-NM, f-M and t-M decreased rapidly after the resection of PHEO and reached the normal level on the third day after the surgery. In CRF patients, f-NM, t-NM, f-M and t-M decreased after hemodialysis despite an increase of f-NE. From these results, it was suggested that plasma NM and M levels reflected plasma NE and E to a certain extent in normal subjects and patients with normal renal function, and that the impaired renal function provoked an elevation of plasma NM and M due to the accumulation of them.

[The phase IV studies with Estracyt in prostatic cancer].

Both anticancer and adverse reactions of Estracyt were investigated by oral administration to 200 patients with prostatic cancer. Two capsules of Estracyt were given twice a day and the administration was principally continued for more than six months. The 200 patients consisted of 68 and 132 patients who were previously untreated and treated, respectively. Thirty seven cases had been treated only with Estracyt and 132 cases also received other treatments. Seventy-five cases were of primary therapy, 71 cases were of maintenance therapy, and 27 cases were of the re-activated stage therapy and 27 cases were of other categories. In conclusion, among the 190 cases for which the due judgement of the effect was possible, Estracyt was markedly effective in 40 cases (21.1%), effective in 43 cases (22.6%), slightly effective in 38 cases (20.0%) and ineffective in 69 cases (36.3%). Adverse reactions were observed in 67 cases (33.5%), among which the administration was discontinued in 18 cases.

[Watery diarrhea, hypokalemia, achlorhydria syndrome due to recurrent malignant pheochromocytoma].

A case of malignant pheochromocytoma, with a recurrence 15 years after adrenalectomy and with an associated watery diarrhea, hypokalemia, achlorhydria syndrome, is reported. Histological evaluation of the tumors revealed composite malignant pheochromocytoma-ganglioneuroblastoma (well differentiated type). Vasoactive intestinal polypeptide and catecholamine levels were high both in the plasma and in the tumors. Somatostatin was also rich in the metastatic tumor of the liver, but not in the plasma. Immunohistochemical studies have demonstrated that immunoreactive vasoactive intestinal polypeptide is present in the ganglioneuroblastoma component, and that immunoreactive somatostatin is present in the pheochromocytoma component. Literature on the watery diarrhea, hypokalemia, achlorhydria syndrome associated with pheochromocytoma was reviewed.

Achlorhydria

[Diurnal variation and responses of corticoids to dexamethasone and ACTH before and after adrenal surgery in primary aldosteronism].

We studied diurnal variation and the responses of plasma corticoids to dexamethasone and ACTH before and after adrenal surgery in 11 patients with primary aldosteronism. Diurnal variation of plasma corticoids was examined in all cases. Before adrenal surgery, plasma aldosterone (Ald) was higher at all times, deoxycorticosterone (DOC) was high value at 5:00 and then normal value, 11-deoxycortisol (S) was high value at 5:00 and 9:00, thereafter normal value, while corticosterone (B) and cortisol (F) were almost normal value at all times. The circadian rhythm was observed in these corticoids. Dexamethasone (2 mg/day) was administered to 8 patients for 10 days. Plasma Ald and DOC were significantly suppressed only at 5:00, while B, S and F almost suppressed at all times. After dexa suppression, the circadian rhythm of Ald still observed, while the diurnal variation of the other corticoids was even. ACTH (1 microgram) was injected intravenously to 9 patients. Responses of DOC, B and Ald were higher, and of S and F were almost normal. Diurnal variation of plasma corticoids was observed after 1 month of adrenal surgery. Ald was lower, the other corticoids were normal values. ACTH (1 microgram) was injected to the same case before surgery. Responses of these corticoids to ACTH were slightly lower. Contents of Ald in adenoma tissues were higher than those in adjacent and normal adrenal tissues, and the contents of the other corticoids in adenoma tissue were almost normal value. As these results showed, before surgery these corticoids were secreted from adenoma without ACTH, although these were responsible to ACTH, and the presence of regulative factor of circadian rhythm except ACTH was suggested. After surgery, not only the responses to ACTH of mineralocorticoids but these of glucocorticoids decreased.

A monoclonal antibody to human transitional cell carcinoma of the bladder: production and characterization.

In order to detect bladder tumor specific antigens, monoclonal antibodies (MoAbs) to human transitional cell carcinoma of the bladder (TCCB) were obtained. Hybridomas were cloned after being prepared by cell fusion between mouse myeloma cell line X63Ag 8.653 and the spleen cells of BALB/c mice hyperimmune to the bladder cancer cells (grade 2) from a patient. Subsequently 12 MoAb-producing clones were obtained for the panel screening by enzyme immunoassay (EIA) and three MoAbs (no. 10, 11 and 14) were selected for reactivity to bladder cancer cells from patients, including normal epithelia. Finally No. 10 was selected as the most appropriate MoAb for this study and determined IgM with kappa-light chains by EIA. We accurately tested MoAb No. 10's reactivity with 71 malignant tumors by immunoperoxidase staining (IIP). No. 10 reacted to most TCCB cells (36/43), but did not react to any of the other tissues, including five normal epithelia, two brain tumors and three sarcomas. It was also shown by both IIP and immunofluorescent staining (IIF) that No. 10's reactivity was limited to the cancer cell membrane. These results suggest that the antigen recognized by MoAb No. 10 is one of the tumor-associated antigens and that some heterogeneity exists in its distribution.

A case of recurrent malignant pheochromocytoma complicated by watery diarrhea, hypokalemia, achlorhydria syndrome.

A patient is reported who had undergone right adrenalectomy for pheochromocytoma and 15 yr later developed a recurrence in the same site complicated by the watery diarrhea, hypokalemia, achlorhydria syndrome. This tumor was histologically defined as a composite malignant pheochromocytoma-ganglioneuroblastoma (well differentiated type). Vasoactive intestinal polypeptide and catecholamine concentrations were elevated in both plasma and the tumor. The tumor somatostatin content also was high. The tumor was immunohistochemically determined to contain both vasoactive intestinal polypeptide and somatostatin.

Production of a monoclonal antibody selective to human transitional cell carcinoma.

Psychiatric complications with chronic hemodialysis--importance of psychological and social care.

Because of a desire to provide the most suitable treatment for hemodialysis patients, psychological, physical and social problems influencing psychiatric conditions of the patients were studied. One hundred hemodialysis inpatients were compared in each group divided by their characteristics, i.e. age, presence of family problems, occupation, physical complications and their character traits. Psychiatric conditions were most favorable between 3 and 12 months after the introduction of hemodialysis, whereas in other hemodialysis terms depression was noticed in more than half of the subjects. In most of the patients, irritation and anger, and loss of brightness and sympathy were observed during the first 3 months or later than 12 months after the introduction of therapy, respectively. Some of the patients required explanations about the necessity of hemodialysis repeatedly and/or through consecutive psychotherapeutic interviews. This study suggested that a poor outcome of psychological conditions after the introduction of hemodialysis was indicated by the presence of family problems, symptoms of physical origin, the absence of full-time work, having a young child or children, age in the 20-40 year group, B and E type character traits classified by the Yatabe and Gilford test and a long duration of therapy.
