The effect of intravenous nitroglycerin on coronary blood flow and infarct size during myocardial infarction in conscious dogs.

Nitroglycerin (TNG), based on electrocardiographic evidence, has been shown to reduce myocardial ischemia, but its effect on morphometrically and enzymatically estimated infarct size has not been defined. Accordingly, coronary occlusion was produced in 50 conscious dogs without LV failure. Twenty-five received TNG (200-300 micrograms/min i.v. for 8 h) and the results compared with those in 25 untreated dogs. Coronary blood flow was measured with 141Ce, 85Sr, and 95Nb (9 microns) after occlusion before TNG, 30 min after TNG, and again at 8 h. Mean blood pressure decreased from 103 to 84 mmHg with TNG vs. 99 to 94 mmHg in controls (p < 0.02). Average heart rates were similar [135 +/- 26 vs. 120 +/- 33 beats/min (SD)]. TNG Did not increase total transmural coronary flow in any region but increased subendocardial flow in the central ischemic areas by 45% (0.09 ml/min/g vs. 0.13 ml/min/g). Animals were sacrificed after 24 h. Infarct size estimated morphometrically (25 +/- 1.5 vs. 26 +/- 1.5 of LV weight) and from myocardial CK depletion (23 +/- 2 vs. 23 +/- 2) was similar for the two groups. Thus, despite increased subendocardial flow, prolonged i.v. TNG did not decrease infarct size even though a difference of 15% would have been detected with this sample size. TNG May relieve coronary spasm but does not appear to be beneficial with sustained coronary occlusion.

The effects of intravenous nitroglycerin on hemodynamics, coronary blood flow and morphologically and enzymatically estimated infarct size in conscious dogs.

Nitroglycerin (TNG) decreases ST-segment elevation accompanying myocardial ischemia, but its effect on morphometrically and enzymatically estimated infarct size (IS) has not been defined. Accordingly, coronary occlusion was produced in 92 conscious dogs; 65 survived for 24 hours. Thirty-three received TNG (200-300 microgram/min i.v. for 8 hours) and the results were compared with those in 32 untreated dogs. Coronary blood flow (CBF) was measured with tracer microspheres (141Ce, 85Sr and 95Nb) 5 minutes after occlusion before TNG, 20 minutes after TNG and again at 8 hours. Mean blood pressure decreased from 103 to 84 mm Hg with TNG, vs 99 to 94 mm Hg in controls (p > 0.02). Nitroglycerin increased CBF in the subendocardium of ischemic areas by 45% (0.09 to 0.13 ml/min/g). The dogs were sacrificed after 24 hours and IS was estimated morphometrically (25 +/- 1% vs 27 +/- 1% of left ventricular weight) and from myocardial CK depletion (23 +/- 1% vs 24 +/- 1%) were similar for the two groups. Thus, despite increased subendocardial CBF, prolonged i.v. TNG did not decrease infarct size, although a 15% difference would have been detected with this sample size. TNG may relieve coronary spasm but does not appear to be beneficial with sustained coronary occlusion.

Mouse spleen cell populations involved in the mitogenic response to protein A.

In this paper, we examined the cellular components involved in the mitogenic response of mouse spleen cells to protein A. The mitogenic response was almost abrogated by the removal of macrophages. The abrogated response could be restored by the addition of peritoneal exudate macrophages from syngeneic mice. The reconstitution experiments were carried out for studying the populations of lymphocytes responding to protein A. In these experiments, spleen cells were passed through a Sephadex G-10 column and then they were separated into nylon-wool-adherent and nonadherent cells. The nylon-wool-nonadherent cells were used as T cells. The nylon-wool-adherent cells were used as B cells after they were treated with anti-BAT and complement. These results indicated that both B and T cells responded to protein A, and T-B cell cooperation could possibly play some role in the response.

[Clinical efficacy of S-6436 in urinary tract infection (author's transl)].

S-6436 was orally given to the patients mainly with uncomplicated acute cystitis at two divided doses for 7 days with the following results: 1) One or 2g/day of S-6436 were administered to the patients with 93.7% of effectiveness. There is almost no difference in effectiveness between 1 g and 2g dosages. 2) Bacteria in urine disappeared in 74% of the patients treated and superinfection occurred in 22%. The disappearance of the bacteria was observed in 83.3% of the patients with E. coli and 60% with Staph. epidermidis. The superinfection however was observed in 16.5% with E. coli and 33.3% with Staph. epidermidis. The disappearance of bacteria was observed in each one case with Klebsiella, Staph. aureus and Ps. aeruginosa. The superinfection was observed in 2 cases with Proteus. 3) Sensitivity tests on the organisms indicated that 94.4% of E. coli, 40% of Staph. epidermidis, Klebsiella and Staph. aureus were susceptible to the drug but Proteus and Ps. aeruginosa were not susceptible. 4) No remarkable side effects were observed, though digestive was used in several patients. From the above results, S-6436 is considered to be a useful drug which is expected to have as much effectiveness as regular cephalexin at two divided doses after breakfast and dinner.

Chloroquine resistance of Plasmodium falciparum in West Irian and East Kalimantan.

Evidence is presented of the chloroquine-resistance in Plasmodium falciparum in West Irian and East Kalimantan as shown by clinical observation and quantitative parasite counts in two patients hospitalized in Tokyo where no natural transmission of malaria occurs. The evidence is supported by the determination of plasma chloroquine levels, and in vitro sensitivity tests. The highest plasma level of chloroquine in the patient infected in West Irian was 197 ng/ml (fourth treatment) and 435 ng/ml (fifth treatment), and that in the patient infected in East Kalimantan was 312 ng/ml. Fifty percent inhibition of maturation was obtained in vitro at 0-6 and 0-62 nM/ml of chloroquine, respectively. Both patients were readically cured by 2 g of sulphadoxine and 100 mg of pyrimethamine given in a single dose.

Protein nutrition and aging.

Twenty-four female rats consisting of 6 sets of litters were used for the experiment. After weaning, rats were divided into 4 groups and fed with 10, 18, 27 and 36% casein diet. Effect of protein nutrition on aging was examined from the anthropometric and biochemical viewpoint. The difference of growth in body weight, and that of the urinary excretion of creatinine and 17-ketosteroids observed in the early period of growing disappeared by the end of growth period. From 1 year after birth, serum alkaline phosphatase activity and serum cholesterol were also measured. However, effect of protein nutrition on these parameters was not clear due to the scatter of data. The difference in diet did not affect life span in the present experiments, but the effect of the variance of litters on it seemed to be significant between the rats fed 18% casein diet and those fed 10% casein diet. Rats fed high-protein diet had a great number of lesions in the kidney and hypophysis, and often an incidence of tumors.

A study of the effects of propranolol and/or K 1366 on the distribution of Rb86 clearance in the canine heart under selective coronary hypotension.

Effects of beta-blockers o-[2-hydroxy-3-(tert.-butylamino)-propoxy]-benzonitrile (Kö 136 1366) and propranolol, on regional Rb86 clearance in the dog myocardium under selective coronary hypotension were investigated. Selective coronary hypotension of the left circumflex artery below 50 mmHg significantly decreased the Rb86 clearance and its I/O ratio of the involved myocardium. Kö 1366 and propranolol had a general trend to decrease the regional Rb86 clearance in the non-ischaemic region and to increase it in the ischaemic region when compared with that of the control group. Therefore, the gradient of regional Rb86 clearance between the non-ischaemic and ischaemic region may decrease. However, these changes were not statistically significant. Although both Kö 1366 and propranolol groups demonstrated similar haemodynamic changes, a reduction in heart rate was larger with propranolol infusion than with Kö 1366. On the contrary, a decreasing rate in systemic arterial blood pressure was marked with Kö 1366. Epicardial ECG ST elevation was reduced after propranolol infusion, not after saline or KO 1366 infusion.
